This study was designed to investigate the feasibility to utilize the electromyogram (EMG) for estimating the muscle torque. The muscle torque estimation plays an important role in functional electrical stimulation because electrical stimulation causes muscles to fatigue much faster than voluntary contraction, and the stimulation intensity should then be modified to keep the muscle torque within the desired range. We employed the neural network method which was trained using the major EMG parameters and the corresponding knee extensor torque measured and extracted during isometric contractions. The experimental results suggested that (1) our neural network algorithm and protocol was feasible to be adopted in a real-time feedback control of the stimulation intensity, (2) the training data needed to cover the entire range of the measured value, (3) different amplitudes and frequencies made little difference to the estimation quality, and (4) a single input to the neural network led to a better estimation rather than a combination of two or three. Since this study was done under a limited contraction condition, the results need more experiments under many different contraction conditions, such as during walking, for justification.
Purpose: This study was to identify the symptom severity, interference and their psychological predictors in thyroid cancer patients hospitalized for radioactive iodine administration. Methods: One hundred seventy-seven thyroid cancer patients admitted to the isolation room for Iodine ($I^{131}$) therapy were recruited. Subjects were asked to complete the questionnaire on core symptoms, thyroid cancer symptoms, interference, depression and state anxiety in the evening after receiving radioactive iodine therapy. Data was analyzed using frequency, percentage, mean, Pearson's correlation, and multiple regression with SPSS vs. 19. Results: Lack of appetite, drowsiness, sleep disturbance, fatigue, and nausea were the 5 most core symptoms. More than 20% of patients experienced moderate to severe thyroid cancer symptoms including feeling cold, hoarseness, swallowing difficulty, and feeling hot. More than 30% of subjects experienced moderate to severe interferences in mood, general activity, and 22% in walking. Depression and state anxiety were identified as predictors of core symptoms, thyroid symptom severity and interference. Conclusion: Nursing interventions to reduce the symptom severity and interference need to be developed by considering thyroid cancer patients' depression and anxiety when hospitalized in the isolation room for radioactive iodine administration.
Alcohol has extensive effects on sleep and daytime sleepiness. Alcohol has a sleep inducing effect and the effect of increased non-REM sleep and suppressed REM sleep during the first half portion of night sleep, but alcohol induces the effect of decreased non-REM sleep and increased light sleep and frequent awakenings and REM rebound during the second half portion of night sleep. Alcohol provokes chronobiological change such as the changes of amplitude or the phase shifts of hormones or core body temperature. The sleep disruption resulting from alcohol drinking may lead to daytime fatigue and sleepiness. The elderly are at particular in the increased risk of alcohol-related sleep disorders because they achieve higher levels of alcohol in the blood and brain than do younger adults after consuming an equivalent dose. Bedtime alcohol consumption among older adults may lead to unsteadiness if walking is attempted during the night, with increased risk of falls and injuries. Continued alcohol use for sleep induction often induces aggravation of insomnia, alcoholism or sleep related breathing disorders such as obstructive sleep apnea. Alcohol should not be used as substitution of sleep pill because of the dependence and tolerance for sleep inducing effect, and the sleep disruption produced by alcohol withdrawal.
본 논문은 족하수 환자를 대강으로 휴대가 간편하고 사용이 편리한 피드백 제어형 전기자극기를 제안하는 데 그 목적이 있다. 이 시스템은 교통사고, 산업재해, 뇌졸중 등으로 인하여 하지마비를 동반한 환자에서 마비된 하지근의 위축 및 괴사를 방지하고 혈액순환을 증가시킬 뿐만아니라 보행을 보조하여 안정된 활동을 가능케 한다 피드백 제어형 전기자극기는 환자가 오랜 시간 보행시 전극의 임피던스 변화나 근피로에 의해 족하수증이 다시 발생하게 되면 관절각의 변화가 관절각 센서에 의해 검출되고 이때의 오차는 PID 제어에 의한 자극전압의 변화를 가져와 설정된 위치에 발목이 도달하도륵 자극레벨이 자동 조절된다. 자극조건은 자극주파수 40 Hz, 자극펄스 0.2 ms, 30∼80 V이다. 족하수 환자 5명을 대상으로 42일 동안 첫째 1주일 동안에는 하루에 15분을 자극하고, 다음 주기는 1주일 단위로 하루에 30분, 60분, 70분, 120분씩 점차적으로 시간을 증가하면서 자극하였다. 이때 근력은 27.7%가 향상되었고, 근 피로도는 22.9%가 감소되었다. 이런 결과를 종합할 때 피드백 제어형 전기자극기는 족하수 환자의 보행기능 및 운동기능을 회복하는 데 효과적이라 하겠다.
