Emotional State and Personality Characteristics in Patients with Vitiligo (백반증 환자의 정서상태와 성격특성)

  • Objectives : This study was aimed to investigate the emotional state and personality factors of patients with vitiligo, and the impacts of clinical characteristics. Methods : Twenty one patients with vitiligo and 20 normal controls were studied. Beck Depression Inventory(BDI), Spielberg's State and Trait Anxiety Inventory(STAI) and Cattell's 16 Personality Factors Questionnaire(16-PF) were used for assessment. Statistically, t-test, Oneway ANOVA, Mann-Whitney U test and Pearson's correlation test were used. Results : The Results were as follows. 1) The comparison of total score of BDI, state anxiety and trait anxiety between the patients with vitiligo and normal controls were not significantly different. 2) In first-stratum source traits of 16-PF, normal control group was significantly higher than the patients with vitiligo in H-factor(Boldness) and I-factor(Tender-mindedness). In second-stratum source traits of 16-PF, there was no significant difference between the patients with vitiligo and normal control in all factors. 3) According to sex, the comparison of total scores of BDI, STAI between male and female patients with vitiligo were not significantly different. In the comparison of 16-PF, female patients were higher than male patients in I-factor only. 4) As according to age(under and over 30 years of age), the comparison of total scores of BDI, STAI between patients over 30 years of age and under 30 years of age were not significantly different. In the comparison of 16-PF, patients over 30 years of age were higher than patients under 30 years of age in E-, G-, L-, M-, Q2-, Q3-, IND-, SUP-, and CRE-factor. 5) As according to the presence of progression of vitiligo and the presence of visible lesion, the comparison of total scores of BDI, STAI and 16-PF were not significantly different. 6) There were no significant correlation between the duration of the vitiligo and BDI, STAI-S, STAI-T, and 16-PF. Conclusions : There were no significant difference between patients with vitiligo and normal controls on the emotional state and personality characteristics. But patients with vitiligo were more bolder and tender-minded than normal controls on 16-PF. The author propose that methods of treatment to resolve problems of the interpersonal relations will be helpful.

  • Arranging 63 kinds of separate volumes and papers published on Oriental medicine, I could get the result as follows. 1. The herbs for internal application used commonly in vitiligo are 155 kinds totally. The herbs for external application are 67 kinds. The herbs for external and internal both application examined into 23 sorts. 2. Herbs for weakness syndrome in vitiligo are 49 kinds. Another type, excess syndrome is 105 sorts, the'latter is roughly twice as many as the former. 3. It is as follows that the results of study in relations to kinds, factors, and medical treatments of herbs about vitiligo in and out of the country with the division of former times to 1900, 1901 to 1980, 1981 to 1990, and 1991 to the latest day. In comparative study of inner and outer of thc country about factors and medical treatments of vitiligo in chronicle classification, its factors in the internal documents are classified by outside factors. Although there arc the differences of factors by each epoch, however, the factors of vitiligo according to external documents are blood stasis(血熱), deficiency of um of the liver and kidneys(肝腎陰虛), deficiency of blood(血虛), excess of exhaustion(勞倦過多) etc. Moreover, the medical treatment is more diverse and the differences by each epoch as to the medical treatment is also put down more saliantiy than in internal documents. 4. In comparison with herbs in experimental and no experimental documents, herbs applied for weakness syndromes in experimental method are 40 kinds totally. The herbs in no experimental methods are 35 kinds. The herbs used by experimental method are 65 kinds. The common herbs for excess syndrome by no experimental method are 78 kinds. We can see comparable difference from kinds of herbs used by experimental method. In brief, there are the differences classified by each epoch in Oriental medicine for treatment. Especially one of the most important feature, the frequency in use of weakness syndrome herbs has increased more than that of excess syndrome herbs. In external documents (china) and experimental study, generally the differences of common herbs and factors have disappeared through many experimentsitudy. The classification of its factors have been fractionalized clinically. Besides, in Western medicine and Orienal medicine, vitiligo tends to be prescribed not to simple skin disease but a mental and physical disease, a whole body and an internal disease.

