Purpose:The purpose of this study was to investigate of the change of the forward head posture(FHP) according to computer using time. Methods:Subjects were 30 male and 30 female. The factors of FHP were measured cranial vertical angle, cranial rotation angle, and head, upper back, neck flexion/extension angle according to computer using time. Change of FHP used to Digital Inclinometry(JTech, Dualer IQTM Dual Inclinometer). The data were collected by data logger(Logger Teknologi HB, Akarp, Sweden). Results:Cranial vertical angle, cranial rotation angle, and head, upper back, neck flexion/extension were increased according to computer using time(p<.05). But cranial vertical angle and cranial rotation angle, and head, upper back, neck flexion/extension were not significant differences between male and female group(p>.05). Conclusion:The effects of cranial vertical angle, cranial rotation angl and head, neck and upper back angle depend on the computer task time. Increased of FHP may result increased tension in posture muscles of cervical spine, resulting in a risk of musculoskeletal disorders.

Purpose : This research intended to find out the effects of scalenus medius muscle relaxed posture on the increase of head-neck rotation for general adults. Methods : This research was conducted on 30 subjects whose both sides head neck rotation angles are not the same and that agreed to participate in the experiment. In a posture of sitting erectly on a mat, both sides head neck rotation angles were measured with C-ROM equipment, and while supporting arm in direction opposite the side where head neck rotation limited and leaning over the body at 45 degree and neck at vertical condition against ground, head-neck rotation angles were measured each with C-ROM equipment. Results : The Head-Neck rotation angle on the side of limited head-neck rotation demonstrated more increase in scalenus medius relaxed posture than in erect sitting posture, showing significant difference statistically(p<0.05). The head-neck rotation angle on the side of non-limited Head-Neck rotation demonstrated more decrease in scalenus medius relaxed posture than in erect sitting posture, not showing any significant difference statistically (p>0.05). Conclusions : This posture may be used for preventing limit of head-neck rotation caused of scalenus medius muscle tension and increasing head-neck rotation.
Background: Limitations of shoulder range of motion (ROM), particularly shoulder internal rotation (SIR), are commonly associated with musculoskeletal disorders in both the general population and athletes. The limitation can result in connective tissue lesions such as superior labrum tears and symptoms such as rotator cuff tears and shoulder impingement syndrome. Maintaining the center of rotation of the glenohumeral joint during SIR can be challenging due to the compensatory scapulothoracic movement and anterior displacement of the humeral head. Therefore, observing the path of the instantaneous center of rotation (PICR) using the olecranon as a marker during SIR may provide valuable insights into understanding the dynamics of the shoulder joint. Objects: The aim of the study was to compare the displacement of the olecranon to measure the rotation control of the humeral head during SIR in individuals with and without restricted SIR ROM. Methods: Twenty-four participants with and without restricted SIR ROM participated in this study. The displacement of olecranon was measured during the shoulder internal rotation control test (SIRCT) using a Kinovea (ver. 0.8.15, Kinovea), the 2-dimensional marker tracking analysis system. An independent t-test was used to compare the horizontal and vertical displacement of the olecranon marker between individuals with and without restricted SIR ROM. The statistical significance was set at p < 0.05. Results: Vertical displacement of the olecranon was significantly greater in the restricted SIR group than in the control group (p < 0.05). However, no significant difference was observed in the horizontal displacement of the olecranon (p > 0.05). Conclusion: The findings of this study indicated that individuals with restricted SIR ROM had significantly greater vertical displacement of the olecranon. The results suggest that the limitation of SIR ROM may lead to difficulty in rotation control of the humeral head.
This study was performed to find out how much projection errors in the cephalometric measurements were made by vertical head rotation in taking posteroanterior cephalograms. 25 adults without any apparent facial asymmetry or severe sagittal skeletal discrepancy were selected and the posteroanterior cephalograms were taken with the head rotated $5^{\circ},\;10^{\circ}$ superior and inferior each to the reference $position(0^{\circ})$. The 7 height, 5 width and 6 angular measurements were taken at each 5 positions. Through the statistical analysis of all measurements taken at each rotated position, folowing results were obtained.1. The projection errors of height measurements were remarkably target than those of width or angular measure nents. f. Among the height measurements, the farther to the rotation axis the measurements were, the larger the projection errors were. 3. Among the width measurements, mandibular width and mandibular width of mandibular first molars showed significant differences between the values taken at each rotated position, while nasal width, maxillary width and intermolar width of maxillary first molars did not. 4. Among the angular measurements, the angle between horizontal reference line and the line that is connected to crista galli and antegonion or maxillare showed significant differences between the values taken at each rotated Position. The above results suggest that it is needed to the effort to keep constant head position for taking the useful posteroanterior cephalogra, because projection errors are caused by vertical head rotation.

