Comparative Histologic Study of 3-Root Canal Filling Materials for Dog's Teeth (성견 치아의 치수절제술 후 3가지 근관충전재에 따른 조직학적 반응의 비교)

  • Endodontic treatment of primary molars is a great challenge to the dentists because of their complex morphology. However, there have been limited controlled in vivo studies about filling materials for primary teeth. Hence, the aim of this study is to evaluate and compare histologic responses of different calcium hydroxide canal filling materials that are used in daily clinical practice. Pulpectomies were performed in 132 teeth of the dogs. The root canals were randomly filled with either $Vitapex^{(R)}$ (n = 44), $Metapex^{(R)}$ (n = 44), or $Metapaste^{(R)}$ (n = 44). After 4 or 13 weeks, respectively after operation, the dogs were sacrificed, and teeth were processed for histologic examinations. Inflammatory reaction was evaluated and scored in a blind manner. The Fisher's exact test and Kruskal-Wallis test were used to compare the tested groups. In 4-week-group, all inflammatory responses were in normal range except for fibrosis. In 13-week-group, increased response to various inflammation reaction was shown compared to the 4-week-group. However, there were no statically significant differences between the tested groups in all inflammatory reaction. Overall, there were no significant differences among three materials in responses of apical tissues and all of them showed favorable tissue responses.

Comparison of the Ability to Mask the Color of Endodontic Filling Materials Using Several Types of Base Materials

  • In clinical pediatric dentistry, a base material with optical properties, including transparency, that can mask the color of the material used for root canal-filling is preferred. This study aimed to examine the optical properties of various base materials by thickness. The disk-shaped specimens were photopolymerized and fabricated using Ionosit (IN), TheraCal LC (TL), TheraCal PT (PT), and A2 shade of FiltekTM Supreme Flowable Restorative (FZ), Fuji II LC (FL), and KetacTM Fil (KF) with 1 and 2 mm thickness. The color parameters of these specimens were measured using a spectrophotometer on a black and white background and were measured using the same method on a mold containing Vitapex® and gutta-percha. The translucency parameter (TP) and color difference were calculated for each group. The Kruskal-Wallis and Bonferroni tests were used in the statistical analyses. The TP decreased when the thickness was 2 mm compared with 1 mm. The TP values of TL and PT were the lowest at all thicknesses. The TP values of 2 mm thickness in all molds filled with Vitapex® and gutta-percha were the lowest for TL, PT, KF, and IN. In TL and PT, the color difference before and after the application of the canal-filling material was the smallest, regardless of material thickness. Within the limits of this in vitro study, TL, PT, KF, and IN demonstrated better masking of the color of canal-filling material.

ANTIMICROBIAL EFFECT OF ANTIBIOTICS AND ROOT CANAL CEMENTS ON THE PREDOMINANT PATHOGENIC ANAEROBIC MICROFLORA IN ROOT CANALS (근관내 주요 혐기성 병인균에 대한 수종 항생제와 근관충전용 세멘트의 항균효과에 관한 연구)