Pimecrolimus increases the melanogenesis and migration of melanocytes in vitro

  • Vitiligo is an intriguing depigmentary disorder and is notoriously difficult to be treated. The ultimate goal of vitiligo treatment is to replenish the lost melanocytes by immigration from hair follicle and to restore the normal function of melanogenesis by residual melanocytes. There are two types of topical calcineurin inhibitors called tacrolimus and pimecrolimus, and are recommended as the first-line treatments in vitiligo. Although pimecrolimus is efficacious for the repigmentation of vitiligo, its intrinsic mechanisms have never been investigated in vitro. This research aimed to study the ability of pimecrolimus on stimulating melanogenesis, melanocyte migration and MITF (microphthalmia associated transcription factor) protein expression. Results showed that pimecrolimus at the dosages of 1, 10, $10^2$nM were neither mitogenic nor cytotoxic to melanocytes. The addition of pimecrolimus at 10, $10^2$ and $10^3nM$ significantly increased intracellular tyrosinase activity, which was consistent with the elevated content of melanin content at the same concentrations. The peak effect was seen at 72 h in response to $10^2$nM pimecrolimus. Results of the wound scratch assay and Transwell assays indicate that pimecrolimus is effective in facilitating melanocyte migration on a collagen IV-coated surface. In addition, MITF protein yield reached the highest by pimecrolimus at $10^2nM$. In brief, pimecrolimus enhances melanin synthesis as well as promotes migration of melanocytes directly, possibly via their effects on MITF protein expression.

Rview of Recent Clinical Research for Herbal Medicine Treatment on Vitiligo in Children - Focused on Chinese Randomized Controlled Trials - (소아 백반증의 한약 치료에 대한 최신 임상 연구 동향 - 중국 무작위 대조군 연구를 중심으로 -)

  • Objective The purpose of this study is to gather information from clinical studies conducted in China and analyze the effects of herbal medicine treatment for childhood vitiligo. Methods The randomized controlled trials (RCTs) with herbal medicine treatment for childhood vitiligo from the Chinese Academic Journal (CAJ) from China National Knowledge Infrastructure (CNKI) were searched. Then, each study was analyzed by demographics, disease durations, interventions, treatment periods, outcomes, adverse events, and compositions of the herbal medicine used. Result A total of 6 RCTs were included. All the trials except for one used a combination of Chinese herbal medicine with Western medicine as their treatment groups. The total efficacy rate was used to measure the effectiveness, and the treatment groups reported a significantly higher total efficacy rate compared to the control groups in all studies. In one study, a control group treated with compound glycyrrhizin capsule and topical compound kaliziran tincture combination treatment was compared with a treatment group which used the same treatment that the control group received in addition to oral herbal medicine. This study also showed higher total effectiveness in the treatment group than the control group. Conclusions In pediatric vitiligo, a combination of herbal medicine treatment rather than Western medical treatment alone can improve symptoms. In addition, it was suggested that the therapeutic effect can be enhanced when oral herbal medicine is used in combination with other herbal medicine treatments.

The effects of Hominis Placenta extract on Melanin synthesis of B16 melanoma cells (자하거(紫何車) 약침액(藥針液)이 B16 melanoma 세포주의 멜라닌 합성에 미치는 영향)

  • Objective : This research was carried out for the development of medicine for vitiligo treatment and focused on the effect of Hominis Placenta extract on Melanin synthesis of B16 melanoma cells. Methods : Acitivity of tyrosinase playing a vital role in synthesis of Melanin and the quantity of Melanin, which is the final product in cultured B16 melanoma cells, effects of Hominis Placenta extract were measured. Results: The results indicated that Hominis Placenta extract increased both the amount of Melanin and the activity of tyrosinase according to the concentration, and they also supported by western blot analysis. Conclusion : The results suggests that Hominis Placenta extract has an advantageous effect on the promotion of Melanin synthesis and will contribute to the development of vitiligo treatment through further related studies.

The Effects of Psoraleae fructus Extract on Melanin Synthesis of B16 Melanoma Cells (보골지 추출물이 B16 melanoma 세포주의 멜라닌 합성에 미치는 영향)

  • Objectives : This study was carried out for the development of medicine for vitiligo treatment and focused on the effect of Psoraleae fructus extract on melanin synthesis of B16 melanoma cells. Methods : Activity of tyrosinase playing a vital role in the synthesis and quantity of melanin, which is the final product in cultured B16 melanoma cells, the effects of Psoraleae fructus extract were measured. Results : The results indicated that Psoraleae fructus extract increased beth the amount of melanin and the activity of tyrosinase according to concentration, also supported by western blot analysis. Conclusions : The results suggest that Psoraleae fructus extract has an advantageous effect on the promotion of melanin synthesis and will contribute to the development of vitiligo treatment through further related studies.

