Evaluation of Airborne Bacteria and Fungi in Surgical Areas at the Animal Hospital

  • Studies on the concentration of airborne microorganisms in human medicine as a part of a study on the nosocomial infections have been conducted properly, but in veterinary medicine, there has been rarely performed in Korea to the best of study's knowledge. The purpose of this study was to evaluate the distribution of airborne microorganisms and to identify their species in different places in the animal hospital to alert the necessity of thorough cleanliness management. This study evaluated the concentrations of airborne bacteria and fungi in hospital areas, such as patient waiting room, internal medicine ward, surgical ward and radiological diagnostic ward. The concentration of bacteria and fungi was significantly lower (p < 0.05) in two operating rooms and higher in the patient waiting room. The dominant species of bacteria were Micrococcus spp., Staphylococcus spp., and fungi were Penicillium spp., Dermatophyte mold. Animal hospitals need to perform proper procedures for disinfection, sterilization, and environmental cleaning as well as appropriate employee training and monitoring in order to the maximum prevention of the risk of nosocomial and surgical infections.

Characteristics of PM10 and PM2.5 Concentrations at Subway Stations in Busan for 3 years (2015~2017) (부산지역 지하역사의 최근 3년간(2015~2017년) PM10과 PM2.5 농도 특성)

  • This research investigated the characteristics of $PM_{10}$ and $PM_{2.5}$ concentrations at the main subway stations in Busan. Annual mean $PM_{10}$ concentrations at the Seomyeon 1- waiting room and platform were $51.3{\mu}g/m^3$ and $47.5{\mu}g/m^3$, respectively, and the annual $PM_{2.5}$ concentration at the Seomyeon 1- platform was $28.8{\mu}g/m^3$. $PM_{2.5}$/$PM_{10}$ ratio at Seomyeon 1-platform and Dongnae station were 0.58 and 0.53, respectively. Diurnal variation of $PM_{10}$ concentration at subway stations in Busan was categorized into four types, depending on the number of peaks and the times at which the peaks occurred. Unlike the areas outside of the subway stations which reported maximum $PM_{10}$ concentration mostly in spring across the entire locations, the interiors of the subway stations reported the maximum $PM_{10}$ concentration in spring, winter, and even summer, depending on their location. $PM_{10}$ concentration was highest on Saturday and lowest on Sunday. The numbers of days when $PM_{10}$ concentration exceeded $100{\mu}g/m^3$ and $80{\mu}g/m^3$ per day over the last three years at the subway stations in Busan were 36 and 239, respectively. The findings of this research are expected to enhace the understanding of the fine particle characteristics at subway stations in Busan and be useful for developing a strategy for controlling urban indoor air quality.

  • This research investigated the characteristics of CO, $CO_2$, and $NO_2$ concentrations at main subway stations in Busan. The annual mean CO concentrations at the Suyeong and Nampo stations were 0.75 ppm and 0.48 ppm, respectively. Annual $CO_2$ concentration at the Seomyeon 1- platform was 649 ppm. The $NO_2$ concentrations at the Seomyeon 2- waiting room and the Yeonsan station were 0.048 ppm and 0.037 ppm, respectively. CO concentration was highest at two times of the day, and was proportional to the number of passengers commuting to and from work. The CO and $CO_2$ concentrations were highest in winter, but $NO_2$ concentration was highest in spring. CO and $CO_2$ concentrations were highest on Saturday and lowest on Sunday. The correlation of CO and $NO_2$ concentrations measured at the subway stations with those at the ambient air quality station were highest at the Seomyeon 1 and 2- waiting room and Jeonpodong. The correlation was lowest at the Yeonsan and Yeonsandong station. The number of days when $CO_2$ concentration exceeded 700 ppm over the last three years at the Seomyeon 1- platform was 174. The findings of this research are expected to deepen understanding of the fine particle characteristics at subway stations in Busan and be useful for developing a strategy for controlling urban indoor air quality.

Distribution of Hospital Airborne Microorganisms in Seoul, Korea (서울시내 종합 병원 공기중의 미생물 분포)

  • This study was carried out to investigate the airborne microbial pollution in hospital environment. Using a mechanical air sampler, microbiological samples were taken from intensive care unit, general ward room, patients wailing room and outdoor of 20 hospitals in Seoul, Korea. The concentration of airborne bacteria and fungi ranged 97-410 cfu/㎥ and 37-77 cfu/㎥, respectively and patients waiting room had highest bacterial count. 10 genera of molds were identified and the most frequently recovered molds were Aspergillus, followed by Penicillium, Alternaria and Cladosporium. Among Staphylococcus species, S. haemolyticus and S. epidermidis were predominant and 47% of Staphylococcus species were isolated from intensive rare unit.

