Individualized diabetes nutrition education improves compliance with diet prescription

  • This study was designed to evaluate the effect of individualized diabetes nutrition education. The nutrition education program was open to all type 2 diabetes patients visiting the clinic center and finally 67 patients agreed to join the program. To compare with 67 education group subjects, 34 subjects were selected by medical record review. The education program consisted of one class session for 1-2 hours long in a small group of 4~5 patients. A meal planning using the food exchange system was provided according to the diet prescription and food habits of each subject. Measurements of clinical outcomes and dietary intakes were performed at baseline and 3 months after the education session. After 3 months, subjects in education group showed improvement in dietary behavior and food exchange knowledge. In education group, intakes of protein, calcium, phosphorus, vitamin $B_2$, and folate per 1,000 kcal/day were significantly increased and cholesterol intake was significantly decreased. They also showed significant reductions in body weight, body mass index (BMI), and fasting blood concentrations of glucose (FBS), HbA1c, total cholesterol, and triglyceride. However, no such improvements were observed in control group. To evaluate telephone consultation effect, after the nutrition education session, 34 subjects of the 67 education group received telephone follow-up consultation once a month for 3 months. The others (33 subjects) had no further contact after the nutrition education session. Subjects in the telephone follow-up group showed a decrease in BMI, FBS, and HbA1c. Moreover, the subjects who did not receive telephone follow-up also showed significant decreases in BMI and FBS. These results indicated that our individually planned education program for one session was effective in rectifying dietary behavior problems and improving food exchange knowledge, and quality of diet, leading to an improvement in the clinical outcomes. In conclusion, our individualized nutrition education was effective in adherence to diet recommendation and in improving glycemic control and lipid concentrations, while follow-up by telephone helped to encourage the adherence to diet prescription.

흡입마취제 투여시 내독소혈증흰쥐 대동맥 수축반응에 미치는 Hydroxocobalamin의 효과 (Effect of Hydroxocobalamin on Contractile Responses to Phenylephrine during Administration of Inhalational Anesthetics in Lipopolysaccharide-Treated Rat Aortae)

  • 패혈증 환자에 흡입마취제를 투여하는 것은 혈역동학에 심각한 영향을 초래할 수 있다. 흡입마취제 투여시 내독소혈증흰쥐 대동맥 수축반응에 미치는 hydroxocobalamin의 영향을 알아보기위해, 내독소 (1.5 mg/kg)를 복강내 투여한 뒤 18시간 후에 대동맥을 적출하여 phenylephrine에 대한 수축반응과 이에 대한 흡입마취제 및 hydroxocobalamin의 작용을 알아보았다. 패혈증이 적절히 유발되었는 지를 확인하기 위해 RT-PCR을 이용하여 혈관 평활근에서 iNOS유도를 확인하였다. 내독소 처치에 의해 수축기 및 확장기 혈압이 대조군에 비해 유의하게 감소되었으나, 심박동수는 영향이 없었다. Phenylephrine에 대한 수축반응은 $10^{-8}\;M$부터 $10^{-5}\;M$까지 용량 의존적으로 증가했으며, 내독소혈증흰쥐 대동맥에서 수축반응이 억제되었다. halothane과 enflurane은 1 MAC 농도에서 phenylephrine에 의한 수축반응을 유의하게 억제했으나 isoflurane은 영향이 없었다. Hydroxocobalamin $(10^{-5}\;M)$은 흡입마취제의 종류 및 투여 유무에 관계없이 내독소혈증흰쥐 대동맥 수축반응을 증가시켰다. 이상의 결과로 미루어보아 hydroxocobalamin은 패혈증 환자의 흡입마취시 혈역동을 개선시킬 것으로 사료된다.

