Because arrhythmia occurs irregularly, it should be examined for at least 24 hours for accurate diagnosis. For this reason, this paper developed firmware software for arrhythmia detection and prevented consumption of temporal and human resources and enabled continuous management and early diagnosis. Prior to the experiment, the interval between the R peaks of the QRS Complex was calculated using the Pan-Tompkins algorithm. The developed firmware software designed and implemented an algorithm to detect arrhythmia such as tachycardia, bradycardia, ventricular tachycardia, persistent tachycardia, and non-persistent tachycardia, and a data transmission format to monitor the collected data based on BLE. As a result of the experiment, arrhythmia was found in real time according to the change in BPM as designed in this paper. And the data quality for BLE communication was verified by comparing the sensor's serial communication value with the Android application reception value. In the future, wearable devices for real-time arrhythmia detection will be lightweight and developed firmware software will be applied.
의료현장에서는 최근 디지털 헬스케어의 중요성이 대두되면서, 다양한 형태의 생체신호 측정 관련 연구가 활발히 진행되고 있다. 생체신호 중 가장 중요한 신호로 심전도를 들 수 있으며, 특히 부정맥 환자에 있어 심전도 신호의 연속 모니터링은 매우 중요하다. 부정맥은 동결절(sinus node), 동빈맥(sinus tachycardia), 심방조기수축(atrial premature beat, APB), 심실세동 (ventricular fibrillation) 등으로 그 발병원에 따른 형태가 다양하며, 발병 이후의 예후가 좋지 않으므로 일상 중 연속 모니터링은 부정맥의 조기 진단과 치료방향 설정에서 매우 중요하다. 부정맥 환자의 심전도 신호는 매우 불안정하며, 부정맥을 자동 검출하기 위한 주요 특징점으로 작용하는 정확한 R-peak 포인트의 검출이 어렵다. 본 연구에서는 연속 측정하는 홀터 심전도 모니터링 기기와 분석용 소프트웨어를 개발하였으며, 부정맥 데이터베이스를 통해 심전도 신호의 R-peak 효용성을 확인하였다. 향후 연구에서는 다양한 발병원인으로 인한 부정맥의 형태적 구분 및 예측을 위한 알고리즘과 임상 데이터에 근거한 유효성 검증에 관한 추가 연구가 필요하다.
Large variation in electrocardiogram (ECG) waveforms continues to present challenges in defining R-wave locations in ECG signals. This research presents a procedure to extract the R-wave locations by forward-backward (FB) algorithm and classify the arrhythmic beat conditions by using RR intervals. The FB algorithm shows forward and backward searching rules from QRS onset and eliminates lower-amplitude signals near the baseline using a statistical process control concept. The proposed algorithm was trained the optimal parameters by using MIT-BIH arrhythmia database (MITDB), and it was verified by actual Holter ECG signals from a local hospital. The signals are classified into normal (N) and three arrhythmia beat types including premature ventricular contraction (PVC), ventricular flutter/fibrillation (VF), and second-degree heart block (BII) beat. This work produces 98.54% accuracy in the detection of R-wave location; 98.68% for N beats; 91.17% for PVC beats; and 87.2% for VF beats in the collected Holter ECG signals, and the results are better than what are reported in literature.
Tetralogy of Fallot was repaired by a transatrial-transpulmonary approach in 91 of 250 patients treated surgically [including redo operations] between April 1986 and December 1989. Their age ranged from 6 months to 14 years [mean 39.7 months]. Associated cardiovascular anomalies were right aortic arch [n=22], ASD [n=12], PDA [n=5], persistent left SVC [n=5], and others [n=6]. PA index was measured pre-operatively since 1987 to estimate pulmonary artery size and safe total correction[mean 289$\pm$110mm2/BSA]. Eight patients received previous shunt take down procedure concomitantly. Pulmonary arteriotomy was extended through small pulmonary annulus to a minimal distance upon the right ventricular infundibulum and transannular patch was applied in 38 patients [41.3%], in 31 of them monocuspid patch was utilized. pRV/LV was measured at operation room in 77 patients [mean 0.58$\pm$0.36]. Operative mortality was 6.6% [6/91]. The causes of death were low cardiac output [n=5], arrhythmia[n=1] and respiratory failure [n=1]. At follow-up between 12 months and 57 months [mean 30.8 months] most patients were in New York Heart Association class I without cardiac medication. There was no late death, but reoperations were required in 3 patients to relieve residual right ventricular outflow obstruction. Thus successful repair of tetralogy of Fallot can be accomplished in most patients including infants by transatrial-transpulmonary approach and the better result can be anticipated with respect to postoperative right ventricular function and arrhythmia than the conventional transventricular approach.

