  • We report a case of difficult endotracheal intubation in a patient with tracheobronchopathia osteochondroplastica. A 65-year-old man was scheduled to undergo ulnar nerve decompression and ganglion excisional biopsy under general anesthesia. During induction of general anesthesia, an endotracheal tube could not be advanced through the vocal cords due to resistance. A large number of nodules were identified below the vocal cords using a $Glidescope^{(R)}$ video-laryngoscopy, and fiberoptic bronchoscopy revealed irregular nodules on the surface of the entire trachea and the main bronchus below the vocal cords. Use of a small endotracheal tube was attempted and failed. a laryngeal mask airway (LMA $Supreme^{TM}$) rather than further intubation was successfully used to maintain the airway.

  • Vocal cord dysfunction (VCD) is respiratory disorder characterized by paradoxical closure of the vocal cord during the respiratory cycle leading to obstructive airway symptoms. The clinical presentation of VCD is often dramatic and its misdiagnosis as asthma or exercise-induced brochospasm(EIB) has led to inappropriate treatment including high dose corticosteroids, intubation, and tracheostomy. Many VCD patients are asymptomatic at rest and require exercise challenge to elicit symptoms and vocal cord abnormalities. The "gold standard" for the diagnosis of VCD remains laryngoscopy or bronchoscopy with direct visualization of paradoxical adduction of the vocal cords. We report a case of exercise-induced Vocal cord masqueraded as exercise-induced asthma unresponsive to corticosteroids. And bronchodilator confirmed by typical bronchoscopic findings with paradoxial adduction of the vocal cords.

  • The human speech sounds are use to diagnosis in oriental medicine with ‘0-sung’theory. In general, human voice are sound waves which generated by phonation. Two major parts of phonation are vocal cords and vocal tract. The uniqueness of individual vocal sound depend on structure and usage of their vocal cords and tract. In the oriental medicine, “0-sung (5-tones)” has been used to classify constitution of human body In order to characterize the “0-sung”, their frequency characteristics are investigated, and a principal frequency component is extracted. Then, the principal component is applied to classify sounds into “0-sung.”

  • Lipomas are benign tumors and most commonly occurs in trunk, upper extremities and lower extremities. About 13-15% of lipomas are located in the head and neck area. However, lipomas of larynx are very rare and only about 100 cases have been reported. Laryngeal lipomas occur mainly in epiglottis, aryepiglottic fold and false vocal cords, which have adipose tissue. Author experienced an unusual presentation of laryngeal lipoma. Tumor seemed to be located in the supraglottis in the preoperative laryngoscopy, but it was found to be located at the free margin of the true vocal cord. To date, only one case has been reported in the world literature. We report this case with a review of the literature.

  • Human vocal cords vibrate as quickly as 100-250 times per second, so it is impossible to observe them with normal endoscopic diagnostic equipment. High-speed videolaryngoscopy (HSV) allows the visualization of non-periodic vibratory motion of vocal fold beyond the limitation of videostroboscopy. New developed post-processing methods that converts HSV to two-dimensional videokymography (2D VKG) using U-medical image-processing software can provide quantitative information on vocal fold mucosa vibration. Multifunctional laryngeal examination system is composed of 3 kinds of examinations such as HSV, 2D scanning digital kymography (2D DKG) and line scanning digital kymography (DKG). Evaluation of entire vocal cord vibratory pattern in each cord is possible using 2D DKG and a faster and more reliable quantitative information can be obtained. As this system is used in clinical and research, it is expected to bring much advances to the diagnosis of voice disorders. In this review, I will introduce the principles and advantages on examination of the vocal fold vibration, which is in the spotlight recently, and proceed with the literature review.

  • Nasotracheal intubation is commonly performed under general anesthesia in oral and maxillofacial surgery. For the convenience of surgery, nasal Ring-Adair-Elwyn (RAE) tubes are mainly used. Because the nasal RAE tubes were bent in an "L" shape, the insertion depth was limited. Particularly, it is necessary to accurately determine the appropriate depth of the RAE tubes in children. Several types of nasal RAE tubes are used in the medical market, which vary in material and length. We performed endotracheal intubation using a nasal RAE tube for double-jaw surgery, but air leakage persisted even when the air pressure in the cuff was increased. When checked with a laryngoscope, it was confirmed that the tube was pushed out, and the cuff was caught on the vocal cords, causing air leakage. Since inserting the tube deeply did not solve the problem, replacing it with a nasal RAE tube (PolarTM, Preformed Tracheal Tube, Smith Medical, Inc., USA) did not cause air leakage; thus, we reported this case.

