Low-Tube-Voltage CT Urography Using Low-Concentration-Iodine Contrast Media and Iterative Reconstruction: A Multi-Institutional Randomized Controlled Trial for Comparison with Conventional CT Urography

  • Objective: To compare the image quality of low-tube-voltage and low-iodine-concentration-contrast-medium (LVLC) computed tomography urography (CTU) with iterative reconstruction (IR) with that of conventional CTU. Materials and Methods: This prospective, multi-institutional, randomized controlled trial was performed at 16 hospitals using CT scanners from various vendors. Patients were randomly assigned to the following groups: 1) the LVLC-CTU (80 kVp and 240 mgI/mL) with IR group and 2) the conventional CTU (120 kVp and 350 mgI/mL) with filtered-back projection group. The overall diagnostic acceptability, sharpness, and noise were assessed. Additionally, the mean attenuation, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and figure of merit (FOM) in the urinary tract were evaluated. Results: The study included 299 patients (LVLC-CTU group: 150 patients; conventional CTU group: 149 patients). The LVLC-CTU group had a significantly lower effective radiation dose ($5.73{\pm}4.04$ vs. $8.43{\pm}4.38mSv$) compared to the conventional CTU group. LVLC-CTU showed at least standard diagnostic acceptability (score ${\geq}3$), but it was non-inferior when compared to conventional CTU. The mean attenuation value, mean SNR, CNR, and FOM in all pre-defined segments of the urinary tract were significantly higher in the LVLC-CTU group than in the conventional CTU group. Conclusion: The diagnostic acceptability and quantitative image quality of LVLC-CTU with IR are not inferior to those of conventional CTU. Additionally, LVLC-CTU with IR is beneficial because both radiation exposure and total iodine load are reduced.

  • Shear modulus (or rigidity) of dam material is an important parameter which can be directly associated with the deformation of dam. Seepage or leakage of water can cause the defects or cracks of dam body. The existence of cracks and rigidity of dam body are decisive information for the estimation of dam safety. Rigidity of material is mainly determined from S-wave velocity and the defects of dam body can be detected by seismic reflection survey. Therefore, seismic reflection survey will be a desirable method which can give a solution about dam safety problem. Among various physical properties of dam body, S-wave velocity is the most important information but it is not easy to get the information. In this study, diverse measuring techniques of S-wave reflection survey were attempted to get the information about S-wave velocity of dam body. Ultimately, S-wave velocity could be estimated by the analysis of SH reflection events which can be easily observed in shot gather data obtained from SH measuring technique. Meanwhile, P-wave reflection survey was also performed at the same profile. P-beam radiation technique which can reduce the surface waves and reinforce the P-wave reflection events was applied for giving a help to analyse P-wave velocity. In the end, P-and S-wave velocity, Vs/Vp, Poisson's ratio distribution of the vertical section under the profile could be acquired.

Data Analysis of Suspension P-S Velocity Logging in Banded Gneiss Area around Hanam, Gyeonggi Province (경기도 하남시 인근 호상편마암 지역에서 Suspension P-S 속도검층 자료분석)

  • In this paper, dynamic elastic module of banded gneiss were calculated on the basis of SPS velocity logging data obtained from a geotechnical test-hole in Pungsan-dong, Hanam, Gyeonggi Province, Korea. This study mainly focuses on the velocity analysis, Q factor calculation relative to attenuation factor, and generation of crack information and its relation with seismic velocity. As a result, P-wave and S-wave velocity of fresh hard rock was 5,559m/s and 3,063m/s, respectively, with Poisson's ratio being 0.28. With these results, dynamic modules were prepared, and crack information analyzed by acoustic televiewer was incorporated to identify the correlation among and between delay of first arrival by crack amplitude ratio, and velocity. The results of this study revealed that the analyzed logging hole mainly consisted of micro crack and a number of cracks and the size of crack aperture, functioned as a variable to seismic velocity in the micro crack area of this type of hard rock.

