A review of recent evidence of dietary protein intake and health

  • The Korea National Health and Nutrition Examination Survey of 2013 to 2017 reported that the average protein consumption of the Korean population is above the current recommended nutrient intake of protein proposed by the Dietary Reference Intakes for Koreans. Some health professionals and the media often advise consuming diets high in protein for promoting metabolic regulation, weight control, and muscle synthesis. However, due to lack of scientific evidence, the validity and safety of high protein consumption are yet to be fully ascertained. The present review assesses recent evidence published in 2014-2020 from human studies, focusing on adequate protein intake and protein sources for the prevention of chronic diseases, particularly metabolic disorders and sarcopenia.

Overweight, Obesity, Oxidative Stress and the Risk of Breast Cancer

  • There is growing scientific evidence linking excess body weight to breast cancer risk. However, there is no common consensus on this relation due partly to methodologies used, populations studied and the cancer subtype. We report here a summary of the present state of knowledge on the role of overweight and obesity in pathogenesis of breast cancer and possible mechanisms through which excess body weight might influence the risk, focusing on the role of oxidative stress in breast cancer etiology. The findings demonstrate duality of excess body weight action in dependence on menopausal status: a statistically significant increased risk in postmenopausal overweight/ obese women and non-significant preventive effect among premenopausal women. Due to several gaps in the literature on this topic, additional studies are needed. Future research should address factors influencing the excess body weight - breast cancer relationship, such as race/ethnicity, tumor subtype, receptor status, the most appropriate measure of adiposity, reproductive characteristics, and lifestyle components.

  • Reactive oxygen species(ROS) are produced at a high rate continuously as a by-product of aerobic metabolism. Several lines of evidence provided that ROS appears to cause to develop aging and various diseases. High level of hyaluronic acid with decreased molecular weight has been detected in patients with inflammatory diseases including rheumatoid arthritis. Hyaluronidase is an endohexosaminidase that initiates the degradation of hyaluronic acid with high molecular weight. Prunus persica Batsch var. davidiana Maximowicz has been known as a korean folk medicine for treatment of neuritis and rheumatism. (omitted)

  • This study was performed to develop a tailored multi-disciplinary obesity management program, as well as an obesity assessment sheet identified the general characteristics and psychological and environmental factors related to weight loss success for obese premenopausal women participating in obesity management programs performed at weight reduction health centers. The results are as follows. The primary general characteristics related to successful weight reduction in the obesity management programs were education level, occupational status, reason for obesity onset, aids used in weight control, basis of frequency, and reason for weight reduction. The initial psychological factors related to successful weight reduction were less stress and disordered eating. The initial environmental factors included an expression of ones opinion and the eating environment. Animal protein and animal fat consumption were significantly less in the unsuccessful group than in those who were unsuccessful at weight loss. The consumption of crude fiber was significantly greater in the successful group than in the unsuccessful group. Psychological factors such as stress, depression, and disordered eating, and the environmental factor of eating environment were significantly better in the successful group than in the unsuccessful group. In conclusion, we have found evidence to suggest what is required in determining the types of programs most suitable for obese women prior to starting an obesity management program.

마황성분 체중감량 한약제제가 심혈관계에 미치는 영향: 3차원 맥영상 검사 연구 (Hemodynamic Effects of Herbal Prescription Containing Ephedra on Weight Loss: A 3-Dimensional Radial Pulse Tonometry Device Study)

