Full mouth prosthetic rehabilitation based on systemic analysis, diagnosis and treatment plan: a case report (체계적 분석, 진단, 치료 계획을 통한 전악 보철 재건 증례)

  • Data collection including checking the problem list and analyzing the etiology cannot be overemphasized in full mouth rehabilitation cases. It is well reported that favorable prognosis of prosthodontic treatment depends on accurate diagnosis. Seventy seven year old female patient presented with chief complaints of chewing pain on maxillary and mandibular left premolars and unesthetic existing prosthesis. Clinical evaluation, radiographic examination and cast analysis were performed to gather comprehensive data and problem lists. Treatment planning was based on collected data and related articles. Abutment evaluation, prognosis depending on various types of prosthesis, and maintenance were considered as well. Occlusal vertical dimension was increased to solve esthetic dissatisfaction and insufficient interocclusal space. Patient adaptation was verified with provisional restorations followed by diagnostic wax up. Function, esthetics and occlusal stability were verified during 8 week follow-up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory result after delivery of definitive prosthesis.

  • Loss of posterior support may cause overloading and excessive wear of remaining teeth. Moreover, the extrusion of antagonistic teeth leads to the destruction of the occlusal plane. The loss of vertical dimension of occlusion (VDO) also emerges clinically, which may bring the loss of esthetic appearance and function. These patients who suffer from the loss of posterior support, often require vigorous periodontal treatments (osteotomy, crown lengthening) and extensive oral rehabilitation. Sixty three years old female patient visited for the prosthetic treatment of the posterior edentulous area. She had no other systematic disease and parafuctional habits for prosthetic treatment. Intraoral and radiographic examinations were done. The evaluation of VDO and vertical dimension of rest position were evaluated for proper prosthetic procedures and diagnostic wax up was done. As a result of diagnosis, VDO was increased by 2 mm considering the loss of VDO and space for the prosthetic treatment. After the pretreatments, initial preparation of teeth and provisionalization were carried out. Six weeks later of provisionalizaion, final preparation and impression was performed. Using the duralay resin copings, jaw relation was registered. The master cast was mounted and definitive restoration was fabricated. After the evaluation of esthetic and function, pick up impression for clinical remounting was done. Lucia jig was made for new jaw relation and occlusal adjustment on the articulator. Definitive restoration was delivered and the patient was periodically recalled for additional occlusal adjustment. From this case, the satisfactory functional and esthetic results through full mouth rehabilitation with increase vertical dimension were achieved.

Apolipophorin-III uptake by the adult testes in the wax moth, Galleria mellonella (꿀벌부채명나방 성충 정소에 의한 아포리포포린-III의 흡수)

  • Apolipophorin-III (apoLp-III) was isolated and purified from the last larval hemolymph of Galleria mellonella by KBr gradient ultracentrifugation and gel chromatography (Sephadex G-100). After KBr gradient ultracentrifugation, the lipophorin-free fractions were used as the samples for gel chromatography. The purity of the finally purified apoLp-III was confirmed by SDS-PAGE after gel chromatography. In this study, we found that apoLp-III is taken up into the adult testes in Galleria mellonella. The testes were dissected from day-1 or -2 adults in cold Ringer's solution and used for tissue culture. The protein moiety of apoLp-III was labeled with FITC dissolved in dimethyl sulfoxide (DMSO) at room temperature under conditions of continuous stirring for 1 h. The FITC-labeled apoLp-III was purified with a Sephadex G-25 PD-10 column. The tissues of the adult testes were incubated at room temperature for 30 min with fluorescein isothiocyanate (FITC)-labeled apoLp-III. Fluorescein microscopy and sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis (PAGE) revealed that the adult testis tissues internalize the FITC-labeled apoLp-III. The results showed that apoLp-III is taken up by the adult testes.

The Clinical Usefulness of Translabyrinthine Approach for Removal of the Vestibular Schwannomas (청신경 초종 수술에서 골미로를 통한 접근법의 유용성)

  • Objective : To determine the feasibility of translabyrinthine approach in the vestibular schwannoma patients, the authors reviewed eighteen consecutive cases, focusing at their functional outcome and operative complications. Materials and Method : To evaluate the functional outcome, we reviewed preoperative radiological findings such as size of tumors and location of jugular bulb as well as the preoperative neurological status including audiometric analysis and cranial nerve function in 18 patients, diagnosed as vestibular schwannoma. Also the surgical outcome was evaluated according to the functional preservation of facial nerve and incidence of the surgical complication as well as the extent of surgical resection. Results : The age of patients ranged from 21 to 62 years, with a mean of 50 years. Of 18 patients operated in our center by the translabyrinthine approach, wide exposure with total removal of the mass was possible in 16 cases (88.8%). The facial nerve was anatomically preserved in 88.8%. At six-month follow-up, facial nerve function was good(Grade I-II) in 15 patients(83%) and acceptable(I-IV) in all patients. Although the jugular bulb was highly placed is five patients, gross total resection was possible without facial nerve injury in all patients by the translabyrinthine approach. One patient experienced CSF leakage after surgery, but there was no patient with disabling deficit. Conclusion : Use of the translabyrinthine approach for removal of vestibular schwannomas resulted in good anatomical and functional preservation of the facial nerve, with minimal incidence of morbidity and no mortality. In cases of high jugular bulb impacted into mastoid bone, total removal was possible by displacing the jugular bulb with Surgicel cellulose and placement of bone wax.

