Sex-related and racial variations in orbital floor anatomy

  • Background: Repair of the orbital floor following trauma or tumor removal remains a challenge because of its complex three-dimensional shape. The purpose of the present study is to understand normal orbital floor anatomy by investigating its differences across four groups (Caucasian American and East Asian, males and females) via facial bone computed tomography (CT). Methods: A total of 48 orbits in 24 patients between 20 and 60 years of age were evaluated. Although most patients underwent CT scanning following trauma, the orbital walls were intact in all patients. Linear and angular measurements of the orbital floor were obtained from CT images. Results: Orbital floor width, length, angle between the orbital floor and medial wall, and distance from the inferior orbital rim to the lowest point of the orbital floor did not show a statistically significant difference between groups. Angles made by the infraorbital rim, the lowest point of the floor, and the anterior border of the infraorbital fissure were statistically significantly wider in East Asian females than in male groups. The floor depth in East Asian females was significantly smaller compared to all the other groups. Conclusion: East Asian female population had smaller curvature and depth of an orbital floor than the other groups, which means racial and sex-related differences should be considered in the orbital floor reconstruction.

Spontaneous Perforation of the Bile Duct (담관의 자연 천공)

  • Spontaneous perforation of the bile duct in children is a very rare disorder. We experienced a 6 year-old girl with spontaneous perforation of the right hepatic duct. The patient was initially misdiagnosed as hepatitis because of elevation of liver enzyme and then as appendicitis because of fluid collection in the pelvic cavity demonstrated by ultrasonogram. A laparoscopic exploration was done and no abnormal findings were detected except bile-stained ascites. Peritoneal drainage was performed and the patients seemed to improve clinically. Abdominal pain, distention and high fever developed after removal of the drains. DISIDA scan showed a possible of bile leak into the peritoneal cavity. ERCP demonstrated free spill of dye from the right hepatic duct. At laparotomy, the leak was seen in the anterior wall of the right hepatic duct 2cm above the junction of the cystic duct and common hepatic duct. The perforation was linear in shape and 0.8cm in size. The patient underwent cholecystectomy, primary closure of the perforation and T-tube choedochostomy. We could not identify the cause of the perforation; however, the T-tube cholangiography taken on the 42nd postoperative day showed a little more dilatation of the proximal common bile duct compared with the cholangiography taken on the 14th day. Long-term follow-up of the patient will be necessary because of the possibility for further change of the duct.

A Case of Nasofrontal Dermoid Cyst Via Transcollumelar Approach (경비주 접근법으로 안면부 손상없이 제거한 비전두 유피낭종 1예)

  • The midline nasal dermoid cysts are rare congenital neoplasms, which are diagnosed frequently in childhood. Masses are often noticeable at birth gaining size over time with recurrent infections and usually arise from the nasal cavity or lower 1/3 of the nasal dorsum. CT scan as the primary investigation is helpful to determine accurately the size and extent of the lesion as well as the integrity of adjacent bony structures. MRI scan is recommended to rule out an intracranial extension or sinus tracts. Treatment of choice is the complete surgical excision preserving the cyst wall. Here in, we present an unusual case of nasofrontal dermoid cyst in a 19-year-old boy without radiographic evidence of transcranial extension. In this case, we surgically removed nasofrontal dermoid cyst via transcolumellar approach. We also corrected saddle nose deformity after mass removal. Therefore, in this case, we experienced a successful case in which the nasofrontal dermoid cyst was totally removed without facial scar and deformity.

  • Three cases of extrahepatic bile duct disorder were presented. Two cases, a dog and a cat, were related to the obstruction of the extrhepatic bile duct, the other dog was affected with the gall bladder rupture. The clinical signs included anorexia, abdominal distension and vomiting. The laboratory test represented increased hepatic enzymes. On the radiography, hepatomegaly was seen in the obstructive cases, and ascites could be seen in the ruptured case. On the Ultrasonography, dilated gall bladder and extrahepatic bile duct were found in the obstructive cases, and there were ascites, indistinct gall bladder wall, dilation of gall bladder and extrabiliary tract, increased mesenteric echogenicity in the ruptured case. All presented were taken medication, surgical foreign material removal, or cholecystectomy showed complete recovery.

A Case of Imperforate Hymen with Hydrocolpos in a Holstein Heifer (홀스타인 미경산우에서 질수증을 동반한 무공질판막 일례)

  • A vaginal speculum examination of a 17-month-old Holstein heifer with a history of repeated artificial insemination (AI) failures via the cranial vagina revealed the presence of an intact hymen (imperforate hymen) beyond the external urethral orifice, which resembled the vaginal wall. An ultrasonographic examination detected hypoechoic fluid within the vaginal cavity between the intact hymen and the cervix, indicating the presence of hydrocolpos. After sedation and epidural anesthesia, the intact hymen was incised using a scalpel and scissors and removed, while the incised portions were sutured using a simple interrupted method. The heifer became pregnant after three consecutive estruses with the intervals of approximately 20 days and concomitant AIs. The heifer gave birth to a healthy male calf with no complications after a 291-day gestation period. This case reports the presence of an imperforate hymen with hydrocolpos in a Holstein heifer, followed by successful insemination, conception and delivery after removal of the imperforate hymen.

Restrictive Cardiomyopathy in a Korean Domestic Short-haired Cat (코리안 숏헤어 고양이의 제한성 심근병증)

  • A 3-year-old castrated male Korean domestic short-haired cat (weighing 5.2 kg) was referred to the Kangwon National University Veterinary Teaching Hospital, with primary complaints of ascites, pleural effusion and respiratory distress. Diagnostic studies revealed marked chylous and hemorrhagic pleural effusion, cardiomegaly, restrictive filling pattern of transmitral flow and mitral annular tissue Doppler profiles and minimally thickened left ventricular free wall. Based on the echocardiographic findings, the case was tentatively diagnosed as restrictive cardiomyopathy. The case was treated with removal of pleural effusion and medical therapy including furosemide, enalapril, sildenafil, clopidogrel. This is the first case report of restrictive cardiomyopathy in Korea.