Folate Deficiency and FHIT Hypermethylation and HPV 16 Infection Promote Cervical Cancerization

  • Fragile histidine triad (FHIT) is a suppressor gene related to cervical cancer through CpG island hypermethylation. Folate is a water-soluble B-vitamin and an important cofactor in one-carbon metabolism. It may play an essential role in cervical lesions through effects on DNA methylation. The purpose of this study was to observe effects of folate and FHIT methylation and HPV 16 on cervical cancer progression. In this study, DNA methylation of FHIT, serum folate level and HPV16 status were measured using methylation-specific polymerase chain reaction (MSP), radioimmunoassay (RIA) and polymerase chain reaction (PCR), respectively, in 310 women with a diagnosis of normal cervix (NC, n=109), cervical intraepithelial neoplasia (CIN, n=101) and squamous cell carcinoma of the cervix (SCC, n=101). There were significant differences in HPV16 status (${\chi}^2=36.64$, P<0.001), CpG island methylation of FHIT (${\chi}^2=71.31$, P<0.001) and serum folate level (F=4.57, P=0.011) across the cervical histologic groups. Interaction analysis showed that the ORs only with FHIT methylation (OR=11.47) or only with HPV 16 positive (OR=4.63) or with serum folate level lower than 3.19ng/ml (OR=1.68) in SCC group were all higher than the control status of HPV 16 negative and FHIT unmethylation and serum folate level more than 3.19ng/ml (OR=1). The ORs only with HPV 16 positive (OR=2.58) or with serum folate level lower than 3.19ng/ml (OR=1.28) in CIN group were all higher than the control status, but the OR only with FHIT methylation (OR=0.53) in CIN group was lower than the control status. HPV 16 positivity was associated with a 7.60-fold increased risk of SCC with folate deficiency and with a 1.84-fold increased risk of CIN. The patients with FHIT methylation and folate deficiency or with FHIT methylation and HPV 16 positive were SCC or CIN, and the patients with HPV 16 positive and FHIT methylation and folate deficiency were all SCC. In conclusion, HPV 16 infection, FHIT methylation and folate deficiency might promote cervical cancer progression. This suggests that FHIT may be an effective target for prevention and treatment of cervical cancer.

4배성 휘커스(Ficus-4x)오디품종의 영양성분 및 항산화능 검색 (Study on the Nutritional Composition and Antioxidative Capacity of Mulberry Fruit(Ficus-4x))

  • 뽕나무 가운데 가장 큰 품종인 4배성 휘커스 오디품종(Ficus-4x)의 일반성분함량을 베리류에 속하는 몇 종류의 과일과 비교시 유사한 수준이었으나, 조회분 함량은 딸기, 복분자, 블루베리 등에 비해 높은 수준을 보였다. 비타민 C함량의 경우도 복분자와 블루베리 보다 높은 수준이었다. 무기질함량 분석결과 칼슘함량은 딸기와 유사한 수준이었으나, 블루베리에 비해서는 2배 이상 높았다. 특히 철분함량은 딸기, 복분자, 블루베리에 비해 월등히 높게 나타나 한방에서 보혈제로 사용해온 근거가 본 연구를 통해 입증되었다고 볼 수 있다. 물성적 특성의 경우 응집성, 검성, 씹힘성이 딸기에 비해 높게 나타나 저장성 면에서 딸기에 비해 우수할 것으로 생각된다. 색도 분석 결과 명도가 낮게 나타나 딸기보다 비교적 어두운 붉은색을 띠었고, 적색도, 황색도 또한 딸기에 비해 다소 낮은 결과를 나타내었다. 4배성 휘커스오디품종의 항산화능을 검색하고자 DPPH소거능 검색시 BHA를 기준(100%)으로 했을때 오디메탄올추출물은 35.7%로 cyanidin 3-glucoside의 40%의 소거능력을 보였다. 그리고 유지 저장시 항산화능을 알아본 결과 옥수수기름 저장 15일 이후부터는 오디의 에탄올추출물과 물추출물 모두에서 토코페롤과 BHA 보다도 안정한 경향을 나타내었다. 이는 최근 천연물에서 추출된 유리한 성분을 첨가물로 이용하려는 목적과 잘 일치된다고 할 수 있으며 유지의 유도기간을 연장하여 장기 보존을 위한 항산화제로서의 가치가 있다고 사료된다.

