Clinical Analysis of Persistent and Recurrent Postoperative Dysphonia (후두미세수술후 지속적인 음성장애환자에 대한 임상적 고찰)

  • The persistent and recurrent dysphonia after microlaryngeal surgery was noted in tweleve patients. We reviewed the results of laryngostroboscopy, psychoacoustic evaluation, aerodynamic study and acoustic analysis according to the treatment modality. The causes of persistent dysphonia were attributed to vocal cord scarring, recurrent mass lesion, residual mass lesion, persistent inflammation, and hyperfunctional voice disorder. We noticed the better vocal function in the group treated with voice therapy or surgical therapy than the group treated with voice rest and medication. Therefore, we concluded that vocal function can be improved with the use of active, multidisciplinary approach which includes voice therapy, medical treatment and selected surgical resection according to the laryngeal lesions.

  • Voice is made by systemic interaction of respiration, vocalization, articulation and resonation. There is no existing multi-channel voice analysis system to assess voice and respiration simultaneously. The most existing systems consist of. vocal fold vibration measurement such as stroboscopy, EGG (electroglottography) or laryngeal electromyography and voice analysis system. Since respiration has close relationship with voice simultaneous analysis of both vocal cord vibration and respiration are essential. In this study, a four channel integrated system are developed for acoustic analysis through microphone, vocal fold vibratory analysis using EGG, and respiratory analysis using two channel RIP (respiratory inductive plethysmography).

  • Change of voice, espicially hoarseness show irregular vibration of vocal cord. So, computerized acoustic analysis has presented many acoustic parameters for objective evaluation of voice. We objectively investigated the vocal vibration of normal persons and hoarseness patients in Korea. The RAP(relative average perturbation), PPQ(pitch period perturbation quotient) and APQ(amplitude perturbation quotient) of normal persons were compared with that of hoarseness patients with multidimensional voice program for the possibility of distinguishing the pathologic vocal vibration from normal. Authors agree that RAP, PPQ and APQ showed interesting differences between the normal and the hoarseness patients by the multivariate statistical analysis. In conculusion, relative average perturbation, pitch period perturbation and amplitude perturbation quotient might be meangingful screening parameters distinguishing hoarseness patients from normal.

  • Measurement technology related to vocal cord disease is in a late state of technology accumulation, and electroglotograph(EGG) measuring equipment is dependent on foreign production. This paper seeks to design a wearable wireless electrical gate conductivity measurement terminal capable of easily measuring and monitoring the vibration state of the vocal cords, that is, the contact pattern of the vocal cords, which is important in vocalization.

Recurrent T-cell lymphoma occurred in the vocal fold: a case report (성대에서 발견된 재발성 T세포 림프종에 대한 증례 보고)

  • Background. Lymphoma of the vocal fold is extremely rare due to low lymphoid content in the larynx. To date, fewer than 100 cases of laryngeal lymphoma have been reported; however, none of these literatures are concerned about exclusive laryngeal involvement of recurred lymphoma which initially appeared in other body sites. Specific consensus about management for these patients yet to exist, due to its rare occurrence; however, the main modality of treatment is chemotherapy alone or in combination with radiation therapy. Case. Herein, we report a case of a 51-year-old female patient who had recurrent T-cell lymphoma developing in bilateral vocal folds. The patient was originally diagnosed of T-cell lymphoma in right colic flexure 10 years ago, and was cured by chemotherapy. Immunohistochemical stain revealed the histologic type of recurred tumor in the vocal folds that are identical to the previously cured lymphoma. Conclusion. To the best of our knowledge, this was the first case that recurrent lymphoma occurred solely in the vocal folds. Despite its rarity, lymphoma should be put in the index of suspicion among those patients with decreased vocal fold mucosal wave without definite vocal fold mass who had a history of cured lymphoma.