  • Anaerobic treatment of landfill leachate was studied to investigate the behaviors of pollutant and the characteristics of microorganism for 10 months. The upflow anaerobic sludge blanket (UASB) reactor achieved about 90% chemical oxygen demand (COD) removal at organic loading rates(OLR) up to $20kgCOD/m^3.d$. At higher OLR ($8-20kgCOD/m^3.d$), the propionate concentration increased, indicating that converting propionate to acetate was the rate-limiting step. Nevertheless, increase in the precipitate inside and on the surface of granules as well as on the wall of the reactor resulted in operational problems. The main inorganic precipitate in the granule was calcium compound. Although specific methanogenic activity (SMA) was not affected seriously in this study, metals had to be removed prior to anaerobic treatment so as to be free from the excessive inorganic accumulation that resulted in operational problems.

Analysis of Unintended Lake Formation Problem and Its Environmental Effects a Case Study

  • Waterlogging and unintended lake formation become the main problem in some parts of the world. Starting from 1989, the waterlogging problem was observed in the farmland of the Jarso community of Konso Woreda adjacent to the Segen River in Ethiopia. Therefore, the objectives are determining the extent and causes of unintended lake formation using GIS/RS in addition to a preliminary field survey to mitigate the problem. The analysis of satellite images revealed that over the years invasion of the irrigable land by unwanted water had increased, as, in 1989, the size of the wetland area was about 8 Km2; in 2000 the size of the un-intended lake was only 8.23 ㎢. Alarmingly the size of the lake increased to 19.68 Km2 in 2014. Silting up of Weir and changing the flow of River Segen and Human Intervention and changing the natural flow of River Yanda were the main causes of this unwanted prolonged water-logging. The ecological and social environment has been degrading as the people of the waterlogged area have been experiencing some settlement and losing their land. Another problem encountered was flooding from River Barka and invasion of the farmland. Sediment control best management practices (BMPs) i.e, Removal of sediment, providing sandbags and well-scheduled maintenance; Changing the junction point of Yanda and Segen River were suggested for the long-term and short term possible remedial measures. Gabion retaining wall on the bank of the Segen River to the face of Barka River was suggested to protect the farmland from flooding.

Treatment of Intraoperative Thrombosis and Spasm in Free Tissue Transfers Using the Fogarty Catheter (포가티 카테터를 이용한 유리조직 전이술 시 수술 중 혈전 및 연축의 치료)

  • Purpose: Microvascular free tissue transfer has become a reliable technique for the reconstruction of complex wounds. Occasionally, unexpected intraoperative thrombosis and/or spasm of recipient artery might be annoying problems even for the technically competent microvascular surgeons. If such problems are not treated properly, they will inevitably cause to flap failure. Methods: From January 2006 to February 2007, soft tissue reconstructions by free tissue transfers were performed on 21 patients having complex defects in the lower extremity. Although segmental revision and various pharmacologic agents were repeatedly applied, arterial occlusions were not managed in 6 cases. For removal of thrombi and release of spasm, Fogarty No. 2 or No. 3 catheters were inserted into the lumen to the proximal recipient artery. Its balloon was then inflated after passing through a resistant area. Next, the catheter was gently withdrawn backward. Results: After the Fogarty catheter was inserted two or three times, the pulsatile arterial flow was restored. When the catheter was inserted into the lumen, a feeling of resistance existed in a 5-10cm more proximal portion that could not be easily accessed from the vascular end. After the reestablishment of blood flow, successful anastomoses were achieved and immediate rethrombosis or spasm did not occur. No long-term sequelae associated with balloon trauma to the arterial wall were observed. Conclusion: The use of the Fogarty catheter can be an effective method in treating pedicle thrombosis and spasm. This is a very simple and rapid technique that offers microvascular surgeons another option to increase the success rate of microvascular anastomosis in free tissue transfers.

Studies on Improving Preservative Treatability of Japanese Larch Heartwood by Presteaming (증기(蒸氣) 전처리(前處理)에 의(依)한 낙엽송(落葉松) 심재(心材)의 방부제(防腐劑) 처리도(處理度) 개선(改善)에 관(關)한 연구(硏究))

  • The effectiveness of presteaming for improving CCA treatability on refractory Japanese larch heartwood was investigated in this study. Presteaming was effective on improving treatability, and the extent of improvement was dependent on moisture contents of wood specimen and steaming conditions. Green wood showed higher average value in both preservative retention and penetration than dry wood, and steaming under pressure conditions also had higher treatability than steaming at atmospheric conditions. The degree of improvement for treatability was increased with the extension of steaming period. Treatability of dry wood pres teamed under pressure conditions more than 6 hours and green wood for 3 hours was similar to that enhanced by conventional incising. Presteaming green wood under pressure conditions more than 6 hours was more effective than conventional incising in improvement of CCA treatability, and resultant treatability satisfied a minimum value required for CCA-treated wood for being used at the regions of hazard class H3 and H4. In addition, an improvement of treatability by presteaming was due to an increase in permeability resulted from the degradation of hemicelluloses within aspirated pit membrane and cell wall, not the removal of extractives from pit membrane. The reduction in strength, measured as longitudinal compressive strength, due to pres teaming was related with the degradation of hemicelluloses, and was increased as steaming conditions were severe. The degree of strength reduction associated with presteaming treatment to obtain required treatability could be quantified from the relatively good relation between the increase in treatability and the decrease in strength.
