Herbal Medicine for Pediatric Epilepsy: Clinical Research Trends in Traditional Chinese Medicine

  • Pediatric epilepsy, a chronic, recurrent brain disorder, is the most common neurological disorder in children. Its prevalence is increasing. Early management is very important since 30~40% of cases persist into adulthood. To provide basic data for future clinical research on pediatric epilepsy using Korean medicine treatment and cooperation between Western medicine doctors and Korean medicine doctors, we reviewed recent clinical research in traditional Chinese medicine (TCM) using herbal medicine for pediatric epilepsy. A total of 23 articles (1 clinical practice guideline, 3 systematic reviews, 15 randomized controlled trials (RCTs), and 4 non-RCTs) were reviewed in this study. The authors summarized characteristics of included studies regarding study subjects, diagnostic tools, pattern identification tools, treatment period, evaluation tools, detail of herbal medicines, treatment effects, and adverse events. Combination therapy using both herbal medicine (HM) and anti-epileptic drugs (AEDs) was performed more frequently than herbal medicine alone. Liver-pacifying medicinal, water-draining medicine, and orifice-opening medicine were frequently used. The main single HMs were Cheonma, Boglyeong, Jogudeung, and Seogchangpo. Combined therapy using HM and AEDs had significant benefits in improving total effective rate. It also appeared to be safer than AEDs. However, since the quality of clinical trials was poor and only studies in the last 10 years were included, the clinical evidence was uncertain. Finally, the authors provided limitations of this study and several suggestions for future research based on our analysis results.

The Management Methods of Multi-Purpose Ecological Reservoir by System Thinking - Focused on Anteo Eco Park - (시스템 사고를 통한 다기능 생태저류지의 관리방안 - 광명 안터생태공원을 중심으로 -)

  • Ecological reservoir is a multifunctional space where provides the functions of retention, animal habitat and improvement of ecosystem health and landscape. The ecological reservoir of Anteo Eco Park located in Gwangmyeong-si has established to functions for water purification, maintenance of healthy aquatic ecosystem. Because the Anteo Eco Park is located in the site where nonpoint pollutant materials flow in, Anteo Eco Park has potential factors which aquatic ecosystem health deteriorates and damages the habitat of golden frog(Rana plancyi chosenica) which is restoration target species. Therefore, the purpose of this study is to suggest the plan to manage the variables which impede the right functions of aquatic ecosystem by understanding the causal loop diagram for the change of water quality environment and the interaction of predator-prey through system thinking. The results are as follows. First, the study showed that the individual number of golden frog which is an indicator species of Anteo Eco Park is threatened by snakeheaded fish, which is an upper predator. Therefore, balanced food chain should be hold to protect golden frog by capturing the snakeheaded fish which is individual number's density is high, and the monitoring management of the individual number for predator(snakeheaded fish)-prey(golden frog) should be performed. Second, the study represented that water pollution and carnification is caused by the sediment as the dead body of the large emergent vegetation in the winter cumulates as sediment. Ecological reservoir in Anteo Eco Park has been managed by eliminating the dead body of the large emergent vegetation, but the guideline for the proper density maintenance of vegetation community is additionally needed. Lastly, the study showed that aquatic ecosystem of Anteo Eco Park where is contaminated from the inflow of nonpoint pollutants affects the individual number's decline of golden frog and snakeheaded fish. Accordingly, the creation of a buffer area and a substitution wetland is needed in the periphery of the Anteo Eco Park to control the inflow of nonpoint pollutants including organic matters, nutrients and heavy metals. This study will be helpful that Anteo Eco Park improves the regional landscape and maintain healthy aquatic ecosystem space for the park visitors including local residents.

Estimation of the Impact of the Inclusion of Sharks in the Appendices of the 'Convention on International Trade in Endangered Species of Wild Fauna and Flora (CITES)' on Korean Fisheries and Recommendations of the Domestic Measures on the Trade of the Listed Shark Species ('멸종위기에 처한 야생 동식물의 국제거래에 관한 협약'의 상어류 등재가 한국 수산업에 끼칠 영향과 대응 전략)