In this paper, we proposed an algorithm for arrhythmia classification, which is associated with the reduction of feature dimensions by linear discriminant analysis (LDA) and a support vector machine (SVM) based classifier. Seventeen original input features were extracted from preprocessed signals by wavelet transform, and attempts were then made to reduce these to 4 features, the linear combination of original features, by LDA. The performance of the SVM classifier with reduced features by LDA showed higher than with that by principal component analysis (PCA) and even with original features. For a cross-validation procedure, this SVM classifier was compared with Multilayer Perceptrons (MLP) and Fuzzy Inference System (FIS) classifiers. When all classifiers used the same reduced features, the overall performance of the SVM classifier was comprehensively superior to all others. Especially, the accuracy of discrimination of normal sinus rhythm (NSR), arterial premature contraction (APC), supraventricular tachycardia (SVT), premature ventricular contraction (PVC), ventricular tachycardia (VT) and ventricular fibrillation (VF) were $99.307\%,\;99.274\%,\;99.854\%,\;98.344\%,\;99.441\%\;and\;99.883\%$, respectively. And, even with smaller learning data, the SVM classifier offered better performance than the MLP classifier.
심전도 신호 분석 및 부정맥 분류는 환자를 진단하고 치료하는데 중요한 역할을 한다. 부정맥은 맥박이 불규칙한 상태로 심실빈맥(VT)이나 심실세동(VF) 환자에게 심각한 위협이 될 수 있다. 심방조기수축(APC)과 상심실성빈맥(SVT), 심실조기수축(PVC)은 심실빈맥(VT)만큼 치명적이지는 않지만 심장질환을 진단하는데 중요한 부정맥이다. 본 논문은 2~3개의 부정맥 분류만을 고려한 기존의 방법을 극복하고 다양한 부정맥을 분류하기 위한 새로운 방법을 제시한다. 심전도 신호의 특징 추출을 위해서 EMD 방법으로 신호를 분해하여 IMFs를 얻는다. 입력 데이터의 양은 분류기 성능에 영향을 미치므로 신호 데이터의 차원을 감소시키기 위해 Burg 알고리즘을 IMFs에 적용하여 AR 계수를 구하고 여러 개의 이진 분류기를 결합한 다중 클래스 SVM의 입력으로 사용한다. 최적의 SVM 성능 파라미터를 선택하고 부정맥 분류에 적용한 결과 검출의 정확성은 96.8%~99.5%였다. 실험 결과는 제안한 EMD 방법에 의한 전처리 및 특징 추출과 다중 클래스 SVM에 의한 부정맥 분류의 유용성을 보여준다.
본 논문은 심전도(ECG) 신호로부터 조기심실수축(PVC)을 자동 탐지하는 방법으로 이산 웨이블릿 변환과 퍼지 신경망을 이용하는 방안을 제시하고 있다. 심전도 신호를 이산 웨이블릿 변환(DWT)으로 특징을 추출한 후, 퍼지 신경망으로 학습하여 정상 비트와 PVC 비트를 분류한다. 윈도우 크기는 R파를 기준으로 $-31/360{\sim}+32/360$초를 사용하며, 웨이블릿 변환은 d3, d4, d5의 웨이블릿 계수 14개를 사용한다. 퍼지 신경망은 가중 퍼지소속함수 기반 신경망을 사용한다. 본 논문은 벤치마킹 데이터로 MIT-BIH 부정맥 데이터베이스를 사용하여 Shyu 실험군(7개 레코드)에서는 전체 분류율에서 97.04% 보다 높은 99.91%의 신뢰성 있는 결과를 나타내었고, Inan 실험군(40개 레코드)에서는 각각 SE는 82.57% 보다 높은 84.67%, SP는 98.33% 보다 높은 99.39%, 전체 분류율은 96.85% 보다 높은 98.01%의 신뢰성 있는 결과를 나타내었다.