Fructus Amomi Cardamomi Extract Inhibits Coxsackievirus-B3 Induced Myocarditis in a Murine Myocarditis Model

  • Coxsackievirus B3 (CVB3) is the main cause of acute myocarditis and dilated cardiomyopathy. Plant extracts are considered as useful materials to develop new antiviral drugs. We had previously selected candidate plant extracts, which showed anti-inflammatory effects. We examined the antiviral effects by using a HeLa cell survival assay. Among these extracts, we chose the Amomi Cardamomi (Amomi) extract, which showed strong antiviral effect and preserved cell survival in CVB3 infection. We investigated the mechanisms underlying the ability of Amomi extract to inhibit CVB3 infection and replication. HeLa cells were infected by CVB3 with or without Amomi extract. Erk and Akt activities, and their correlation with virus replication were observed. Live virus titers in cell supernatants and viral positive- and negative-strand RNA amplification were measured. Amomi extract significantly increased HeLa cell survival in different concentrations ($100-10{\mu}g/ml$). CVB3 capsid protein VP1 expression (76%) and viral protease 2A-induced eIF4G1 cleavage (70%) were significantly decreased in Amomi extract ($100{\mu}g/ml$) treated cells. The levels of positive- (20%) and negative-strand (80%) RNA were dramatically decreased compared with the control, as revealed by reverse transcription-PCR. In addition, Amomi extract improved mice survival (51% vs 26%) and dramatically reduced heart inflammation in a CVB3-induced myocarditis mouse model. These results suggested that Amomi extract significantly inhibited Enterovirus replication and myocarditis damage. Amomi may be developed as a therapeutic drug for Enterovirus.

A Study on the Preliminary 3-D Structure Model around East Sea and Its Vicinity

  • 본 연구는 ILP(International Lithosphere Project) Task Group II-4가 진행하고 있는 상부맨틀에 대한 3차원 구조도 작성 연구의 일환으로 수행되어졌으며 구조도 작성을 위한 데이터 베이스의 구조는 task group의 표준안을 따랐다. 기존 문헌과 기존의 데이터 베이스를 통해서 획득된 자료를 이용해 동해와 그 주변을 대상으로 하는 지역의 ($32-45^{\circ}$E, $122-148^{\circ}$N) 상부 670km까지의 3차원 구조도 작성을 위한 초기 모델을 구축하였으며, 이 절차를 최대한 자동화하는 프로그램을 포트란을 이용해 만들어보았다. 연구 지역에 대한 곡율을 계산하기 위해 표준타원체 모델인 WGS84과 geoid undulation 모델인 EGM96을 사용했으며 지형 고도 자료는 GTOPO30, GLOBE 1.0, 그리고 Smith and Sandwell 데이터베이스를 사용하여 지구 중심으로부터 지표까지의 거리를 구하였다. 연구지역은 $0.25^{\circ}$간격으로 나누었으며 총 5777개의 격자점을 정의하였으며 각각의 격자점에 1차원 수직구조를 부여함으로써 3차원 모델을 구축하였다. 그리고 지형적으로나 지질학적으로 유사한 지역을 하나의 구역으로 정의하고 동일한 수직구조를 부여함으로써 모든 격자점에 1차원 수직구조를 정의하지 않도록 하였다. 본 연구에서는 지표 지질은 모델에 고려하지 않았지만 지형학적으로 의미가 있는 분지나 수평적으로 불균질성이 뚜렷한 지역을 중심으로 연구 지역의 리젼을 정의하였다. 중요 리젼에 대한 지각구조에 대해서는 기존의 문헌을 통해 모델치를 정의하였으며 지각 하부부터 상부 670km에 대한 부분은 Task Group에서 제시한 표준 모델을 이용했다. 모델을 정의하기 위해 주어진 격자점에 대한 지구 중심으로부터 지오이드까지의 거리, 지오이드로부터 지표까지의 거리를 정의해주었으며, 각 격자점의 수직구조를 정의하기 위해 깊이에 따른 각 매질의 밀도, P파의 속도, S파의 속도, P파에 대한 Q값, S파에 대한 Q값을 정의 해주었다. S파의 속도를 구하기 위해서 지구 내부 물질을 포아송 매질이라는 가정 하에, 관계식을 $Vp{\;}={\;}SQRT(3){\;}{\times}{\;}Vs$ 이용하였다. 획득한 모델치들을 이용해 동해와 동해 인근 지역에 대한 초기모델을 구축하였다.

