접촉스포츠 참여가 은퇴 선수의 신경인지 점수와 이중과제 보행에 미치는 영향 (Effects of Participation in Contact Sports on Neurocognitive Scores and Dual-Task Walking in Retired Athletes)

  • Objective: The aim of this study was to investigate the effect of participation in contact sports on neurocognitive scores, dual-task walking velocity, and cognitive costs in retired athletes. Method: Forty-four retired athletes (mean age = 26.4±5.5 yrs) and thirty-eight controls (mean age = 26.1±4.9 yrs) participated in this study. Neurocognitive score was collected using computerized neurocognitive testing using RehaCom. Gait velocity was collected one single task, four dual-tasks, and two multi-tasks using Optogait. Mann-Whitney U test was performed to compared differences in cognitive scores among groups. A mixed-design two-way ANOVA and Bonferroni posthoc test were used to assess the effect of group and walking tasks for each condition. Results: The auditory divided attention of neurocognitive score of retired athletes was higher than the control group (p < 0.05). No statistical differences were observed in the other neurocognitive scores between groups. The changes in walking velocity and cognitive costs according to the dual-task walking tests differed between the two groups (p < 0.05). Conclusion: Although participation in contact sports did not affect the neurocognitive results of retired athletes, it could be confirmed that the reduction in walking velocity and an increase in cognitive costs during dual-task walking. Rather than observing only neurocognitive scores as a single evaluation item for cognitive evaluation of retired athletes in relation to daily life, the application of the dual-task gait test may provide useful information.

  • This paper presented a design and control of a biped walking RGO(robotic gait orthosis) and its simulation. The biped walking RGO was distinguished from the other one by which had a very light-weight and a new RGO system will be made of 12-servo motors and 12-controllers. The vibration evaluation of the dynamic PLS(posterior leaf splint) on the biped walking RGO was used to access by the 3-axis accelerometer with a low frequency vibration of less than 30 Hz. The galt of the biped walking RGO depends on the constrains of mechanical kinematics and the initial posture. The stability of dynamic walking was investigated by analyzing the ZMP (zero moment point) of the biped walking RGO. It was designed according to the human wear type and was able to accomodate itself to the environments of S.C.I. Patients. The Joints of each leg were adopted with a good kinematic characteristics. To analyse joint kinematic properties. we made the strain stress analysis of the dynamic PLS and the analysis study of FEM with a dynamic PLS.

The Effects of Walking and Turning and Treadmill Training on Postural Balance and Walking in People with Parkinson's Disease

  • Purpose: The aim of our study was to evaluate the therapeutic effects of walking and turning plus treadmill training on the functional balance and walking ability of individuals with Parkinson's disease (PD). Methods: Twenty-four participants with Stage 1 to 3 ($2.13{\pm}0.64$) PD based on the Hoehn and Yahr scale were randomly allocated to the experimental group (EG) and control group (CG), with 12 participants in each group. The measured outcomes included the motor subscale of the unified Parkinson's disease rating scale (UPDRS-M), the Berg balance scale (BBS), the Timed Up and Go (TUG) test, the 10-meter walk test (10MWT), and the 6-minute walk test (6MWT). Results: Pre-to-post intervention improvements were noted for all the outcome measures for both groups (p < 0.05). Post intervention, there was a significant improvement in the EG compared to the CG for the following measured outcomes (p < 0.05): UPDRS-M (p = 0.021; 95% CI, 0.081-6.519), BBS (p = 0.042; 95% CI, 1.375-4.541), TUG (p = 0.034; 95% CI, -3.315--0.143), 10MWT (p = 0.011; 95% CI, -2.032--0.289), and 6MWT (p = 0.002; 95% CI, 24.39-91.273). Conclusion: Our study suggests that walking and turning plus treadmill training improves balance and walking compared to treadmill training only in patients with PD.

측방보행 훈련이 뇌졸중 환자의 균형 및 보행에 미치는 영향 (Effects of Side Walking Training on Balance and Gait in Stroke Patients)

  • 본 연구의 목적은 뇌졸중 환자를 대상으로 측방보행 훈련이 균형(기능적 보행 검사, 일어나 걸어가기 검사) 및 보행(10 미터 보행검사)에 미치는 영향을 알아보고자 하였다. 28명의 뇌졸중 환자를 대상으로 실험군(측방 보행군)과 대조군(전방 보행군)에 각각 14명씩 무작위로 배정하였다. 두 그룹 모두 재활 물리치료를 4주 동안 실시하였다. 실험군에는 4주 동안 1회당 20분씩 주 3회 측방보행 훈련을 실시하였고, 대조군에도 4주 동안 1회당 20분씩 주 3회 전방보행 훈련을 실시하였다. 측방보행 후 기능적 보행평가를 측정한 결과 실험군에서 16.86점에서 18.64점으로 유의하게 향상되었고(p<.05). 일어나 걸어가기 검사에서도 26.03초에서 22.43초로 감소하여 유의하게 향상되었다(p<.05). 10미터 보행검사에 있어서도 실험군에서 21.90초에서 19.10초 감소하여 유의하게 향상되었다(p<.05). 따라서 측방보행 훈련이 뇌졸중 환자의 균형 및 보행을 증진시키는데 유용한 훈련으로 제안할 수 있을 것이다.

