A case of vocal cord gnathostomiasis diagnosed with sectional morphologies in a histopathological specimen from a Chinese woman living in Korea

  • This study aimed to describe a rare case of gnathostomiasis in the vocal cord. A 54-yearold Chinese woman living in Korea visited with a chief complaint of voice change at the outpatient department of otorhinolaryngology in Hallym Sacred Heart Hospital, Hallym University on August 2, 2021. She had eaten raw conger a few weeks before the voice change developed, but her medical history and physical examinations demonstrated neither gastrointestinal symptoms nor other health problems. A round and red cystic lesion, recognized in the anterior part of the right vocal cord, was removed using forceps and scissors through laryngeal microsurgery. The histopathological specimen of the cyst revealed 3 cross-sections of a nematode larva in the lumen of the cyst wall composed of inflammatory cells and fibrotic tissues. They differ in diameter, from 190 ㎛ to 235 ㎛. They showed characteristic cuticular layers with tegumental spines, somatic muscle layers, and gastrointestinal tracts such as the esophagus and intestine. Notably, intestinal sections consisted of 27-28 lining cells containing 0-4 nuclei per cell. We tentatively identified the nematode larva recovered from the vocal cord cystic lesion as the thirdstage larva of Gnathostoma, probably G. nipponicum or G. hispidum, based on the sectional morphologies.

Voice Analysis and Videostroboscopic Findings before and after Laryngomicrosurgery of Intracordal Cysts (성대낭종환자에서의 후두미세수술전후의 음성언어분석비교)

  • Background and Objectives : Dysphonia may be secondary to many different type of benign vocal cord lesions such as vocal polyp, vocal nodule, Reinke's edema, and intracordal cyst. Diagnosis and treatment of intracordal cysts are more difficult than other benign vocal cord lesions. But postoperative voice analysis of intracordal cyst have rarely been reported in the literature. The purpose of this study is to analyze aerodynamic and acoustic results and videostroboscopic findings before and after laryngomicrosurgery. Materials and Methods : We reviewed the pre and post-operative voice analysis results and videostroboscopic findings of 15 surgically treated patients of intracordal cysts at Severance hospital from Jun. 1997 to Nov. 1999 retrospectively. They were diagnosed with videostroboscopic findings, surgical findings, and pathologic reports. Their pre and post-operative speech were analyzed with MDVP(Multi Dimension Voice Analysis Program) of CSL(Computerized Speech Lab) and Aerophone II. Their pre and post-operative mucosal wave of true vocal cord was analyzed with videostroboscopy. In order to compare this results with normal group, 10 of normal persons were evaluated with same methods. Results : After the operation, mucosal wave of true vocal cord was improved in all patients. Postoperative acoustic and aerodynamic results were improved in almost parameters, but they did not reach the normal value. Conclusions : Videostroboscopy was essential in diagnosing intracordal cysts. By comparing the acoustic and aerodynamic results and video-stroboscopic findings before and after the laryngomicrosurgery, postoperative vocal function was defined more accurately and objectively. Almost parameters may be useful in assessing the quantitative changes in vocal quality before and after the laryngomicrosurgery.

Vocal sulcus accompanying with benign vocal cord disease (양성 성대질환을 동반하는 성대구 (Vocal Sulcus))

  • A retrospective review of 158 patients who had microlaryngeal surgery for benign vocal cord disease, at the Department of Otolaryngology, Kang-Nam St. Mary Hospital, over 2-year period from Mar. 1991 to Feb. 1993, was completed. Of 158 patients, 16 patients(10.1%, male 8 & female 8, most common in fifth decade) had concomitant vocal sulcus unilaterally (10) or bilaterally (6). Vocal sulcus was accompanied with 7.5% (7/93) of vocal polyp, 15.8% (6/38) of vocal nodule, 11.1% (1/9) of Reinke's edema and 8.3% (1/12) of vocal cyst. Vocal sulcus located above the associated disease in 13(59.1%), below the disease in 7(31.8%) and in the lesion in 2(9.0%). Vocal sulcus was found mainly on the midportion of the membranous vocal cord and the length was less than half of membranous vocal cord in 16(72.7%), more than half in 6(27.3%). In 12 cases (54.5%), vocal sulcus was deep enough to attach to the vocal ligament. We thought it is important to consider the concomitant vocal sulcus during the microlaryngeal surgery for benign vocal cord disease.

Squamous Cell Carcinoma of the Larynx Arising in the Intracordal Cyst (성대 낭종 내부에 발생한 후두 편평세포암종)

  • More than half of patients presenting with hoarseness show benign vocal cord changes. However, in rare cases with benign mucosal lesions, it can be diagnosed as a malignant disease after histopathological examination. A 53-year-old man with a 30-pack-year smoking history was admitted for the evaluation of hoarseness, and using a laryngoscope, an enclosed, sac-like cystic lesion was detected on the midpoint of the right true vocal cord. The cystic lesion was deemed to be an intracordal cyst and treated with laryngeal microsurgery. However, pathological findings showed squamous cell carcinoma of the larynx arising in the intracordal cyst, which is exceptionally rare. Therefore, even if a benign lesion is initially suspected, a biopsy must be performed on a patient with smoking history to confirm the diagnosis. In conclusion, we report a case of squamous cell carcinoma of the larynx arising in the intracordal cyst.