폐경후 여성들의 골밀도에 따른 영양섭취상태와 생활습관과의 관련성 연구 - 충남일부 지역을 중심으로 - (Association of Bone Mineral Density with Nutrient Intake and Lifestyles of Postmenopausal Women in Chungnam)

  • This study was peformed to assess the relationships among bone mineral density(BMD), nutrient intake and lifestyle factors in postmenopausal women. A trained nutritionist examined the interviewed usual dietary intake and physical activity of 85 subjects in Chungnam with a questionnaire interview, and BMDs of the lumbar spines($L_2-L_4$), femoral necks(FN), ward's triangles(WT) and trochanters(TR) were measured by dual energy X-ray absorptiometry (DEXA). The BMDs(T-score) of $L_2-L_4$, FN, and WT were $0.996g/cm^2(-1.601),\;0.697g/cm^2(-1.697)$, and $0.793g/cm^2(-1.512)$, respectively, which were assessed as osteopenia by T-score,. and TR was normal at $0.718g/cm^2(-0.675)$. Subjects included 85 in Chungnam were divided into three groups according to the BMD measurement of the $L_2-L_4$ and FN assessed by T-score. The percentages of the osteoporosis, osteopenia, and normal groups were $32.9\%,\;42.4\%$ and $24.7\%$ respectively. The average age was significantly the highest of the osteoporosis group than in the other two osteopenia or normal groups(P<0.001). Among lifestyle factors, the BMD of lifestyles was significantly related with exercise but it was not significant with medication., salt. intake, bone fracture, coffee consumption, drinking and smoking. The nutrient intake of the subjects was most nutrient intake adequate to the Korean RDA level for most nutrients excepting energy intakes and calcium. The BMD of lumbar spines $L_2-L_4$ showed significant positive association with the intake of most nutrients except carbohydrate, especially in the normal group. The Higher BMD levels were found for those with high intake of protein and vitamin B1. The BMDs of $L_2-L_4$, and FN were positively correlated with showed energy expenditure, exercise, miscellaneous activity, per weekend and week(P<0.05) in normal group but that was showed negatively correlated in the osteopenia group. Therefore, this study confirmed that one of the most effective ways to minimize bone less in postmenopausal women is to maintain an adequate intake of calcium and other nutrients and regular physical activity.

모발의 아연과 납 합량 : 학령 전 아동의 영양소 섭취 상태와 신장 및 체중과의 관계 (Hair Zinc and Lead: Relationship to Nutrient Intake and Height and Body Weight in Korean Preschool Children)

  • The objective was to evaluate the factors influencing on the growth of children aged 3 - 6 years by mini dietary assessment and anthropometric indices in 556 children and by analyzing mineral contents of hair from 273 children by inductively coupled plasma mass spectrometry (ICP-MS). The mean concentration of zinc (Zn) in hair was 68.9 $\pm$ 31.1 ppm and the zinc level of 91 % children was less than 60% of the reference value of 180 ppm. The mean content of lead (Pb) in hair was 1.76 $\pm$ 1.20 ppm, but only 67% of 273 children was within the reference value of 2 ppm and the lead level of 33% of children was 2 - 4 times higher than the reference value. The level of Zn was negatively correlated with Pb content (r = -0.305, p 〈 0.01). Hair Zn content, regardless of age, was significantly positively correlated with height (r = 0.214) and body weight (r = 0.159). The height of 95.3% children was belonged to the normal range of 90 - 109.9% of standard value and the body weight of 49.8% children was in the range of 90 - 109.9% of standard value. However, 8.1 % children was overweighted (110 - 119.9% of standard) and 9.4% children was obese (120 - 149.9% of standard) and 0.9% children was severe obese (〉 150%) which resulted in total 10.3% obese. Contrary to our expectation, 32.8% children was underweight (70 - 89.9% of standard) and 36% of the underweight children showed the low content of zinc (10 - 49 ppm) in hair. There was no significant correlation between dietary intake of Zn and hair Zn content. Hair Zn content was low even though children consumed greater than 75% of their requirement for zinc. Regardless of dietary intake of Zn, hair Zn level was rather negatively correlated to the content of Pb in hair. 38.8% children had an unbalanced diet which resulted in lower intake of calorie, Ca, Zn, vitamin B$_2$ compared with those consuming variety of foods. Therefore, it would be needed that new approach for nutrition education to improve eating habit having the unbalanced diet in preschool children. It might be also suggested to develop new food products to complement the nutrients of Zn, Ca, Fe as snacks or dietary supplement for Korean preschool children.