  • The purpose of this study was to evaluate the susceptibility of anaerobic microorganisms to certain antibiotics and root canal cements. Prevotella intermedia(Bacteroides intermedius) ATCC 25611(serotype A), Fusobacterium nucleatum ATCC 25586, Actinomyces viscosus ATCC 15987 which are the predominant pathogenic anaerobes in dental root canals were cultured in BHI for 48 hours(Fig.1). After each $200{\mu}l$ of those broths with microorganisms was streaked on each surface of blood agar plate, 2 to 5 antibiotic discs which are impregnated with Tetrncycline, Erythromycin, Ampicillin, Clindamycin, or Vancomycin were applied on each surface of blood agar plate and cultured for 5 days anaerobically in the anaerobic chamber (Fig.2). 15 antibiotic discs for each kind of antibiotics and each species of microorganisms were tested. Also each kind of root canal cement tubes which include Zinc oxide eugenol cement, Zinc phosphate cement, Calcium hydroxide powder+DD.W., Calcium hydroxide paste(Pulpdent Tempcanal), or Vitapex(Table 1) were applied on the inoculated BAPs after $200{\mu}l$ of each experimental species of microorganisms was streaked on the surface of blood agar plates, and they were cultured for 5 days anaerobically in the anaerobic chamber(Fig.3). The sensitivity(antimicrobial effect) was determined by the diameter of the inhibition zone. The results are as follows: 1. The results of antibiotic susceptibility test(Table 2) 1) All of the tested antibiotics had antimicrobial activity with various degrees. 2) In Prevotella intermedia (old Bacteroides intermedius), the diameter of inhibition zone to Erythromycin($37.87mm{\pm}2.20$) was largest, those to Tetracycline($26.20mm{\pm}2.96$), Vancomycin($21.53mm{\pm}1.96$), Clindamycin($18.73mm{\pm}0.96$) was smaller than former orderly, and That to Ampicillin ($7.87mm{\pm}0.83$) was smallest. 3) In Actinomyces viscosus, the diameter of inhibition zone to Erythromycin($28.73mm{\pm}1.22$) was largest, those to Ampicillin($21.73mm{\pm}1.03$), Clindamycin($21.33mm{\pm}1.59$) was similarly next order, that to Vancomycin($19.00mm{\pm}1.96$) was smaller than Clindamycin, and that to Tetracycline($11.93mm{\pm}0.70$) was smallest. 4) In Fusobacterium nucleatum, the diameter of inhibition zone to Ampicillin($31.07mm{\pm}1.91$) was largest, that to Erythromycin($28.87mm{\pm}0.92$), Clindamycin($20.47mm{\pm}1.51$), Vancomycin ($16.73mm{\pm}0.96$), Tetracycline ($12.13mm{\pm}1.06$) are smaller than former orderly. 2. The results of root canal cements and pastes(Table 3) 1) The external diameter of tube is 4mm, so 4mm of the inhibition zone diameter means non-susceptable. Prevotella intermedia (old Bacteroides intermedius) was non-susceptable to Calcium hydroxide powder+D.D.W., Calcium hydroxide paste(pulpdent Tempcanal), and Actinomyces viscosus was non-susceptable to Zinc phosphate cement, Calcium hydroxide powder + D.D.W., Calcium hydroxide paste(pulpdent Tempcanal). 2) In Prevotella intermedia (old Bacteroides intermedius), the diameter of inhibition zone to Zinc oxide eugenol cement($13.67mm{\pm}3.30$) was largest, that to Vitapex($9.20mm{\pm}2.96$), Zinc phosphate cement($6.13mm{\pm}2.07$) was smaller than former. 3) In Actinomyces viscosus, the diameter of inhibition zone to Zinc oxide eugenol cement($17.40mm{\pm}5.20$) was largest and that to Vitapex($8.80mm{\pm}1.70$) was next order. 4) In Fusobacterium nucleatum, the diameter of inhibition zone to Vitapex($42.33mm{\pm}17.2$) was largest and those to Calcium hydroxide paste(Pulpdent Tempcanal)($14.47mm{\pm}3.72$) and Zinc oxide eugenol cement($8.93mm{\pm}2.71$), Zinc phosphate cement($8.20mm{\pm}2.27$), Calcium hydroxide powder+D.D.W.($5.53mm{\pm}2.10$)was next orderly. And then In Zinc oxide eugenol cement and Zinc phosphate cement group, two of fifteen samples showed no inhibition zone, in Calcium hydroxide powder + D.D.W. group, 8 of 15 samples showed no inhibition zone.

AN EXPERIMENTAL STUDY ON THE EFFECT OF Ca(OH)2 UPON THE HEALING PROCESS OF THE PULP AND PERIAPICAL TISSUE IN THE DOGS' TEETH (수산화칼슘이 손상치수조직 및 치근조직의 치유에 미치는 영향에 관한 연구)

