A fulminant hepatitis is associated with massive liver cell necrosis and a high mortality rate. But survivors regenerate a normal liver and do not have chronic liver disease. This clinical course suggests that the acutely injured livers release a factor that allows a recovery from chronic hepatitis or cirrhosis. The objective of this study was to isolate and characterize an anti-fibrotic factor from acutely damaged rat livers. The liver cell necrosis was prepared from rat by warm ischemical perfusion and the perfusates were assessed against the growth inhibition of fat-storing cells (FSC). A liver-derived growth inhibitory factor (LDGIF) was purified from ischemically damaged rat livers by chromatographies on Sephacryl S-300, CM Sepharose, hydroxyapatite, and Superose 12. The LDGIF was isolated with an overall purification of 194-fold and 40% recovery. Although LDGIF was identified as the rat liver arginase by Nterminal sequence analysis, LDGIF exists as a monomer and the purified native arginase has a trimer form. Furthermore, LDGIF has a lower enzyme activity on the hydrolysis of L-arginine and a higher inhibitory effect on proliferation of FSC than the normal rat liver arginase. The catalytic activity of LDGIF is ascribed to the monomeric characteristics of the LDGIF. Therefore, the inhibitory action of LDGIF might not be due to the arginine depletion by the catalytic activity of arginase. In conclusion, the presence of the LDGIF could interpret the clinical course that serious fibrosis is not found in the liver of patients recovering from severe hepatic necrosis due to fulminant hepatitis, suggesting that this LDGIF may provide a novel target for the prevention and treatment of hepatic fibrosis.
Objective: To know the recurrence rate of paranasal sinusitis in adults using follow-up investigations. Method: Data was collected from 37 patients who were treated in the Department of Ophthalmology & Otolaryngology & Dermatology, Kangnam Oriental Medical Hospital from 2001-10-01 to 2005-05-31 for chronic paranasal sinusitis in adults over 21 years of age. Results & conclusions: 1. The males made up 51.4% and females made up 48.6%. The recurrence rate was 78.4%. 2. By residence, 78.4% lived in apartments; 21.6% lived in houses. 3. Of the main symptoms, nasal obstruction was the most common. 4. The duration of the disease was within 1 month to 40 years; the group with difficulties within the last 5 years made up the largest group. 5. The patients who had chronic nasal family histories made up 40.5%; Patients who didn't have chronic nasal family histories made up 59.5%. 6. In regards to the period of Tx distribution. the group treated within 4 weeks made up the largest group at 54.1%. 7. As far as methods of Tx distribution, the group of patients treated with herbal medicine and acupuncture were larger (62.2%) than the group treated with only herbal medicine (37.8%). 8. The most common nasal condition symptom in patient histories was chronic rhinitis. 9. The most frequent areas affected by paranasal sinusitis on PNS images were the lateral maxillary sinus, the unilateral maxillary sinus and the ethmoidal sinus. 10. The number of patients who drank little was larger (75.7%) than the number of drinking patients(24.3%); There were more non-smoking patients (78.4%) than smoking patients(16.2%). 11. The patients who slept for 6-7 hours made up the larger group (40.5%). 12. The number of patients who preferred warm water was larger making up 54.1 %; the number of patients that preferred cool water made up 21.6%. 13. The patients who had a common cold 3-5 times a year was the largest group at 54.1%.
In this study we examined the concerned literature for clinical use of Balsaminaceae plants focusing on the latest literature "zhong hua ben cao"(chinese materia medica, 1998). The findings were summarized as follows: 1. As an original plant, 27 species have been reviewed. 2. As a result of investigating oriental drug names by medicinal using part, a survey reported that the total of 22 species of oriental drug names, including 14 species of herba, 3 species of flos, 4 species of radix and rhizoma, and 1 species of semen, are being used for clinical use. 3. When we examined oriental drugs with five flavors by calculating and arranging their nature, effect, toxicity with points for conveniences sake, pungent and bitte showed 13 points, respectively, as main flavors. In drug nature, warm property of drug indicated 10 points and cold property of drug obtained 6 points. But as for other oriental drug, it is thought that we can recognize their significance. 4. With respect to efficacy, activating blood drug reached 10 kinds and wind- damp-dispelling 7 kinds, according to the examination. 5. In the examination on components and medicinal action, we examined that components are recorded in only impatiens balsamina's whole grass, flowers, root, root stems, and seeds, and Impatiens noli-tangere's whole grass. In medical action, we found out that the components are recorded in impatiens balsamina' flowers and seeds; impatiens noli-tangere's whole grass and flowers; and Impatiens textori' whole grass. 6. As a result of examining clinical prescription, the total of 84 kinds of clinical prescriptions by disease nature were examined, of which the prescription of Impatiens balsamina showed 19 cases in total, which was remarkable.