만성 폐쇄성 폐질환 환자에서 병기에 따른 영양상태 평가 (Nutritional Status of Chronic Obstructive Pulmonary Disease Patients according to the Severity of Disease)

  • The purpose of the study was to investigate nutritional status of chronic obstructive pulmonary disease (COPD) patients and to find out the differences according to the stages of disease. From March to October, 2006, 41 stable male patients of mild to severe COPD patients were recruited from Seoul National University hospital. The patients' of body weight and fat free mass were assessed by bioelectrical impedance analysis. The nutritional status of the patients was also assessed by 3-day recall, index of nutritional quality (INQ), dietary diversity score (DDS), dietary variety score (DVS), food group index pattern and dietary quality index (DQI). The total of 41 patients were classified into three groups, stage I, stage II and stage III groups according to the classification of Global Initiative for Chronic Obstructive Lung Disease (GOLD) standard. The mean age of the patients in each stage were 67.2-66.9 years showing no significant difference. The ratio of $FEV_1$/FVC were $57.5{\pm}7.3$, $46.9{\pm}7.6$ and $38.2{\pm}6.8%$, respectively showing significant differences according to the stages of disease. The fat free mass of the stage II ($48.2{\pm}4.7kg$) and III ($47.3{\pm}4.5kg$) was significantly lower than that of stage I ($53.1{\pm}6.9kg$) patients. There were significant correlation of fat free mass with $FEV_{1}$, and BMI (body mass index) with $FEV_{1}$/FVC ratio (p < 0.05). COPD patients showed the diet-related clinical symptoms of anorexia, dyspnea, dyspepsia, and chewing difficulty. Daily intakes of calorie, K, vitamin $B_2$ and folate of the patients were very low ($83.8{\pm}20.7%$, $58.9{\pm}14.4%$, $70.7{\pm}19.6%$ and $74.4{\pm}10.2%$, respectively) however, they did not significantly different according to the stages of disease. Daily intake of calcium was significantly lower in the stage III patients (p < 0.05). The mean scores of dietary variety score was significantly lower in the stage III patients (p < 0.001). Dietary quality index of the patients were not different among the stages of disease and the scores indicated poor quality of diet. As a summary, we found that body fat free mass, regularity of exercise, frequency of having snacks and dietary variety score were significantly associated with the severity of chronic obstructive pulmonary disease.

APo E3 Genotype 고지혈증 환자에서 사상체질에 따른 의학영양치료 혈중 지질 농도에 미치는 영향 (The Effects of Medical Nutrition Therapy on Plasma Lipid Levels of Apo E3 genotype hyperlipidemic Patients according to Sasang Constitutions)

