Purpose: This study aimed at identifying the factors affecting the service satisfaction of urban elderly, focusing on the outreach community health service in Seoul, and suggesting policy directions for the successful implementation of community care. Method: Individuals aged 65 and 70 who used the outreach community health service from July 2017 to June 2019 were eligible for the survey. A total of 2,028 people were sampled using a proportional allocation method for each autonomous district in the survey which covered 25 districts. A multi-level logistic regression analysis was conducted, taking into account the individual's socioeconomic level, health status, type of service provided, and the healthcare-related environment and service provision period of the autonomous district. Result: The results revealed that the health status of the urban elderly, the type of services provided (health screening, linkage to community health center and clinic/hospital, medical checkup results counseling, frailty evaluation), and personal experience of the service were the major factors associated with the satisfaction with the outreach health services. Conclusion: The development of customized health services based on the close relationship between visiting nurses and the elderly may be considered to promote a sustainable community health care model.
Objectives: This study was aimed to figure out korean medicine doctors' opinion and treatment of Korean medicine clinical practice guidelines (CPG) for premenstrual syndrome (PMS). Methods: We conducted a questionnaire survey targeting korean medicine doctors belonging to the Korean Association of Oriental Medicine by e-mail and text and analyzed the answers. Results: 1. The average monthly number of patients visiting the respondents' clinics for PMS was 2.7, the main age group was 20-30s (80.8%) and the treatments the patients given before were mostly western treatments such as painkiller and combined oral contraceptives. 2. The treatment period for PMS was mostly less than 3 months (69%), and the frequency of acupuncture treatment was the most twice per week (41.1%). 3. The most effective herbal medicine for affective symptoms was Soyo-san, and for physical symptoms was Gyejibokryung-hwan. The most commonly used acupuncture point was 三陰交 (SP6). 4. Other opinions of the clinicians about Korean medicine clinical practice guideline of PMS were the need for promotion of PMS, convenience of taking herbal medicines and price competitiveness. Conclusions: We figured out korean medicine doctors' opinion of PMS CPG, clinical diagnosis and treatment.
Background: To prepare for future work stoppages in the medical industry, this study aimed to identify the effects of healthcare worker strikes on the mortality rate of patients visiting the emergency department (ED) at six training hospitals in Daegu, South Korea. Methods: We used a retrospective, cross-sectional, multicenter design to analyze the medical records of patients who visited six training hospitals in Daegu (August 21-September 8, 2020). For comparison, control period 1 was set as the same period in the previous year (August 21-September 8, 2019) and control period 2 was set as July 1-19, 2020. Patient characteristics including age, sex, and time of ED visit were investigated along with mode of arrival, length of ED stay, and in-hospital mortality. The experimental and control groups were compared using t-tests, and Mann-Whitney U-test, chi-square test, and Fisher exact tests, as appropriate. Univariate logistic regression was performed to identify significant factors, followed by multivariate logistic regression analysis. Results: During the study period, 31,357 patients visited the ED, of which 7,749 belonged to the experimental group. Control periods 1 and 2 included 13,100 and 10,243 patients, respectively. No significant in-hospital mortality differences were found between strike periods; however, the results showed statistically significant differences in the length of ED stay. Conclusion: The ED resident strike did not influence the mortality rate of patients who visited the EDs of six training hospitals in Daegu. Furthermore, the number of patients admitted and the length of ED stay decreased during the strike period.
Objectives: The purpose of this study was to provide basic data that would inform the direction of oral rehabilitation and how to expand of the role of dental hygienists in Japan. Methods: A systematic literature review was conducted on the role of dental hygienists in the field of oral rehabilitation in Japan. Results: Japan has been making academic and practical developments in the field of oral rehabilitation for over 30 years, and has been gradually implementing a fee support policy since the 1990s. In addition, Japan has been operating a dental hygienist system specializing in oral rehabilitation since 2006. The related work was being carried out with the dental hygienist's expertise in the field of rehabilitation medical treatment secured. Dental hygienists work full-time at long-term care facilities for the elderly in addition to conducting oral care activities under the local comprehensive care system, in the areas of convalescence and acute rehabilitation, as well as in the field of visiting rehabilitation. It can be seen that, in the field of nursing care, they are specialized in oral care tasks for the elderly. Conclusions: In the future, a policy and related fee system should be gradually prepared to expand the role of dental hygienists in the field of oral rehabilitation that can contribute to improving oral health linked to systemic diseases.
