Analysis of Time Taken for Visiting Nursing Activities by Visiting Nurses (방문 간호사의 방문 보건 활동 소요시간 분석)

  • Purpose: The purpose of this study is to analyse time taken for visiting nursing activities by visiting nurses in health centers. Method: A questionnaire was developed by a research team for the technological support of visiting nursing activities and visiting nurses. A total of 481 questionnaires were recovered by five visiting nurses from May to October 2003. Result: A visiting nurse's total length of time for visiting activities was 532.2 minutes per day and the number of households a visiting nurse visits a day was 4.5. A visiting nurse spent 133.3 minutes for actual nursing services and 119.1 minutes for travelling. Time for actual nursing activities was more than half of the total working hours. Time for travelling was 42.9 minutes on the average when using public transportation means and 25.6 minutes when using a car. Conclusion: The results of this study is expected to be used as basic data in establishing plans for the adequate supply of visiting nurses based on demands for visiting nursing services.

Testing for Self-care Ability Measurement in an Aged Health Center - A Delphi Technique - (일개 노인보건센터에서의 자가관리능력 측정도구 검증 - 델파이기법을 중심으로 -)

  • Purpose: To find out whether the self-care ability measurement used for visiting health care provide an appropriate standard to select service methods using a Delphi survey and to offer basic data that can improve the measurement. Method: A Delphi survey was done with 39 people consisting of theoretical professionals, field managers, and practitioners engaged in visiting health care. Result: It was found that items and weighted points in the currently used self-care ability measurement needed adjustment. Therefore, a modified evaluation standard was presented by domains, items and detailed items. Finally, the self-care ability measurement is composed of 4 domains, 11 items and 19 detailed items. Conclusion: This study is significant in that it presented an evaluation standard applicable directly to business practice by supplementing problems in the existing self-care ability measurement. This study suggests the need to develop various measurements for elderly households based on community.

The Development and Effect of Doctor-Visiting Nurses ICT Communication System for the Elderly: Focusing on Visiting Nursing Services for Long-term Care Patients (고령자 대상 의사-의료인간 ICT활용 협진 모델개발 및 실증에 관한 연구: 장기요양서비스 중 재가노인대상 방문간호서비스를 중심으로)

  • Purpose: The purpose of this study was to examine effectiveness and usefulness of Information Communication Technology (ICT) in communication between physician and visiting nurses who provide visiting nursing services under long-term care insurance. Methods: Structured questionnaires were used to measure usefulness and satisfaction of the system, both accessibility and convenience to visiting nurses (31 people) and users (182 people). Results: From the user perspective, accessibility and convenience in terms of service users were both satisfactory as shown by users' percentage. No statistically significant difference was found for satisfaction between before and after using the system. The usefulness of the system for visiting nurses was satisfactory for most the nurses. Also most nurses answered that the system is needed and is very useful. Most of the participants (both visiting nurse and service users) were satisfied with use of the ICT system. However, there was no statistically significant difference in satisfaction between the pre and post service because the service provision period was too short (three months). Conclusion: The consensus from both users and service providers is that an ICT based visiting nursing system needs to be introduced but a more user - friendly environment for system development will be needed.

A Study on Health Education Needs of the Multicultural Family as Perceived by Visiting Nurses (방문간호사가 지각하는 다문화가족의 건강교육 요구)

  • Purpose: The purpose of this study was to identify health education needs of the multicultural family as perceived by visiting nurses. Methods: Mixed method was applied. A questionnaire was distributed to 184 visiting nurses. Three focus group interviews were subsequently conducted in order to obtain a greater understanding of nurses' experience of health education. Results: Visiting nurses were mainly providing education about pregnancy, delivery and baby care. Health education needs of the multicultural family as perceived by visiting nurses were classified according to four categories : diet, hygiene, contraception and family planning, and communication. Barriers to health education of visiting nurses were a lack of adequate health education materials and communication problems. Conclusion: To achieve effective health care of the multicultural family by visiting nurses, customized multi-language health education material focusing on five subjects, pregnancy and delivery, diet, hygiene, contraception and family planning, and communication, with easy terms and simple paragraphs is required.

Violence Victimization of Visiting Nurses and Prevention Strategies Adopted by Public Institutions in Korea (방문간호사가 경험하는 폭력의 특성과 예방대책)

  • Purpose: This study aims to reveal violence victimization of visiting nurses and investigate the coping and prevention strategies adopted by public institutions. Method: The data were collected over three weeks in 2017 using self-report questionnaires from 237 visiting nurses. Data were collected between August 23 and September 15, 2017. Result: Among the respondents, 74.7% had experienced some types of work-related violence during the past year. Verbal abuse (65.4%) was most common, followed by physical threat (46.8%), sexual abuse (43.9%), and physical violence (5.5%). The nurses perceived the most common cause of violation as socio-economic frustration of the offenders vented out on the nurses. Besides, most reported offenders had mental health problems. The two most effective measures to prevent violence faced by the nurses are terminating visiting services for the offenders and implementing the buddy system, which has rarely been used in practice. Conclusion: These findings show that visiting nurses are exposed to a risk of serious abuse in everyday work life, and the nurses themselves or institutions have failed to handle the violence, showing shaky responses at best. Therefore, it is necessary to establish internal regulations and systems at the institutional level that can help prevent violence against visiting nurses.