Objectives Based on related literature, cold and heat, deficiency and excess, true and false, and actually used formulas, paradoxical treatments presented in the 『Plain Questions of Inner Canon of Yellow Emperor』 including ‘treating pseudo-heat symptoms and real cold syndrome with cold herbs, treating pseudo-heat symptoms and real cold syndrome with cold herbs’ were analyzed.Methods Out of literature, paradoxical treatment related classics and papers were investigated and analyzed. Among diseases and syndromes, real cold syndrome with pseudo-heat symptoms, real heat syndrome with pseudo-cold symptoms, real deficiency syndrome with pseudo-excess symptoms, and real excess syndrome with pseudo-deficiency symptoms were reviewed. Among formulas, typical examples of the above mentioned paradoxical treatments were used as examples to analyze paradoxical treatments.Results Treating pseudo-heat symptoms and real cold syndrome with cold herbs is a method that uses herbs with cool and cold nature to treat real cold syndrome with pseudo-heat symptoms and Tongmaeksayeokgajeodamjeuptang is suitable for this method. Treating pseudo-cold symptoms and real heat syndrome with hot herbs is a method that uses herbs with warm and hot nature to treat real heat syndrome with pseudo-cold symptoms and Baekhogainsamtang is suitable for this method.Conclusions Based on the related literature, cold and heat, deficiency and excess, true and false, and actually used formulas examined as mentioned above, the paradoxical treatments presented in the 『Plain Questions of Inner Canon of Yellow Emperor』 are thought to be reasonable paradoxical treatments that fit the diseases and syndromes that actually appeared in our bodies.
Objectives: To grasp the traditional stream of cancer treatment inherited from the previous doctors. Methods: The author's research has been performed cancer treatment based on JIJU(積聚) taking the original text as a reference. Results & Conclusion: The general three outlines and five detailed rules of cancer treatment have been obtained as follows. The first outline of cancer treatment is that reinforced vital function makes cancer reduce naturally. the second is that Harmless cancer can coexist in human beings and aging with them. and the third is that Elimination and reinforcing therapies should be executed in appropriate era and those therapies should utilize appropriate methods. The first detailed rule of cancer treatment is when using reinforcing therapy, it must applicate mildly and when using elimination therapy, it must applicate calmly. The second detailed rule is that the methods of cancer treatment are different from each cancer stage. The concentration should be made on reinforcing therapy at early stage while reinforcing and elimination therapies must be conducted together at middle stage. At terminal stage reinforcing therapy is the sole method to be taken. The third detailed rule is that the basis property of cancer drug is warm nature and extremely biased property should be avoided and when complication arises (eg. inflamatory disease, cancer fever, etc), cold or cool nature can be applied. The fourth detailed rule is that Cancer drug must have the effect eliminating the blood stasis, phlegm and excessive fluid, all together. Objectives : The aim of this paper is to compare the mechanisms of the OP/LB method and the SSD treatment in 『Piweilun』. Methods : Wenbing texts, articles on the OP/LB method, and the 『Piweilun』 was examined for comparison of treatment mechanisms of SSD. Results : The mechanism of the OP/LB method in treating SSD was to treat the Spleen and Stomach separately, to restore the ascending/descending pattern of qi through simultaneous use of pungent and bitter flavors which raises and lowers, respectively. Moreover, the use of medicinals with contrary properties regulate the other's biased nature, and none of the medicinals create dampness. The pungent and bitter flavors play central roles, where the pungent flavor opens and communicates and the bitter flavor clears and lowers. The treatment method of SSD in the 『Piweilun』 treat the Spleen and Stomach together, with a focus on raising and dispersing through upraising yang and reinforcing qi by means of Wind medicinals with pungent and bitter flavors added to sweet and warm medicinals. Conclusions : Owing to the expansion of the OP/LB method concept by modern-contemporary scholars, the potential for treating SSD as can be seen in the 『Piweilun』 with the OP/LB method has emerged. The similarity between the OP/LB method and treatment of SSD in the 『Piweilun』 is that the objective of the formulas is to lower fire heat and communicate qi to help qi movement. This common objective allows for treatment of SSD within Wenbing and those in 『Piweilun』 with the said formulas through appropriate modification fit for each situation.