Objectives: This study assessed the validity and reliability of the Health-related Quality of Life Instrument with 8 Items (HINT-8) in patients with diabetes. HINT-8 is a newly-developed, generic health-related quality of life (HRQoL) instrument. Methods: Three HRQoL instruments-HINT-8, EuroQoL 5-Dimension 5-Level (EQ-5D-5L), and Short Form Health Survey version 2.0 (SF-36v2)-were provided to 300 patients with diabetes visiting a tertiary hospital for follow-up visits in Korea. The HRQoL scores obtained using the HINT-8 were evaluated for subgroups with known differences based on demographics and diabetes-related characteristics (known-group validity). The mean scores of the instruments were compared between groups segmented by their responses to the HINT-8 (discriminatory ability). Correlation coefficients of the HINT-8 with other instruments were calculated (convergent and divergent validity). The Cohen kappa and intra-class correlation coefficient (ICC) were also evaluated (test-retest reliability). Results: The average HINT-8 indexes were lower among women, older, and less-educated subjects. Subjects who did not list any problems on the HINT-8 had significantly higher HRQoL scores than those who did. The correlation coefficients of the HINT-8 with the EQ-5D-5L index and EuroQoL visual analogue scale were 0.715 (p<0.001) and 0.517 (p<0.001), respectively. The correlation coefficients between the HINT-8 index and the scores of 8 domains of the SF-36v2 ranged from 0.478 (p<0.001) to 0.669 (p<0.001). The Cohen kappa values for the HINT-8 ranged from 0.268 to 0.601, and the ICC of the HINT-8 index was 0.800 (95% confidence interval [CI], 0.720 to 0.860). Conclusions: This study showed that the HINT-8 is a valid and reliable HRQoL instrument for patients with diabetes.
Background: As South Korea enters an aged society, the government has emphasized the need for a soft landing of the older adults into the community after the acute and recovery periods under a national policy of "community care." However, the institutionalization of community rehabilitation services to implement this is insufficient. Japan had already entered an aged society when the Long-Term Care Insurance System was introduced in 2000. Thus, the case of Japan's institutionalization of the system is expected to have implications for us in supplementing a suitable system for the aged society. Objects: This study compared the institutionalization process of the Long-Term Care Insurance System in South Korea and Japan and the services currently being implemented in each country. Methods: To examine the institutionalization process and services of the system, related legal rules and regulations, government reports, and articles were reviewed. To examine the operation status of the system, statistical data provided by each country's government were analyzed. Results: Japan recognized the importance of community rehabilitation even before the enactment of Long-Term Care Insurance. Thus, community rehabilitation services, such as home-visit rehabilitation and health facilities, were already stipulated in the law. Under such institutional legacy, Long-Term Care Insurance was able to establish a service system, which balanced welfare and health-related services, including various types of services with enhanced rehabilitation functions. In South Korea, rehabilitation policies were not much considered in the process of institutionalizing the system; thus, it was composed mainly of services focusing on care and recuperation. Conclusion: In order to realize community care, rehabilitation services need to be developed in Long-Term Care Insurance System in various forms such as home-visit services, daily services, short stay, and facility services.
Background: There were a few studies which were conducted to know about the behavior of the chronic obstructive pulmonary diseases (COPD) patients. The aims of this study was to explore the behaviour of COPD patients, such as awareness and impact of disease, the pathway of visiting doctors, and the treatment pattern and preference. Methods: A face-to-face interview of 300 subjects with COPD was conducted. Results: The most concerned symptom which made the respondents to visit the hospital was 'breathlessness' (78%). Only 58% of them knew the exact diagnosis. Seventy-three percent of them visited the hospital 'once a month' or 'once every 2 month'. They have made 12.8 prescheduled visits to the hospital in the past 1 year. Unscheduled visits and hospital stay figured to two in the past year. Only 11% of respondents felt they were currently in good health. 'Severe' and 'very severe' COPD patients perceived their health to be in a worse condition than 'mild' and 'moderate' COPD patients. When conditions worsened, 42% of patients were hospitalized. The most common prescription treatment was a fixed combination of inhaled corticosteroids and long-acting ${\beta}2$ agonists (48%), followed by a long acting anticholinergics (38%). Conclusion: Over forty percent of the patients didn't know exactly about their condition. Most of them had a negative attitude toward their current health status. Doctors need to know more about COPD patients in terms of their attitude toward the disease, impact of the disease, interaction with healthcare professionals and treatment related problems.
