Anterior Epidural Irrigation in a Lumbar Spinal Stenosis Patient (척추관 협착증 환자에서 전 경막외강 세정요법)

  • Background: Pain due to spinal stenosis is one of the major causes of low back pain, and it sometimes does not respond to conservative treatment. We postulated that anterior epidural irrigation could be a good choice for treating lumbar spinal stenosis patients. However, the effects and theoretical basis of anterior epidural irrigation have not been investigated for spinal stenosis. So, this study was designed to identify the clinical efficacy of anterior epidural irrigation for treating lumbar spinal stenosis. Methods: 71 patients with lumbar spinal stenosis underwent anterior epidural irrigation. Anterior epidural irrigation was performed with normal saline and a mixture of local anesthetic with steroid through a catheter under fluoroscopic guidance. The findings were evaluated with using visual analogue scales for low back pain and leg symptoms. Results: Relief from low back pain and leg pain was observed for up to 12 months after anterior epidural irrigation. None of the patients showed any deterioration of their motor or sensory deficits during follow up. Conclusions: Anterior epidural irrigation is effective for the relief of back and leg pain in spinal stenosis patients.

Combined Effects of PMV and Acoustics on Indoor Environmental Perception (PMV와 음환경의 복합 작용이 실내 환경 지각에 미치는 영향)

  • Purpose: The purposes of this paper are to investigate effects of indoor thermal environment on acoustical perception and effects of acoustics on indoor thermal perception, and to understand basic human perception on indoor environment. Method: Subjective assessment was performed in an indoor environmental chamber with 24 university students. Thermal conditions with PMV -1.53, 0.03, 1.53, 1.83 were simulated with a VRF system, a humidifier, a dehumidifier, and a ventilation system. Six noise sources - Cafe, Fan, Traffic, Birds, Music, Water- with sound levels of 45, 50, 55, 60 dBA were played for 2 minutes in random order. Temperature sensation, temperature preference, humidity sensation, humidity preference, noisiness, loudness, annoyance, and acoustic preference were assessed using bipolar visual analogue scales. The ANOVA and Turkey's post hoc test were used for data analysis. Result: Thermal environmental perceptions were not altered through 2 minutes noise exposure. Acoustical perceptions were altered by thermal conditions. The results were consistent with previous papers, however, the noise exposure time should be carefully considered for further development.

Pain Management Survey of Psychiatric Unit Nurses (정신과 근무하는 간호사의 통증관리 지식 및 태도에 대한 조사)

  • Purpose: This study was intended to measure the knowledge and attitudes of nurses working in a psychiatric ward regarding the subject of pain. Methods: In this descriptive study, data were collected in psychiatric wards(or hospitals) located in the Kyungnam and Kyungpook area from December 2013 to January 2014, using a modified version of the McCaffrey and Pasero Clinical Manual Survey Regarding Pain. The content of the survey follows a logical progression from common pain management problems through mechanisms of pain, assessment and pharmacology. Results: The average score of knowledge regarding pain was roughly $21.59{\pm}2.93$ among the 181 participants. The nurses selected Tylenol as the preferred choice (81.2%). Seventy-one percent of respondents didn't use Pain Intensity Instruments during the pain assessment. The subjects preferred the Numerical Rating Scale rather than Visual Analogue Scales. Pharmacology questions were the least likely to be answered correctly. No specific correlations were found between score and demographic variables of age and experience. No specific correlation between score and educational level was noted. Conclusion: Nurses require ongoing education on pain management, particularly regarding pharmaceutical agents.

A Case Report of a 7-year-old Hemophilic Arthropathy Patient Treated by Oriental Medical Treatment (혈우병성 관절증 7세 환아의 한방적 처치에 대한 증례보고)

  • Objectives: The purpose of this study is to demonstrate the therapeutic effect of oriental medical treatment on Hemophilic arthropathy. Methods: The patient was treated by Herb medicine, Acupuncture, Moxibustion, and Cupping. As a result, improvement in the ROM of joints was shown. ROM(Range of motion), Lequesne's functional index(LFI), WOMAC index and Visual analogue scale(VAS) were used to measure the progression. Results The ROM, LFI, WOMAC index and VAS were evaluated during admission. All scales improved; ROM of knee joint : $60^{\circ}$($105^{\circ}$ to $165^{\circ}$), ROM of ankle joint : $40^{\circ}$($20^{\circ}$ to $60^{\circ}$), LFI : 15 to 4, WOMAC index : 55 to 12 and VAS : 5.0 to 1.0. We also improved the patient's symptoms with oriental medical treatment. Conclusions: According to the results, oriental medical treatment is considered to be effective on Hemophilic arthropathy and further study is needed.