  • 본 연구는 고지혈증 환자를 대상으로 apo E 유전자 다형성에 따른 사상체질의 분포양상을 살펴보고 apo E3 타입 고지혈증 환자에서 사상체질에 따른 의학영양 치료가 혈중 지질 농도에 미치는 영향을 살펴보았다. (1) 본 연구에 참여한 고지혈증 환자 33명으로 남자의 평균 연령은 $47.8{\pm}6.2$세였고, 여자의 평균 연령은 $50.5{\pm}5.7$세였다. apo E 유전자 다형성의 분포는 apo E2는 6.1%(2명), apo E3는 78.8%(26명), apo E4는 15.2%(5명)였고, 사상체질의 분포는 태양인이 0%(0명), 태음인이 60.6%(20명), 소양인이 21.2%(7명), 소음인이 18.2%(6명)였다. (2) 신체 계측 결과는 apo E3 타입 고지혈증 환자 중 남자의 경우는 신장, 체중, 체지방, 제지방, TBW, BMI, 허리둘레, 엉덩이둘레, 허리둘레/엉덩이둘레 비율 모두에서 MNT 실시 전과 후의 변화가 나타나지 않았다. 여자의 경우는 소양인에서 체중, BMI가 MNT 전과 비교하여 후에 유의적인 감소가 있었고, 태음인과 소음인에서는 MNT 실시 전과 후의 변화가 나타나지 않았다. (3) 영양소 섭취 상태는 남자의 경우 태음인에서 총열량, 지방, 동물성 지방, 식물성 지방, 섬유소, 비타민 $B_1$, 비타민 $B_2$, niacin의 섭취가 유의적으로 감소하였고 소양인에서는 섭취량의 변화가 없었으며, 소음인에서는 인의 섭취가 유의적으로 증가하였다. 여자의 경우 태음인에서 칼슘, 인의 섭취가 유의적으로 증가하였고, 소양인과 소음인에서는 섭취량의 변화가 없었다. MNT 실시 전과 후의 영양소 섭취 상태는 총열량의 섭취량이 처방열량에 적절하게 개선되었고, MNT 후에 태음인에서는 칼슘, 철분, 비타민 A의 섭취가 개선되었으며, 소음인에서는 단백질, 비타민 A의 섭취가 개선되었다. 식사 전반의 질은 태음인, 소양인, 소음인 모두에서 잘 유지되었으나 식품 섭취 패턴을 조사한 결과는 콜레스테롤이 많은 음식, 소고기, 돼지고기 등 육류, 단음식, 튀긴 음식의 섭취 빈도에서 변화가 관찰되지 않았다. (4) 혈중 지질 농도의 변화는 태음인에서 HDL-콜레스테롤의 농도는 유의적으로 증가하였고, 호모시스테인의 농도는 유의적으로 감소하였다. 소양이네서 HDL-콜레스테롤의 농도가 유의적으로 증가하였다. 소음인에서는 혈중 지질 농도에서 유의적인 변화가 없었다.

2020 한국인 영양소 섭취기준: 리보플라빈 (2020 Dietary Reference Intakes for Koreans: riboflavin)