  • The purpose of this study was to observe the responses of the remaining pulp tissue after pulpotomy upon the several kinds of $Ca(OH)_2$ products and the responses of periapical tissue upon some root canal filling materials after extirpation. For pulpotomy, the class V cavities were prepared on the premolars, molars and upper canines, and the pulp was amputated. Each drug was placed over the amputated tissue and cavity was sealed with zinc oxide eugenol cement. The drugs which were used for the study were Dycal (Caulk Co. U.S.A.), Cavitec (Kerr Co. U.S.A.), Calvital, Nobudyne and Neodyne (Neo Dental Chemical Products). For extirpation, the endodontic cavities were prepared on the lingual surfaces of anterior teeth, and the pulp tissues were extirpated as routine method. After enlarging, irrigation, and measuring of root length by taking X-ray, each root canal filling material was filled in the canal with gutta percha cone, and endodontic cavity was sealed with zinc oxide eugenol cement. Zinc oxide eugenol, $Ca(OH)_2$ (Eli Lilly Co. U.S.A.) and Vitapex (Neo Dental Chemical Products) were used as root canal filling materials. Animals were sacrificed after 1, 3 and 6 weeks following the operation. The teeth were decalcified in formic acid, sectioned and stained with hematoxylin eosin. Microscopic examination revealed as follows. 1. Dycal: The dentin bridge formation was observed at the 3rd week after pulpotomy. Inflammatory conditions which were infiltration of inflammatory cells and dilatation of blood vessels were kept in remaining pulp tissue at the 6th week. 2. Calvital: The dentin bridge was observed at the 1st week after pulpotomy. As the time clasped, the pulp tended to be the fibrous degeneration. 3. Cavitec, Nobudyne and Neodyne: In the case of Cavitec and Nobudyne, the incompleted and irregular dentin bridge was observed at the 6th week, and in Neodyne, was observed at the 3rd week. The severe inflammatory changes were seen in the remaining pulp tissue. As the time clasped, the fibrous degeneration tended to spread in the remaining pulp tissue. 4. $Ca(OH)_2$: Osteocementum was formed at the 3rd week, the matrix of cementum and dentin were resorted, and infiltration of lymphocytes was seen in periapical tissue when $Ca(OH)_2$ was used as canal-filling materials. S. ZOE and Vitapex The cementum like substance was seen in periapical portion at the 1st week, when ZOE and Vitapex were used as root canal filling materials. As the time elapsed, the matrix of cementum and dentin tended to be resorted. At the 6th week, the inflammatory condition of periapical tissue was continued in the case of ZOE, but was reduced in the case of Vitapex.

Effectiveness of Sodium Iodide Root Canal Filling Pastes in Primary Teeth (요오드화 나트륨을 사용한 유치 근관 충전재의 효과)

  • Objectives: This study aimed to compare the physical properties and antibacterial effectiveness of iodoform based root filling pastes, Vitapex® and Metapex®, with sodium iodide root filling paste (NaI paste) for primary teeth. Materials and Methods: The physical properties (flowability, film thickness, radiopacity) of the pastes were evaluated according to ISO 6876:2012. The antibacterial activity against Enterococcus faecalis strain (ATCC 6538) was evaluated using a direct contact test. Results: There was no significant statistical difference (p > 0.05) observed in the flow and film thickness of NaI paste when compared to the currently available root canal filling materials. The average flow capacities for Vitapex®, Metapex®, and NaI paste were 15.40 mm, 21.25 mm, and 20.01 mm, respectively. The average film thickness for Vitapex®, Metapex®, and NaI paste were 33.3 ㎕, 22.6 ㎕, and 25.0 ㎕, respectively. However, NaI paste showed lower radiopacity than the existing materials, and this difference was statistically significant (p < 0.05) NaI paste demonstrated higher antimicrobial activity than the available materials, and this difference was also statistically significant (p < 0.05). Conclusion: Compared to the existing commercialized root canal filling materials, NaI paste exhibited similar performance in terms of flow and film thickness, and superior antimicrobial activity against E. faecalis. Hence, NaI paste could be a promising root filling material for primary teeth and may be a potential alternative to existing materials.

  • Calcium hydroxide is unavoidably left on the root canal wall when used as an intracanal medicament, and is assumed to have some undesirable influence on the canal sealers. The purpose of this study was to determine whether the $Ca(OH)_2$ change physical properties (Exp. I) and sealing ability (Exp. II) of root canal sealers. Exp. I: As calcium hydroxide agents, $Calcipex^{\circledR}, {\;}Vitapex^{\circledR}, {\;}Calxyl^{\circledR}, {\;}and{\;}Ca(OH)_2$ were used.(omitted)

Effect of Calcium Hydroxide on the Microhardness of Root Dentin of Primary Tooth (수산화칼슘 근관충전제의 적용이 유치 치근상아질의 미세경도에 미치는 영향)

  • Calcium hydroxide mixture medicaments can nearly be considered to be the ideal primary tooth filling material. However, long-term application of calcium hydroxide combinations as an intra canal medicament softens dentin. The aim of this study was to evaluate the effect of calcium hydroxide on the microhardness of root dentin of primary tooth. For the study, 60 extractedprimary incisors were divided into 3 groups (no medicament, calcium hydroxide/iodorform mixture, and calcium hydroxide/distilled water mixture). After the cleansing and shaping of canals, calcium hydroxide medicaments were applied and stored for different periods of time (1, 7, 30, 90 days). The root was horizontally sectioned into 2 mm thick specimens and the microhardness was measured using Vickers microhardness tester. The results were as follows : Root dentin microhardness of primary teeth decreased with long term exposure to calcium hydroxide medicaments according to the experimental period and showed statistically significance (p < 0.05). Root dentin microhardness of primary tooth filled with calcium hydroxide mixed with distilled water showed more decrease than filled with Vitapex and showed statistically significance (p < 0.05). Root dentin microhardness of a control group without exposure to calcium hydroxide decreased according to the experimental period and showed statistically significance (p < 0.05).