Clinical Characteristics of the Intracordal Cysts (성대낭종의 임상적 특성)

  • Background and Objectives : Intracordal cysts may occur secondary to voice abuse and overuse or may be secondary to a remnant of epithelium trapped within the lamina propria. They nay occur spontaneously or may be associated with poor vocal hygiene. As the cyst enlarges it can start to significantly affect the vibratory region of the vocal fold. Recently, with the advancement of the microsurgical technique and the laryngeal stroboscopy, correct diagnosis of intracordal cyst have been increased. The aims of this study is to review the important clinical characteristics of the intracordal cyst. Materials and Method : In the present study, 121 cases of the intracordal cyst were treated by the microsurgical technique. These lesions were diagnosed before the operation with indirect laryngoscopy, laryngeal endoscopy, laryngeal stroboscopy and confirmed with the findings observed during operations and the results of the biopsies. Results : The intracordal cysts were 121 cases in the 2595 patients who underwent laryngeal micorosurgery(4.7%). Ductal cyst were 88 cases and epidermoid cyst were 33 cases. The lesions are more frequent in women and anterior third of true vocal cord is more frequently involved site. With the indirect laryngoscopic examination, the ductal cysts are more frequently misdiagnosed as other diseases of the vocal cord such as vocal polyps or nodules. The degree of postoperative voice satisfaction is similar to that of vocal polyps. Conclusion : Intracordal cysts are very similar to the other mucosal disorders of the vocal cord and it may be misdiagnosed as vocal polyps or nodules, frequently. Therefore careful preoperative examinations for the vocal cord lesions with stroboscopy and other endoscopic instruments are important part of the correct diagnosis. An ideal treatment is enucleation of the cysts without upture of the cystic wall or injury of the lamina propria.

The lesions are more frequent in women and anterior third of true vocal cord is more frequently involved site. With the preoperative laryngoscopic examination, the intracordal cysts were mostly misdiagnosed as other disease of the vocal cord such as vocal polyps or nodules. And main cause of intracordal cysts was thought of vocal abuse. From view of the surgical approach, Ductal cysts was difficult to remove completely than epidermoid cyst without cystic wall rupture. Conclusion : Intracordal cysts are very similar to the other mucosal disorders of the vocal cord and it may be misdiagnosed as vocal polyps or nodules, frequently. Therefore careful preoperative examinations for the vocal cord lesions with stroboscopy and other endoscopic instruments are important part of the correct diagnosis. An ideal treatment is enucleation of the cysts without rupture of the cystic wall or injury of the lamina propria. And marsupialization is meaningful to ductal cyst that cannot be enucleated completely.

  • Primary laryngeal aspergillosis is rare, It is most often found in immunocompromised patient, such as leukemia, malignant disease, diabetes or immunosuppressive drugs. These days the occurrences of laryngeal aspergillosis in immunocompetent patients are increasing. The cause of laryngeal aspergillosis in immunocompetent patients is not clear, but a few factors are considered such as iatrogenic factors, vocal abuse, vocal fold cyst and occupational factors. The histopathologic characteristics are somewhat different between that of immunocompromised patients and immunocompetent patients. We report a case of primary vocal cord aspergillosis in immunocompetent patient who had treated with only surgery and brief review of the pertinent literature.

Clinical Characteristics and Treatment Result of Laryngeal Cysts (후두에 발생한 낭종의 임상양상 및 치료 결과)

  • Background and Objectives: The purposes of this study are to find out clinical causes, clinical characteristics and treatment outcomes in relation to anatomical location of laryngeal cyst. Subjects and Method: A retrospective study of medical records was carried out for 170 patients with cysts on vocal cord, epiglottis, vallecula, arytenoid and aryepiglottic fold. Results: There were 83 cases of epiglottic cysts, 41 cases of vallecular cysts, 35 cases of intracordal cysts, 3 cases of arytenoid cysts and 2 cases of aryepiglottic cysts. Laryngeal cysts were more common in men than in women, and the ratio between men and women was 2:1. The age of patients ranged from 7 to 90 years, with their average age being 52 years. The most common symptom was voice change at intracordal cysts and globus sensation at vallecular and epiglottic cysts. The most common cause of intracordal cysts were voice abuse. But other location of laryngeal cysts doesn't have common causes. The average size of cysts was 0.3cm at vocal cord, 1.43cm at epiglottis, 1.4cm at vallecula, 0.9cm at arytenoid and 1cm at aryepiglottis. Recurrence was observed in 7 cases from 1 months to 18 months following the operation. Size of all recurred cysts was over the average. Conclusion: Physicians should be aware of changes in clinical patterns of laryngeal cysts according to location and have long follow-up period at large cysts after operation.

Change of Acoustic Parameter and Voice Handicap Index after Laryngeal Microsurgery (후두미세수술 후 음향지표의 변화와 환자의 만족도 비교)

  • Background and Object: The aim of this study is to evaluate the change of patient's subjective voice handicap index (VHI) and acoustic parameters before and after laryngeal microsurgery for benign vocal cord disease. Materials and Method: We analyzed 78 patients who received laryngeal microsurgery for benign vocal cord disease from January 2004 to February 2007 retrospectively. There were 28 vocal polyp, 40 vocal nodule, 5 intracordal cyst and 5 Reinke's edema. Jitter, shimmer, harmony to noise ratio (HNR) were analyzed before surgery and 2-3months after surgery using the Doctor's speech science program. The voice handicap index introduced by the Pittsburgh Voice Center was used to examine patient's subjective change of voice quality. Results: Acoustic parameters of jitter, shimmer and HNR were improved in patients with vocal polyp and vocal nodule after surgery. The acoustic parameters were not improved in patients with Reinke's edema, statistically. Only jitter was improved significantly in patients with intracordal cyst (p<0.05). The VHI was significantly improved after surgery. The change of jitter and shimmer was significantly correlated with the change of VHI after surgery. Conclusion: The acoustic parameters and VHI were significantly improved in patients with benign vocal disease after laryngeal microsurgery.
