Purpose: The purpose of this study was to investigate the effect of microcurrent on fatigue of muscles in people who were flat-footed during gait. Methods: 10 flat-footed university students volunteered to participate in this study. 10 flat-footed subjects were divided into 2 groups, one group was experimental group of 5subjects(This group put on microcurrent induction shoes but the subjects were not able to feel the current.) and the other group was the control group of 5subjects(This group put on the general shoes which were similar in shape but microcurrent was not induced.) to perform double blind test and random sampling. Their gait muscle fatigue of 6 regions (vastus medialis, gastrocnemius, tibialis anterior, biceps femoris, erector spinae, and rectus abdominis muscle.) was measured by EMG MP150, Delsys Inc Boston, USA during walking and then they carried out the Harvard step with a platform (It was a arbitrarily made wooden platform of 100cm long, 50cm wide, 60cm high. They carried out climbing it for one second and descending it for one second by using the Metronome program, total 5minutes) for 5minutes. Right after that, the subjects walked on a treadmill at a speed of 4km/h for 10minutes and then their gait muscle fatigue of 6regions was assessed while they were walking on the ground as equally as before exercise. Results: The experimental group has resulted in lower average differences in gait muscle fatigue before and after exercise than those of the control group average 12.24Hz(P=0.009) at vastus medialis, average 8.52Hz(P=0.016) at gastrocnemius, average 9.16Hz(P=0.009) at tibialis anterior, average 8.66Hz(P=0.047) at biceps femoris, average 7.53Hz(P=0.016) at erector spinae, and average 7.80Hz(P=0.047) at rectus abdominis. All of the assessments of muscles have shown significant difference statistically. Conclusions: This result has shown that the use of micro current could decrease gait muscle fatigue of flat-footed people. It is recommended to use a microcurrent to reduce their gait muscle fatigue.

Study Design: Prospective observational study. Purpose: To evaluate the disability domains relevant to Indian patients with low backache and propose a modified disability questionnaire for such patients. Overview of Literature: The Oswestry Disability Index (ODI) is a self-reported measurement tool that measures both pain and functional status and is used for evaluating disability caused by lower backache. Although ODI remains a good tool for disability assessment, from the Indian perspective questions related to weight lifting and sexual activity of ODI are questioned in some of the earlier studies. Activities of daily living in Indian patients vary substantially from those in other populations and include activities like bending forwards, sitting in floor and squatting which are not represented in the ODI. Methods: In this prospective observational study, a seven-step approach was used for the development of a questionnaire. Thirty patients were interviewed to identify the most challenging issue they faced while performing their daily activities (by free listing) and understand how important the questionnaire items were in terms of the standard ODI. Thus, a comprehensive disability questionnaire comprising 14 questions was developed and administered to 88 patients. Both qualitative (interviews) and quantitative methods (to establish the validity, reliability, and correlation with the Visual Analog Scale [VAS] and Rolland Morris disability questionnaire) were used to identify the 10 questions that best addressed the disability domains relevant to Indian patients. Results: According to free listing, four new questions pertaining to bending forward, sitting on the floor, walking on uneven surfaces, and work-related disabilities were included. In the second phase, wherein the questionnaire with 14 items was used, 56.8% patients did not answer the questions related to sexual activity, whereas 23.8% did not answer those related to walking on uneven surfaces. The modified questionnaire demonstrated good internal consistency (Cronbach's alpha=0.892) and correlation with the Rolland Morris questionnaire (Cronbach's alpha=0.850, p>0.05), as well as with the VAS score for disability (Cronbach's alpha=0.712, p>0.05) and pain (Cronbach's alpha=0.625, p>0.05). Conclusions: A modified disability questionnaire that was designed by adding two questions related to bending forward and work status and removing questions related to sexual activity and weight lifting or traveling (depending on the occupation) can help evaluate disability caused by back pain in Indian population.
The purpose of this study was to develop the inertial sensor module system to detect gait event using single angular rate sensor(gyroscope), and evaluate the accuracy of this system. This sensor module is attached at the heel and gait events such as heel strike, foot flat, heel off, toe off are detected by using proposed automatic event detection algorithm. The developed algorithm detect characteristics of pitch data of the gyroscope to find gait event. To evaluate the accuracy of system, 3D motion capture system was used and synchronized with sensor module system for comparison of gait event timings. In experiment, 6 subjects performed 5 trials level walking with 3 different conditions such as slow, preferred and fast. Results showed that gait event timings by sensor module system are similar to that by kinematic data, because maximum absolute errors were under 37.4msec regardless of gait velocity. Therefore, this system can be used to detect gait events. Although this system has advantages of small, light weight, long-term monitoring and high accuracy, it is necessary to improve the system to get other gait information such as gait velocity, stride length, step width and joint angles.