조기심실수축(PVC)은 가장 보편적인 부정맥으로 심실세동, 심실빈맥 등과 같은 위험한 상황을 유발할 수 있는 가능성을 가지고 있기 때문에 이의 조기 검출은 매우 중요하다. 특히 일반인들의 건강상태를 지속적으로 모니터링 해야 하는 헬스케어 시스템에서는 이를 위한 심전도 신호의 실시간 처리가 필요하다. 즉, 최소한의 연산량으로 정확한 R파를 검출하고, 대상 환자의 특징을 파악하여 PVC를 분류할 수 있는 적합한 알고리즘의 설계가 필요하다. 따라서 본 연구에서는 PVC 실시간 분류를 위한 환자 적응형 패턴 매칭 기법을 제안한다. 이를 위해 전 처리 과정과 적응 가변형 문턱 값과 윈도우를 통해 R파를 검출하였으며, 검출 대상에 따른 정상신호 군을 선별하고 이를 벗어나는 신호를 이상신호로 분류하기 위해 해쉬 함수를 통한 패턴 매칭 기법을 적용하였다. 제안한 알고리즘의 R파 검출 및 정상신호 분류 성능을 평가하기 위해서 MIT-BIH 부정맥 데이터베이스를 사용하였다. 성능평가 결과, R파는 평균 99.33%, 이상신호 분류에 대한 에러율은 0.32%로 나타났다.
The present study was performed to evaluate antiarrhythmic effects of KR-32570, a novel inhibitor of sodium hydrogen exchanger subtype-1 (NHE-1), in rat arrhythmia induced by focal ischemia and reperfusion. During ischemia, KR-32570 significantly decreased the number of premature ventricular contraction (PVC) from 152.0 times to 75.5, 52.4 and 20.0 times for 0.1, 0.3 and 1.0 mg/kg, respectively (p<0.05) and the duration of ventricular tachycardia (VT) from 88.1 s to 35.8, 7.7 and 1.3 s, respectively(p<0.05) in anesthetized rats subjected to 10-min coronary occlusion of coronary artery. Similarlt to ischemia-induced arrhythmia, KR-32570 significantly decreased reperfusion-induced arrhythmia including PVC (41.3, 21.5, 11.3 and 6.6 times at vehicle, 0.1, 0.3 and 1.0 mg/kg, respectively, p<0.05) and VT (100.5, 64.2, 25.8 and 25.2 s, respectively, p<0.05), and VF (86.9, 27.5, 6.9 and 0 s, respectively, p<0.05). Moreover, KR-32570 dose-dependently decreased the incidence of mortality occurring after reperfusion (41, 27, 18 and 0% at vehicle, 0.1, 0.3, 1.0 mg/kg, respectively). These results suggest that KR-32570 has a potent antiarrhythmic effect in rat arrhythmia induced by ischemia and reperfusion.
Computer assisted cardiac mapping system has made it possible to display local activation times of the heart using a simultaneous multi-point data aquisition system, and opened an era in electrophyslology guided cardiac arrhythmia surgery especially in ventricular tachycardia. In this study, we have developed a 64 channel computerized cardiacmapping system us:ng a micro-computer for basic reasearch of electrophysiology and electrical propagation in cardiac arrhythmias. The significant tasks of this study were the simultaneous acquisition of large amount of data from 64 sites, accurate and rapid analysis, and the effective display of the analyzed data. To solve these problems, we made a 64 channel signal pre-processing board in order to amplify and fitter the raw signals. And we developed the soflu'are Yor cardiac isochronous mapping whictl is presented immediately ama computer-generated graphics. This system is expected 4o enable us to study pathophyslology of cardiac arrhythmia and to improve the results of diagnosis and surgical treatments for cardiac arrhythmia.