사람의 인지기능 중 주의력은 감각기관을 통해 방대한 자극 중 필요한 자극에만 의식을 집중하는 과정이다. 주의력은 실내 온도 등 노출 환경에 영향을 받는 것으로 알려져 있다. 스포츠 분야에서는 냉각 팩 등을 사용한 직접 전도 방식의 냉각이 광범위하게 사용되어 효과를 보고 있으나 전도에 의한 냉각이 주의력과 같은 인지 활동에 미치는 영향에 대해서는 알려진 바가 없다. 본 연구에서는 경추부 후면 부위에 특수 제작된 냉각 패드를 적용할 때 주의력에 미치는 영향을 살펴보기 위해 수행되었다. 40명의 피검사자를 네 집단으로 나누고 독서 및 산책 중 냉각 패드의 적용에 따라 주의력이 차이를 보이는지 살펴보았다. 주의력은 FAIR 주의력 검사를 통해 평가되었으며, 선택적 주의력을 의미하는 능력지수, 주의력의 정확도를 나타내는 통제지수, 균일한 주의력의 유지 기간을 나타내는 지속성지수에 대한 분산분석과 사후분석을 실시하였다. 결과적으로, 독서 및 학습 등 시각적 주의력이 지속적으로 요구되는 경우 냉각 패드의 적용을 통한 전도 냉각이 선택적 주의력을 확대시키고 균일한 주의력을 유지하는데 도움이 된다. 또한, 선택적 주의력의 향상과 균일한 주의력의 유지를 위해 가벼운 신체 활동이 도움이 된다. 다만 이 경우 전체 독서 시간의 감소를 전제로 하여야 한다.
최근 다양한 환경에서의 보행과 특정 동작에서 해당 모드를 지원하는 지능형 대퇴 의족이 개발되고 있다. 계단 상행, 하행과 같은 특정 제어는 동작 전 모드를 변경해야하기 때문에 사용자의 의도를 기반으로 제어명령을 구분하는 기법이 필요하다. 따라서 본 논문에서는 지능형 대퇴 의족 시스템에서 사용자의 의도에 기반하여 다양한 제어명령을 구분할 수 있는 기법을 제안한다. 제안하는 기법은 근전도 신호의 보상이 필요하다고 판단되는 경우, 평상시의 근전도 신호의 세기 및 주파수 성분과 압력센서로 추정한 근육의 부피 정도의 상관관계를 이용하여 근전도 신호를 보상하는 것이며 실험을 통해 근피로가 축적되어 있는 상황에도 사용자의 의도를 정확하게 검출하는 것을 확인하였다. 향상된 사용자 의도 검출 기법을 통해 정해진 시간 내 근육의 수축 횟수를 기반으로 5개의 제어모드를 구분할 수 있도록 하였으며 실험 결과 운동 후 근피로로 인해 근신호의 세기가 평시와 다를 경우에도 근신호 보상을 통해 97.5%의 정확도를 갖는 것을 확인하였다.