  • 리보플라빈 (비타민 B2)은 한국인에게 섭취 부족의 우려가 있는 비타민으로서 에너지 대사를 포함한 산화/환원 반응의 조효소로서 작용한다. 적절한 섭취수준을 평가하는 방법으로는 적혈구에서의 리보플라빈 농도나 EGRAC, 혹은 소변의 리보플라빈 농도를 측정하는 것이다. 적혈구 리보플라빈 수준은 400 nmol/L (15 ㎍/100 mL) 이상을 적정수준, 270 nmol/L (10 ㎍/100 mL) 이하를 결핍으로 하거나 EGRAC 값이 1.2 이하인 경우 적정수준, 1.4이상이면 결핍으로 판정한다. 2020 한국인 영양소 섭취기준 개정 시 19-64세 성인의 2020 리보플라빈 평균필요량은 남자 1.3 mg/d, 여자 1.0 mg/d로 설정되었으며, 64-74세 남자 1.2 mg/d, 여자 0.9 mg/d, 75세 이상 남자 1.1 mg/d, 여자 0.8 mg/d로 성인 보다 낮게 설정되었다. 2020 국민건강영양조사 결과 19세 이상 우리나라 성인의 리보플라빈 평균 섭취량은 1.69 mg/d이며, 권장섭취량 대비 124.9% 였고, 보충제 섭취자들의 보충제로부터의 리보플라빈 섭취량은 평균 10.26 mg/d로 식품으로부터 섭취하는 1.50 mg에 비해 약 6.8배 높은 수준이었다 [18]. 2020 한국인 영양소 섭취기준에서 연령별, 성별 리보플라빈 권장섭취량은 각 생애주기 구간별 평균필요량에 변이계수 10%를 적용하여 평균필요량의 120% 수준으로 설정되었다 리보플라빈의 대표적 식품 급원으로는 유제품, 난류, 육류, 가금류와 생선류의 동물성 식품과 두류, 녹색채소류, 곡류 등이 있으며 2020 국민건강영양조사에 따르면 한국인의 리보플라빈 급원 식품으로 기여도가 높은 식품은 달걀, 라면, 돼지고기, 우유, 간장, 쇠고기, 배추김치, 닭고기, 고추가루, 시리얼 순이다. 최근 리보플라빈의 경우 비타민 보충제 외에도 다양한 건강기능성 식품섭취로 인해 보충제를 섭취하는 사람들의 경우 보충제로부터의 섭취수준이 식품으로부터의 섭취량을 훨씬 초과하고 있어 리보플라빈 영양상태 평가 시 보충제로부터 섭취하는 수준에 대한 평가가 향후 반드시 필요하다. 또한 2020 한국인 영양소 섭취기준 개정에서는 만성 질환 예방에 대한 부분은 아직 과학적 증거의 불충분으로 고려되지 않았으나 향후 만성질환과 관련된 역학연구 및 중재연구가 더 필요하다고 판단된다.

묵은 김치 제조과정에서의 이화학적 및 미생물학적 특성변화 (Changes in Physicochemical and Microbiological Properties in Low-Temperature and Long-Term Fermented Kimchi during Fermentation)

  • To develop the low-temperature and long-term fermented kimchi, kimchi was prepared according to the recipe of a specific ratio of major and minor ingredients and adjusted its salinity to 3.7%. Prepared kimchi fermented at $15{\pm}1^{\circ}C$ for 24 hours and transferred and fermented in a refrigerator only used to make low-temperature and long-term fermented kimchi at $-1{\pm}1^{\circ}C$ for 30 weeks. During 30 weeks of fermentation the changes in physicochemical and microbiological properties of low-temperature and long-term fermented kimchi were studied. The initial pH of 6.47 decrease gradually and dropped to pH 4.0 after 14 weeks of fermentation, and then it maintained at same level. Acidity increased to 0.49% on 2 weeks of fermentation and kept at 0.47 $\sim$0.50% during 2 to 30 weeks fermentation. Salinity was slightly increased at early stage and started to decrease on 4 weeks of fermentation, and then it did not change. The change of reducing sugar content was closely related to the trend of pH change with a very high correlation coefficient(r =0.912). Lactic acid, citric acid, malic acid, succinic acid and acetic acid were major organic acids contained in low-temperature and long-term fermented kimchi. Vitamin C content decreased at initial stage of fermentation and then slightly increased up to the maximum of 22.3 mg% on 8weeks of fermentation. In color measurement, L value continued to increase during the fermentation and reached at the highest of 55.45 on 22 weeks of fermentation, and a and b values of 3.62 and 4.54 also increased to 31.26 and 37.32 on 30 weeks of fermentation, respectively. Total microbial count increased slowly from beginning and was the highest on 4 weeks of fermentation, and then began to decrease slowly. Count of Lactobacillus spp. was highest after 6weeks, but count of Lactobacillus spp. was highest on 2 weeks of fermentation, and then both showed a slow decrease. Yeast count wasn't increased until 4 weeks of fermentation and then increased rapidly to get the highest on 10 weeks of fermentation.