Quantitative Analysis of Bone Mineral Measurements in Different Types of Dual-energy Absorptiometry Systems: Comparison of CT vs DEXA (이중 에너지 조사 방식의 장비별 골밀도 측정의 정량적 비교 분석: CT vs DEXA 비교)

  • Generally assessing bone mineral density (BMD) were performed on dual energy X-ray absorptiometry (DEXA) the same as dual energy CT (DECT) with a rapid-kVp switching. The purpose of this study is to compare the different of BMD value between DEXA and DECT method, and evaluate usefulness of DECT method. Using scanner for BMD measurements were GE, Healthcare Discovery 750 HD for DECT and Hologic QDR 4500W for DEXA. For compare BMD value in each method, scanned lumbar spine phantom and subjects visiting Korean National Cancer Center from April 2015 to December 2015, records of 50 patients. This study was approved by the Institutional Review Board. The mean BMD value measures for spine phantom and for subjects in each scanners presented strong correlation (r=0.948 with p<0.05 for phantom; r=0.635 with p<0.05 and Kendall's tau $({\tau})=0.46$ with p<0.05 for subjects) and linear relationship between DECT and conventional DEXA. DECT technique for BMD measurement will provide a very useful methodology without additional radiation dose.

Seismic Data Processing and Inversion for Characterization of CO2 Storage Prospect in Ulleung Basin, East Sea (동해 울릉분지 CO2 저장소 특성 분석을 위한 탄성파 자료처리 및 역산)

  • $CO_2$ geological storage plays an important role in reduction of greenhouse gas emissions, but there is a lack of research for CCS demonstration. To achieve the goal of CCS, storing $CO_2$ safely and permanently in underground geological formations, it is essential to understand the characteristics of them, such as total storage capacity, stability, etc. and establish an injection strategy. We perform the impedance inversion for the seismic data acquired from the Ulleung Basin in 2012. To review the possibility of $CO_2$ storage, we also construct porosity models and extract attributes of the prospects from the seismic data. To improve the quality of seismic data, amplitude preserved processing methods, SWD(Shallow Water Demultiple), SRME(Surface Related Multiple Elimination) and Radon Demultiple, are applied. Three well log data are also analysed, and the log correlations of each well are 0.648, 0.574 and 0.342, respectively. All wells are used in building the low-frequency model to generate more robust initial model. Simultaneous pre-stack inversion is performed on all of the 2D profiles and inverted P-impedance, S-impedance and Vp/Vs ratio are generated from the inversion process. With the porosity profiles generated from the seismic inversion process, the porous and non-porous zones can be identified for the purpose of the $CO_2$ sequestration initiative. More detailed characterization of the geological storage and the simulation of $CO_2$ migration might be an essential for the CCS demonstration.

Analysis of the Imaging Dose for IGRT/Gated Treatments (영상유도 및 호흡동조 방사선치료에서의 영상장비에 의한 흡수선량 분석)

