Muscle Activities in the Lower Limbs for the Different Movement Patterns on an Unstable Platform

  • We performed experimental studies on the muscle activities in the lower limbs for the different movement patterns on an unstable platform. A training system for postural control using an unstable platform that we previously developed was applied for the experiments. This unstable platform provides 360 degrees of movement allowing for training of posture in various directions and provides simultaneous excitations to visual sensory, somatic sensation and vestibular organs. Compare with the stable platform, keeping body balance on the unstable platform requests more effective sensation from vision, vestibular sense and somatic sense. Especially, the somatosensory inputs from the muscle proprioceptors and muscle force are crucial. To study the muscle activities for the different movement patterns and find the best training method for improving the ability of postural control through training and improving the lower extremity muscular strength, fifteen young healthy participants went through trainings and experiments. The participants were instructed to move the center of pressure following the appointed movement pattern while standing on the unstable platform. The electromyographies of the muscles in the lower limbs were recorded and analyzed in the time and the frequency domain. Our experimental results showed the significant differences in muscle activities for the different movement patterns. Especially, the spectral energy of electromyography signals in muscle for the movement pattern in anterior-posterior direction was significantly higher than those occurred in the other patterns. The muscles in the lower leg, especially tibialis anterior and gastrocnemius were more activated compared to the others for controlling the balance of body on the unstable platform. The experimental results suggest that, through the choice of different movement pattern, the training for lower extremity strength could be performed on specific muscles in different intensity. And, the ability of postural control could be improved by the training for lower extremity strength.

The Results of the Endoscopic Decompression for the Treatment of Haglund's Disease (Haglund씨 병에서 시행한 내시경적 감압술의 결과)

  • Purpose: Haglund's disease represents a painful heel caused by mechanically induced inflammation of the retrocalcaneal bursa and insertional Achilles tendinosis may coexist. Traditional open surgery can cause complications such as skin breakdown, painful scar and altered sensation around the heel. Endoscopic treatment offers the advantages that are related to minimally invasive procedure and we evaluate the clinical results and operative techniques of endoscopic decompression of retrocalcaneal space for Haglund's disease. Materials and Methods: Our retrospective study included seven heels in six consecutive patients for which nonoperative treatment had failed and endoscopic decompression was performed. The mean age was forty-one years (range, 28 to 53 years). All of the patients had typical complaints of inflammation of the retrocalcaneal bursa and Fowler-Philip angle of more than $75^{\circ}$ and positive parallel pitch lines were present on the lateral calcaneal radiograph. The endoscopic procedure consists of the resection of inflamed retrocalcaneal bursa and enough bone to prevent impingement of the bursa between the calcaneus and Achilles tendon. All patients were evaluated with radiologic angle, visual analogue scale (VAS) for pain and Ogilvie-Harris functional score. The mean follow-up was 18 months (range, 15 to 21 months). Results: The mean operation time was 61 minutes (range, 50 to 85 minutes). VAS for pain and Fowler-Philip angle were decreased from preoperative 8.7 and $82^{\circ}$ to postoperative 2.3 and $57^{\circ}$, respectively. One patient with ankylosing spondylitis had a fair result, 2 patients had good results and the remaining 3 patients had excellent results according to Ogilvie-Harris functional score. There were no surgical complications such as infection, Achilles tendon avulsion or abnormal heel sensation. Conclusion: The endoscopic decompression for Haglund's disease was demonstrated to have several advantages including low morbidity, allowance of functional rehabilitation, short recovery time and quick sports resumption. However a comparative study is needed to determine the value of endoscopic decompression and particular caution should be exerted for the enthesiopathy.