Community Care and Nurse Care Coordinator (지역사회 통합 돌봄과 간호사 케어 코디네이터)

  • Purpose: This study examined the definition of care coordination, and the activities and roles of nurse care coordinators. The study also proposed suggestions for establishing and expanding the role of nurse care coordinators in community health practice. Methods: This study derived its conclusions by conducting a literature review. Results: The definition of care coordination is a comprehensive concept that includes case management and transitional care, and can be summarized as organizer and integrator of care. According to the literature review, 12 activities and 6 roles of nurse care coordinator were identified such as a collaborator, an individualized care planer, an educator/counselor, a direct nursing care providers, a population care coordinator, and a program evaluator. Training for nurse care coordinators is currently addressed in the Primary Medical Chronic Disease Management Program. Visiting nurses working at the Eupyeong-dong visiting health and welfare service are expected to act as nurse care coordinators, and the role of nurse care coordinators will, in the future, gradually increase in various nursing facilities. Conclusion: In addition to developing competencies to act as a nurse care coordinator, there is need for approaches to health policy that develop both independent role and population focused role as care coordinators.

A Proposal of a Nursing Model Applicable to Group Occupational Health Service (보건관리대행 간호모형의 제안)

  • This study was conducted to propose a model for nursing performance in group occupational health service. To achieve such a goal, the writer choose and analysed two kinds of journals which were thought to be the most suitable for that purpose and authoritative on community health nursing: The Journal of Korean Community Nursing and the Journal of Korean Academic Society of Industrial Nursing. The total number of the articles analysed in this study was 16 and in the analysis of them the writer was specially concerned with the following questions: 1) Have there been any different performances in occupational health management between staying nurses and visiting ones? 2) What are the levels of knowledge, attitude, and practice of workers and how can we, on the basis of them, develop the program that workers are able to participate positively in ? the obtained conclusions are as follows. 1) Different performances are revealed between staying nurses and visiting ones: Common symptom management frequently appeared in staying nurses. Counseling, health education and follow-up care after medical examination were more frequently revealed in visiting nurses. 2) As for the lovel of knowledge, attitude, and practice of occupational health, workers have average one respectively. The major factors influencing the level of KAP of workers were area, age, sex and status. Counseling, health education, follow-up care after medical examination, and health promotion should be reinforced as core programs in group occupational health services. And for the part of visiting nurses, it is necessary to put more emphasis on efficiency training using the method of nursing process.

Correlations between a Flexible Parental Visiting Environment and Parental Stress in Neonatal Intensive Care Units (신생아집중치료실 미숙아 부모의 자율면회 방문 현황과 부모 스트레스와의 상관관계)

  • Purpose: The purpose of this study was to examine the correlation between a flexible parental visiting environment and parental stress in neonatal intensive care units (NICUs). Methods: The study participants included 60 parents of premature infants in NICUs. Structured questionnaires and interviews, as well as observations by researchers using a caregiving behavior checklist, were used to measure the flexibility of the parental visiting environment and parents' stress levels. Quantitative and qualitative data were collected concurrently and were initially analyzed as separate data sets. Data collection extended from March 11, 2018 to June 30. 2018 and the data were analyzed using descriptive statistics, the independent t-test, one-way analysis of variance, and Pearson correlation coefficients. Results: There was a negative correlation (r=-.30, p=.021) between parental stress and the total number of visits in 7 days. We also found that the average duration of each visit and the number of caregiving behaviors performed by parents were positively correlated (r=.73, p<.001). Conclusion: When designing a flexible visiting environment for parents, parents should be encouraged to visit their babies. By doing so, stress can be reduced both for babies and for parents. Therefore, it is suggested that the related polices and regulations in South Korea should be changed to provide more a flexible visiting environment to promote better parent-child attachment and family adjustment.

Development and Application of Cost Management Program for Visiting Nursing Centers Using Time-Driven Activity-Based Costing (시간동인 활동기준원가계산 기반 재가 방문간호센터 원가관리프로그램 개발 및 적용)

  • Purpose: This study aimed to develop a web-based cost management program for visiting nursing centers (CMP-VNC), using time-driven activity-based costing (TD-ABC), and to analyze effects of the program. Methods: The CMP-VNC was developed using the combined prototyping approach and system developing life cycle method following four stages: need analysis with comprehensive literature reviews and focus group interviews, design and development of program algorithm, evaluation of the developed program validity using experts and users group, and application and effects analysis. The non-equivalent control group pretest-posttest design was used to analyze the effects of the program. The program demonstration was conducted for four weeks with 60 visiting nurses in 35 visiting centers. Results: The web-based program was developed. It has five interfaces with basic and special functions using TD-ABC, namely, input, visiting nursing activity, visiting nursing activity cost, cost efficiency, and cost calculation report. The experimental group showed significantly higher cost perception and cost confidence than control group. Conclusion: We found that the CMP-VNC can be an effective tool to increase visiting nurses' competency of costing and enhance efficiencies of visiting nursing centers.

Effects of Visiting Nursing Services in Long-term Care Insurance on Utilization of Health Care (노인장기요양보험의 방문간호 서비스가 의료이용에 미치는 효과)

  • Purpose: This is a comparative study using secondary data from the Korean national long term care (LTC) insurance. Methods: Visiting nursing (VN) service users (n=666) and non-users (n=4,375) were extracted and compared in terms of medical expenditures, length of hospitalization, and annual number of ambulatory care visits to investigate effects of VN services in LTC. Results: Total health care expenditures were compared between the two groups and it was found that VN service users spent about $ 1700 than non-users for their medical costs between 2009 and 2011. The average length of in-hospital stay for VN service users was 19.4 days shorter than that of non-users. However, using VN services did not significantly influence the annual number of ambulatory care visits. Conclusion: The study has found that VN services are effective ways of providing community-based LTC services. We recommend LTC policy makers to further utilize VN services to deliver cost effective health care services.