585-nm Pulsed Dye Laser Treatment of Vocal Polyp (585-nm Pulsed Dye LASER를 이용한 성대 폴립의 치료)

  • Background and Objectives: The 585-nm pulsed dye laser (PDL) has recently been adopted by otolaryngologists because of its epithelial-sparing properties. Many authors have reported the use of PDL for treatment of various vocal cord lesions. This purpose of this study is to examine the effectiveness of 585-nm PDL in the treatment of vocal polyp. Materials and Methods: Eight patients with vocal polyp were treated with 585-nm PDL from Sep. 2006 to Nov. 2006 in Yong-dong Sevrance hospital. 5 of them went through local anesthesia and 3 of them went through general anesthesia. In order to control laser fiber, flexible digital transnasal laryngoscope was applied under local anesthesia and general anesthesia using LMA, and micromanipulator was used under general anesthesia using endotracheal tube. The evaluations of vocal function was done at pre-and postoperation. Results: All patients improved in the perceptual evaluation of voice after PDL surgery. The aerodynamic study revealed that 5 of 8 patients showed improvement in maximal phonation time, and 6 of 8 showed improvement in mean airflow rate during phonation. The acoustic analysis revealed that all patients showed improvement in Jitter and Shimmer, and 7 of 8 showed improvement in noise to harmony ratio. Conclusion: This study demonstrates promising results in the efficacy of 585-nm PDL for the treatment of vocal polyps, and it illustrates a new option for vocal polyp treatment as well as the advantage of PDL surgery.

Postoperative Phonetic Evaluation of Two Cases of Verrucous Carcinoma of the Larynx after Partial Laryngectomy with False Vocal Cord Advancement Flap (후두 우상암에서 후두 부분적출술 및 가성대 전진피판에 의한 성대 재건술 전후의 음성변화 : 2례)

  • Verrucous carcinoma is a variant of highly differentiated squamous cell carcinoma and has a predilection for the oral cavity and larynx. It is known to be of good prognosis due to its low regional and distant metastatic potential. Peak incidence is at ages 40-69 while predominantly occuring in males and its association with tobacco smoking has been implicated. The verrucous carcinoma have accordingly been treated by radiotherapy or surgical procedures but there has been considerable controversy regarding the effect of radiotherapy due to the possibility of anaplastic transformation Laser surgery and partial resection are among the commonly used surgical procedures but laser surgery has not yet shown results in improving phonetic abilities. Authors experienced two cases of verrucous carcinoma of the larynx treated by partial laryngectomy and false vocal cord advancement flap who showed no recurrence of disease under stroboscopic examination after two year follow up and when compared to preoperative aerodynamic studies, the expiratory air pressure was decreased and mean air flow rate increased which improved glottal efficiency markedly.

Clinical Analysis of Cases of Segmental Resection and Primary Anastomosis in Tracheal Stenosis (기관 절제 및 단단 문합술에 의한 기관 협착증의 치료)

  • Despite improvement in respiratory care, including use of low pressure and high volume cuffed tubes, tracheal stenosis remains a serious complication after a long-term tracheal intubation and tracheostomy. In such patients, tracheal resection and primary anastomosis is still considered ideal therapeutic modality. Between 1989 and 1997, we performed tracheal resections with end-to-end anastomosis on 14 patients with no operative mortality and some morbidity. Tracheal stenosis was caused by tracheostomy in nine patients, by endotracheal intubation in three patients and by thyroid carcinoma in two patients. The length of stenosis was various from 2cm to 4.5cm. All patient underwent segmental tracheal resection and primary anastomosis(14 patients) and additional procedures were cricoid cartilage reconstruction(2 patients), suprahyoid laryngeal release(3patients), carinal release technique(2 patients) and arytenoidectomy(2 patients). We have nine complications: granulona at anastomosis site in four patients, vocal cord palsy in two patients and restenosis, pneumonia, skin necrosis in each of those patients. The granuloma was removed by bronchoscopic forceps(4 patients). Vocal cord palsy was treated by arytenoidectorny(2 patients), restenosis by T-tube insertion, pneumonia by antibiotics and skin necrosis was treated by skin graft. We reviews our expenence of clinical features of tracheal stenosis and surgical treatment by tracheal one-to-end anastomosis with additional procedures to avoid postoperative complications for sucessful results.

  • Herpes zoster oticus is one of complication of varicella zoster virus (VZV) reactivation in the geniculate ganglion of the facial nerve, which is the most common presentation of herpes zoster in the head and neck region. However, VZV infection of the larynx has rarely been described in the literature compared with Herpes zoster oticus. Moreover, zoster laryngopharyngitis simultaneously occurred with recurred Herpes zoster oticus which has no newly developing motor dysfunction has not been reported yet. Therefore, these diseases are difficult to diagnose due to its rareness. However, distinctive appearances such as unilateral herpetic mucosal eruptions and vesicles are useful and essential in making a quick and accurate diagnosis. Thus, we report a characteristic case of zoster laryngopharyngitis simultaneously occurred with recurred Herpes zoster oticus not accompanied by any newly developing motor palsy.