Development of Concrete Sensory Information for Patients Undergoing Cardiac Catheterization (심도자 검사 환자를 위한 구체적 감각 정보의 개발)

  • Cardiac catheterization is a diagnostic procedure which is intrusive and anxiety provoking. Patient education and information offer have been suggested as effective nursing interventions to reduce patients' anxiety and stress. Main objectives of this study are : 1) to develop concrete objective information for patients undergoing cardiac catheterization : 2) to analyze the types of information included in the newly developed concrete objective information. The subjects of this study are 11 patients who were admitted to the CCU of a general hospital in Seoul. The subjects were provided with preparatory information about cardiac catheterization by using interview and a booklet. After the procedure, the subjects were asked to describe additional information which they suggest to be added into the booklet and the physical sensations felt during the procedure. The results of the study are summarized as follows : Most subjects were satisfied with the preparatory information that was provided by interview and the booklet before the procedure. But patients suggested several problems related to the content of the booklet. First, they reported difficulty to differentiate the terms -coronary angio gram and cardiac catheterization. Also, some patients expressed that they feared after reading the information about the incision of inguinal area. Subjects responded that the information about the direct process of the test did not reduce their anxiety and the information was not detail enough. Next, most subjects would want to know about the monitor and the sound from monitor. They said that they could not hear instructions from doctor or nurse during the procedure due to tension. Considering above response results, the need for more effective way to provide information, like visual and auditory information through video tape for giving information is suggested. Sensations related to the procedure were the smell coming from sterilization of inguinal area, stinging pain in groin when the doctor inserts a needle into artery, and the sensation of pressure and moving of vessels surrounding neck when the catheter was inserted and visualized on fluoroscopy. Besides, subject reported hot sensation and burning feeling in face and chest area, and nausea when dye is injected by hand. In the analysis of information content, there was 79% agreement on the actual units of analysis that were coded. In the analysis of type of information, procedural information was 60.4%. Concrete objective information was 28.1%. and other information was 11.5%. Agreement of the coders in categorizing the units of information was determined by using Cohen's kappa which corrects for chance agreement. Cohen's kappa was .84.

Credibility of a Newly Developed Sham Moxibustion (피부 열전도 온도에 근거를 둔 거짓 뜸 개발 및 평가 연구)

  • Background : To demonstrate the efficacy of moxibustion therapy, randomized controled trials are required. But, clinical trials of moxibustion had limitations due to the absence of a sham moxibustion model for an appropriate placebo. Objectives : To develop a new sham moxibustion model based on the thermal characteristics of commercial indirect moxibustion, especially temperature, and to evaluate whether it could be applied in clinical trials. Methods : By applying heat insulation, we created a sham moxibustion device that was indistinguishable from a real one with the naked eye. It also stimulated heat but had inert remedial value. A clinical trial was performed on subjects to test double blinding. The subjects were randomly assigned into two groups, a treatment group and a sham group. Acupoint Zusanli($ST_{36}$) was used in each group for 3 times. A sham acupuncture credibility questionnaire was modified into a moxibustion credibility questionnaire and was filled out after treatment. Results : No major difference was detected in the subjects' baseline data. Most subjects and practitioners could not distinguish the sham moxibustion device from the real one. But, subjects who had experience of moxibustion therapy more likely to distinguish the sham moxibustion device from the real one than subjects who didn't have experience of moxibustion therapy. The treatment group showed a significant difference in the VAS(Visual Analog Scale) for intensity of sensation during treatment than that of the sham group. Conclusions : The sham moxibustion device in this study is proved sufficient and credible to be applied in investigations of the effect of moxibustion. But it is more appropriate for the people who don't have experience of moxibustion therapy.

