A Study on the Theory of Latent Qi Wenbing -Focusing on Its Background and Logicality- (역대 복기온병설(伏氣溫病說)에 대한 고찰 -학설의 배경과 논리성을 중심으로-)

  • Objectives : The theory of Latent Qi Wenbing and its counter-arguments were examined in terms of each argument's theoretical background and logicality. Methods :Arguments for and against the theory of Latent Qi Wenbing in medical texts were examined closely. Results & Conclusions : Each doctor's theory on Latent Qi Wenbing was examined in the following four aspects. First, to what kind of pathogenic qi was the patient exposed during Winter? Second, how did the pathogenic qi affect the body in the time of exposure? Third, how did the latent qi change over the Spring and Summer within the body? Fourth, what kind of weather generates the disease during Spring and Summer? Each argument had different opinions on each aspect. The main argument made against the theory of Latent Qi Wenbing is that it is impossible for the pathogenic qi that initially damaged the body to stay latent until it generates disease after some time. The theory of Latent Qi Wenbing could be assumed to have been created based on situations in which such explanation was adept. Factors that contribute to this theory could be explained through the condition of the healthy qi, state of cultivation during the past season, and constitutional factors.

  • The Wenbing tiaobian(온병조변) is the first systematic disease monograph on warm factor disease(온병), written by Wu Jutong(오국통). It was most influenced by the Linzheng zhinan yian(임증지남의안), which was a book of gathering medical case records of Ye tianshi(엽천사). Therefore, there are plenty of quotations from this book in the Wenbing tiaobian. This study investigates the quotations from Linzheng zhinan yian, especially focusing on the way of transformation and correction of the original medical case records on the warm factor disease. The results are as follows: About 104 of 265 provisions in the Wenbing tiaobian were directly quoted from the Linzheng zhinan yian. The provisions quoted from Ye's case records were rearranged according to the categories of the triple burners pattern differentiation(삼초변증) and the causes of warm factor disease. And eve case record was transformed into more general descriptive form in order to put it into the book. For example, the specific figures, and some patients' peculiar symptoms, causes, sex and disease names were omitted. On the other hand, the tongue moss, pulse shape and some symptoms, which were necessary for differentiating patterns, were added. In the case of the formula, some formulas originated from Ye's case records were named newly. And the dose of each herb consisting a formula was determined, and therapeutic principle, taking method and detailed explanation was added to every formula.

  • Developing of antibiotic, bacterial meningitis is one of the disease of high mortality. Especially in case of gram negative, pneumococal meningitis, they have high mortality and neurological disorders after treatment. Main symptoms of bacterial meningitis are fever, headache, vomit, neck stiffness and coma etc. In oriental medicine, acute feverish infectious diseases have been treated as wenbing(溫病). We can divide wenbing into 8 kinds. Bacterial meningitis is included as Chunwen(春溫), fengwen(風溫) in the sight of similarity on the symptoms and falling ill. Comparing with CVA, we have too rare cases of treating bacterial meningitis with oriental medicine. A case of sequelae of bacterial meningitis patient diagnosed as Chunon, pungon showed prominent improvement by herb med and acupuncture treatment etc.

  • Objectives : Zheng Zhi Xin Chuan(證治心傳) is a compact compilation of 14 discussions on medicine, written by Yuan Ban(袁班) in the late Ming-early Qing period. Methods : To survey the scholarly position of Yuan Ban(袁班) in the history of Wenbing by systematically analyzing and organizing Wenbing related theories in Zheng Zhi Xin Chuan (證治心傳). Results & Conclusions : In his book, YB suggests concepts such as 'mainly attack upper heater(多犯上焦)', 'transmission to the nutrient phase(轉入營分)', 'sequential transmission(順傳)', 'abnormal transmission(逆傳)', 'dryness invasion of human body in autumn(秋傷於燥)'. In the history of Wenbing, it has been widely acknowledged that the concept of weiqiyingxiebianzheng(衛氣營血辨證) and abnormal transmission to the pericardium(逆傳心包) were originally suggested by Ye Tianshi(葉天士). However, according to the findings of this study, these concepts are traced back to the contents of YB's publication, nearly a century before Ye's time. In addition, YB's discussion on '秋燥' was highly advanced than any other scholar of his time, hinting his influence on medicine thereafter.