Toxoplasma gondiiis a widespread apicomplexan parasite which is able to infect virtually all warm-blooded vertebrates. Twenty-two percent of the U.S. population is infected, but severe disease in adults is mainly limited to immunosuppressed patients. In patients with acquired immunodeficiency syndrome(AIDS), T. gondii causes a life-threatening opportunistic infection, with Toxoplasma encephalitis as its most severe manifestations. T. gondii is also known to cause congenital infection and is among the pathogens with the highest incidence of complications in pregnancies. Despite its clinical importance, only very few therapeutic drugs against T. gondii are available, all of which target the rapidly dividing tachyzoites, leaving the dormant encysted bradyzoite stage unaffected. We searched 15 traditional medicines that have anti-inflammatory effect from dongyibogam and Traditional Chinese medicine. In vitro studies were performed with HeLa cell cultures, with quantification of Toxoplasma growth by a cell proliferation assay. The result of experiment shows the selectivity of Citrus unshiu Markovich is 6.0. This is higher than sulfadiazine (selectivity was 1.63). For in vivo studies, mice were acutely infected intraperitoneally with $10^5$ tachyzoites of the virulent RH strain and then treated per orally for 4 days from 6 hours postinfection. Efficacy was assessed by sequential determination of parasite burdens in peritoneal cavity. In vivo, Citrus unshiu Markoviche inhibited Toxoplasma growth at a concentration of 10㎎/㎏ of body weight per day, the inhibition ratio was estimated to be 64.01%.

Stroke is known as a detrimental disease that leaves serious sequelae. The stroke patients suffer from limitations of physical and social activities. The patients try to adapt themselves to the limitations in order to achieve rehabilitation. This study was performed to clarify the lived experience of rehabilitation from the stroke patients. In collecting data for this study, in-depth personal interviews were made by a researcher from February to April in 2001 at rehabilitation clinics and physical therapy centers locate in Iksan city. The methodological approach was van Manen's hermeneutic phenomenological methodology in order to understand the meaning and nature of stroke patients' experiences in rehabilitating their physical limitations. Collected data were analyzed with phenomenological way of study that was develope by van Manen. The 8 patients who agreed to participate in this research were inter viewed by researcher under the private and comfortable environment. Personal interviews were done three or five times per each patients and each interview took 70 to 90 minutes. The statements were analyzed and finally revealed three essential themes : Theme 1 - Desperateness to achieve freedom again Theme 2 - Seizing hope of recovering Theme 3 - Seeking support from family members, relatives and friends Based on these themes, stroke patients' rehabilitation experience are described as following. The stroke patients have strong desire to achieve freedom again in order to escape from social isolation. The stroke patients want to go back to the state of their previous health state. The stroke patients strongly try to do anything for their recovering and also to have positive thinking. In the other hand, they feel pity for themselves through desperation, fear, sorrow, and self-pity. The stroke patients have dissatisfaction about neighbors' rumor and attitude toward themselves. The stroke patients have experiences to rely on neighbors' support seeking a warm word of consolation. This research showed us that van Manen's hermeneutic phenomenological methodology leads us to understand stroke patients' rehabilitation process more comprehensibly. Based on this research, it is suggested that further studies provide a foundation for the development of a rehabilitation theory for Korean stroke patients.

The objective was to evaluate the effectiveness and safety of acupuncture for patients with rotator cuff diseases. There were 12 electronic databases and 3 trial registries searched up to November 30th, 2019. All randomized trials were eligible, regardless of language, date of publication, or settings. The primary outcomes were pain, shoulder function, and proportion of improved participants assessed within 12 weeks of randomization of the trial. The Cochrane risk of bias for the studies was assessed. Effects sizes were presented as a risk ratio, mean difference, or standardized mean difference with a 95% confidence intervals. Grading of Recommendations Assessment, Development and Evaluation approach was adopted to rate certainty of evidence. Of the 3,686 records screened, 28 randomized trials (2,216 participants) were included in this review. The types of acupuncture included manual acupuncture, dry needling, electroacupuncture, acupotomy, warm needle acupuncture, and fire needle acupuncture. All of the studies had an unclear or high risk of bias related to more than 1 domain. Significant benefits of acupuncture in terms of pain and shoulder function were observed in all comparisons, however, the proportion of improved participants was not described in 2 comparisons. There was substantial heterogeneity among meta-analyzed trials. No serious harm was observed. For primary outcomes, the overall certainty of evidence was very low. There was very low certainty of evidence for the benefits of acupuncture for patients with rotator cuff diseases. The safety of acupuncture remains unclear due to the incompleteness of reporting. Future well-designed randomized trials with transparent reporting are required.
