A Case Study of a Patient with Diplopia and Bilateral Facial Palsy Due to Atypical Miller Fisher Syndrome: Treatment with Complex Korean Medicine

  • Miller Fisher syndrome (MFS) is a rare variant of Guillain-Barré syndrome characterized by ocular paralysis, ataxia, and insensitivity. This report describes the effect of Complex Korean Medicine Treatment (CKMT) on a patient previously diagnosed with MFS presenting with diplopia and facial palsy. The distance at which diplopia occurs, the diplopia questionnaire, the range of diplopia, the degree of facial paralysis, and the degree of ptosis were evaluated at the time of admission and weekly for 1 month. After receiving CKMT for 4 weeks the 62-year-old female had improved symptoms of diplopia, bilateral facial palsy and ptosis caused by MFS. These results show the significant association of MFS with facial paralysis and the improvement achieved with CKMT.

Implementation of structured trauma training for firstyear surgical residents in Ethiopia: a novel pilot program in a low income country

  • Purpose: Curricula for surgical residents should include training in trauma care; however, such training is absent in many low income countries. At the largest surgical training institution in Ethiopia, a trauma training program was developed, integrated into the existing surgical curriculum, and implemented. This study was conducted to evaluate the trainees' response to the new program. Methods: Over a 5-month period, 35 first-year surgical residents participated in weekly trauma care training sessions. The program included journal clubs, practical sessions, didactic sessions, and case-based discussions. Six months after the conclusion of the training, changes in knowledge, attitude, and practices were evaluated through a self-report survey. Results: For knowledge-based items, the survey data revealed reported improvements in 83.8% to 96.8% of students. Furthermore, 90.3% to 93.5% of participants indicated improvements in practice, while 96.7% exhibited a change in attitude. Respondents reported that attending didactic courses improved their presentation skills and facilitated the acquisition of knowledge. They suggested the inclusion of additional practical sessions. Conclusions: Training structures that are simple to implement are crucial for residency programs with limited resources. Such programs can be developed using existing academic staff and can aid residents in delivering improved care to trauma patients.

Influence of Rainfall on Cyanobacterial Bloom in Daechung Reservoir

  • The water quality and algal communities in the Daechung Reservoir, Korea, were monitored from summer to autumn in 1999 and 2001. Although the average weekly precipitations during June and July were very similar in 1999 and 2001, they were much different during August and September, the so-called blooming season. The rainfall in 1999 increased about 70% after late August, whereas it decreased to the one-fifth level in 2001. The higher concentrations of chlorophyll- a, phycocyanin, and cyanobacteria were observed in 2001, which resulted in the dense algal bloom. In addition, in 2001, the cyanobacterial percentage remained above 80% during the investigation period, and the cyanobacteria were exclusively composed of Microcystis spp. Conversely, there was no report on the algal bloom in 1999. However, the peak bloom seasons were the same for both years, from late August to early September, irrespective of the amount of precipitation. These results suggest that the magnitude and duration of rainfall before bloom season are important factors determining the extent of cyanobacterial bloom in this system.

  • A 60-year-old man was diagnosed with stage IV squamous cell carcinoma of lung and treated with weekly doses of docetaxel and cisplatin. Tumor mass and mediastinal lymphadenopathy disappeared after 4.5 cycles of chemotherapy. At one week post final chemotherapy, the patients developed sudden shortness of breath. New, multifocal infiltrations developed on both lungs without definitive evidence of infection. Despite administration of broad spectrum antibiotics, the lung lesion did not improve, so bronchoalveolar lavage and computed tomography-guided lung biopsy were performed. The proportion of lymphocytes was increased markedly and histopathology revealed squamous cell carcinoma combined with bronchiolitis obliterans organizing pneumonia. After high dose corticosteroid therapy, dyspnea and the newly developed consolidation had decreased slightly. However, dyspnea and hypoxemia increased again because of aggravated lung cancer since chemotherapy had stopped. Chemotherapy couldn't be restarted due to the poor performance status of the patient. Later, patient died of respiratory failure from poor general condition and progression of lung cancer.