  • Laryngeal framework surgery to improve change the voice is a challenging development in phoniatric surgery. Basically two categories can be distinguished : (1) attempted medialization of the vocal fold, as for the treatment of paralytic dysphonias (2) adjustment of the vocal fold's tension of transsexuals or mutational dysphonia. Vocal pitch can be elevated by various surgical technique 1) cricothyroid approximation 2) A-P expansion of the thyroid ala 3) longitudinal incision in the cords 4) intrachondral injection of the steroid, and 5) evaporation of the cords by $CO_2$ laser. We have experienced a case of pitch elevation procedure after transsexual operation. After transsexual operation, he had received anterior commissure laryngoplasty modified from Le Jeune with no change in voice pitch(Fo=110Hz). 8 monthes later, he had received cricothyroid approximation resulting in pitch elevation(Fo=160Hz).

  • Measurement technology related to vocal cord disease is in a late state of technology accumulation, and electroglotograph(EGG) measuring equipment is dependent on foreign production. This paper seeks to design a wearable wireless electrical gate conductivity measurement terminal capable of easily measuring and monitoring the vibration state of the vocal cords, that is, the contact pattern of the vocal cords, which is important in vocalization.

  • The voice of aged persons is known generally to be somewhat different from that of other adults, suggesting that laryngeal change occurs with advancing age. However, because knowledge of the voice characteristics of aged persons is limited, it is difficult to judge whether their voices arc normal. Chart review and laryngoscopic examination from ninety-one patients with hoarseness over the age of 60(1st group) and one hundred sixteen patients with hoarseness below the age of 50(2nd group) were done to define aging related voice disorders. The following results were obtained. 1) Associated diseases related to laryngeal disease were hypertension(12%), pulmonary disease(4.4%), thyroid disease(1.1%) in 1st group and hypertension(9.5%), thyroid disease(1.7%) in 2nd group. 2) The underlying diseases causing hoarseness in order of frequency were benign vocal fold lesion(37.7%), inflammatory disease(36.8%), functional dysphonia(17%) in 1st group and benign vocal fold lesion(43.6%), functional dysphonia(26.3%), inflammatory disease(16.5%) in 2nd group. 3) In stroboscopic findings, atrophy and sulcus of vocal cords are more prevalent in males than in females and edema of vocal cords is more common in females. Generally the voice characteristics of aged persons depend on the mass of the vocal folds which may be decreased through atrophy or be increased by edema. However, other factors such as systemic diseases, drug side effects and compensatory mechanism to presbylaryngis must be taken into account in diagnosing and treating voice disorders in aged persons.

  • Background and Objectives : Though voice therapy is proven to be effective in academia, treatment results and satisfaction level are inconsistent in clinics. The inconsistency may rise from diverse vocal cords misuse patterns in patient population as they suffer from different diseases or have bad phonation habits. In order to overcome the limitation, we uniformized patients' vocal cords with injection laryngoplasty prior to voice therapy. The efficacy of voice therapy and consistency in treatment results after injection laryngoplasty were assessed. Materials and Methods : Patients diagnosed with vocal nodules were either treated with injection laryngoplasty followed by voice therapy (combined treatment group) or voice therapy only (voice therapy group). Each group consisted of 15 patients. Acoustic measures (jitter, shimmer, NHR), aerodynamic measures (MPT, Psub, MAFR), and subjective auditory conscious measures (K-VHI, K-VRQOL) between two groups were analyzed. Results : After treatments, both groups improved in terms of acoustic, aerodynamic, and subjective auditory conscious measures compared to pre-treatment. Combined treatment group had statistically significantly greater improvement in shimmer, P in K-VHI, and PF in K-VRQOL compared to voice therapy group. Conclusion : Injection laryngoplasty treatment prior to voice therapy synergizes in treating patients compared to voice therapy only.
