두경부 세기견조방사선치료계획 최적화 조건에서 주요 인자들의 영향 분석 (Analysis of the major factors of influence on the conditions of the Intensity Modulated Radiation Therapy planning optimization in Head and Neck)

  • 목 적 : 최적화 알고리즘에 적용되는 최적화 인자들의 영향을 고려하여, 가장 적합한 인자 값을 도출함으로써 이상적인 치료계획을 쉽게 설계할 수 있도록 하고자 한다. 대상 및 방법 : 본 연구의 세기조절방사선치료에서 선량계산 알고리즘은 PBC(Pencil Beam Convolution)이고, 최적화 알고리즘은 DVO(Dose Volume Optimizer 10.0.28)이다. 두경부 환자의 세기조절방사선치료에서 치료계획용적의 처방선량은 동시에 2.2 Gy와 2.0 Gy가 될 수 있도록 하였다. 치료계획은 6 MV, 7개의 조사야로 역선량계산방법으로 수립하였다. 최적화 알고리즘 인자는 용적선량-조건강도(Priority, Constrain), 선량부 드럼강도(Smooth)로 선정하고, 각 인자들의 변화량에 따른 치료계획의 영향을 분석하였다. 용적선량-조건강도는 기준 조건강도를 정하고, 비율은 같지만 절대 값은 다른 최적화 과정을 실시하였다. 또한 조건강도의 절대 값에 변화에 따른 치료용적과 주변 정상장기들을 평가하였다. 선량부드럼강도는 기준 조건의 단순 변화와 용적선량-조건강도와 관련시킨 변화를 치료계획에 반영시켰다. 치료계획은 처방선량지수(Conformal Index, CI), 처방선량포함지수(Paddick's Conformal Index, PCI), 선량균질지수(Homogeneity Index, HI)와 각 장기의 평균선량으로 평가하였다. 결 과 : 용적선량-조건강도의 비율을 동일하게 하고 절대 값을 변화 시켰을 때 CI값은 다르지만, PCI는 $1.299{\pm}0.006$, HI는 $1.095{\pm}0.004$, D5%/D95%는 $1.090{\pm}1.011$으로 처방선량에 대한 영향은 유사하였다. 이하선의 평균선량은 용적선량-조건강도의 절대 값이 40, 60, 70, 90으로 증가될 때, 67.4, 50.3, 51.2, 47.1 Gy로 감소하였다. 각각의 치료계획에서 선량부드럼강도를 증가시켰을 때, PCI는 $1.338{\pm}0.006$로 증가된 값을 보였다. 결 론 : 용적선량-조건강도는 절대적인 값보다 각 조건의 비율에 따라 최적화 알고리즘에 영향을 주었다. 절대 값이 다르더라도 같은 비율을 유지하면 유사한 치료계획이 수립되었다. 성공적인 치료계획을 수립하기 위해 특히 보호해야할 정상장기의 용적선량-조건강도는 치료용적의 용적선량-조건강도의 50%이상 되어야한다. 선량부드럼강도는 용적선량-조건강도에 따라 비례하여 증가하거나 감소하여야 한다. 단순히 절대 값으로 적용하면 용적선량-조건강도는 그 조건을 충분히 만족시키지 못한다.

토모테라피를 이용한 간암환자의 정위적 방사선치료시 복부압박장치의 유용성 평가 (Usefulness of Abdominal Compressor Using Stereotactic Body Radiotherapy with Hepatocellular Carcinoma Patients)