Effects of Recent Life Changes on the Temporomandibular Disorders and Treatment Course (생활변화가 측두하악기능장애와 치료과정에 끼치는 영향)

  • 44 Temporomandibular Disorders(TMDs) patients with non-trauma and non-iatrogenic origin pressented at Wonkwang University Dental Hospital. They were grouped into experimental subjects and 85 persons without TMSDs were classified into control groups. The objectives of this study was to investigate the effects of recent life changes on the symptom severity and treatment course of TMDs. For that purpose, the author used several scales and indices, namely, Social Readjustment Rating Scale(SRRS) devised by Holmes and Rahe, SRRS-Korean revision by Hong and Jeong, Helkimos Anamnestic index, Clinical Dysfunction index, and Visual Analogue Scale treatment index(VAS Ti) transformed from VAS by the author. Data resulted from the investigation were collected by scale or index and processed with SPSS. The obtained results were as follows : 1. Life change units(LCU) and values of indices of experimental group were higher than those of control group. 2. Life change units recorded with SRRS-Korean revision were higher than those with original SRRS in all cases and significant positive correlations existed, between he two Therefore, clinical use of original SRRS in Korea is reliable and valid. 3. In experimental group, LCU were positively correlated with Helkimos Clinical Dysfunction index and VAS treatment index, but in control group LCU were not correlated with any items. From this, increase of life changes in patient with TMDs is likely to aggravate TMJ dysfunction and more likely to complicate treatment course.

A Report about the Result and Satisfaction of Korean Medicine Treatment on Dysmenorrhea Support Program of Jeollabuk-do (전라북도 월경통 지원사업의 한방치료 결과 및 만족도 보고)

  • Objectives: The purpose of this study was to report the result and the satisfaction of Korean medicine treatment for patients with dysmenorrhea who participated in support program of Jeollabuk-do. Methods: The subjects of this study were 33 volunteers, suffering from primary dysmenorrhea. As the 'Dysmenorrhea Treatment Project', we conducted a dysmenorrhea program at 22 Korean medicine clinic and 1 Korean medicine hospital in Jeollabuk-do. From May to December 2018, we conducted the program and assessed the effect and satisfaction twice after first two menstrual cycles after Korean medicine treatment through questionnaire. They were treated by Korean medicine treatment such as herbal medicine, acupuncture, moxibustion, heat therapy, physical therapy and others in 2017. The results were measured by the visual analogue scale (VAS) and multidimensional verbal rating scale (MVRS) of overall pain. The disturbance in school activity for menstrual period and satisfaction of Korean medicine treatment were investigated by continuous rating scales. Results: The VAS score of overall pain was decreased significantly. Also the amount of the analgesic taken for menstrual period and the degree of disturbance in school activity were decreased significantly. Besides, the satisfaction of Korean medicine treatment was increased. Conclusions: This study might suggest that the Korean medicine treatment on dysmenorrhea is effective.

Effects of Thoracic Expansion and Diaphragm Breathing Exercises on Pain and Function in Patients with Rotator Cuff Repair (가슴우리팽창과 가로막 호흡운동이 돌림근띠 복원술 환자의 통증과 기능에 미치는 효과)

  • Purpose: The study aims to determine the effect of a breathing exercise on shoulder pain, range of motion (ROM), and forward head posture in patients with rotator cuff repair. Methods: In total, 25 patients with rotator cuff repair were included in this study. The experimental group (n = 13) underwent a breathing exercise, while the control group (n = 12) received traditional physical therapy. The visual analogue scales (VASs) for pain, flexion and abduction ROM, and the craniovertebral angle (CVA) of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention compared with the pre-intervention period, and independent t-tests were used to analyze differences in dependent variables between the two groups. Results: After the two-week intervention, the experimental group experienced a significantly decreased VAS (p < 0.05) and significantly increased ROM and CVA (p < 0.05), while the control group experienced a significantly decreased VAS (p < 0.05). Further, the experimental group that underwent the breathing exercise showed greater improvements in flexion and abduction ROM and in the CVA than the control group (p < 0.05). Conclusion: The results suggested that a breathing exercise can reduce shoulder pain and enhance ROM and posture in patients with rotator cuff repair.

