The Effect of Gamisungmagalguntang(加味升麻葛根湯) on Chronic Urticaria Identified as "Wind Heat" Pattern (풍열형(風熱型) 만성 두드러기에 대한 가미승마갈근탕(加味升麻葛根湯)의 치료효과)

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.

  • This study was clinically performed with the 73 outpatients of urticaria who had visited the Dept. of Dermartology, Oriental Medical Hospital, Dong-eui University from January 1st 2000 to June 30th 2003. They have all taken the oriental herb medicine as Sungmagalguntanggamibang(SGT, 升麻葛根湯加味方) more than 1 time. Urticaria is a common disease and a well known reaction pattern of the skin charaterized by erythema, wheals and edema, that is the results of various causes. Urticaria is classified into acute or chronic type. The etilogy of acute uricaria has been determined in many cases, but the cause of chronic urticaria has been determined in 10$\%$ to 20$\%$ of cases. Character of chronic type urticaria is recurrent pruritus and erythematous skin disease by stimulating factor. Through checking up the present patient state, we verified the effect of SGT in various ways. The results were summarized as follows; 1. An increase of taking a medicine(SGT) and attending a hospital in number let down repeated urticaria occurence. 2. SGT is more effective when the symptoms with the severe pruritic wheals are spread on the whole body in daily occurrence. 3. In urticaria treatment method, gradually reducing a western medicine and going side by side SGT, that is good at symptoms improvement. 4. Etiology of urticaria is classified into intestine-stomach accumulated heat by indigestion(食滯腸胃積熱), wind-moisture(風濕), wind-heat(風熱) etc. The SGT is available in the order of frequency of use.

Four Case Reports of Chronic Urticaria (만성 두드러기 환자 치험 4례)

  • Urticaria is a common disease and a well known reaction pattern of the skin characterized by erythema or wheal and edema, that is result of a local leakage of plasma from minute vessel into the connective tissue of the dermis. The urticaria that lasts for more than six of eight weeks, is categorized as chronic urticaria. In Oriental medicine, urticaria is classified as Yeun-jin(慇疹) and similar to Poong-yeun-jin(風慇疹), Poong-so-yeun-jin(風瘙慇疹), Poong-yum-leu, Poong-dan(風丹) and Poong-jin-geu(風疹塊). We report four improved cases of chronic urticaria that had lasted for more than eight weeks. We regarded their chronic urticaria as wind-heat(風熱) and prescribed Bangpungtongsungsan-gami(防風通聖散加味) and Seunggaltang-gami(升葛湯加味) to these patients. These patients showed improvement of chronic urticaria.

  • Objectives : The purpose of this study is to report 1 clinical experience of korean medicine treatment on chronic urticaria identified as "Wind Heat" pattern. Methods : We treated a 63-year old woman patient with Gagamtongcheong-san, acupuncture and pharmacopuncture. The improvement of patients evaluated using CU-Q2oL, UAS, and photography. Results : CU-Q2oL score decreased from 95 to 18, UAS decreased from 6 to 0. Conclusions : The complex treatments of korean medicine and was effective to improve of chronic urticaria. It is necessary to have more observations and case reports.