A Study on the Concept and Clinical Application of "冬不藏精, 春必病溫" ("동부장정(冬不藏精), 춘필병온(春必病溫)"의 개념 및 임상활용에 대한 고찰)

  • Objectives : The verse, "冬不藏精, 春必病溫", which had much influence, has been falsely referenced as contents of "Neijing". This study aims to examine this process, while looking at its theoretical meaning within clinical practice. Methods : The origins of the concept "冬不藏精, 春必病溫" are explained through annotations of "Neijing" and Li Dong Yuan(李東垣)'s interpretation of Shanghan(傷寒), while its clinical meaning is examined through theories of doctors's who applied the concept in their practices. Results : The beginning of "冬不藏精, 春必病溫" could be traced back to Li's annotation of the verse "冬傷于寒, 春必病溫" from the "Neijing", where he based his understanding on the concept "少陰不藏." Since then, the discourse on "冬傷于寒, 春必病溫" has been replaced by "冬不藏精, 春必病溫" as a new concept to explain the pathology of Wenbing(溫病). These discussions followed the line of thought that by failure to store Jing(精) during the winter, Cold pathogen would infiltrate, hiding itself in Shaoyin(少陰). Various arguments on how the Cold pathogen would develop into Wenbing and its treatment followed. Conclusions : "冬不藏精, 春必病溫" can be understood as the result of a new interpretation of "冬傷于寒, 春必病溫" through the perspective of Cold pathogen infiltration based on "少陰不藏".

Structuralistic Analysis for Establishment of Concept and Variables of Shanghan Diseases (상한병(傷寒病)의 개념(槪念)과 논치근거(論治根據) 확립을 위한 구조주의적 분석)

  • In order to grasp the clear concepts and variables to treat Shanghan diseases which has complex meaning by each medical literature, several concepts and analytic method of structuralism from early to late stage encompassing Saussure and Derrida were used. Main concepts are langue and parole, signifiant and signifie, syntagme and paradigme, denotation and connotation, synchronie and diachronie, identity of structure and differance etc. and methods are substituting these concepts to historical Shanghan theories from Zhongjing to Ming-Qing dynasty and comparisons of synchronie about their era. Essential qualities of Shanghan diseases are pathologic phenomena under the order of unification of nature and man formed through concrescence between individual human body and geo-climatico-socio-cultural environmental conditions, neither injuries by cold pathogen nor five types of exopathogenic febrile diseases. The former environmental elements can be inferred from general traits of desires and public pathological aspects of social members, and the latter personal factors from corresponding features to those pathogenic variables. In addition, the concepts of Shanghan disease are added successively up to now via Jin-Yuan's four great masters and Wenbing masters, and thus the new concepts of denotation became another connotation obtaining new signifiant. In this way, for the treatment of Shanghan diseases, new theories should be made for ranging prescription over the wenbing field; reflecting not only climatical variables but also each patient's physio-pathological features and sociocultural variables. Thereby an appropriate and reasonable therapeutical systems can be designed and can guide research direction hereafter.

A Study on Yinyangjiao (음양교(陰陽交)에 관한 고찰(考察))

  • Objectives : This study focuses on the condition Yinynagjiao, which was first mentioned in 『Huangdineijing』, on the cause, mechanism and reason for it being described as fatal. Later doctors's understanding and treatment of this condition were examined subsequently. Methods : Verses related to Yinyangjiao from texts such as 『Huangdineijing』, 『Jinkuiyuhanjing』, 『Maijing』 were cross-examined focusing on their context and difference in expression with reference to annotations and later texts that mention Yinyangjiao. Based on the findings, its mechanism and treatment methods as found in Wenbing texts were compared with descriptions from previous texts. Results & Conclusions : According to 『Huangdineijing』, heat disease belongs to the Shanghan category, treated through promoting perspiration. In the confrontational position between JingQi and XieQi, perspiration could be understood as the result of JingQi's victory that lead to the discharge of XieQi in the form of sweat. Yinyangjiao is the opposite situation where Jing is discharged while heat toxin sinks into the body, thus being fatal, and treatment aimed for perspiration not adequately addressing the main problem which was ultimately expected to resolve through means of solid Yin Jing that would stabilize the pulse in due time. On the other hand, Wenbing scholars saw the pathogen as heat, shifting previous perspectives entirely. As a result, instead of applying pungent/warm medicinals to promote sweating, they suggested the use of cool, sweet/moist medicinals to cool the heat and nourish Yin as means of treating the condition.

