Individualized ibuprofen treatment using serial B-type natriuretic peptide measurement for symptomatic patent ductus arteriosus in very preterm infants

  • Purpose: Plasma level of B-type natriuretic peptide (BNP), an emerging, sensitive, and specific biomarker of hemodynamically significant patent ductus arteriosus (PDA), rapidly decreases in infants receiving cyclooxygenase inhibitors for ductal closure. We investigated the usefulness of serial BNP measurement as a guide for individual identification of early constrictive responses to ibuprofen in preterm infants with symptomatic PDA (sPDA). Methods: Before March 2010, the standard course of pharmacological treatment was initiated with indomethacin (or ibuprofen) and routinely followed by 2 additional doses at intervals of 24 hours. After April 2010, individualized pharmacological treatment was used, starting with the first dose of ibuprofen and withholding additional ibuprofen doses if the BNP concentration was <600 pg/mL and clinical symptoms of PDA improved. Results: The BNP-guided group received significantly fewer doses of ibuprofen than the standard group did during the first course of treatment and the entire study period. The need for further doses of cyclooxygenase inhibitors and for surgical ligation was not significantly different between the 2 groups. No significant differences were seen in clinical outcomes and/or complications related to sPDA and/or pharmacological treatment. Conclusion: Individualized BNP-guided pharmacological treatment may be used clinically to avoid unnecessary doses of cyclooxygenase inhibitors without increasing the ductal closure failure and the short-term morbidity related to sPDA.

  • This study was performed to investigate the physiological responses of Oplopanax elatus by water condition. Drought stress was induced by withholding water for 26 days. The results show that $P_{N\;max}$, SPAD, gs, E and Ci were significantly decreased with decreasing of soil moisture contents. However, AQY and WUE were decreased slightly only at 26 day. This implies that photosynthetic rate is reduced due to an inability to regulate water and $CO_2$ exchange through the stomatal. According to JIP analysis, ${\Phi}_{PO}$, ${\Psi}_O$, ${\Phi}_{EO}$ and $PI_{ABS}$ were dramatically decreased at 21 day and 26 day, which reflects the relative reduction state of the photosystem II. On the other hand, the relative activities per reaction center such as ABS/RC, TRo/RC were significantly increased at 26 day. Particularly, Dio/RC and DIo/CS increased substantially under drought stress, indicating that excessive energy was consumed by heat dissipation. These results of chlorophyll a fluorescence show that the sensitivity changes photosystem II activity. Thus, according to the results, O. elatus was exhibited a strong reduction of photosynthetic activity to approximately 10% soil moisture contents, and JIP parameters could be useful indicator to monitor the physiological states of O. elatus under drought stress.

Cross-Tolerance and Responses of Antioxidative Enzymes of Rice to Various Environmental Stresse

  • In order to examine the cross-tolerance of two chilling-tolerant cultivars (Donganbyeo and Heukhyangbyeo) and two chilling-susceptible cultivars (Hyangmibyeo and Taekbaekbyeo) to salt, paraquat, and drought, changes of physiological response and antioxidant enzymes were investigated. The seedlings were grown in a growth chamber until the 4-leaf stage. The seedlings were exposed to chilling at $5^{\circ}C$ for 3 days. For drought treatment, the seedlings were subjected to drought by withholding water from plants for 5 days. For paraquat study, plants were sprayed with $300{\mu}M$ paraquat. For the salt stress, the seedlings were transferred to the Hoagland's nutrient solution containing 0.6% (w/v) NaCl for 4 days. Chilling-tolerant cultivars showed cross-tolerant to other stresses, salt, paraquat, and drought in physiological parameters, such as leaf injury, chlorophyll a fluorescence, and lipid peroxidation. The baseline levels of antioxidative enzyme activities, catalase (CAT) and peroxidase (POX) activities in chilling-tolerant cultivars were higher than in the chilling-susceptible cultivars. However, there were no differences in ascorbate peroxidase (APX) and glutathione reductase (GR) activities between chilling-tolerant and -susceptible cultivars in untreated control. CAT activity in chilling-tolerant cultivars was higher than that in chilling-susceptible cultivars during chilling, salt, and drought treatments, but not during paraquat treatment. However, other antioxidative enzymes, APX, POX, and GR activities showed no significant differences between chilling-tolerant and -susceptible cultivars during chilling, salt, paraquat, and drought treatments. Thus, it was assumed that CAT contribute to cross-tolerance mechanism of chilling, salt, and drought in rice plants.

