The Characteristics related to Pulmonary Rehabilitation in Patients with Chronic Obstructive Pulmonary Disease: A Cross-sectional Study, Data from the Korea National Health and Nutrition Examination Survey 2015-2019.

  • Objective: Patients with chronic obstructive pulmonary disease (COPD) may experience reduced physical activity and quality of life (QoL) due to decreased pulmonary function. The purpose of this study was to investigate the level of pulmonary function, physical activity, and QoL of COPD patients. Design: Cross-sectional observational study. Methods: This study examined the published data of the Korea National Health and Nutrition Examination Survey in 2015-2019. Among 39,759 subjects who participated for 5 years, data from 151 patients diagnosed with COPD were analyzed separately. For the pulmonary function, the results of forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV6, forced expiratory flow 25-75%, and peak expiratory flow were observed. Physical activity was identified as frequency and duration. For the QoL, EQ-5D-3L evaluation results were examined, and the frequency and index of the Korean version were investigated. Results: In pulmonary function, all variables were found to be lower than age and weighted matched normal values. COPD patients showed to perform very low levels of high/medium physical activity and sitting time was confirmed to be more than 8 hours a day. In QoL, it was found that the highest reporting rate of some problems was the "pain and discomfort" and "mobility". Conclusions: It was found that COPD patients showed that the prevalence of circulatory disease was relatively high, lowered pulmonary function, and QoL. These can be improved through regular physical activity, and it is thought that this can be achieved through optimization of pulmonary rehabilitation.

The Effect of Shoulder Stabilization Exercise and Core Stabilization Exercise on the Shoulder Height and Respiratory Function in Young Adults with Round Shoulder Posture

  • PURPOSE: The purpose of this study was to compare how the shoulder height and respiratory function are affected by applying shoulder stabilization exercises and core stabilization exercises that are effective for strengthening the trunk muscles and postural stability for adults with a round shoulder posture (RSP). METHODS: The participants were 28 young adults with RSP. They were assigned randomly to two groups: shoulder stabilization exercise and core stabilization exercise. They performed the exercises for 30 minutes twice a week for four weeks. They measured the shoulder height and respiratory function before and after exercise. RESULTS: No significant difference in shoulder height was found between the groups. A significant decrease in shoulder height was found in the shoulder stabilization exercise group after exercise. The core stabilization exercise group showed a significant decrease after exercise. In respiratory function, no significant difference was found between the groups. The forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were increased significantly in the shoulder stabilization exercise group before and after exercise. The FEV1, FEV1/FVC, and peak expiratory flow were significantly higher in the shoulder stabilization exercise group after exercise than in the core stabilization exercise group. CONCLUSION: Shoulder stabilization exercise and core stabilization exercise improved the postural alignment and pulmonary function, and the exercises could be helpful in shoulder rehabilitation as well as the clinical part of the treatment of rounded shoulder posture.

PNF의 어깨뼈 골반 대칭 상호 패턴이 노인의 호흡기능과 삶의 질에 미치는 효과 (The Effects of Scapula and Pelvis Symmetrical Reciprocal Pattern of PNF Concept on Respiratory Function and Quality of Life in the Elderly)

  • Purpose : The purpose of this study is to determine the effects of scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept on respiratory function and quality of life in elderly subjects. Methods : Nineteen elderly subjects with healthy were recruited. Subjects performed scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept. Exercise was appied 30 minutes three times per week for four weeks. The respiratory function and quality of life (QOL) test three times (before, two weeks, and four weeks). Respiratory function includes forced vital capacity (FVC), forced expiratory volume at one second (FEV1), Peak Expiratory Flow (PEF) and chest cage expansion test (CCET). The QOL test was measured Korean WHOQOL-BRIEF. The analysis method was analyzed through the one-way ANOVA repeated methods, and the statistical significance is α=.05. It was analyzed through the post test Bonferroni test. Results : After the 4 week scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept showed statistically significant differences in the respiratory function (FVC, FEV1, PEF, and CCET), and QOL (p<.05). As a result of the post-hoc test, FVC showed a significant increase in the mid test and post test compared to the pre test (p<.05), FEV1 showed a significant increase in the post test compared to the pre test (p<.05). PEF showed a significant increase in all pre-test, mid-test, and post-test sections (p<.05), CCET showed a significant increase in all pre-test, mid-test, and post-test sections (p<.05). QOL showed a significant increase in all pre-test, mid-test, and post-test sections (p<.05). Conclusion : In this study, the scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept improved respiratory function and QOL. The findings suggest that this intervention could be beneficial in improving respiratory function and QOL in the elderly.