The correlation of depression with Internet use and body image in Korean adolescents

  • Purpose: To examine the correlation of depression with Internet use and body image perception, and to analyze the risk factors of depression in a total of 920 students in Seoul, Korea. Methods: Students were recruited by contacting school principals and teachers and were encouraged to fill out a self-report questionnaire designed specifically for this study in July of 2008. Results: Female participants had an increased risk for depression than did male participants (adjusted odds ratio [aOR], 1.790; 95% confidence interval [CI], 1.330-2.410, P<0.001). Older students were more susceptible to depression (aOR, 1.246; 95% CI, 1.115-1.392, P<0.001). Longer daily Internet use and more frequent Internet use were analyzed as risk factors for depression. No physical activity was a risk factor for depression (aOR, 0.392; 95% CI, 1.264-4.526, P=0.014). Dissatisfaction with one's body image increased the risk for depression (aOR, 1.373; 95% CI, 1.169-1.613; P<0.001). Obesity and perception of body image showed no significant relationship with increased risk for depression. Conclusion: Prevalence of depression was 13.8% in adolescents in Seoul, Korea, in July 2008. Female sex, age, daily Internet use duration, weekly Internet use frequency, physical activity, and dissatisfaction with one's body image independently increased risk of depression.

A Case of Unknown-Primary Small Cell Carcinoma of the Neck (경부에 발생한 원발불명의 소세포암 1례)

  • Small cell carcinoma usually occurs in lung, but extrapulmonary small cell carcinomas can occur in any sites of body. Most sites of extrapulmonary small cell carcinoma reported were esophagus. And small cell carcinomas occurred in head and neck area were reported rarely. Extrapulmonary small cell carcinoma could be diagnosed when there is no evidence of primary lung lesion on chest X-ray, CT scan of chest and bronchoscopy. The authors experienced a case of small cell carcinoma of left submandibular lymph node in 64-year-old male patient. Biopsy specimen showed poorly differentiated carcinoma but immunohistochemical study showed small cell carcinoma. The chest X-ray and CT scan of chest showed no evidence of primary lung lesion. The patient received chemotherapy(etoposide plus cisplatin) and concurrent chemoradiotherapy using weekly taxol which resulted in good clinical remission. He is still alive 8 months after diagnosis without evidence of lung disease. We report our case with a brief review of literatures.

Volume Calculation for Filling Up of Rubbish Using Stereo Camera and Uniform Mesh (스테레오 카메라와 균일 매시를 이용한 매립지의 환경감시를 위한 체적 계산 알고리즘)

  • For the construction of safe and clear urban environment, it is necessary that we identify the rubbish waste volume and we know the accuracy volume. In this paper, we developed the algorithm which computes the waste volume using the stereo camera for enhancing the environment of waste repository. Using the stereo vision camera, we first computed the distortion parameters of stereo camera and then we obtained the points cloud of the object surface by measuring the target object. Regarding the points cloud as the input of the volume calculation algorithm, we obtained the waste volume of the target object. For this purpose, we suggested two volume calculation algorithm based on the uniform meshing method. The difference between the measured volume such as today's one and yesterday's one gives the reposit of waste volume. Using this approach, we can get the change of the waste volume repository by reading the volume reports weekly, monthly and yearly, so we can get quantitative statistics report of waste volume.

Effect of a Physical Activity Promoting Program Based on the IMB Model on Obese-Metabolic Health Outcomes among Obese Older Adults with Knee Osteoarthritis (슬관절염 비만노인을 위한 IMB 모델 기반 신체활동 증진 프로그램의 효과)