  • The Sixteenth Meeting of the Conferences of Parties of the Convention on International Trade in Endangered Species of Wild Fauna and Flora (CITES), which was held in March 2013 in Bangkok, Thailand, listed five shark species and one genus, and uplisted one sawfish species. All new species listings will be come into force of the eighteen months delay, on the fourteenth of September, 2014. The purpose of the delay is to support the preparation of the domestic measures on shark trade by the parties, as there has been no previous experience in trading the commercially exploited aquatic species in the CITES. The CITES Secretary-General has visited several potential shark trade countries to encourage and facilitate the implementation of the new CITES listings. The newly listed sharks have been caught as a target or non-target species by Korean distant water fisheries and introduced into Korea. So the establishment or change of the domestic laws, regulations, and measures for the implementation of the new CITES species must be prepared before the effective entry date. This paper is prepared to assess the effects of the trade of the CITES listed shark species, and to suggest effective government service measures for the management of Korean distant water fisheries. The Ministry of Environment (MOE) is the general Management Authority of the CITES, and the Ministry of Food and Drug Safety (MFDS) is in charge of the medical trade of CITES listed species in Korea. By law, all imported medicines and medical materials must be inspected by the MFDS during the customs examination; this kind of authority sharing is reasonable and effective way of providing government service. Similarly, the designation of new CITES Management Authority for the trade of commercially exploited aquatic CITES species is critical and the Ministry of Oceans and Fisheries (MOF), which is now in charge of the trade of fisheries products, is the most appropriate governing body for this purpose in Korea. The revision of the National Plan of Action for Conservation and Management of Sharks, initially submitted to the Food and Agriculture Organization (FAO) of the United Nations in 2011 as a practical guideline for shark conservation in all Korean fisheries, could be a effective measure to achieve unification of conservation of endangered species and sustainable use of fisheries stocks. The proper CITES measures for the trade of listed species, such as the establishment of the documenting system for Non-detriment Findings, domestic measures suitable for the "Introduction from the Sea" clause, species specific Harmonized System Codes for the customs service, and an effective shark catch data reporting system should be in place prior to the fourteenth of September 2014.

Analysis of Sediment Contamination Levels in the Giheung Reservoir (기흥저수지 퇴적물에 대한 오염도 분석)

  • In order to analyze the effects of sediment on the occurrence of algal bloom on the Giheung Reservoir, the contamination levels of sediments were evaluated. The concentrations of various organic compounds (ignition loss), as well as the total nitrogen, total phosphorus, and heavy metals (Zn, Cr, Co, Ni, Pb, As, Hg, Cd) were analyzed in the sediments taken at eighteen sites of the reservoir. The concentrations of ignition loss and total nitrogen tended to increase from upstream to downstream, and ranged from 4.38 to 12.93% and 2,153 to 4,723 mg/kg, respectively. Heavy metals were in the order of Zn>Cr>Co>Ni>Pb>As>Hg, and the contamination level of the heavy metals was not high as a whole. The concentrations of the total phosphorus were in the range of 765 ~ 3,238 mg/kg, which exceeded the contamination level of the "Sediment Quality Assessment Guideline of River and Lake Sediment (Rule No. 2015-687 of the National Institute of Environmental Research, Korea)" at two upstream sites, four downstream sites, and all downstream sites. These results indicated that the pollution level of the total phosphorus, which is the main factor related to algal bloom, was found to be serious. Therefore, it is necessary to establish a countermeasure for sediment management in order to control the algal bloom which occurs periodically in the reservoir.

A study on the survey and reduction of indoor asbestos concentration (실내의 석면농도 실태 조사 및 저감에 관한 연구)

  • The research on the actual condition of indoor asbestos concentration in Korea has not been thoroughly accomplished up to now. In this research the ratio of asbestos-containing buildings and indoor asbestos concentration was studied. This investigation was conducted in 2012 and 2013 and buildings were categorized based on region, building type by use, existing space(ground or basement), and construction year, respectively. Also the indoor asbestos concentration change was monitored to evaluation the efficiencies of two types of asbestos-concentration abatement devices. As a result, the ratio of asbestos-containing buildings in Seoul was largely decreased. The ratio of asbestos-containing buildings was higher in hospitals and schools regionally and in ground buildings than in basement. The average indoor asbestos concentrations were 0.0011, 0.0008 piece/cc in 2012 and 2013 investigation, respectively. Those values were much lower than standards(0.01 piece/cc), therefore the threat of indoor asbestos concentration might be negligible. In asbestos-concentration abatement experiments, the circulation velocity of ventilator were changed 2-6.7 m/sec. With 6.7 m/sec of velocity of ventilator, the concentration of indoor asbestos was fluctuated and maximum value was 2.4 piece/cc. With 4.5 and 2 m/sec of velocities of ventilator, the maximum concentration of indoor asbestos was fluctuated and maximum value was 0.9 piece/cc. This indicated that the concentration of indoor asbestos was decreased partly due to the free drop of asbestos. From these results, the proper velocity of ventilator seems to be between 4.7 and 6.5m/sec under this circumstance and further research is required. These research results may be used to guideline of asbestos management policy.