  • 목 적: 간에 발생한 절제 불가능한 원발성 및 전이성 종양에서 토모테라피를 이용한 정위적 방사선치료를 시행하기 위해 사용된 복부압박장치의 유용성을 평가하고자 하였다. 대상 및 방법: 2011년 11월부터 2012년 3월까지 토모테라피(Hi-Art Tomotherapy, USA)를 시행받기 위해 본원에 내원한 간암환자 중 복부압박장치(diaphragm control, elekta, sweden)를 사용하였을 때 움직임이 1 cm 이상 줄어든 환자를 대상으로 하였다. 4D CT (somatom sensation, siemens, germany)를 통해 치료계획영상과 4차원 단층촬영영상을 촬영하고, 육안적으로 보이는 종양과 종양의 움직임을 고려하여 육안적 종양체적(gross tumor volume, GTV)으로 설정하였고, GTV 주변으로 균일하게 5~7 mm의 여유를 주어 치료계획용 종양체적(planning target volume, PTV)으로 설정하였다. 손상위험장기(organs at risk) 중 십이지장, 위, 대장의 거리가 종양으로부터 최소 1 cm 이상인 환자들을 1군($d{\geq}1$)으로, 1 cm 미만인 환자들을 2군(d<1)으로 분류하고 각각 4~5회의 정위적 방사선치료와 20회의 방사선치료를 계획하였다. Mega-voltage computed tomograpy (MVCT)와 kilo-voltage computed tomograpy (KVCT)를 일차적으로 골격구조셋팅(bone-technique)으로 융합(fusion)한 뒤, 이차적으로 간을 보며 영상을 재조정 하였다. 치료 후 얻은 MVCT 영상을 영상변형이 가능한 Mim_vista (Mimsoftware, ver. 5.4 USA)로 보내고, 간을 비교하여 다시 묘사(delineate)하고, 손상위험장기 중 십이지장, 위, 소장, 대장을 합쳐서 대장장기(bowel_organ)로 정의하여 다시 묘사하였다. 보정방사선 치료계획시스템을 통하여 보정영상의 치료계획 선량과 보정된 선량의 차이를 평가하였다. 첫 번째, 치료 시작일부터 각각 1군($d{\geq}1$)은 4회, 2군(d<1)은 10회까지의 MVCT와 KVCT간의 영상 융합을 통한 셋업오차를 분석하였다. 두 번째, 보정영상에서 종양 즉, GTV, PTV의 치료계획선량과 보정선량의 3%의 선량차이를 나타내는 체적($V_{diff3%}$)과 5%의 선량차이를 나타내는 체적($V_{diff3%}$)을 비교하였고, 손상위험장기 중 대장장기의 최대선량의 차이율을 비교하였다. 결 과: MVCT를 통해 분석한 평균 셋업오차는 $-0.66{\pm}1.53$ mm (좌-우), $0.39{\pm}4.17$ mm (상-하), $0.71{\pm}1.74$ mm (전-후), $-0.18{\pm}0.30$ degrees (roll)였다. 1군($d{\geq}1$)과 2군(d<1)의 셋업오차는 유사하였다. 1군($d{\geq}1$)에서 보정방사선 치료계획을 통한 $V_{diff3%}$ 중 GTV는 $0.78{\pm}0.05%$, PTV는 $9.97{\pm}3.62%$, $V_{diff5%}$ 중 GTV는 0.0%, PTV는 $2.9{\pm}0.95%$, 대장장기의 최대선량의 차이율은 $-6.85{\pm}1.11%$였다. 2군(d<1)에서 $V_{diff3%}$ 중 GTV는 $1.62{\pm}0.55%$, PTV는 $8.61{\pm}2.01%$, $V_{diff5%}$ 중 GTV는 0.0%, PTV는 $5.33{\pm}2.32%$, 대장장기의 최대선량의 차이율은 $28.33{\pm}24.41%$였다. 결 론: 복부압박장치를 통한 간암의 방사선치료시 MVCT를 통한 환자 셋업오차는 평균 ${\pm}5$ mm 이하였고, 복부 압박장치를 사용하고 투시영상을 통해 확인한 횡경막의 움직임이 최소 5 mm 이상이라는 것을 감안하면, 환자 셋업오차는 그 안에 있음을 알 수 있었다. 1군($d{\geq}1$)과 2군(d<1)에서 GTV, PTV의 선량차이율은 오차범위 안에 있었고, 1군($d{\geq}1$)과 2군(d<1)의 대장장기 최대선량의 차이율은 최대 35% 이상의 차이를 보였고, 1군($d{\geq}1$)이 2군(d<1)보다 오차범위가 작았다. 따라서 간내 종양과 손상위험장기의 거리가 최소 1 cm 이상 유지된다면 정위적 방사선치료를 진행함에 복부압박장치가 도움이 될 수 있을 것으로 사료된다.