  • It is reported that the causes of unaesthetic proportion of anterior teeth vary widely. Especially, when the unaesthetic tooth proportion of the mandibular incisors arises due to the wear of the anterior teeth accompanied by the compensation of the alveolar bone, it may cause serious functional and aesthetic problems. In such case, it should be considered that the evaluation of vertical dimension and tooth proportion as well as smile line, soft tissue and hard tissue morphology. And, increase of vertical dimension or clinical crown lengthening followed by prosthodontic restorations is needed to improve the interdental mesial/distal, width/length ratio considering the anterior guidance. This case report demonstrates functional and aesthetic improvements through systematic diagnosis and treatment procedures in a 48-year-old male patient with unaesthetic anterior teeth proportion because of tooth wear accompanied by the compensation of alveolar bone and defect of several central incisors due to chronic periodontitis.

  • The treatment of areas demanding esthetic requirements, such as maxillary anterior teeth, should take into account the achievement of a healthy, harmonious to the surrounding tissue, and an attractive smile line. In this case report, smile line, soft tissue and hard tissue morphology, and the anatomy and proportion of the tooth, must be considered. In patients with unesthetic maxillary anterior ratio due to inadequate gingival contour and diastema caused by peg lateralis, it would be challenging to achieve an esthetic restoration by orthodontic treatment alone. In such case, after orthodontic treatment, dento-gingivo-facial compositional diagnosis and analysis, followed by gingivectomy and prosthodontic restoration is needed to improve the interdental mesial/distal, width/length ratio to achieve a satisfactory esthetic result. In addition, when improving the tooth proportion of peg lateralis by prosthodontic treatment, Porcelain laminate veneer (PLV), which results in less tooth structure loss, reproduction of similar shade to that of the proximal tooth and high transparency, is recommended. This case report demonstrates esthetic improvements by prosthodontic restoration through systematic diagnosis and treatment procedure in patients with unesthetic maxillary anterior proportion after orthodontic treatment due to peg lateralis by means of two female patients aged twenty years old.

  • Excessive occlusal wear causes loss of tooth structure, occlusal plane disharmony, impaired function and esthetic problems. Although the decrease of occlusal vertical dimension may be compensated by the growth of alveolar bone and tooth eruption, minimal increase of occlusal vertical dimension may be required for esthetics and retention of prosthesis. In this case, a 44-year-old male patient visited Seoul National University Dental Hospital with chief complaint of severe tooth wear and shade disharmony. Based on assessment of diagnostic wax-up, 3 mm increase of occlusal vertical dimension was determined. Removable occlusal splint and interim prosthesis was used to ascertain patient's comfort and adaptation. After the adaptation period, definitive prosthesis fabricated with full-contour monolithic zirconia were delivered and the patient was recommended to wear a nightguard device for prosthesis protection. This report presents a case of full mouth rehabilitation with the elevation of patient's occlusal vertical height, resulting in satisfactory esthetics and functions.

  • The progressive attrition of teeth is a normal process by aging. However, excessive tooth wear with decreased vertical dimension of occlusion and collapse of occlusal plane may cause pathologic pulpal condition, occlusal disharmony, functional disorders and esthetic problems. The change of vertical dimension is required in case that there is no sufficient clearance for restoration or in case that the occlusal relationship is modified. For gaining the vertical dimension, a careful diagnosis is essential prior to starting the restoration treatment. After evaluating adaptation of neuromuscular system of patient during provisional phase, the final restorations can be fabricated. In this case, a 78 year old male with severely worn down dentition was treated. To improve the esthetic appearance and to achieve the ideal occlusal relationship, the full mouth rehabilitation with minimal increase of vertical dimension is planned and diagnostic wax-up was performed at the increased vertical dimension. After evaluation of provisional restorations for 12 weeks, final restorations were fabricated and routine clinical assessments were made. After 1 year, the restorations with newly established occlusal scheme are well maintained without significant complications and esthetically and functionally satisfactory results were obtained.