  • Unrestricted measurement method of three-dimensional walking distance utilizing body acceleration and terrestrial magnetism is discussed. The three-dimensional walking distance is derived by the integration of the three dimensional acceleration of foot during swing phase. Since the sensor system attached on the foot rotates during swing phase, the acceleration data measured on the foot include acceleration of gravity which causes inaccurate calculation of the velocity and the distance.   • A wireless control system was designed and constructed to control the rice transplanter of walking type with remote control. VHF(very high freqency) was sent from wireless transmitter to wireless reciever by usig 6 channels to control main clutch, steering clutch, plant lever and throttle lever within 200 meter distance between remote control and rice transplanter. The rice transplanter with wireless control system showed good performance for accuracy of travel speed, traveling mobility and turning diameter in the concrete, paddy and dry field. It is concluded that the developed wireless control system could be adapted to control not only conventional rice transplanters but also most of the agricultural machine by changing some parts of the control system.

근육피로도를 고려한 하반신 마비환자의 보행 자동제어 FES 시스템에 관한 연구 (A Study on an Automatic FES Control System for Paraplegic Walking Against Muscle Fatigue)

  • In this paper, a DSP and microcomputer-based EMG controlled functional electrical stimulation (FES) system, for restoring walking of paraplegics at the patients' own command, is presented. The above-lesion EMG is a time-varying nonstationary signal and its autoregressive (AR) parameters are identified by the nonstationary identification algorithm using a DSP chip. The identified AR parameters are used for the cloassification of the function and the control of the movement. The below-lesion response-EMG signal is used as a measure of muscle fatigue. This FES system is designed to measure muscle fatigue and control the stimulation intensity according to the amplitude of the response-EMG signal. While the automatic electrical intensity control is obtained by identifying the movement, the proposed FES system is suitable for the automatic control of paraplegic walking.

  • 본 논문은 보행보조기 사용자의 사용자 의지 파악을 통해 보행보조기의 최적 구동 방법을 제시하고자 한다. 최근에 노인인구의 증가로 인해 노인 및 장애인을 위한 보행 보조기에 대한 관심이 증가되고 있다. 그러나 대부분의 경우 동력이 없는 시스템으로써 경사 등의 공간에서 취약성을 가지고 있다. 이에 동력형 보행보조기에 대한 관심이 증가되고 있으나, 대부분의 경우 보행보조기 조종이 여의치 않다. 이에 본 논문에서는 사용자의 보행 의지를 파악할 수 있는 시스템을 도입하고 이를 기초로 보행자의 안정적인 구동을 수행할 수 있도록 퍼지 기반의 알고리즘을 적용한다. 먼저 사용자의 의지력은 FSR 센서를 이용하여 파악하고, 이를 기초로 사용자가 이동하고자 하는 이동 방향과 이동속도에 대해 퍼지 알고리즘을 적용하였다. 퍼지알고리즘으로부터 나온 데이터를 기초로 보행보조기의 차량 속도와 방향에 대해 구동 바퀴의 차동 구동을 통해 사용자의 의지에 맞춰 구동할 수 있도록 하였다. 이를 통해 사용자의 이동하려는 의지에 대해 안정적으로 차량을 이동시킬 수 있도록 최적의 보행 시스템을 구축하였다. 이 모든 과정을 실험을 통해 검증하였다.

지리적 가중회귀모형을 이용한 지역별 걷기실천율의 지역적 변이 및 영향요인 탐색 (Exploring Spatial Variations and Factors associated with Walking Practice in Korea: An Empirical Study based on Geographically Weighted Regression)

  • Purpose: Walking practice is a representative indicator of the level of physical activity of local residents. Although the world health organization addressed reduction in prevalence of insufficient physical activity as a global target, the rate of walking practice in Korea has not improved and there are large regional disparities. Therefore, this study aimed to explore the spatial variations of walking practice and its associated factors in Korea. Methods: A secondary analysis was conducted using Community Health Outcome and Health Determinants Database 1.3 from Korea Centers for Disease Control and Prevention. A total of 229 districts was included in the analysis. We compared the ordinary least squares (OLS) and the geographically weighted regression (GWR) to explore the associated factors of walking practice. MGWR 2.2.1 software was used to explore the spatial distribution of walking practice and modeling the GWR. Results: Walking practice had spatial variations across the country. The results showed that the GWR model had better accommodation of spatial autocorrelation than the OLS model. The GWR results indicated that different predictors of walking practice across regions of Korea. Conclusion: The findings of this study may provide insight to nursing researchers, health professionals, and policy makers in planning health programs to promote walking practices in their respective communities.

불안정 지면에 앉아 추가적으로 수행한 몸통운동이 만성 뇌졸중 환자의 균형과 보행 능력에 미치는 영향 (Effects of Additional Trunk Exercises on an Unstable Surface on the Balance and Walking Ability of Individuals with Chronic Stroke)

  • Purpose: This study aimed to explore the effects of additional trunk exercises on an unstable surface on the balance and walking ability of individuals with chronic stroke. Methods: Sixteen patients with chronic stroke participated in this study. The participants were randomly assigned to two groups: experimental group (n=8) and control group (n=8). All the participants underwent a typical physical therapy program for 30 min a day. Moreover, the experimental group participated in a 30 min trunk exercise program on an unstable surface, whereas the control group participated in a 30 min trunk exercise program on a stable surface. Both groups performed the exercises five times a week for three weeks. The Berg Balance Scale (BBS) was used to measure changes in balance. The gait variables were measured using the GAITRite system (CIR System Inc., Clifton, NJ, USA) to examine changes in walking ability. Results: Both groups showed a significant intragroup improvement in balance, gait speed, cadence, stride length, and double support period (p<0.05). In the intergroup comparisons after the intervention, the experimental group showed significant improvements over the control group in balance, gait speed, cadence, stride length, and double support period (p<0.05). Conclusion: This study applied additional trunk exercises on an unstable surface to chronic stroke patients, and the results showed a significant improvement in the patients' balance and walking abilities. Therefore, trunk exercise on an unstable surface may be applicable as an intervention method to improve the balance and walking ability of chronic stroke patients.