Tyrosinase Activity and Melanogenic Effects of Rhododendron schlippenbachii Extract In vivo and In vitro

  • Rhododendron schlippenbachii have been used as a medicine because of their various biological activities. In this study, R. schlippenbachii ethanol extract was evaluated for the treatment of vitiligo. The R. schlippenbachii ethanol extract did not show any cell cytotoxicity. The effect on mushroom tyrosinase and cellular tyrosinase activities were further assessed. In addition, the determination of melanin content in melanocytes was measured using both the B16 melanoma cells and C57BL/6J Ler-vit/vit mice. Finally, the existence of quercetin in R. schlippenbachii was confirmed by qualitative analysis using HPLC. The results clearly demonstrated the R. schlippenbachii extract enhanced melanogenesis and also increased tyrosinase activity in cultured melanoma cells and C57BL/6J Ler-vit/vit mice. In addition, treatment with R. schlippenbachii extract led to a higher content of melanin and eumelanin in C57BL/6J Ler-vit/vit mice hair than in control (untreated) mice, which demonstrated the therapeutic effect of hair-graying associated with vitiligo. Finally, we confirmed a notable increase in melanocytes in the skin of C57BL/6J Ler-vit/vit mice treated with R. schlippenbachii extract compared with the control. Extracts of R. schlippenbachii was shown to be potent tyrosinase and melanin synthesis activator in B16 melanoma cells. The R. schlippenbachii extract have significantly higher melanin content than the untreated control in C57BL/6J Ler-vit/vit mice hair. The results suggest that R. schlippenbachii extract might be considered as an alternative treatment for improvement of vitiligo.

A Review of Clinical Researches for Herbal Medicine Treatment on Vitiligo (백반증의 한약 치료에 대한 임상 연구 고찰)

  • Objectives The purpose of this study is to analyze clinical studies on the efficacy and safety of herbal medicine in vitiligo by analyzing randomized controlled trials (RCTs). Methods Three electronic databases including the PubMed, EMBASE, and Cochrane library were used to search for randomized controlled trials, by using specific key words and criteria up to January 4th, 2020. Data in regards to years of publication, nation, demographic information, disease characteristics, duration of diseases, treatment methods, treatment period, outcome measures, results and adverse events were collected for this study. Results A total of 11 randomized controlled trials were selected and analyzed. The total effective rate of the treatment group treated with herbal medicine was significantly higher than that of the control group. In the other outcome measures, the treatment group also showed statistically significant differences in improving the outcome measures compared to the control group, or showed similar treatment effects to the control group. The most commonly used herbal medicines were Carthami Flos (紅花), Angelicae Gigantis Radix (當歸), Angelica dahurica Bentham et Hooker f. (白芷), Astragali Radix (黃芪), Glycyrrhizae Radix et Rhizoma (甘草), Salviae Miltiorrhizae Radix (丹參), Persicae Semen (桃仁), Araliae Continentalis Radix (獨活), Tribuli Fructus (白蒺藜), Psoraleae Semen (補骨脂) etc. Hardly any severe adverse events were reported from the trials selected. Conclusions Based on the results of the clinical studies, herbal medicine treatment could be an effective and safe option for vitiligo treatment and symptom improvement.

  • Wolf's isotopic response is defined as the occurrence of a new skin disorder at the site of another unrelated skin disease that has already healed. In most cases of isotopic response, the initial dermatosis is herpes infection, and the most frequent second dermatoses are granulomatous reactions. Various interpretations of this phenomenon have been attempted. However, the exact mechanism has not been identified yet. Herein, we report a case in which the secondary disease was segmental vitiligo that appeared over the same dermatomes of herpes zoster. A 71-year-old woman presented with well-defined, depigmented patches on the left chest and back. She had been diagnosed with herpes zoster on the same dermatomes and treated with an antiviral agent 3 years ago. Histological examination showed decreased basal melanin pigments and melanocytes. Consequently, the patient was diagnosed with segmental vitiligo based on the clinical and histological findings.

Four cases of vitiligo patients treated by Oriental medical treatment who have experienced Excimer Laser treatment (엑시머레이저 치료를 경험했던 백반증 환자가 한방복합치료와 엑시머레이저 병행치료로 호전된 치험 4례)

  • Purpose: This study aims to prove effects of an oriental medical complex therapy on vitiligo patients who have experienced the Eximer laser therapy. Method: The study conducted its investigation targeting these four patients who have not been observed with any satisfactory reaction to the Eximer laser therapy. The study analyzed the findings through the comparison on the pictures before and after the therapy. Result: After comparing the pictures, the study confirmed positive changes in the conditions of the patients. The oriental medical complex therapy turned out to be effective enough to treat the patients who have gone through the Eximer laser therapy before. Conclusion: Such result leads the study to a conclusion that the oriental medical complex therapy can be an effective way to treat these patients who do not have a satisfactory reaction to the Eximer laser therapy. In addition, the author of the study suggests that more clinical trials should be conducted.