  • Purpose: The introduction of image guided radiation therapy/four-dimensional radiation therapy (IGRT/4DRT) potentially increases the accumulated dose to patients from imaging and verification processes as compared to conventional practice. It is therefore essential to investigate the level of the imaging dose to patients when IGRT/4DRT devices are installed. The imaging dose level was monitored and was compared with the use of pre-IGRT practice. Materials and Methods: A four-dimensional CT (4DCT) unit (GE, Ultra Light Speed 16), a simulator (Varian Acuity) and Varian IX unit with an on-board imager (OBI) and cone beam CT (CBCT) were installed. The surface doses to a RANDO phantom (The Phantom Laboratory, Salem, NY USA) were measured with the newly installed devices and with pre-existing devices including a single slice CT scanner (GE, Light Speed), a simulator (Varian Ximatron) and L-gram linear accelerator (Varian, 2100C Linac). The surface doses were measured using thermo luminescent dosimeters (TLDs) at eight sites-the brain, eye, thyroid, chest, abdomen, ovary, prostate and pelvis. Results: Compared to imaging with the use of single slice non-gated CT, the use of 4DCT imaging increased the dose to the chest and abdomen approximately ten-fold ($1.74{\pm}0.34$ cGy versus $23.23{\pm}3.67$cGy). Imaging doses with the use of the Acuity simulator were smaller than doses with the use of the Ximatron simulator, which were $0.91{\pm}0.89$ cGy versus $6.77{\pm}3.56$ cGy, respectively. The dose with the use of the electronic portal imaging device (EPID; Varian IX unit) was approximately 50% of the dose with the use of the L-gram linear accelerator ($1.83{\pm}0.36$ cGy versus $3.80{\pm}1.67$ cGy). The dose from the OBI for fluoroscopy and low-dose mode CBCT were $0.97{\pm}0.34$ cGy and $2.3{\pm}0.67$ cGy, respectively. Conclusion: The use of 4DCT is the major source of an increase of the radiation (imaging) dose to patients. OBI and CBCT doses were small, but the accumulated dose associated with everyday verification need to be considered.

Determination of Appropriate Exposure Angles for the Reverse Water's View using a Head Phantom (두부 팬텀을 이용한 Reverse Water's View에 관한 적절한 촬영 각도 분석)

  • Early diagnosis for upper facial trauma is difficult by using the standard Water's view (S-Water's) in general radiograph due to overlapping of anatomical structures, the uncertainty of patient positioning, and specific patients with obese, pediatric, old, or high-risk. The purpose of this study was to analyze appropriate exposure angles through a comparison of two different protocols (S-Water's vs. reverse Water's view (R-Water's)) by using a head phantom. A head phantom and general radiograph with 75 kVp, 400 mA, 45 ms 18 mAs, and SID 100 cm. Images of R-Water's were obtained by different angles in the range of $0^{\circ}$ to $50^{\circ}$, which adjusted an angle at 1 degree interval in supine position. Survey elements were developed and three observers were evaluated with four elements including the maxillary sinus, zygomatic arch, petrous ridge, and image distortion. Statistical significant analysis were used the Krippendorff's alpha and Fleiss' kappa. The intra-class correlation (ICC) coefficient for three observers were high with maxillary, 0.957 (0.903, 0.995); zygomatic arch, 0.939 (0.866, 0.987); petrous ridge, 0.972 (0.897, 1.000); and image distortion, 0.949 (0.830, 1.000). The high-quality image (HI) and perfect agreement (PA) for acquired exposure angles were high in range of the maxillary sinus ($36^{\circ}-44^{\circ}C$), zygomatic arch ($33^{\circ}-40^{\circ}$), petrous ridge ($32^{\circ}-50^{\circ}$), and image distortion ($44^{\circ}-50^{\circ}$). Consequently, an appropriate exposure angles for the R-Water's view in the supine position for patients with facial trauma are in the from $36^{\circ}$ to $40^{\circ}$ in this phantom study. The results of this study will be helpful for the rapid diagnosis of facial fractures by simple radiography.

Die Korrelation zwischen der DG und GB-Theorie (의존문법과 생성문법 간의 상호관계)