  • Objectives: This study was aimed to evaluate the hemodynamic feasibility using pulse parameters as a way to establish safe dose guidelines for herbal prescription containing Ephedra herb (Ephedra intermedia Schrenk & C.A.Mey) on weight loss, and to provide a foundation for developing evidence-based guidelines for clinical use. Methods: Forty-two volunteers were recruited to participate in a study examining the changes in pulse wave characteristics following the ingestion of Gambi-hwan, a herbal prescription containing ephedra, over a period of 4 weeks, and pulse wave measurements were taken before and after the administration. Pulse wave parameters were measured in this study using a 3-dimensional radial pulse tonometry device (DMP-Lifeplus). In addition, questionnaire, blood pressure, temperature, and body composition were also measured as secondary measures. Results: Fifteen minutes after administration of Gambi-hwan, the non-adverse event group (non-AE) exhibited a statistically significant increase in several power and pressure-related parameters, including h1, h5, systolic area, pulse area, and pulse width, while the AE group showed a trend of decreasing stroke volume and increasing Radial Augmentation Index (RAI), w, and w/t. After 4 weeks of administration, both groups exhibited significant changes in pulse rate, w/t, RAI, t3/t, stroke volume (SV), and stroke volume Index (SVI). Notably, there are significant differences between AE group and non-AE group in w/t, SV, and SVI. Conclusions: These findings suggest that pulse parameters may be a useful way to establish safe dosing guidelines for weight loss herbal prescription containing ephedra. Further research is needed to confirm these results and to develop evidence-based guidelines for clinical use.

식이 내 비타민 D 강화가 성장기 흰쥐의 골밀도에 미치는 영향 (Effects of Vitamin D Supplementation on Bone Mineral Density in Growing Rats)

  • Modification of the diet during childhood and adolescence may be an effective strategy for maximizing the peak bone mass. Many supplementation studies have suggested a positive effect of the increased vitamin D intake on the bone mineral status in the elderly. However to date all studies have been conducted on old men and postmenopausal women. The aim of this study was to examine the effects of vitamin D supplementation on the bone mineral density and bone mineral content in growing rats. Twenty Sprague-Dawley female rats were divided into two groups; Control, and vitamin D supplementation. The bone mineral density(BMD) and bone mineral content(BMC) were measured using PIXImus in the spine and femur. Vitamin D supplementation did not affect the level of weight gain, mean food intake and food efficiency ratio. In addition, vitamin D supplementation had no added effect on the spine and femur BMD, and BMC. There were no significant differences in the spine BMD/weight and BMC/weight between the groups, but the spine BMD/weight and BMC/weight was 11 % higher in the vitamin D supplementation group. The femur BMD/weight and femur BMC/weight were significantly higher in the vitamin D supplementation group 9 weeks after the experiment. These results provide evidence of the beneficial effects of vitamin D supplementation on the BMD during the growth period.

간헐적 단식의 임상적 적용 (Clinical Implications of Intermittent Fasting)

  • Dietary therapy is one of the most important treatments for obesity. In general, it is difficult to maintain the recommended diet for weight control for a long time; therefore, it is difficult to achieve weight loss or maintain weight. Intermittent fasting has recently become one of the most popular diets for weight loss. Intermittent fasting is a strategy of repeating intermittent energy restriction and eating, unlike conventional diets of continuous calorie restriction. Studies on intermittent fasting have shown positive results not only in terms of weight loss but also improvement in metabolic indicators, such as glucose control and reduction of blood pressure. Therefore, it is important to maintain a long-term dietary strategy to prevent weight loss in obese individuals. The effect on weight loss was similar to that of an existing continuous energy-restricted diet. However, long term studies and safety data are still lacking, and large-scale studies with various populations are needed. If more evidence is secured for various individuals, it can be expected that intermittent fasting, including time-restricted eating, will be applied clinically in the future.

Fluconazole prophylaxis against invasive candidiasis in very low and extremely low birth weight preterm neonates: a systematic review and meta-analysis