진사탁(陳士鐸) 임상 이론의 특징에 관한 연구 (A Study on Characteristics of Jinsatak(陳士鐸)'s Clinic Theory)

  • The characteristics of Jin's ideas on clinic theory can be arranged as follows. 1. Jin emphasized warming and tonifying[溫補] in treatment and the part that shows this the best is the taking care of[調理] the Vital gate[命門], kidney, liver, and spleen. His ideas were based on his understanding of a human life's origin, and was influenced by Seolgi(薛己), Joheon-ga(趙獻可) and Janggaebin(張介賓)'s Vital gate and source Gi theory(元氣說) so scholastically, he has that in common with them but was later criticized by later doctors such as Oksamjon(玉三尊) as an 'literary doctor(文字醫)' who followed the ideas of "Uigwan(醫貫)". 2. The warming and tonifying school[溫補學派], who were influenced by Taoism, said in their theory of disease outbreak[發病學說] that since one must not hurt one's Yin essence and Yang fire [陰精陽火] there is more deficiency than excess, so that was why they used tonifying methods. Jin was also like them and this point of view is universal in internal medicine, gynecology, pediatric medicine and surgery and so on. 3. Jin, who saw the negative form of pulse diagnosis[診脈] emphasized following symptoms over pulse diagnosis using the spirit of ‘finding truth based on truth[實事求是]' in "Maekgyeolcheonmi(脈訣闡微)", but emphasized 'the combination of pulse and symptoms[脈證合參]'. He understood pulse diagnosis as a defining tool for symptoms, and in "Seoksilbirok(石室秘錄)" simplified pulse diagnosis into 10 methods : floating/sunken(浮沉), slow/fast(遲數), large/fine(大小), vacuous/replete(虛實) and slippery/rough(滑澀). 4. Jin used 'large formulas(大方)' a lot that usually featured a large dose, and in " Bonchosinpyeon(本草新編)" he thought of the seven formulas(七方) and ten preparations(十劑) as the standard when using medicine. He did away with old customs and presented a 'new(新)' and 'extra(奇)' point of view. He especially used a lot of Insam(人蔘) when tonifying Gi and Geumeunhwa(金銀花) when treating sores and ulcers. 5. In the area of surgery Jin gave priority to the early finding and treatment of disease with internal treatment[內治] and was against the overuse of acupuncture. However records of surgical measures in a special situation like lung abscesses(肺癰) and liver abscesses(肝癰), and anesthetic measures using 'Manghyeongju(忘形酒)' and 'Singoiyak(神膏異藥)' and opening the abdomen or skull, and organ transplants using a dog's tongue are important data. 6. Jin stated the diseases of Gi and blood broadly. Especially in the principles of treating blood, blood diseases had to be forwarded[順] and Gi regulation[理氣] was the number one priority and stated the following two treatments. First, in "Jeonggiinhyeolpyeon(精氣引血篇)" of volume 6 of "Oegyeongmieon(外經微言)", for the rules for treating blood he stated the pattern identification of finding Gi in blood and blood in Gi. Second, he emphasized Gi regulation(理氣) in blood diseases and stated that the Gi must be tonifyed after finding the source of the loss of blood.

Initial Risk Assessment of Acetanilide in OECD High Production Volume Chemical Program

  • In Korea, 2,320 tonnes of acetanilide were mostly wed as intermediates for synthesis in phar-maceuticals or additives in synthesizing hydrogen peroxide, varnishes, polymers and rubber. Only small amount of 120 kg were wed as a stabilizer for hydrogen peroxide solution for hair colouring agents in 1998. Readily available environmental or human exposure data do not exist in Korea at the present time. However, potential human exposures from drinking water, food, ambient water and in work places are expected to be negligible because this chemical is produced in the closed system in only one company in Korea and the processing factory is equipped with local ventilation and air filtering system. Acetanilide could be distributed mainly to water based on EQC model. This substance is readily biodegradable and its bioaccumulation is low. Acute toxicity of acetanilide is low since the L $D_{50}$ of oral exposure in rats is 1,959 mg/kg bw. The chemical is not irritating to skin, but slightly irritating to the eyes of rabbits. horn repeated dose toxicity, the adverse effects in rats were red pulp hyperplasia of spleen, bone marrow hyperplasia of femur and decreased hemoglobin, hematocrit and mean corpuscular hemoglobin concentration. The LOAEL for repeated dose toxicity in rats was 22 mg/kg/day for both sexes. Acetanilide is not considered to be genotoxic. In a reproductive/developmental toxicity study, no treatment-related changes in precoital time and rate of copulation, impregnation, pregnancy were shown in all treated groups. The NOAELs for reproduction and developmental toxicity (off-spring toxicity) are considered to be 200 mg/kg bw/day and 67 mg/kg bw/day, respectively. Ecotoxicity data has been generated in a limited number of aquatic species of algae (72 hr- $E_{b}$ $C_{50}$; 13.5 mg/l), daphnid (48hr-E $C_{50}$ > 100 mg/l) and fish (Oryzias latipes, 96hr-L $C_{50}$; 100 mg/l). Form the acute toxicity values, the predicted no effect concentration (PNEC) of 0.135 mg/1 was derived win an assessment factor of 100. On the basis of these data, acetanilide was suggested as currently of low priority for further post-SIDS work in OECD.in OECD.D.