This study was conducted to provide a counterplan for preventing so celled “plastic house syndrome” revealed among farmers spending much time in the plastic houses. For this, working environment inside a plastic house was observed. Then, experiments were carried out mostly in a climatic chamber with three kinds of working posture on uneven($D_1-F_1$) or even($D_2-F_2$) ground surface. Tested work loads with three kinds of working posture were : moving in a sitting posture with attaching breast to legs and waving arms ($A_1$), moving in a bending posture with waving arms ($B_1$), and moving a 6kg weighting luggage in a standing posture ($C_1$) Physiological responses in the workers to three different work loads were observed in a climatic chamber, with or without using some instruments, to evaluate work efficiencies. The results obtained are summerized as follows. 1. $C_1$ was the hardest work and $B_1$ was harder than $A_1$ on the even ground. 2. Worker's physiological fatigue and physical loads remarkably decreased when using the instruments such as a chair and a cart with some rollers on the even ground. 3. Working with pushing a cart($F_1$) was the hardest work, and standing($D_1$) was harder than walking($E_1$) on the uneven ground. 4. Worker's physiological fatigues and physical loads remarkably decreased on the even ground. 5. Similar results were obtained when the same experiment was carried out in a plastic house.
Background: Functional status is the ability to perform daily activities. Little is known about quality of life and health status of gynaecological cancer patients. The present study therefore aimed to evaluate the functional status of women receiving treatment for gynecological oncological disease while not hospitalised. Materials and Methods: This descriptive study covered 42 patients monitored by the Gynecological Oncology Unit in 2011. Data were collected using the Functional Living Index-Cancer and analysed with the chi square test, independent samples t-test, Mann-Whitney U test, one-way ANOVA test and the Kruskal-Wallis H test. Results: Of the 42 cases, 66.7% had been diagnosed within the previous year and 90.5% were undergoing chemotherapy. The most severe symptoms experienced were pain (35.7%), fatigue-weakness (40.5%) and nausea and vomiting (56.5%). Daily activities where the most difficulty was experienced were housework (28.6%), average pace walking (31.0%), carrying more than 5 kg (28.6%). The mean Functional Living Index score was quite high ($103.5{\pm}24.1$). FLIC-C scale scores did not vary with the educational status, diagnosis duration, and family history of cancer (p>0.05). Conclusions: Evaluation of the functional status of gynecological cancer patients and how they cope with problems should indicate to healthcare professionals what help can be given to maintain quality of life.
The purpose of this study was to evaluate the effects of forefoot rocker shoes equipped with a metatarsal bar on lower extremity muscle activity and plantar pressure distribution. Ten healthy women in the age of twenties were participated in this study as the subjects. All subjects walked on a treadmill(Gait Trainer, BIODEX, USA) wearing normal shoes and metatarsal bar shoes, during which the plantar pressure distribution and muscle activity were measured. Using Pedar-X system(Novel Gmbh, Germany), the plantar pressure was measured for six regions of the foot: forefoot, midfoot, rearfoot, 1st metatarsal, 2-3th metatarsal, and 4-5th metatarsal, and for each sub-region, 4 features such as maximum force, contact area, peak pressure, and mean pressure were analyzed based on the plantar pressure. EMG(Electromyography) activity was measured by attaching surface electrodes to the rectus femoris, biceps femoris, tibialis anterior, and gastrocnemius medial head, and magnitude of muscle contraction was analyzed in IEMG(Integrated EMG) value. The results show that the maximum force, contact area, peak pressure, and mean pressure in the midfoot all increased while maximum force, peak pressure, contact area, mean pressure in the 1st metatarsal and 2-3th metatarsal all decreased when wearing functional shoes. Also, muscle activities in the four muscles were all decreased when wearing the functional shoes. This paper suggests that forfoot rocker shoes equipped with a metatarsal bar can help disperse the high pressure and absorb the shock to the foot as well as give positive influence on gait pattern and postural stability by reducing muscle fatigue during walking.

As a result, I showed improved results significantly discount rate after wearing in the Center of Position area and distance to assess the balance ability. Therefore, the correction insole function is to support the Longitudinal arch and Transverse arch to an important role in the foot. It may be that it has a functional element for improving the balance of the function of preventing collapse of the arch during walking, to disperse the weight of the entire foot, us reduce fatigue in the end.