  • Background: This study aimed to determine the effects of training on breathing re-education on pain and dysfunction levels, posture, quality of life (QoL), and pulmonary function in patients. Methods: This study included 34 patients with chronic neck pain and upper thoracic breathing pattern were included. The participants were assigned to either the routine physical therapy program (RPTP) (CG; n=17) or RPTP and breathing pattern re-education training (EG; n=17). The CG and EG groups performed RPTP for 40 minutes, and only the EG groups performed breathing re-education training for 10 minutes. Exercises were performed thrice weekly for both groups. Level of pain and dysfunction, posture, QoL and pulmonary function status were assessed before and after the intervention. Results: After four weeks of intervention, numeric pain rating scale (NPRS), Korean version of neck disability index, cranio-vertebral angle, cranial rotation angle, and 12-item short form health survey-physical and mental component summaries had significant differences before and after intervention in both groups (p<.01). However, only NPRS, forced vital capacity, forced expiratory volume in 1s, and maximum voluntary ventilation showed significant interactions between the two groups and measurement time (p<.01). Conclusion: Breathing re-education training and RPTP may be optimal for patients with chronic neck pain and may be more effective in improving neck pain and pulmonary function.

Effects of Inspiratory Training on Respiratory Function and Balance in Stroke Patients: A Randomized Controlled Trial

  • Objective: The aim of this study is to explore how using inspiratory training affects the respiratory function and balance of stroke patients. We also plan to compare the results with a control group that does not receive the intervention. Design: A Randomized Controlled Trial Methods: In this study, 27 chronic stroke patients were randomly assigned to either a control group (n=14) or an experimental group (n=13). Both groups underwent six weeks of common interventions involving standard physiotherapy and treadmill training. Additionally, the experimental group received inspiratory training. Respiratory function and balance were evaluated using Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 second (FEV1), Maximal Inspiratory Pressure (MIP), Maximal Expiratory Pressure (MEP), Peak Expiratory Flow (PEF), Five times Sit-to-Stand (FTSTS), Seated Center of Pressure (S-COP), and Timed Up and Go (TUG) tests. Results: Respiratory function and balance were compared within each group before and after intervention. The experimental group, which received inspiratory training, showed significant improvements in FVC (0.26±0.18), FEV1 (0.35±0.32), MIP (11.54±12.39), PEF (1.12±1.52), and TUG (-3.39±2.45) compared to pre-intervention values (p<0.05). When comparing changes between groups post-intervention, the experimental group demonstrated significant increases in FVC, FEV1, MIP, PEF, and TUG compared to the control group (p<0.05). However, there were no significant differences in MEP, FTSTS, and S-COP. Conclusions: The results of this study indicate a positive effect of inspiratory training on chronic stroke patients. These findings suggest that with further research involving a larger sample size and enhanced intervention methods, inspiratory training could be employed positively in the rehabilitation of stroke patients.

The Reliability and Validity of a Portable Hand-held Spirometer for the Measurement of Various Lung Functions in Healthy Adults

  • Objective: This study aims to assess the reliability and validity of the new hand-held spirometer as a potential substitute for traditional pulmonary function testing (PFT) devices. Design: Cross-sectional study. Methods: In this study, thirty healthy adults underwent spirometry using both the new hand-held spirometer and the MIR spirometer, which is a standard PFT device. Parameters including peak expiratory flow (PEF), forced expiratory volume in one second (FEV1), and forced vital capacity (FVC) were measured and analyzed for validity and reliability. Inter-rater reliability and validity were evaluated through 95% limits of agreement (LOA) and intraclass correlation coefficients (ICC). Statistical analyses, including the Bland-Altman plots and the ICC, were utilized to assess agreement between the two devices. Results: The new hand-held spirometer exhibited a good agreement with intra-class coefficient (ICC [2,1]) ranging 0.762 to 0.956 and 95% LOA of -1.94 to 1.80 when compared with MIR. The test-retest reliability of the hand-held spirometer analyzed using - ICC [2,1] demonstrated a good level of consistency (ICC [2,1] =0.849-0.934). Conclusions: In conclusion, the study aimed to assess the potential of the new hand-held spirometer as a viable alternative to traditional PFT devices, with a specific focus on its reliability and validity in spirometric measurements. The new hand-held spirometer exhibited good test-retest reliability across all measured variables, suggesting its potential as a valid and reliable tool for simultaneous PFT measurements.