Interim Report about The Effect of Bunsimgi-eum(Fenxinqiyin) on the Chest Discomfort of Hwa-byung's Major Symptom (화병 핵심증상 중 흉민에 대한 분심기음의 효능 : 무작위배정, 이중맹검, 위약대조군연구의 중간보고 - 피험자 특성을 중심으로)

  • Objectives : The purpose of this research is to examine the effect of Bunsimgi-eum on the chest discomfort of Hwa-byung's major symptom. Methods : In this randomized, double blinded, placebo-controlled study, we planned to give Bunsimgi-eum or controlled medication for major symptoms of Hwa-byung. As preparatory research, 100 mm Visual Analogue Scale(VAS) for chest discomfort, the Hwa-byung' major symptom was measured as the 1st evaluative instrument, and Likert scale for major symptom of Hwa-byung, Korean State-Trait Anger Expression Inventory(STAXI-K), Korean State-Trait Anxiety Inventory(STAI-K), Korean Beck's Depression Inventory(BDI-K) and Heart Rate Variability(HRV) were also measured as the 2nd evaluative instrument at the before treatment. Results : Clinical characteristics-vital signs & demographic characteristics showed no significant difference between both groups. The characteristics of disease-chief complaint, pattern Identification, period, etiological factor, etc, also showed no significant difference between both groups. There were almost no Coincidence between pattern identification & weight of Hwa-byung's major symptoms. The results of Chest PA, EKG and clinicopathologic examination showed no significant difference between both groups. There were no significant difference between both groups in all valuation scales; 100 mm Visual Analogue Scale for chest discomfort, the Hwa-byung' major symptom was measured as the 1st evaluative instrument, and Likert scale for major symptom of Hwa-byung, STAXI-K, STAI-K, BDI-K and HRV. Conclusions : We considered that establishment of experimental group and controlled group was objective and worth conducting this research. And this methodology is expected to be applied to the subsequent research. And also, we hope to make up for this study through various study and discussion.

The Clinical Effects of Ahnjeonbaekho-tang (AJBHT) on Graves' Disease: A Prospective Clinical Study

  • Objectives: Graves' disease is the most common cause of hyperthyroidism, and its pathogenesis includes thyroid specific autoimmunity. Anti-thyroid drugs are widely used for regulating the thyroid function. However, in spite of long-term therapy with anti-thyroid drugs, about 40$\sim$$70\%$ of the treated patients have a relapse, and some suffer adverse effects. In this study, to evaluate the clinical efficacy of Ahnjeonbaekho-tang(AJBHT) on Graves' disease patients, we performed the clinical study prospectively. Methods: Through the thyroid function test(TFT) of 54 patients diagnosed as Graves' disease in other hospitals, 21 patients were assigned into the study. After the withdrawal of anti-thyroid drugs, AJBHT was administerd to patients for 2 months. At the same time, TFT, TSH binding inhibiting immunoglobulin(TEII) level and visual analogue scales (VAS) about fatigue and palpitation were measured before and after administration. Thirteen patients have completed the entire follow-up of this study over two months. Results: Serum levels of T3 and FT4 were significantly improved by AJBHT(T3: 298.85$\pm$79.60 ng/dl 181.15$\pm$33.92 ng/dl p<0.0001, FT4: 2.78$\pm$1.06 ng/dl$\rightarrow$1.78$\pm$0.83 ng/dl p<0.05). However, there were no significant changes in TSH and TBII values. And the VAS scores of fatigue and palpitation also were significantly improved(5.80$\pm$3.01$\rightarrow$3.60$\pm$2.63, p<0.05; 6.19$\pm$2.09$\rightarrow$3.60$\pm$2.46, p<0.01). Patients' age was related to the post-treatment FT4 values(p<0.05). Conclusions: From these results, we suggest that AJBHT is effective on the TFT and the symptoms of Graves' disease, and is a safe alternative drug for Graves' disease patients.

Clinical Study on Pain Index, Radiological Evaluation and MMPI of Traffic Accident Patient (교통사고 환자의 MMPI 및 통증, 영상평가에 대한 임상연구)

  • This study analyzed the MMPI (Minnesota Multiphashic Personality Inventory), VAS (Visual Analogue Scale), cervical and lumbar curvature (Cervical angle, Ishihara Index, Lumbar angle, Ferguson angle) and satisfaction score of patient of traffic accident. About 59 cases of patients admitted to the Korean Medicine hospital, MMPI and pain levels (VAS) cervical and lumbar curvature (Cervical angle, Ishihara Index, lumbar angle, Ferguson angle) and satisfaction score were measured and statistically analyzed. Depending on the type of accident divided into two groups of In car TA and Out car TA. After hospitalization, patient's pain index was improved as a whole. Pain index and evaluation of the cervical and lumbar curvature, according to the accident types, the difference was not statistically significant. In the analysis of the MMPI validity scales, In car TA group was analyzed by 'V-shaped', Out car TA group was analyzed by umbrella- shaped, 'inverted V-shaped'. Clinical scales of out car TA group were analyzed by 3-1 profile compared to the In car TA group. Results of multiple regression analysis showed no association between factors and MMPI scales. In the case of a traffic accident patients, damage to the body, as well as emotional and psychological factors are thought to affect the condition or prognosis of patients. In the future, for the management and treatment of these parts will be needed to complement the academic or institutional.