Study on The explanation of channel disease in "Tai Su(太素)". ("태소(太素) . 경맥병해(經脈病解)"에 대한 연구)

  • The "Tai Su(太素)" which was published by Yang Shang Shan(楊上善) during the Tang(唐) dynasty does not follow complicating hand down procedures allowing it to be preserved in a relatively satisfactory state, and the book is evaluated as a major article in the study of the "Yellow Emperor's Internal Classic(黃帝內經)" in the modern age. The book of "Tai Su explanation of channel diseases(太素 經脈病解)" offers the detailed description of disease symptoms found in the book of "Miraculous Pivot channels(靈樞 經脈)", and Yang Shang Shan(楊上善) provided relatively complete explanations from the perspective of the wane and wax of shadow and sunshine powers(陰陽消長). The present writing was projected to acknowledge the relationship between the books of "Tai Su explanation of channel diseases(太素 經脈病解)" and "Miraculous Pivot channels(靈樞 經脈)" and substantially attempted to understand the original text of the "Tai Su(太素)" by adopting the perspectives of Yang Shang Shan(楊上善) in explaining the disease symptoms as he attempted in the book of "Miraculous Pivot channels(靈樞 經脈)". Total 50 disease symptoms were explained in the book of "Tai Su explanation of channel diseases(太素 經脈病解)", and 33 of them are common to the symptoms described in the book of "Miraculous Pivot channels(靈樞 經脈)" by taking up about 66% of the book. The monthly attachment(月別配屬) of three shadow and three sunshine power(三陰三陽), which is described in the book of "Tai Su explanation of channel diseases(太素 經脈病解)" provides an important clue in understanding the exterior and interior(表裏) relationship, and the crucial perspective lies on the mutual intensity change of shadow and sunshine powers. Therefore, the monthly attachment of three shadow and three sunshine power provided by the present article helps to understand the meaning of the three shadow and three sunshine power along with the time attachment(時間配屬) described in the other chapter of the "Yellow Emperor's Internal Classic(黃帝內經)". In the method of explaining pathogenesis, the explanation was made by understanding the perspective of mutual intensity change of shadow and sunshine powers, and exterior and interior relationship along with the channel Path(經脈流注) have been used simultaneously. In the comment of the main article, Yang Shang Shan(楊上善) stated the channels of hands and feet(手足經脈) as the scope of the three shadow and three sunshine power, but the symptoms and signs are mainly focused on the channels of feet as in the book of "Miraculous Pivot channels(靈樞 經脈)".

THE SHEAR BOND STRENGTH BETWEEN DICOR AND SEVERAL VENEERING PORCELAINS (Dicor와 수종 전장도재간의 전단결합강도)

  • Dicor has not been prescribed routinely, in spite of many advantages, because of esthetic limitations by excessive translucency and external shading. In an attempt to solve these problems, the technique of veneering Dicor by aluminous poreclain has been used and recently Dicor Plus system was developed. The purpose of this study was to evaluate the compatibility between Dicor and several veneering porcelains by measuring the shear bond strength and observing the failure mode and interface appearance with SEM. Total 55 Dicor disks(10.0mm diam. X 3.0mm thickness) were fabricated by lost wax technique and divided into five groups of 11. Veneering porcelains such as Dicor Plus, Vitadur Alpha, Vitadur N, Vivodent, and Ceramco II were built up over the center of the treated Dicor surface using paper tube(5.0mm diam. X 4.0mm height) and fired according to the manufacturesr’instructions. A representative sample from each group was completely embedded in epoxy resin and crosssectioned, and remaining 50 samples were embedded in epoxy resin with the bonded area perpendicular to table base. The shear bond strengths were measured by applying the shear load parallel to Dicor surface close to the bonded area. Failure modes and interface appearances were observed using SEM at 15 and 1000 magnification respectively. The obtained results were as follows : 1. The mean shear bond strengths showed Dicor-Dicor Plus(10.53 MPa); Dicor-Vitadur Alpha(8.84 MPa); Dicor-Vitadur N(7.37 MPa); Dicor-Vivodent(4.28 MPa); Dicor-Ceramco II(0.89 MPa). 2. The shear bond strength of Dicor-CeramcoII was significantly decreased compared with Dicor-Dicor Plus(p<0.01), but had no significant difference compared with Dicor-Vivodent(p>0.01). 3. The shear bond strengths of Dicor-Vitadur Alpha and Dicor-Vitadur N were not significantly different compared with Dicor-Dicor Plus(p>0.01). 4. SEM examination of bond failure modes revealed that Dicor-Dicor plus, Dicor-Vitadur Alpha, Dicor-Vitadur N exhibited cohesive failure within Dicor and Dicor-Vivodent exhibited adhesive failure. And Dicor-Ceramco III exhibited adhesive failure and cohesive failure within CeramcoIII together. 5. SEM examination of interfaces revealed that Dicor-Dicor Plus exhibited the most tight contact and Dicor-Vitadur Alpha, Dicor-Vitadur N exhibited acceptible contacts. But Vivodent exhibited discontinuous gap and Ceramco II exhibited large continuous gap.