Improvement of National Risk Alarm 4-Stage Criteria for Flood Disaster (홍수재난 대응을 위한 국가위기경보 4단계 설정기준 개선)

  • EAP, which is operated on the frame of Risk Alarm 4-stage of National Risk Management Guideline, is a critical method in order to promptly respond to disasters. Korea Flood Control Office issues major and moderate flood alarm at each river station by respectively 50% and 70% of design flood discharge in terms of watermark or sea level, however, the criteria deciding major and moderate floods are vague for field managers to control the disaster situations. On the other hand, Japan and USA use river water level as a main criterion in order to classify the stage of flood disaster, which is higher design flood level than Korea. Thus, the authors analyzed domestic and oversea EAP guidelines and suggested improved criteria showing easy display method and raising the criteria of flood level for reflecting more effective action plans through testing a simulation training on the test-bed.

A Study on the Feasibility of COBie to the Wastewater Treatment Plant (COBie 기반 하수처리시설 유지관리시스템 구축)

  • With the introduction of COBie (Construction Operation Building Exchange) in BIM technology enabling an automatic transfer of design and construction information to operation and management (O&M) phase, the BIM centric O&M management system development process has been tested on the subject of architectural types of building. However, for now, there is a need to investigate the technical feasibility of COBie application to civil structures including industrial facilities. This study takes both "O&M Guideline for Public Wastewater Treatment Plant" and a real wastewater treatment plant into account for the purpose, in which the latter is intended to supplement the result of the first. The findings are three-folds: (1) COBie, as an asset modeling, is not sufficient enough to encompass commissioning data, (2) more relevant IFC development and family library build-up useful to modeling wastewater treatment plant is imperative, and (3) well-planned coordination and organization of COBie data-set in line with O&M practice will enhance the feasibility of the COBie in industrial facilities. The result could be used for a basis study for COBie application, particularly in industrial facilities.

  • Accurate flood frequency and magnitude estimation has a critical role in flood risk management and damage reduction. In United States, Log Pearson Type-III (LP-III) distribution with method of moments for parameter estimation has been uniformly and consistently employed in estimating design floods. After the first version of flood frequency guidelines (Bulletin 15) was published in 1967, the revised version Bulletin 17B has been employed since 1982 up to now. A new version of flood frequency guidelines, Bulletin 17C, is prepared and about to come out soon. In the current study, we analyzed the new features of the upcoming Bulletin 17C and presented case studies applying its new features. From the presented results, we see what critical components in the new design flood frequency guideline we could learn.

  • The Land Use Suitability Assessment System was recently introduced by the Act on Planning and Utilization of the National Territory to use, manage, and develop the national territory, which integrated the National Land Use and Management Act and the Urban Planning Act. It provides a guideline for land use according to locational characteristics, usability, and developmental conditions of land in the vicinity. The database is constructed with LMIS cadastral data, posted land price data, and the data of related agencies such as the Korea Forest Service, the Ministry of Environment, and the Korea Water Resources Corporation etc. In this paper we describe accurate database construction method fur land use suitability assessment system as classifying and verifying errors deriving from database construction focused on Jinju city. Those data errors have the problems such as accuracy difference among the related agencies data, gap of data acquisition time, and non-consideration of latest updated data etc.

A comparative study on the Activating Factors of domestic and overseas scuba diving resorts using delphi method

  • This study aims to analyze and compare the activating factors of domestic and overseas scuba diving resorts. Our delphi survey was conducted three times in 30 experts who involved in operation and management including scuba diving resort management representative and training team leader. As a result of comparing the activating factors at domestic versus overseas, it was found that common important activating factors included expansion of convenient facilities at public diving places, installation of safety and medical facilities for divers, development of first aid system including AED and oxygen ventilator, requirement of convenient facilities such as water lift and toilet in diving boat, installation of diving boat screw safety system, local boat operation guideline, regular course training program for scuba diving, employment of professional scuba diving instructor and guide, communication and promotion through various SNS portals, promotion by divers' word of mouth, involvement in regional Diving Resort Association, involvement in Korean Diving Association, communication and mutual benefit with local fishing villages, and linkage policy with local tourism industry.