동적 신경근 안정화 운동이 노인들의 폐기능과 뇌파에 미치는 영향 (The Effect of Dynamic Neuromuscular Stabilization Exercise on Pulmonary Function and Electroencephalogram in Elderly)

  • Purpose : After coronavirus disease 2019 pandemic, many people around the world became aware of respiratory diseases and developed increased interest in health and exercise. As the elderly population grows larger, the prevalence of brain diseases and respiratory conditions increases. This study aimed to investigate the impact of dynamic neuromuscular stabilization (DNS) exercises on pulmonary function and electroencephalogram (EEG) findings in the elderly population. Methods : In this study, we measured the pulmonary function and performed EEG of 30 elderly individuals. The participants were randomly divided into an experimental group (n=15) and a control group (n=15). The experimental group completed a DNS exercise training protocol, while the control group performed simple exercise training. After the exercise, the pulmonary function and EEG testing were repeated, and the amounts of change within and between groups were determined. Results : In terms of pulmonary function, the experimental group's forced vital capacity (FVC) and forced expiratory volume at 1 second (FEV1) increased significantly after the exercise (p<.05), while the FVC differed significantly between groups (p<.05). In terms of EEG findings, the relative alpha waves significantly decreased in the F3 region (p<.05), while the relative beta waves significantly increased in the F4 region (p<.05). The relative gamma waves increased significantly in the Fp2 and P3 regions (p<.05), while the values in the F3 region differed significantly between groups (p<.05). Conclusion : DNS exercises performed by this elderly population demonstrated significant clinical value by providing fundamental data about both pulmonary function and EEG activity.

소아 천식에서 호기산화질소와 폐기능 검사의 관계 (Relationship between exhaled nitric oxide and pulmonary function test in children with asthma)

  • 목 적 : 천식의 특징은 기도염증의 결과인 가역적인 기류제한과 기관지과반응성이다. 최근에 호기산화질소(exhaled nitric oxide; FeNO)이 비침습적으로 기도염증을 알 수 있는 지표로서 이에 대한 많은 연구가 있었다. 또한 FeNO는 천식의 중증도를 나타내는 데 이용되는 객담내 호산구수나 메타콜린 기도유발시험과 관련이 있다고 보고되었다. 이에 저자들은 간헐성 천식환자에서 폐기능검사와 호기산화질소와의 관계를 알아보고자 본 연구를 시행하였다. 방 법 : 2005년 4월부터 8월까지 경희의대부속병원 소아과에서 천식으로 진단되어 폐기능검사와 호기산화질소 측정이 가능하였던 5세부터 15세의 80명의 환자를 대상으로 하였다. 최근 4주내에 상기도 감염이 없었고, 부신피질스테로이드를 포함한 조절약물을 사용하지 않은 경우에 연구 대상에 포함하였다. 폐기능검사는 Microspirometer를 사용하였다. Eco Medics사의 Chemiluminescence NO-analyzer(CLD 88 sp, Duernten, Switzerland)를 사용하여 FeNO를 측정하였으며 폐기능 검사와 FeNO와의 상관관계 분석은 Spearman correlation coefficient를 사용 하였다. 결 과 : 1초간 강제호기량(forced expiratory volume in 1 second, $FEV_1$)의 평균은 $0.890{\pm}0.455L$이었으며 강제폐활량 (forced vital capacity; FVC)의 평균은 $1.071{\pm}0.630L$이었다. $FEV_1%pred$의 평균은 $98.39{\pm}34.27%$였으며 $FEV_1/FVC$의 평균은 $88.53{\pm}19.49$이었다. FeNO의 평균치는 16.88 parts per billion(ppb)이었다. FeNO 측정치는 $FEV_1$(r=0.345, P<0.01), FVC(r=0.244, P<0.05)와 유의한 상관관계를 보였으나 $FEV_1%pred$$FEV_1/FVC$는 FeNO와 유의한 상관관계를 나타내지 않았다. 결 론 : FeNO의 측정은 폐기능 검사와 함께 소아천식 환자의 관리에 유용하게 사용될 수 있는 신빙성있는 검사로 생각되며 측정 방법이나 측정치의 참고치 설정에 앞으로 많은 연구가 필요할 것으로 생각된다.