The purpose of this study was to analyse the subtalar joint movement characteristics in human stance phase. The data of subtalar joint movement patterns are collected by CTA(calcaneus to tibia angle) measurements. CTA is defined as a angle formed between the bisect of the posterior shank and bisect of the posterior heel, as determined by placement of the retroreflective markers. The angle measured in degrees. The participants are 74 healthy individuals (37 men and 37 women) who have no orthopedic and neurological impairment, aged from 19 to 29 years(mean 22.95). Prior to participation, each subjecct informed the procedures of experiment from researcher and assistant researcher. The equipments of this study are walking grid, marking tapes, goniometer, video camera, monitor and ink for foot print are used in the study. In order to determine the statistical significance of result, the paired t-test and Pearson correlation were applied at the 0.05 level of significance. The results were as follows : 1. The reliability of measured CTA value are showed a high correlation, ranged from .86 - .94. 2. The mean value of step width are 7.67cm in men and 6cm in women. So, significant difference between men and women in 0.05 level of significance. 3. There was significant difference between man and women's TOA(toe out angle)(p<0.05). 4. The CTA of female's is more higher than male's one, however not statistical difference between man and women(p>0.05). 5. The CTA is reduced according to increasing TOA(p<0.05).
The purpose of this paper is to study and propose playground facilities for physically challenged children to cultivate adaptability in society and advances the opportunity of participate in physical exercise. This study carried out site investigations by visiting and inspecting various facilities: a recreational walking path for physically challenged people, a care facility for physically challenged children, a special school for physically challenged children, and "Madel Park", a private-use playground for physically challenged children. The result of the site investigation found that, overall, play facilities are difficult to use for physically challenged children. In addition, because park space is often small and narrow, children who are restricted to wheelchairs find it impossible to access such facilities. By carefully inspecting each of the facilities, the author found suitable elements to apply for outside play space. These elements include the introduction of an essential slope way to allow for safe movement, the provision of special seats which support the body to outside play equipment and the adaptation of treatment room instruments that assist balance to outdoor playground facilities. On the basis of the site investigation the interview and the indoor education program for physically challenged children the author completed an outside play program. Each item in this program was classified as belonging to either a "sense play" program or and "exercise play" program. Finally, the author designed suitable play equipment for physically challenged children corresponding to each item in the exercise programs in order to research the appropriateness of the equipment to its intended use. This research is the first step. From this point the author expects that specific applications will be developed through a practical space plan and sufficient clinical experiments involving use of the new equipment.
PURPOSES : In this study, we analyzed the road crossing behavior of older pedestrians on a mid-block signalized crosswalk, and compared it to that of younger pedestrians. In addition, we analyzed the correlation between accidents involving older pedestrians while crossing roads and their behavioral characteristics. Finally, we confirmed the reasons for an increase in accidents involving older pedestrians. METHODS : First, 30 areas with the highest incidence of accidents involving older pedestrians while crossing roads were selected as target areas for analysis. Next, we measured the start-up delay (the time elapsed from the moment the signal turns green to the moment the pedestrian starts walking) and head movement (the number of head turns during crossing a road) of 900 (450 older and 450 younger) pedestrians. The next step was to conduct a survey and confirm the differences in judgment between older and younger pedestrians about approaching vehicles. Finally, we analyzed the correlation between the survey results and traffic accidents. RESULTS : The average start-up delay and head movement of the older pedestrians was 1.58 seconds and 3.15 times, respectively. A definite correlation was obtained between head movement and the frequency of pedestrian traffic accidents. The results of our survey indicate that 17.3% of the older pedestrians and 7.8% of the younger pedestrians have a high crash risk. CONCLUSIONS : Behavioral characteristics of older pedestrians were closely correlated with accidents involving older pedestrians while crossing roads in mid-block signalized crosswalks. Our study indicates that in order to reduce the number of accidents involving older pedestrians, it is necessary to develop an improvement plan including measures such as installation of safety facilities taking the behavioral characteristics of older pedestrians into consideration and their safety education.
PURPOSE: The purpose of this study was to determine if virtual reality-based exercise was effective in balance, gait, and falls efficacy in patients with Parkinson's disease (PD). METHODS: Thirty patients with PD were assigned randomly to the experimental (n=15) or control groups (n=15). The experimental group performed virtual reality-based exercise and the control group underwent conventional physical therapy for 30minutes, five times per week for four weeks. A force platform system, the Korean version of the Berg Balance Scale (K-BBS), the six-minute walking test (6MWT), and the Korean Version of the Falls Efficacy Scale (K-FES) were used to evaluate balance, gait, and falls efficacy. Wilcoxon signed-rank test and Mann-Whitney U test were used to examine the within- and between-group differences after training, respectively. RESULTS: Changes in the K-BBS score (p<.001) and fall efficacy (p<.01), following the intervention were significantly greater in the experimental group than in the control group whereas significant group difference were not observed for the anterior-posterior and mediolateral postural sway lengths. The change in the ground reaction force (p<.001) and 6MWT values (p<.05) were significantly greater after intervention in patients in the experimental group than in the control group, whereas a significant group difference was not observed for the step and stride lengths. CONCLUSION: This study indicates that virtual reality-based exercise is an effective intervention for improving balance, gait, and fall efficacy in patients with PD.