A QUANTITATIVE ANALYSIS OF THE IMMUNOGLOBULIN CONTAINING CELLS IN PERIAPICAL LESIONS OF THE HUMAN TEETH (치근단 병소에서 면역글로불린의 분포에 관한 연구)

  • Periapical lesions develop as a result of immunopathologic response to irritants from infected root canal systems. Removal of these irritants from the root canal system and sealing the root canal space may induce he31ing of the periapical lesions. 83 periapical lesions diagnosed as periapical abscess, periapical granuloma, chronic nonspecific inflammation, fibrosis and periapical Cyst were evaluated for the distribution of immunoglobulin containing cells. The influence of the state of root canal treatment on the distribution of immunoglobulin containing cells has evaluated. All lesions were divided into a group with no treatment, a group with canal enlargement, a group filled with gutta percha, and a group filled with Vitapex(calcium hydroxide). The distribution of immunoglobulin-containing cells according to the presence of pain and fistula was also evaluated. The following results were obtained. 1. Statistically significant difference in the distribution of immunoglobulin-containing cells among periapical abscess, periapical granuloma, chronic nonspecific inflammation/fibrosis and periapical cyst were found.(Kruskal-Wallis analysis, P<0.05) The number of immunoglobulin-containing cells in fibrosis was remarkably lower than that of periapical abscess, granuloma and cyst. 2. IgM and IgA containing cells were predominantly observed in periapical abscesses and periapical cysts, respectively. 3. All periapical lesions showed a large number of IgG containing cells followed by IgM, IgA and IgE containing cells. 4. There was a decrease in all Ig-containing cells in the group with canal filling compared to groups without treatment or with enlargement. That is, there is a decrease in Ig-containing cells as treatment progresses. 5. No significant correlation existed between the presence of pain and fistula and the distribution of immunoglobulin containing cells in periapical lesions.(t-test) Results appear to support that immune response are actively involved in the development and progress in periapical lesions. The fact that distribution of immunoglobulins differ according to the state of endodontic treatment suggests that root canal treatment may alter the humoral immune response of the periapical lesions.

  • The pulpectomy or root canal treatment on primary teeth is to be done when there is evidence of chronic in flammation or necrosis in the radicular pulp. Due to the tortuous and ribbon-shaped anatomy of the primary teeth, the instrumentation of endodontic hand files and barbed broaches is not an easy procedure. Recently, many automatic Ni-Ti rotary instruments have been developed and has made endodontic treatment easier and faster. This report describes two cases of root canal treatment on primary molars using Ni-Ti rotary files. The cervical constricture was eliminated by the crown-down method, as smaller file proceeded unhindered into the apical third of the canal. In addition, the crown-down technique enhanced the efficacy of the endodontic irrigant. The use of rotary instrumentation for primary teeth seemed to be a more effective way to debride the uneven walls of primary teeth.

TOOTH REPLANTATION AFTER TRAUMATIC AVULSION: A 8-YEAR FOLLOW UP. (재식된 상악 중절치의 장기간에 걸친 추적례)

  • Tooth avulsion usually causes inflammatory root resorption and ankylosis, and ankylosis cause severe functional and esthetic problems, especially in childhood. A 7-year-old female visited the Dept. of Pediatric Dentistry, Yonsei University with the chief complaint of avulsive trauma to the upper right incisor which was left dry for 40 minutes. Tooth was irrigated with saline and replanted immediately and splinted. Anti bacterial agent and anti inflammatory agent were prescribed. After 4 months of replantation slight external root resorption and apical radiolucency was seen at radiographic examination, therfore pulp extirpation and calcium hydroxide($Vitapex^{TM}$) canal filling were carried out. After 16 months, root canal was filled with gutta-percha, and bleaching treatment was done. Treatment results were satisfactory both esthetically and functionally for 8 years and 5 months.