Intraoral Vertical Ramus Osteotomy,along with Sagittal Split Ramus Osteotomy,is an popular surgical technique performed on mandibular prognathism. However Intraoral Vertical Ramus Osteotomy has been suspected for an initial mobilization at the healing phase of segment because it does not employ the rigid fixation between segments. To execute a study on the healing phase of segment after Intraoral Vertical Ramus Osteotomy on the horizontal plane, 102 patients (204 parts) who were diagnosed mandibular prognathism and took Intraoral Vertical Ramus Osteotomy at the Yonsei University dental hospital were observed during the period of before operation, immediately postoperation, 1 month, 3 months, 6 months, and 12 months. The change in the width of segment and horizontal angle of proximal segment and condylar head on the Submentovertex Cephalogram taken from those patients represented following results. 1. The width of proximal and distal segment decreased with the lapse of time. It decreased into 84.5% between immediate postoperative and 6M and even continued to decrease till 12M. 2. The horizontal angle of the proximal segment did medial rotation according as the lapse of time and rigorously continued till 3M. The rotation angle of condylar head indicated its tendency of recurrence to the original position but the entire recurrence was not allowed. The bigger an initial angle was, the higher was the tendency of recurrence after the operation while the rotation angle remained still bigger. 3. After grouping into group 1, group 2,and group 3 based on the extent of the variation of rotation angle of condylar head at immediate postoperative, the variation of rotation angle was measures in each group. The result presented that the initial rotation angle of condylar head had correlation with that of proximal segment but had no relation with the extent of setback of the mandible. However a quantitative analysis alone is not a sufficient method for analyzing the healing phase of segment on the horizontal plane.Therefore a multilateral analysis using 3 dimensional data such as CT is recommendable for the future study.

PURPOSE: The Unilateral neglect is characterized by difficulty shifting attention to the side of space opposite the brain lesion and frequently reducing use of contralesional extremities. This study was to identify whether the visual deprivation was responsible for head position on unilateral neglect patient after stroke. METHODS: A patient with left middle cerebral artery infarction participated in the study. We assessed neglect using line bisection and star cancellation test. Patient was instructed to maintain correct alignment of trunk and head in a sit position. We evaluated degree of head lateral tilting and rotation. Then, patient was blocked visual input. Also, we evaluated head position in the same way. RESULTS: He scored 3 points in the line bisection test and 9 points star cancellation test. In postural evaluation, he had deviated posture such as lateral head tilting and rotation. After visual cue deprivation, patient showed different head position which was decreased degree of head tilting and rotation. CONCLUSION: For vertical body orientation, it was used multiple sensory references including the vestibular, somatosensory, and visual system. This finding suggested that abnormal posture of neglect patient could be related to the visual input. It has important clinical implications in terms of understanding the neglect.

Objective: Superimposition of frontal cephalograms cannot be performed when the cephalograms are taken with different vertical head rotations. The purpose of the present study was to evaluate the validity of correcting the positional change of frontal cephalometric landmarks caused by vertical head rotation. Methods: In 30 adult individuals, frontal and lateral cephalograms were taken at a $90^{\circ}$ angle. Geometric principles of radiography were used to calculate the possible vertical and horizontal landmark changes if the head should be rotated down $5^{\circ}$ about an ear rod axis. The calculated changes were then compared with cephalometric changes measured on frontal cephalogram actually taken with the head rotated down $5^{\circ}$. Results: When the frontal cephalograms were taken with the head rotated down $5^{\circ}$ about an ear rod axis, significant changes in the vertical position of the landmarks occurred, particularly in the landmarks located farther anteriorly from the ear rod axis. The comparison of calculated changes and real cephalometric changes showed that the differences were less than 0.4 mm in the vertical direction and less than 0.2 mm in the horizontal direction. The differences between calculated and real changes were smaller in the landmarks less affected by vertical head rotation. Conclusions: Even when frontal cephalograms are taken at different vertical head rotations, the concomitant changes in the position of the landmarks can be corrected through calculation using the geometric principle of radiography as long as frontal and lateral cephalograms are taken perpendicular to each other.