  • Purpose: This study examined the effects of a physical activity promoting program based on the Information-Motivation-Behavioral Skills (IMB) model on physical activity and health outcomes among obese older adults with knee osteoarthritis. Methods: This study utilized a randomized controlled trial with a convenience sample of 75 obese older adults with knee osteoarthritis in a university hospital. The older adults in the intervention group participated in a 12-week program involving weekly group sessions and monitoring calls with education booklets and video clips for exercise dances, while those in the control group received an usual care. Outcomes were measured using self-report questionnaires, anthropometrics, and blood analyses. The intervention effects were analyzed using Mann-Whitney U test and ANCOVA. Results: The mean age of participants was 74.9 years with 84.0% women. The intervention group at 12 weeks showed significantly greater improvements in self-efficacy for physical activity (F=81.92, p<.001), physical activity amounts (Z=-2.21, p=.044), knee joint function (F=15.88, p<.001), and health-related quality of life (F=14.89, p<.001) compared to the control group. Among obese-metabolic outcomes, the intervention group at 12 weeks showed a significant decrease in visceral fat mass (F=7.57, p=.008) and improvement in high-density level cholesterol (F=9.51, p=.003) compared to the control group. Conclusion: Study findings support the need for an IMB based physical activity program for promoting physical activity, knee function and health outcomes in obese older adults with knee osteoarthritis. Longitudinal studies are warranted to confirm the persistence of obese-metabolic effects in clinical settings.

Heavy Metals (Hg, Pb, Cd) Content and Risk Assessment of Commercial Dried Laver Porphyra sp. (유통 마른 김(Porphyra sp.)의 중금속(Hg, Pb, Cd) 함량과 위해성 평가)

  • To investigate heavy metals (Hg, Pb and Cd) and their potential health risks in commercial dried laver (Porphyra sp.), we collected 45 samples from the major production areas on the western and southern coasts of Korea (Hwaseong, Seocheon, Gunsan, Muan, Shinan, Jindo, Haenam, Wando, Jangheung, Goheung and Busan). The Hg, Pb and Cd concentrations were measured using inductively coupled plasma spectrometry (ICP-MS) or a mercury analyzer. The average Hg, Pb and Cd concentrations in the dried laver were $0.006{\pm}0.0017$, $0.196{\pm}0.0614$ and $0.894{\pm}0.4882$ mg/kg, respectively. Based on the 2007 Korean Public Nutrition Report, these levels are 0.02, 0.11 and 2.47% of the provisional tolerable weekly intake (PTWI) for Hg, Pb and Cd, respectively, established by the FAO/WHO. The hazard quotient (HQ) determined from the ratio of exposure and safe levels were less than 1.0. Therefore, the levels of overall exposure to Hg, Pb and Cd for dried laver were below the recommended JECFA (Joint FAO/WHO Expert Committee on Food Additives) levels, which indicate safe levels for public health.

A Case of Early Diagnosed Hunter Syndrome Detected by Large Head on Routine Examination (큰 두위를 주소로 내원하여 헌터 증후군으로 일찍 진단된 증례 1례)

  • A 25-month-old boy was referred to the hospital due to large head detected on routine physical examination. At visit, dysmorphic facial appearances, including broad nose, prominent forehead, and coarse face, were noted. Nasal obstruction with nasal voice, prominent adenoids, and bilateral middle ear effusions were detected. His abdomen was distended, and liver and spleen were palpated about 3 finger and 2 finger breadths, respectively. He was operated for bilateral inguinal hernias. The motion of both elbow joints was mildly limited on supination and pronation. Urinary level of glycosaminoglycan was elevated and the enzyme activity of iduronate sulfatase in leukocytes was decreased. The mutational analysis of the gene iduronate 2-sulfatase (IDS) revealed c.263G>A (p.Arg88His) mutation. His developmental scale showed delayed development and there was cardiac valvular involvement (tricuspid regurgitation and mitral valve prolapse). After the diagnosis of Hunter syndrome, enzyme replacement therapy started on a weekly basis without progression of any clinical features. Here we report a case of early diagnosed Hunter syndrome detected by large head on routine examination. Thus, it is important to associate Hunter syndrome in the patient with large head especially, if there is the history of bilateral inguinal hernia and prominent adenoids to increase the possibility of early diagnosis and treatment.