  • Background: Evidence shows that fluconazole prophylaxis is an effective treatment against invasive fungal infections in preterm neonates, however, the most efficient schedule of fluconazole prophylaxis for the colonization and mortality of invasive candidiasis (IC) is unknown. Purpose: This systematic review and meta-analysis aimed to assess the efficiency of different prophylactic fluconazole schedules in controlling IC colonization, infection, and mortality in very low birth weight (VLBW) and extremely low birth weight (ELBW) infants in neonatal intensive care units. Methods: We searched the PubMed, Scopus, Embase, and Cochrane databases using the keywords "candida," "invasive candidiasis," "IC," "fluconazole prophylaxis," "preterm infants," "very low birth weight infants," "VLBW," "extremely low birth weight," and "ELBW." Results: Mortality was significantly decreased in a meta-analysis of studies using different fluconazole prophylaxis regimens. The meta-analysis also indicated a significant decrease in the incidence of IC-associated mortality in ELBW infants using the same fluconazole prophylaxis schedules. Conclusion: Future studies should explore the effectiveness of other different fluconazole prophylaxis schedules on IC colonization, infection, and mortality.

"Cohesiveness of Hyaluronic Acid Fillers": Evaluation Using Multiple Cohesion Tests

  • Background Hyaluronic acid fillers can be manufactured using various processes. They have multiple properties, including their concentration, degree of modification, and rheological data. Cohesion is one such property to evaluate gel integrity; however, there is no standardized method for calculating this parameter. This study aimed to evaluate different tests for calculating hyaluronic acid cohesion and discuss the importance of hyaluronic acid cohesion as a consideration when selecting fillers. Methods The cohesion levels of five different hyaluronic acid fillers with different rheological properties were evaluated and compared using the drop weight, compression, tack, and dispersion time tests. Results The cohesion tests yielded different results in the samples. Samples 2 and 4 showed approximately two times the number of droplets when compared with Sample 5 in drop weight test. Samples 1, 2, 3, and 4 were superior to Sample 5 in tack test. Samples 1, 2, and 3 showed cohesive appearances at 95 seconds in most cases in dispersion test. Rheological test results did not reflect the measures of cohesion. Conclusion Although there are no definite standardized tests to evaluate the cohesion of hyaluronic acid fillers, our proposed tests showed similar results for different hyaluronic acid filler products. Further studies are needed to evaluate the cohesion of hyaluronic acid fillers and determine the clinical use of this distinguishing characteristic for clinicians selecting the product of choice. Effect of Soy Protein and Exercise on Bone Mineral Density and Bone Mineral Content in Growing Male Rats

  • The purpose of this study was to examine the effects of dietary protein and exercise on bone mineral density and bone mineral content of growing male rats. Forty male, Sprague-Dawley rats(age 21 days) were assigned to four groups that underwent 9 weeks of experimental treatment. Animals were assigned to one of two exercise treatments (treadmill running or sedentary). The exercise and nonexercise group were fed a diet containing casein or soy with rich isoflavones (3.4mg/g protein). The exercise group ran on a rodent treadmill(speed of 15m/min for 30min) three days per week during the 9-week study period. All rats were fed an experimental diet and deionized water ad libitum for 9 weeks. Total bone mineral density (BMD), total bone mineral content (BMC), total body calcium, spine BMD and BMC, and femur BMD and BMC were determined by using dual energy x-ray absorptiometry (FIXI-mus, GE Lunar Radiation Cooperation, Madison, WI, USA). The soy diet group appears to have a significantly higher total BMD/weight and total BMC/ weight, spine BMD/weight, spine BMC/weight, femur BMD/weight and femur BMC/weight compared to the casein group in nonexercise and exercise. The exercise group had significantly greater total BMD/weight and BMC/ weight, spine BMD/weight and BMC/weight, femur BMD/weight and BMC/weight compared to the nonexercise group when the protein source was casein. The exercise combined soy group had significantly greater total BMD/weight and BMC/weight, spine BMD/weight and BMC/weight, femur BMD/weight and BMC/weight, compared to the exercise combined casein group. The results indicate that exercise had a positive influence on bone mineral density and bone mineral content and soy significantly affect on bone mineral density and bone mineral content for the 9 weeks experimental period. It can be concluded that exercise combined with a soy diet is most beneficial for acquisition of spine bone mineral density in young growing male rats. This convincing evidence suggests that a change in life style such as increasing exercise and consumption of soy protein is a practical strategy for significantly reducing the incidence of osteoporosis.