A comparative analytical study of the industrial patterns of baggy pants - Focused on men in their early twenties - (배기팬츠의 산업패턴 비교·분석 연구 - 20대 초반 남성을 중심으로 -)

  • This study aims to give practical help to the pattern making of baggy pants produced in large quantity from industrial fields. The functionality of crotch part is a key element to decide wear sensation and comfort of pants as having direct influence on movements of legs in pattern designing of baggy pants. The industrial patterns of baggy pants in various styles that are released currently in Korea were collected and a comparative analysis was conducted on them for the thesis. First, samples of men baggy pants were collected from three different brands and compared the completion sizes. Secondly, it studied the most appropriate patterns as quantifying the appropriateness of movements through fitting tests. Finally, the study verified the differences of visual images by surveys of professional groups. The conclusions obtained from this research are as follows. In terms of aesthetic attribute among the three brands, A baggy pants, which had a small size tolerance, were the most appropriate. Meanwhile concerning the appropriateness of movement, B baggy pants were shown to be the most appropriate. This reveals explicitly the problem of aesthetic attribute and functionality, which are always in conflict during the pattern design. It is necessary to develop patterns and establish a measurement system that may satisfy both of these factors.

Clinical Characteristics and Evaluation of Yin-deficiency Syndrome in Patients with Burning Mouth Syndrome (구강작열감증후군 환자의 임상적 특징 및 음허증 평가)

  • Objectives : This study was designed to investigate the clinical characteristics and usefulness of comprehensive diagnosis of Yin-deficiency and heart rate variability in patients with burning mouth syndrome (BMS). Methods : We surveyed 30 burning mouth syndrome patients visiting the Oral Diseases Clinic of Kyung Hee University Oriental Medicine Hospital from April to September of 2011. The subjects were evaluated on self-assessed severity of burning mouth syndrome and xerostomia using visual analogue scale (VAS) score and Yin-deficiency condition (based on the 10-item Yin-deficiency questionnaire). Salivary function was measured by the unstimulated salivary flow rate (USFR), and heart rate variability (HRV) parameters were recorded by SA-2000E (Medicore Co., Ltd., Korea). Results : There were substantial significant positive correlations between burning sensation VAS scores in mouth and Yin-deficiency scores. There was significant negative correlation between xerostomia VAS score and USFR. Compared to the normal range of total power (TP) in HRV parameters, the burning mouth syndrome patients showed significant lower values of TP. Conclusions : The results of this study suggest that comprehensive diagnosis of Yin-deficiecny and HRV parameters are useful in diagnosing of burning mouth syndrome patients. Therefore, we assume that improvement of Yin-deficiency condition can be a potentially effective way to treat burning mouth syndrome.

The Use of Locally Applied Vibration to Minimize Pain during Fractional CO2 Laser Therapy in Living Liver-Donor Scar Management

  • Background Fractional $CO_2$ laser is an effective treatment for scars, but most patients complain about sharp burning pain, even after the application of lidocaine ointment. This study analyzed the impact of a vibrating device to nonpharmacologically reduce the acute pain of laser treatment, in accordance with the gate control theory of pain management. Methods This is a prospective study performed from May 2013 through March 2014. Fifty-three patients (mean age, 26.7 years; range, 16-44 years) who had donated livers for liver transplantation were treated with a fractional $CO_2$ laser (10,600 nm; model $eCO_2$, Lutronic Corp) for their abdomen scars. Laser treatment was applied 4 months after surgery. A commercially available, locally applied vibrating device (model UM-30M, Unix Electronics Co. Ltd.) was used, in an on-and-off pattern, together with the $CO_2$ laser. A visual analogue scale (VAS; 0, no pain; 10, most severe pain) of pain sensation was assessed and statistically analyzed using a paired t-test. Results The average VAS score for pain with the vibrating device was 4.60 and the average VAS score without the vibrating device was 6.11. The average difference between scores was 1.51 (P=0.001). Conclusions A locally applied vibrating device was demonstrated to be effective in reducing pain when treating with a fractional $CO_2$ laser. Vibration treatment could be helpful when treating scars with fractional $CO_2$ laser in pain-sensitive patients, particularly children.