Comparison of Dosimetrical and Radiobiological Parameters on Three VMAT Techniques for Left-Sided Breast Cancer

  • Purpose: To compare the dosimetrical and radiobiological parameters among various volumetric modulated arc therapy (VMAT) techniques using restricted and continuous arc beams for left-sided breast cancer. Materials and Methods: Ten patients with left-sided breast cancer without regional nodes were retrospectively selected and prescribed the dose of 42.6 Gy in 16 fractions on the planning target volume (PTV). For each patient, three plans were generated using the $Eclipse^{TM}$ system (Varian Medical System, Palo Alto, CA) with one partial arc 1pVMAT, two partial arcs 2pVMAT, and two tangential arcs 2tVMAT. All plans were calculated through anisotropic analytic algorithm and photon optimizer with 6 MV photon beam of $VitalBEAM^{TM}$. The same dose objectives for each plan were used to achieve a fair comparison during optimization. Results: For PTV, dosimetrical parameters such as Homogeneity index, conformity index, and conformal number were superior in 2pVMAT than those in both techniques. $V_{95%}$, which indicates PTV coverage, was 91.86%, 96.60%, and 96.65% for 1pVMAT, 2pVMAT, and 2tVMAT, respectively. In most organs at risk (OARs), 2pVMAT significantly reduced the delivered doses compared with the other techniques, excluding the doses to contralateral lung. For the analysis of radiobiological parameters, a significant difference in normal tissue complication probability was observed in ipsilateral lung while no difference was observed in the other OARs. Conclusions: Our study showed that 2pVMAT had better plan quality and normal tissue sparing than 1pVMAT and 2tVMAT but not for all parameters. Therefore, 2pVMAT could be considered the priority choice for the treatment planning for left breast cancer.

Initial Risk Assessment of Disodium Disulphite in OECD High Production Volume Chemical Program

  • Disodium disulphite, the HPV chemical, was assigned to Korea in order to implement OECD SIDS program in 1999. It was produced about 3,200 ton/year in 1998. This report evaluates the toxic potency of disodium disulphite based on the environmental and mammalian effects as well as human exposure. Oral $LD_{50}$ in rats is 1,540 mg/kg b.w. and effects was observed to the stomach, liver and the GI track that was filled with blood. For repeated dose toxicity, the predominant effect was the induction of stomach lesion due to local irritation. The no observed adverse effect lever for local (stomach irritation) was about 217 mg/kg bw/day. There is no evidence that disodium disulphite is genotoxic in vivo. No reproductive or developmental toxicty of disodium disulphite was observed for the period up to 2 yr and over three generation. In humans, urticaria and asthma with itching, edema, rhinitis, and nasal congestion were reported. Disodium disulphite is unlikely to induce respiratory sensitization but may enhance symptom of asthma in sensitive individuals. This chemical would be mainly transported to water compartment when released to environmental compartments since it is highly water soluble (470 g/l at 20). Low K oc (2.447) indicates disodium disulphite is so mobile in soil that it may not stay in the terrestrial compartment. The chemical has been tested in a limited number of aquatic species. hem acute toxicity test to fish, 96 hr-$LC_{50}$ was > 100 mg/1. For algae, 72 hr-$XC_{50}$ was 48.1 mg/1. For daphnid, the acute toxicity value of 48 hr-$EC_{50}$ was 88.76 mg/1, and chronic value of 21day-NOEC was > 10 mg/1. Therefore, PNEC of 0.1 mg/l for the aquatic organism was obtained from the chronic value of daphnid using the assessment factor of 100. Based on these data the disodium disulphite was recommended as low priority for further post-SIDS work in OECD.