Source-sink Relationships of Soybean as Influenced by Drought Stress during the Pod and Seed-developing Stage

  • This study was conducted to investigate the influence of drought stress during the pod developing and seed filling stage on source-sink relationships of soybean (Glycine max). Drought treatments were imposed by withholding water at the full-pod stage, 19 days after flowering, and then limited watering was relieved at 15 days after the initiation of drought treatment. Soybean seed yield was reduced by 39% mainly due to decreased pod number under drought stress, but the 100-seed weight was relatively less reduced. In spite of the 15-day drought during the full-pod stage, soybean produced good seeds showing similar l00-seed weight, protein, starch and soluble sugar content to those from the well-watered. Although drought during the full-pod stage caused source limitations; i.e. accelerated leaf senescence and reduced leaf soluble sugars, it did not cause limitations of other source characteristics such as SGR and leaf starch level. This is because the reduction in size of sinks, such as pod and seed abortions compensated for source limitations, resulting in balanced source-sink as expressed by LAR and the ratio of leaf area to seed dry weight.   • The object of International License Contract is technology. Technology is means to produce visible goods, which are human's intellectual creations such as Intellectual Property Right - patent, design, trademark- and Know-how. Unlike visible goods which decrease as being used, these technologies are possible to be produced expansively and develop additionally. Therefore, the way to make a contract of goods is a sales contract which transfers ownership while technology follows license contract which gives approval of use for a certain period. International license contract means that licensor has right to possess, allows licensee to use licensed technology for a fixed period and takes royalty. So there are various matters such as selection of the duration of a contract, confirmation of technology range, competition limitation, technique guidance and support, calculation of royalty, withholding tax between parties. This study examines licensor's grant of license and competition limitation. Intellectual property rights fundamentally give exclusive rights to the creator so the licensor use or dispose of his or her intellectual property rights at will. Technology transfer is possible through license contract because of this right. But licensor must exercise his or her intellectual property rights within a reasonable limit. It means, when licensor makes an unreasonable demand abusing his or her position, it is regarded as competition limitation clause and the deal itself may become null. Therefore, restraint on competition needs to be examined in detail as it influences on contract validity. Each country has their own competition laws for establishing a fair market order and inspection guide and guideline for judging whether there is any unfair act related to intellectual property rights. Judgment on intellectual property rights is subject the technology-introduced country's domestic laws and thus, contracting parties each need to precede opposite nation's domestic laws system.

  • 쿠르노 모델은 경쟁적 전력시장을 게임이론 기반으로 모델링하기 위한 대표적인 모델이다. 이전 연구에서도 쿠르노 모델을 이용하여 전력시장을 모델링 하기 위한 다양한 시도가 이루어져 왔다. 쿠르노 모델은 몇 개의 주요 발전사업자들이 경쟁하고 그로 인해 시장지배력이 존재하는 과점 시장모델에 적합하다. 쿠르노 모델로 시장을 모델링함에 있어서는 우하향 하는 수요함수의 존재가 선결되어야 한다. 과점에서 시장참여자들은 시장지배력을 활용하여 그들의 이익을 극대화하려고 노력하지만, 우하향하는 시장수요함수에 의해 매출 역시 하락하기 때문에 적당한 지점에서 이러한 시장지배력의 행사를 제한하여야 한다. 스마트그리드에서는 실시간으로 변동하는 요금제와 다양한 전산기반 툴의 활용으로 인해 이러한 수요반응이 더욱 활성화될 것이고, 이 경우 쿠르노 모델은 수요반응 솔루션의 주요 모델로 활용될 것이다. 이에 본 논문은 실제 시장에서 계량경제학적인 접근으로 전력시장의 수요곡선을 추정하는 방법에 대해 제안한다.

Surgical Decision Making for the Elderly Patients in Severe Head Injuries

  • Objective : Age is a strong predictor of mortality in traumatic brain injuries. A surgical decision making is difficult especially for the elderly patients with severe head injuries. We studied so-called 'withholding a life-saving surgery' over a two year period at a university hospital. Methods : We collected data from 227 elderly patients. In 35 patients with Glasgow Coma Score 3--8, 28 patients had lesions that required operation. A life-saving surgery was withheld in 15 patients either by doctors and/or the families (Group A). Surgery was performed in 13 patients (Group B). We retrospectively examined the medical records and radiological findings of these 28 patients. We calculated the predicted probability of 6 month mortality (IPM) and 6 month unfavorable outcome (IPU) to compare the result of decision by the International Mission for Prognosis and Analysis of Clinical Trials in TBI (IMPACT) calculator. Results : Types of the mass lesion did not affect on the surgical decision making. None of the motor score 1 underwent surgery, while all patients with reactive pupils underwent surgery. Causes of injury or episodes of hypoxia/hypotension might have affected on the decision making, however, their role was not distinct. All patients in the group A died. In the group B, the outcome was unfavorable in 11 of 13 patients. Patients with high IPM or IPU were more common in group A than group B. Wrong decisions brought futile cares. Conclusion : Ethical training and developing decision-making skills are necessary including shared decision making.