A clinical study of two patients on vertigo with Banhabaekchulcheonma-tang gamibang (반하백출천마탕가미방(半夏白朮天麻湯加味方)을 이용한 현훈(眩暈) 치험2례(例))

  • Objectives : Vertigo is a sensation of spinning while stationary. It is commonly associated with nausea or vomiting, unsteadiness (postural instability), falls. It can affect a person's thoughts and life style. Recurrent episodes in those with vertigo are common and frequently impair the quality of life. The purpose of this study was to report the clinical effects of herbal medicine on vertigo.Methods : We employed oriental medical treatments; herbal-medication (Banhabaekchulcheonma-tang gamibang), acupuncture and moxibustion. At the same time, the blood cupping therapy in the neck area was applied. We treated the patients two or three times a week with oriental therapy methods. They took medicine three times a day after a meal. During taking medicine, we let the patients avoid fatty food, flour based food, and alcohol. We determined the course of treatment with visual analogue scale (VAS). We let them respond to a VAS item between 0 and 10 point continuously. The symptoms that we evaluated were vertigo, headache, chest discomfort, nausea and vomiting.Results : After taking treatment - acupuncture and moxibustion during some period and taking herbal-medicine, the VAS scale was gradually getting decreased. Generally the symptom of vomitting and nausea was eased at first, and then the symptom of chest discomfort, headache, vertigo was relieved.Conclusions : Banhabaekchulcheonma-tang gamibang was effective in the treatment of vertigo and its related symptoms, headache, chest discomfort, nausea and vomiting.

A Propensity of Formative Presentation by Line Drawing (라인드로잉에 의한 디자인 조형의 표현성향)

  • During design thinking and developing, its idea into the real world, and we are under pattern recognition and gestalt principles of perceptual organization. Generally originality is a part of creativity which consists an integral factor of the designing. This is a study on the measure system for an ability of originality in design. It is reorganized that the OTLD(Originality Test of Line Drawing) is a measuring system for personal originality. In order to catch the development the thoughts, we presented 10 picture planes as stimuli (each picture plane contained 3-18 dots), recorded the tape displaying eye-mark trajectories and outputting the trajectories with EMR(Eye Mark Recorder), then found the process of visual sensation and perception. From the results of this study, we examined the relationships between connections and complexity of the objects on the picture plane, which could be transformed into some objective measuring parameters. We would suggest this OLTD as a measurement system for an ability of originality in design fields, but we couldn't find any reliability and validity for fine art fields.

Blockade of Ganglion Impar and Superior Hypogastric Plexus Block for Perineal Cancer Pain -Case report- (회음부 암성통증에 대한 외톨이 신경절 블록과 상 하복 신경총 블록 -증례 보고-)

  • Intractable pain arising from disorders of the viscera and somatic structures within the pelvis and perineum often poses difficult problems for the pain pratitioner. The reason for this difficulty is that the region contains diverse anatomic structures with mixed somatic, visceral, and autonomic innervation affecting bladder and bowel control and sexual function. Clinically, sympathetic pain in the perineum has a distinctly vague, burning, and poorly localized quality and is frequently associated with the sensation of urgency. Although various approaches have been proposed for the management of intractable perineal pain, their efficacy and applications are limited. Historically, neurolytic blockade in this region has been focused mainly on somatic rather than sympathetic components. The efficacy of neurolytic ganglion impar block has been demonstrated in treating perineal pain without significant somatovisceral dysfunctions for patient with advanced cancer in 1990. The introduction of superior hypogastric plexus block in 1990 demonstrated its effectiveness in patients with cancer related pelvic pain. In our report, five patients had advanced cancer (rectal caner 3; cervix cancer 1; metastases to sacral portion of renal cell cancer 1). Localized perineal pain was present in all cases and was characterized as burning and urgent with 9~10/10 pain intensity. After neurolytic block of ganglion impar, patients experiened incomplete pain reduction (7~8/10), as determined by the VAS (visual analogue scale), and change in pain site. We then treated with superior hypogastric plexus block, which produced satisfactory pain relief (to less than 4/10), without complication.