Purpose: The purpose of this study was to investigate the effect of head position changes on vertical and horizontal magnification in dental panoramic radiographs. Materials and Methods: Five 4 mm metal balls were placed above alveolar crest of dry skull considering extraction socket and dental arch. Panoramic radiographs were taken by OP-100D (Instrumentarium Imaging Co., Tuusula, Finland) at proper and rotated head position along the sagittal and transverse axis at 3 - $20^{\circ}$ upwardly, downwardly, to the left and to the right rotation. Images were stored in DICOM files and were measured by ruler equipped within INFINITT PACS software. Results: The mean horizontal magnification was $1.22{\pm}0.01-1.44{\pm}0.01$ and mean vertical magnification was $1.29{\pm}0.00-1.35{\pm}0.02$ at standard head position. There was statistical significance of horizontal magnification between the anterior ($1.24{\pm}0.02-1.31{\pm}0.03$) and the posterior area ($1.40{\pm}0.03-1.33{\pm}0.02$) (P < 0.05). Vertical magnification resulted in less variation ($1.24{\pm}0.01-1.37{\pm}0.02$) than horizontal magnification ($0.88{\pm}0.03-3.73{\pm}0.16$) according to the rotation. There was statistical significant difference on horizontal magnification (P < 0.05). Conclusion: In rotated head position, the horizontal magnification should be considered because these can cause distortion on panoramic radiographs.
The Purpose of this study was to compare the degree of vortical head rotation and to evaluate the reproducibility of natural Head posture (NHP) according to two guiding methods, the head posture aligner (HPA) method and the solf balance posture (SBP) method. The subjects consisted of 30 adults. On the first day. lateral and frontal cephalometric radiographs were obtained through the two guiding methods. One mouth later. lateral and frontal cephalometric radiographs were obtained again through both guiding methods. The degrees of vertical head rotation of both guiding methods were compared and the reproducibility was evaluated for each guiding method. A comparison of the degrees of vortical head rotation for the two methods revealed that the vertical head posture was lower in the SBP method than in the HPA method by an average of $2.79^{\circ}$. All measurements obtained using the HPA and SBP methods with a time interval of one north did not show any significant difierence in lateral and frortal cephalometric radiographs. The results of the present study suggest that the SBP method may be used as an alternative to the HPA method in case the HPA method can not be applied.

Purpose: Recently, A Catcher was added to prevent sagging in Radixact® X9. In this study, We quantitatively compared general couch of Tomo-HDA® with catcher couch of Radixact® X9 using the human phantom and evaluated usefulness of catcher. Materials and methods: We used rando phantom for phantom study and set the each iso-center of head and neck region and Pelvis region for region parameter. Furthermore, We used hand made low melting point alloys for weight parameter. MVCT(Mega Voltage Computed Tomography) images were acquired for vertical error and rotation(pitch) error measurement increasing weight(A: 15kg, A+B: 30kg, A+B+C: 45kg). We selected 120 patients who has been treated using Tomotherpy machine for patient study. 60 patients has been treated in Tomo-HDA® and the other 60 patients treated in Radixact® X9. In the patient study methods, vertical error and rotation(pitch) error was measured for mean value calculation using MVCT images acquired on first day of radiation therapy. Result: Result of phantom study, Vertical error and rotation(pitch) error was increased proportionally increased as the weight increases in general couch of Tomo-HDA®. each maximum value was 7.52mm, 0.38° in head and neck region and 11.94mm, 0.92° in pelvis region. However, We could confirm that there was stable error range(0.02~0.1mm, 0~0.04°) in Catcher couch of Radixact®. Result of patient study, The head and neck region was measured 4.79mm 0.33° lower, and the pelvis region was measured 7.66mm, 0.22° lower in Catcher couch of Radixact® X9. Conclusion: In this study, Vertical error and rotation(pitch) error was proportionally increased as the weight increases in general couch of Tomo-HDA®. Especially, The pelvis region error was more increased than the head and neck region error. However, Vertical error and rotation(pitch) error was regularly generated regardless of weight or regions in CatcherTM couch of Radixact® X9 that this study's purpose. In conclusion, CatcherTM couch of Radixact® X9 can minimize mechanical error that couch sagging. Furthermore, The pelvis region is more efficiency than head and neck region. In radiation therapy using Tomotherapy machine, it is regarded that may contribute to minimizing unadjusted pitch error due to characters of Tomotherapy.