  • Purpose: Evidence-based design is the process of making design decisions based on reliable research to achieve the best results, so it is important to accumulate reliable evidence through research. Therefore, the purpose of this study is to present new evidence by applying the evidence-based design process to improve the digital signage for each room spaces in hospitals and verifying its effectiveness. Method: Through the 8-step process of evidence-based design, improved digital signage for each room spaces are installed. It conducted surveys and statistical analysis to prove hypotheses by linking design and research. Results: The hypotheses established in the study are: 1) improvement in the readability of digital signage is correlated with patient satisfaction; 2) Improving the intuitiveness of digital signage correlates with patient satisfaction; 3) Improving the sufficiency of digital signage correlates with patient satisfaction. As a result of satisfaction analysis and correlation analysis, all of the above hypotheses were proven. Implications: Although the hospital sign system is the element that patients rely on most intuitively in the long journey of reception, waiting, examination, and treatment, there is insufficient evidence to refer to or apply it when designing. It is necessary to expand future research to expand the evidence that can be applied to hospital sign design.

A Study on the Air Counts and the Infection of Maternity in n General Hospital (병실 낙하균 및 산모감염에 관한 연구)

  • This research is to prevent the infection of maternity in the hospital by examining the microbes contaminations in maternity through airbone microbes and those who are engaged in the ward of O.B. & G.Y. and to furnish the basic data available to hospital management. The bacterial growth of airbone microbes contaminations in nosocomial air and who thor the nasal cavity of passers by (doctors, nurses, parturient women) who went to the ward of O.B. & G.Y. contaminated or not were examined in“E”Univ. Hospital from July to August, 1979 by using thioglycollate broths and agar plates. The following results were obtained: 1. The average colony number of airborne microbes revealed as follows the pediatric ward (36 colonies), the internal ward (33 colonies), the ward of O.B. & G.Y. (30 colonies), the ward of surgery (24 colonies), delivery-waiting room (11 colonies), and the delivery room (3 colonies). 2. The bacterial growth beforenoon differed from that of afternoon. Namely, the latter (24 colonies) was higher than the former (21 colonies). 3. The type of strains isolated from the air of the ward revealed staphylococci (82%), Gram negative bacilli (18%), fungi (17%), Gram positive diplococci (13%), and Bacillus subtilis (2.8%). 4. The strains isolated in the delivery-waiting room revealed staphylococci (66.7%), Gram negative bacilli (33.6%), and revealed staphylococci (75%), Gram positive diplococci (8.3%), and fungi (8.3%), in delivery room. 5. Most of strains isolated in the ward of O.B. & G.Y. revealed staphylococci (100.0%), Gram positive diplococci (8.3%), and Gram negative bacilli (6.7%). 6. The strain isolated in the surgical ward revealed staphylococci (91.7%), fungi (33.3%), Gram positive diplococci (25%), Gram negative bacilli (25%) and Bacillus subtilis (8.3%). 7. The strain isolated in the pediatric ward revealed staphylococci (75%), fungi (25%), Gram positive diplococci (8.3%), Bacillus subtilis (8.3%), and Gram negative bacilli (8.3%). 8. The strain isolated in the internal ward revealed staphylococci (91.7%), fungi (33.3%), Gram positive diplococci (25%), and negative bacilli (16.7%). The strains isolated from the nasal cavity of those doctors and nurses who and enaged in the ward of O.B. & G.Y. revealed staphylococci (80%), Bacillus subtilis (10%), and Gram negative bacilli (10%), from doctors and Gram positive diplococci (10%), instead of Gram negative bacilli (10%), from nurses. 10. The strain isolated from nasal cavity of parturient women on admission revealed staphylococci (90%), and Gram negative bacilli (10%), but after admission revealed staphylococci (70%), Gram positive diplococci (10%), and Gram negative bacilli (10%). 11. Of the total 91 staphylococci isolated from the air of the ward, the Coagulase pastive was 36 (39.6%), and the negative 55 (60.4%), As a result of the coagulase experiment of the staphylococci isolated from the nasal cavity of those who are engaged in the ward of O.B. & G.Y. all were revealed as negative that belonged to non-pathogenic. 12. Consequence of the biochemic examination of the gram negative bacilli isolated from the air of the ward the aerobacter aerogens revealed was (16.7%) E-coli 5% in the nasal cavity of those came and went to the of O.B. & G.Y. and Aerobacter aerogens 7.5%.