  • This study is a research on the etiology in Wu Youke's wenyi theory. In regard to the etiology of epidemic disease that had been spread on a very large scale at that time, Wu Youke denied the traditional theory which urged the irregular change of climate as the cause of epidemic disease, and proposed the concept of 'zaqi' which was considered by him to be something that could be the real cause of epidemic disease. And He treated the wenyi disease as something that has the same meaning with wenbing, so his concept on wenbing was basically the thing that treats 'zaqi' as the fundamental cause of wenbing and treats the concept of 'wen(溫)' as an environmental cause that could help activate the virulence of 'zaqi'. Such concept like this was the thing somewhat different from the traditional etiological theory that considers the change of climate as the principal cause of waigan(外感)-disease, and it must for the most part have been originated from the experience of Wu Youke himself. But this study, in contrast, based on the thing he denied the traditional theory on the irregular change of climate, has been done in the point of view that fundamental concept of his wenyi theory such as 'zaqi' was not only originated from his clinical experience but also from the influence of paradigm shift in the natural philosophy of that time. There had been so much change in cosmology and natural philosophy from the fundamental basis at that time, and the the most principal concept of it was that there always exists irregular faces in the change of nature. Such concept like this got into its stride from about 17th century, and it was expressed in the form of the severe criticism against the traditional natural philosophy. In regard to this, this study has outlined the academic thought of the leading scholars who made a significant progress in such a paradigm shift, and it includes the scholars like Wang Tingxiang, Wang Fuzhi, Hu Wei, Huang Zongxi, who played their role in the time of the latter period of Ming dynasty and the former period of Qing dynasty.

A Study on the Mechanism of Opening-with-Pungent/Lowering-with-Bitter Method of Wenbing and the Spleen/Stomach Disease Treatment in Piweilun (온병학(溫病學) 신개고강법(辛開苦降法)과 『비위론(脾胃論)』의 비위병(脾胃病) 치료 기전에 대한 고찰)

  • Objectives : The aim of this paper is to compare the mechanisms of the OP/LB method and the SSD treatment in 『Piweilun』. Methods : Wenbing texts, articles on the OP/LB method, and the 『Piweilun』 was examined for comparison of treatment mechanisms of SSD. Results : The mechanism of the OP/LB method in treating SSD was to treat the Spleen and Stomach separately, to restore the ascending/descending pattern of qi through simultaneous use of pungent and bitter flavors which raises and lowers, respectively. Moreover, the use of medicinals with contrary properties regulate the other's biased nature, and none of the medicinals create dampness. The pungent and bitter flavors play central roles, where the pungent flavor opens and communicates and the bitter flavor clears and lowers. The treatment method of SSD in the 『Piweilun』 treat the Spleen and Stomach together, with a focus on raising and dispersing through upraising yang and reinforcing qi by means of Wind medicinals with pungent and bitter flavors added to sweet and warm medicinals. Conclusions : Owing to the expansion of the OP/LB method concept by modern-contemporary scholars, the potential for treating SSD as can be seen in the 『Piweilun』 with the OP/LB method has emerged. The similarity between the OP/LB method and treatment of SSD in the 『Piweilun』 is that the objective of the formulas is to lower fire heat and communicate qi to help qi movement. This common objective allows for treatment of SSD within Wenbing and those in 『Piweilun』 with the said formulas through appropriate modification fit for each situation.

Synchronized Expression of Two Bombyx mori Caspase Family Genes, ice-2 and ice-5 in Cells Induced by Ultraviolet Irradiation

  • The caspase family proteins play an important role in programmed cell death (apoptosis). To date, the expression profiles of the caspase family genes in Bombyx mori (Bm) are poorly known. In this study, we examined the expression profiles of two novel Bm caspase family genes (ice-2 and ice-5), the potential change of the mitochondrial membrane and the morphology in Bm cells after stimulation of ultraviolet (UV) irradiation. The results showed the potential change of the mitochondrial membrane occurred at 5 hours after UV irradiation treatment. Analysis of fluorescent quantitative RT-PCR demonstrated that both the ice-2 and ice-5 might be involved in UV induced apoptosis in Bm cells. Notably, after UV irradiating, expression pattern of ice-2 and ice-5 were remarkably different. The ice-2 gene was highly expressed at two time points, 0.5 and 5 hours after UV stimulating, while the expression level of ice-5 only peaked at 5 hours after UV stimulating. It indicated that apoptosis induced by UV irradiation was involved in the mitochondrial pathway and the two isoforms of Bm ice may act but play different role during the apoptosis of Bm cells.