토양 수분 스트레스와 차광 처리가 천궁의 엽록소 형광반응 및 광합성에 미치는 영향 (Effect of Soil Water and Shading Treatment on Chlorophyll Fluorescence Parameters and Photosynthetic Capacity in Cnidium officinale Makino)

  • Background: Measurement of chlorophyll fluorescence (CF) is useful for detection the ability of plants to tolerate environmental stresses such as drought, and excessive sunlight. Cnidium officinale Makino is highly sensitive to water stress and excessive sunlight. In this study, we evaluated the effect of soil water and shade treatment on the photosynthesis and leaf temperature change of C. officinale. Methods and Results: C. officinale was cultivated under uniform irrigation for 1 week drought stress (no watering) for 6 days. A significant decrease in CF was observed on the 5th day of withholding water (approximately 6% of soil water content) regardless of shading. Notably, the Rfd_lss parameter (CF decrease rates) with and without shade treatment was reduced by 73.1% and 56.5% respectively, at 6 days compared with those at the initial stage (0 day). The patterns of the degree of CF parameters corresponded to those of the soil water content and difference between leaf temperature (Ts) and air temperature (Ta). Meanwhile, CF parameters recovered to the 3 - 4 days levels after re-watering, while the soil water potential was completely restored. The suitable soil water content for C. officinale optimal growth was between -5 kPa and -10 kPa in this experiment. Conclusions: Lack of soil water in the cultivation of C. officinale, even with shading, decreased latent heat cooling through transpiration. As a result, heat dissipation declined, and the plant was subjected to drought stress. Soil water content plays a major role in photosynthesis and leaf temperature in C. officinale.

토양 건조 및 침수처리가 박태기나무의 광계 II 활성에 미치는 영향 (Effects of Soil Drought and Waterlogging on Photosystem II Activities in Cercis Bunge)

  • This study was conducted to investigate the photosystem II activities of Cercis chinensis by soil water condition. Drought stress was induced by withholding water and waterlogging treatments was immerging the pots for 15 days. Results showed that the relative activities per reaction center such as ABS/RC, TRo/RC and Dio/RC were significantly increased compared with the control group after 12 days in waterlogging treatments. Particularly, Dio/RC increased substantially under waterlogging stress, indicating that excessive energy was consumed by heat dissipation. Furthermore, the performance index on absorption basis(PIabs) and responses to structural and functional PS II(SFIabs) were dramatically decreased after 15 days in both the drought and waterlogging treatments, which reflects the relative reduction state of the photosystem II. These results of chlorophyll a fluorescence by OKJIP analysis show that the sensitive changes photosystem II activity. Thus, on the basis of our results that Cercis chinensis was exhibited a strong reduction of photosynthetic activity to waterlogging stress, and OKJIP parameters such as ABS/RC, DIo/RC, PIabs and SFIabs could be useful indicator to monitor the physiological states of Cercis chinensis under soil water condition.

Beliefs and Attitudes toward Physician-assisted Suicide among Korean Adults

  • Purpose: To grasp public opinion accurately, we conducted an opinion poll on beliefs and attitudes toward physician-assisted suicide (PAS). Methods: A randomized telephone survey ensuring a representative sample was conducted, 1,007 participants aged 18 years or older (response rate, 9.5%). Results: The main results are as follows: i) 61.1% of participants thought that the current social support system for terminally ill patients and their families is insufficient; ii) 60% of participants did not recognize the term "hospice and palliative care"; iii) 81.7% of participants would not like to receive life-sustaining treatment if there is no possibility of recovery; iv) 58.4% of participants would like to receive hospice and palliative care if they are diagnosed with a terminal illness; v) the priorities for dignified dying were preparing a support system to reduce the burden of care (28.6%), economic support including reduction of medical expenses (26.7%), expansion of hospice and palliative care services (25.4%), and legalization of PAS (13.6%); and vi) 58.3% of participants agreed that the expansion of hospice and palliative care should precede the legalization of PAS. Conclusion: Koreans currently want other efforts, including expansion of hospice and palliative care services, instead of the legalization of PAS.