  • Das Ziel dieses Aufsatzes liegt darin, auf Grund der Theorien von einigen Grammatikern die Korrelation zwischen der Dependenzgrammatik (=DG) und GB­Theorie(=GB) zu beschreiben. Nach Eroms(2000) sind VPn in eine engere Phrase, die die $Erg\"{a}nzungen$ in obliquen Kasus bindet, und eine $\"{a}u{\ss}ere$, die Subjektsvalenz abbindet, zu trennen. Dem INFL-Phrasenkopf der GB entspricht das Finitheitsmorphem in der DG. In der $herk\"{o}mmlichen$ DG steht das Verb an der Spitze des Satzes, aber Eroms(2000) stellt S an die Satzspitze. Die S(mit S., S?, S!) sind $n\"{a}mlich$ oberstes Element des Satzes, von dem alle anderen $W\"{o}rter\;abh\"{a}ngig$ sind. Die Konjunktionen regieren ihren Satz, bzw. der Satz wird durch sie gesteuert. Damit ist die Analogie zur X-bar-Syntax $vollst\"{a}ndig$. Nach Vater(l996) lasst sich die Struktur der deutschen VP systernatisch nach dem X-bar-Schema darstellen, wobei die Zuordnung der Objekte mehrere $M\"{o}glichkeiten\;zul\"{a}sst$. X-bar-Regeln allein $k\"{o}nnen$ nicht die Vielfalt $m\"{o}glicher$ VP-Strukturen erzeugen. Hier greifen V-bar-Regeln und Valenz ineinander. Vater(l996) $schl\"{a}gt$ vor, den von $\'{A}gel(1993)$ vorgeschlagenen Unterschied zwischen Valenzpotenz und Valenzrealisierung zu $ber\"{u}cksichtigen$, um die verschiedenen syntaktischen und morphologischen Realisierungen zwischen verschiedenen Sprachen in den Griff zu bekommen. $\'{A}gel(1993, 2000)$ nimmt in Anlehnung an $L\'{a}szlo(l988)$ zwei Realisierungsebenen der Valenz, die Mikroebene(=die der morphologischen Aktanten) und die Makroebene(=die der syntaktischen Aktanten) an. Der $Tr\"{a}ger$ auf Mikroebene ist der Mikroaktant, also der Teil des Verbflexivs, der $Tr\"{a}ger$ auf Makroebene ist der Makroaktant, also das syntaktische Subjekt. Die Begriffe 'Mikroebene' und 'Makroebene' werden sowohl statisch und als auch dynamisch interpretiert. Auf Grund dieser Begriffe versucht $\'{A}gel(1993, 2000)$, nach Parallelen der Valenzrealisierung in S und NP zu suchen. Aber die Untersuchung der $Valenzrealisierungsverh\"{a}ltnisse$ in der NP wird zu einem scheinbaren typologischen Gegensatz zwischen S und NP $f\"{u}hren$. Um diesen Gegensatz $aufzul\"{o}sen$, wird das Konzept 'finites Substantiv' $eingef\"{u}hrt$, das analog zum Konzept 'finites Verb' ist. Dabei wird die sog. starke Adjektivflexion in einen Teil der Substantivflexion uminterpretiert. Die GB-Theorie definiert sich als eine Theorie der sog. mentalen Grammatik, des $Sprachverm\"{o}gens$. Da als der Gegenstand der DG die sog. objektive Grammatik angesehen werden kann, scheint der grundlegende Unterschied zwischen den $Gegenst\"{a}nden$ beider Theorien in der Opposition 'intem(=mental vs. extern (=objektiv)' zu liegen. Da die GB eine 'naturwissenschaftlicher' organisierte Theorie ist als die empirische Richtung der DG, ware eine Vergleichsgrundlage in der Tat nicht einfach zu etablieren. Obwohl die beiden Theorien von $v\"{o}llig$ anderen Voraussetzungen und Zielsetzungen ausgehen, gibt es zwischen der DG und der GB $\"{U}berlappungen$ und zunehmende $Ann\"{a}herungen$. Generell ist es notwendig, dass Teileinsichten aus einer Theorie in eine andere Theorie $\"{u}bernommen$ werden. In diesem Aufsatz konnten wir durch die Untersuchung der Korrelation von beiden Theorien partielle Konvergenzen erkennen. Nach Engel(l994) gibt es keine Grammatik 'an sich', sie wird von Grammatikern gemacht. Grammatiken sind Menschenwerk und Linguistenwerk. Wir sind auf der Suche nach 'einer besseren Grammatik.'