이 연구는 웹보메트릭 빅데이터를 활용하여 코로나바이러스 확진 국가(지역)들의 온라인 정보원의 다양성을 조사했다. 구체적으로 2020년 2월에 코로나바이러스 확진자가 발생한 28개국을 대상으로 웹 데이터를 수집한 결과, 호주, 캐나다, 이탈리아 등의 온라인 가시성이 높게 나타나면서 관련 정보를 가장 많이 생산하고 있었다. 국가별 검색건수(hit counts)와 정보채널의 역할을 하는 도메인(domain) 빈도와는 통계적으로 유의한 상관성이 있었다. 한편 데이터 수집도구인 bing.com의 점유률이 평소에도 높은 국가들을 제외하고 다시 검토한 결과, 당시 확진자 수가 많았던 일본, 중국, 싱가포르 등이 코로나바이러스와 관련된 웹데이터를 주도적으로 올리고 있었다. 온라인 정보원은 n-헬릭스를 활용하여 분류되었다. n-헬릭스는 대학-기업-정부의 3주체간 상호작용과 혁신을 강조하는 트리플헬릭스모델(Triple Helix Model)에 기반한 확장된 분석틀이다. 그 결과, 정부기관이 18.1%를 차지하면서 코로나바이러스 정보의 최대 공급자로 나타났다. 2원성 네트워크 분석결과를 보면 언론사, 대학병원, 공중보건에 특화된 조직 등도 코로나바이러스 연구와 방역 정보의 온라인 유통에 적극적이었다. 웹페이지에 포함된 단어들을 중심으로 내용분석을 해 보니 건강, 학교, 가족, 공공, 방안 등의 단어가 중심성이 높게 나타나 코로나바이러스로 인한 개인별 예방수칙뿐만 아니라 생활 불편과 업무장애로 인한 대처방안 등에 관심이 높다는 것을 알 수 있었다.
본 연구는 C시에 소재한 보건소의 방문건강관리 대상 노인 중 만성 관절통증이 있는 65세이상 취약계층 재가 여성노인 234명을 대상으로 삶의 질에 영향을 미치는 주요 요인을 파악하기 위한 서술적 상관관계 연구이다. 본 연구의 자료는 일반적 특성, 통증양상, 통증정도, 지각된 건강상태, 일상생활능력, 수면양상, 우울, 삶의 질로 구성된 구조화된 설문지를 통해 2017년 2월 16일부터 3월 13일에 걸쳐 수집되었으며, t-test, ANOVA, 상관분석, 위계적 다중 회귀분석으로 분석하였다. 연구결과, 대상자의 일반적 특성을 포함하여 대상자의 삶의 질에 영향을 미친 변수는 우울(${\beta}=-.60$, p<.001), 통증정도(${\beta}=-.15$, p=.007), 의료보장 유형(${\beta}=.15$, p=.001), 지각된 건강상태(${\beta}=.14$, p=.007), 통증지속기간(${\beta}=-.10$, p=.019), 결혼상태(${\beta}=.10$, p=.024), 일상생활능력(${\beta}=.09$, p=.036)순으로 나타났으며, 모형의 총 설명력은 63.1%였다. 본 연구를 통해 만성 관절통증이 있는 재가 여성노인의 삶의 질을 향상시키기 위해서는 지역사회 중심의 체계적인 우울 예방 및 관리 프로그램 운영이 우선 실시되어야 하며, 특히 경제적으로 취약한 공적부조 대상 노인의 삶의 질에 관심을 기울여야 하고, 통증 발현 초기부터 통증에 대한 정확한 사정을 토대로 통증완화중재 개입을 시행해야 한다.
목적 : 본 연구는 중소도시 지역 거주 고령자의 건강 증진을 위한 ICT-리빙랩(Living lab) 기반 프로그램의 효과성을 검증하고 이에 따라 현대사회가 직면한 노인 건강과 삶의 질 문제 해결에 대한 기초자료를 제공하고자 한다. 연구방법 : 2019년 10월부터 11월까지 2개월에 걸쳐 강원도 W시에 소재한 경로당을 이용하는 고령자 14명을 대상으로 ICT-리빙랩 기반 프로그램을 실시하였다. 본 프로그램은 노인 관련 전문가 4명, ICT 전문가 1명의 자문과 대상자들의 필요와 요구를 고려하여 구성하였다. 프로그램은 사전 평가 10분, 준비활동 10분, 주활동 30분, 마무리 10분으로, 총 60분의 구성을 주 1회씩 2개월간, 총 8회기를 실시하였다. 결과 : 실험대상자들은 사전평가에 비해 사후평가에서 삶의 질 점수(EQ-5D)가 0.76에서 0.84로 증가하였고(p=.009*), 주관적 건강상태(EQ-VAS)는 36.43에서 65.71로 증가하였다(p=.001*). 인지능력(MMSE-DS)은 21.21에서 24.14로 증가하였고(p=.000*), 우울감(SGDS-K)에서는 3.36에서 3.21로 평균점수가 감소하였지만 유의한 차이는 없었다(p=.854). 결론 : 본 연구의 ICT-리빙랩 프로그램은 현대사회가 직면한 노인문제를 예방 및 개선하여 건강과 삶의 질을 증진시키는 방안 중 하나로써 추후 관련 연구에 대한 기초자료로 활용될 수 있을 것이다.