Utilization Status of Emergency Medical Service for Children (아동의 응급의료서비스 이용실태)

  • Purpose: This study was done to describe utilization status of emergency medical service for children at one university affiliated hospital located in Seoul. Data were obtained from the medical records of patients under 13 years of age who visited the ER from January 1 to December 31, 2006. Method: Medical records missing the time of discharge were excluded in the analysis of waiting time, which resulted in 19,766 cases. Data were analyzed using SPSS WIN 14.0 version. Result: There were slightly more boys (58.4%), average age of the children was 3.97 years of age. More children at the aged 1 to 3 years (51.3%) visited the ER. Fever was the most frequent complaint: 5,180 cases (24.38%). The other complaints were head or facial laceration (10.55%), vomiting (9.63%), abdominal pain (8.06%), cough (7.67%), and painful limb swelling (6.34%). Average waiting time before the first medical examination was 17 minutes, and average ER stay time was 3 hours and 23 minutes. Conclusion: The results suggest the need to assign a nurse specialist for pediatric ER to provide more efficient care for the children. Also, extra staff assignment during the evening shift or extending office hours of local pediatricians should be considered.

Rehabilitative goals for patients undergoing lung retransplantation

  • Lung retransplantation (LRT) involves a second or subsequent lung transplant (LT) in a patient whose first transplanted graft has failed. LRT is the only treatment option for irreversible lung allograft failure caused by acute graft failure, chronic lung allograft dysfunction, or postoperative complications of bronchial anastomosis. Prehabilitation (rehabilitation before LT), while patients are on the waiting list, is recognized as an essential component of the therapeutic regimen and should be offered throughout the waiting period from the moment of listing until transplantation. LRT is particularly fraught with challenges, and prehabilitation to reduce frailty is one of the few opportunities to address modifiable risk factors (such as functional and motor impairments) in a patient population in which there is clearly room to improve outcomes. Although rehabilitative outcomes and quality of life in patients receiving or awaiting LT have gained increased interest, there is a paucity of data on rehabilitation in patients undergoing LRT. Frailty is one of the few modifiable risk factors of retransplantation that is potentially preventable. As such, it is imperative that professionals involved in the field of retransplantation conduct research specifically exploring rehabilitative techniques and outcomes of value for patients receiving LRT, because this area remains unexplored.

  • Railway construction in the random vibration natural phenomena, as well as a relatively regular train loads for dynamic loads, such as a usability and safety should be ensured. Vibration due to train loads and seismic vibrations caused by wind compared to the typically very small in size, rather than the safety of the structure affects the usability. Recently in the downtown area, ground and underground facilities, such as a permanent facility that may cause excessive vibration increases, associated with the construction of these transportation facilities on ground vibrations of structures has been increasing concern and complaint. More recently, high-speed train vibration and noise due to furnace is increasing. In order to solve this problem, such as soundproof considering several feet, but by applying the vibration and noise reduction measures insufficient for the study is Free. In this study, track structure, track, and the inside of the building to support the system, the different forms of neurological history and share about the history cheonanahsan high-speed rail, if passed by the bus stop on the train loads of noise, and the history of interior noise and vibration measurement / analysis of measurement results to assess the relative comparison with the relevant provisions were reviewed. Based on this history, future plans for the design of the bridge to reflect the results of a study is intended to provide information.   • Recently, the Korean medical system changed with diversification & specialization and the field of medical service is also specialized in these days. And healing environment is proposed as a general concept in the hospital architecture. There is a trend that a establishment of women's hospital is increasing as specialized type. In difference from general hospital, they are providing specialized and integrated services in response to the women's disease and demand. Particularly, Outpatient department space in women's hospital has been changed in terms of area, shape and program. However, there used to be not enough research efforts of responding these changes. This study includes the Outpatient department of s women's hospital in the form of small and medium sized building over a certain level in Korea. And the methods of the study adopted the theoric examination based concept and features of women's hospital, the abstraction of distinctions in the Outpatient department through the analysis of the drawing, and the analysis of distinctions in the Outpatient department of women's hospital with the structural space classified and the analysis of space relation. Therefore, the purpose of this is to study investigates and analyzes the spatial characteristics of the Outpatient department space in women's hospital through a analysis of case study. To conclude, the space composition can be classified into the entry space, the clinic space for patients, the support space of patients' check-up, the reception and waiting space, the rest and convenience space, and the link space in the Outpatient department of women's hospital. The level space can account for the waiting space type, the clinic office type, the type based on the relation between the waiting room and the clinic office with its features. As a result, the results will be applied for the planning & design for the future women's hospital.