노인환자의 기도가역성 검사에서 FEV6의 보완적 지표로서의 역할 (The Complementary Role of FEV6 in Bronchodilator Reversibility Test for the Old Age)

  • 연구배경: 기도가역성 측정시, 1초간 노력성 호기량(forced expiratory volume in one second, $FEV_{1}$)과 노력성 폐활량(forced vital capacity, FVC)은 흔히 사용되는 지표인데 양성반응의 권장기준은 기저치보다 증가치가 절대량이 200ml이상 그리고 백분율로는 12%이상의 두 조건을 다 만족하는 것이다. 그러나 노인 환자는 천식에 합당한 병력을 가지고 있어도 기도가역성의 진단기준을 만족시키지 못하는 경우가 종종 있다. 저자들은 6초간 노력성 호기량($FEV_{6}$)이 노인의 기도가역성 검사에서 역할을 알아보고자 하였다. 방 법: 65세 이상의 노인환자 중 폐기능검사에서 $FEV_{1}$/FVC가 80%미만인 236명에게 폐활량측정법으로 기도가역성 검사를 시행하였다. 기저 $FEV_{1}$에 따라 다음과 같이 세 군으로 분류하였으며 각 군에서의 FVC, $FEV_{6}$, $FEV_{1}$의 지표에 따른 기도가역성를 비교분석하였다. 제1군 : $$FEV_{1}{\geq_-}80%$$, 제2군 : $FEV_{1}$ 60~80%, 제3군 : $$FEV_{1}{\leq_-}60%$$ 결 과: 전체 환자에서 기도가역성은 각각의 지표에 따라 $FEV_{1}$에서는 33명(14.0%), $FEV_{6}$는 55명(23.3%), FVC는 55명(23.3%)로 양성반응을 보였고, 제3군($$FEV_{1}{\leq_-}60%$$)에서는 $FEV_{1}$을 지표로 할 때는 15명(22.4%), $FEV_{6}$는 30명(44.8%), FVC는 32명(47.8%)으로 나타났다. 결 론: 본 연구에서는 $FEV_{6}$가 노인환자에서 기도가역성 검사의 새로운 보완적 지표로서의 역할을 보여줬지만 이의 일반화를 위해서는 좀 더 많은 연구가 필요할 것이다.

폐활량측정법의 새로운 정상예측식이 폐활량측정법 장애 양상 및 질병 중증도 해석에 미치는 영향 (Effect of a New Spirometric Reference Equation on the Interpretation of Spirometric Patterns and Disease Severity)

  • 연구 배경: 폐활량측정법 정상예측식이 한국인을 대상으로 개발되었다. 한국인 정상예측식을 실제 진료에 사용하기 위해서 그 동안 많이 사용하던 정상 예측식 중 하나인 Morris 예측식을 적용하였을 때와 한국인 예측식을 적용하였을 때 장애 양상 해석 및 질병 중증도 평가가 어떻게 달라지나 비교하고자 하였다. 방 법: 서울아산병원의 호흡기검사실에서 2004년도 11월 한 달간 폐활량측정법을 시행한 남자 926명과 여자 694명을 대상으로 하였다. 나이, 성, 키, 몸무게, 그리고 폐활량측정법으로 $FEV_1$ [forced expiratory volume in one second], FVC [forced vital capacity], $FEV_1/FVC$ 등을 구하였다. 한국인 예측식과 Morris 예측식을 사용하여 장애 양상 해석과 질병 중증도 평가를 하였고 그 차이를 비교하였다. 폐활량측정법 장애 양상은 $FEV_1/FVC$과 FVC 값에 따라서 정상, 제한성, 폐쇄성, 양상 미정 등으로 정의하였고 질병 중증도는 기류제한이 있는 환자는 $FEV_1$ 값에 따라서 기류제한이 없는 환자는 FVC값에 따라서 정의하였다. 결 과: Morris 예측식에서 한국인 예측식으로 바꾸어 적용하면 장애 양상 해석이 남자 환자의 경우 22.5% (208/926) 달라졌고 여자의 경우 24.8% (172/694) 달라졌다. 한국인 예측식을 적용하였을 때, 기류제한이 있는 환자의 경우 질병의 중증도가 남자에서 30.2% (114/378) 바뀌었고 여자에서 39.4% (37/94) 바뀌었다. 기류제한이 없는 환자의 경우는 질병의 중증도가 남자에서 27.9% (153/548) 바뀌었고 여자에서 30.2% (181/600) 바뀌었다. 결 론: 폐활량측정법 정상예측식이 바뀌면 장애 양상 해석과 질병 중증도 평가에 유의한 영향을 미칠 수 있다.
