Delayed Cerebral Abscess as a Shunt Complication and Endoscopic Removal of the Ventricular Catheter and Abscess

  • Shunt infections are a common complication of ventriculoperitoneal (VP) shunts, but the formation of a brain abscess related to a shunt system is very rare. A 44-year-old woman had a VP shunt inserted for hydrocephalus secondary to a subarachnoid hemorrhage. She suffered an episode of meningitis and sepsis 8 months after the shunt operation. After recovering from the meningitis, she complained of a loss of cognitive function. An enhancing mass was found in the frontal lobe, around the frontal horn of the lateral ventricle, and the ventricular catheter was embedded inside the mass. The ventricular catheter and cerebral abscess were removed using neuroendoscopy. We present an interesting case of a shunt-related brain abscess which illustrates the usefulness of neuroendoscopy.

Influence of Intraventricular cAMP on the Renal Function of the Rabbit (가토(家兎)의 신장기능(腎臟機能)에 미치는 측뇌실내(側腦室內) cAMP의 영향(影響))

  • Cyclic adenosine monophosphate (cAMP), known as a versatile regulator of cellular processes and as a secondary messenger of various hormones and other biogenic agents, such as prostaglandins and histamine, induced prompt and transient antidiuresis followed by mild natriuresis and diuresis, when it was administered into the lateral ventricle of the rabbit in doses ranging from $100\;{\mu}g$ to 1 mg. The initial antidiuresis was brought about by the systemic hypotension, whereas the secondary diuresis seemed to be resulted from the decreased tubular reabsorption of sodium, suggestive of participation of certain endogenous natriuretic agent. This observation suggests that cAMP might be involved in the center-mediated renal action of prostaglandins.

  • There have been few reports documenting the outcome of arterial swich operations(ASO) in selected patients with transposition of great arteries(TGA) and with left ventricular outflow tract obstruction(LVOTO). In the case of TGA with LVOTO, if the atrial septal defect(ASD) is large and the ventricular septal defect(VSD) is restricive, this deprives the left ventricle(LV) of approporiate preload and could lead to underdevelopment of the ventircular mass and lead poor LV performance after the arterial switch operation, dspite a high pressure in the LV preoperatively. Because an increase in the systolic ventricular pressure is not necessarily paralleled by an increase in ventricular mass, which is also essential for optimal ventricular performance after the operation. We report here a case of rapid LV training after ASO in TGA with unprepared LV (because of large ASD and restrictive VSD) despite a high pressure in the LV(due to LVOTO) preoperatively.

  • We applied the ICA method to separate the ventricle and tissue components and to extract left ventricular input function from the H$_2$$^{15}$ O myocardial PET under the assumption that the elementary activities of ventricular pools and myocardium were spatially independent, and that the mixture of them composed dynamic PET frames. ICA-generated left ventricular input functions were compared with the ROI-generated ones, and also with the invasively derived arterial blood samples. Moreover, the rMBF calculated with the ICA-generated input functions and single compartment model was correlated with the results obtained with the radiolabeled microspheres.

  • An algorithm has been proposed for the automatic detection of optimal epiand endocardial left ventricular borders from 2-D short axis echocardiogram which is degraded by noise and echo drop out. For the implementation of the algorithm, we modified Ballard's Generalized Hough Transform which can be applicable only for deterministic object border, and newly proposed Fuzzy Hough Transform method. The algorithm presented here allows detection of object whose exact shapes are unknown. The algorithm only requires an approximate model of target object based on anatomical data. To detect the approximate epicardial contour of left ventricle, Fuzzy Hough Transform was applied to the echocardiogram. The optimal epicardial contour was founded by using graph searching method which contains cost function analysis process. Using this optimal epicardial contour and average thickness imformation of left ventricular wall, the approximate endocardial line was founded, and graph searching method was also used to detect optimal endocardial contour.

  • Since cardiac pacemaker was first totally implanted by Chardack, Gage and Greatbatch [1966], the electrical circuity of the cardiac pacemaker has been improved, modified, and refined. The problem of transvenous electrodes, however, is still remained; this may be due to electrode displacement, exit and/or entrance block, lead fracture and insulation defects. In permanent cardiac pacing, Irreversible loss of function of the transvenous electrode catheter eventually requires insertion of new lead. Authors now report one case that disclosed easy displacement of electrode tip in early phase of implantation and then two years and five months later, malfunctioning electrode could not be withdrawn from the cardiovascular system because it has become firmly enclosed by fibrous tissue along its course from the vein tract to the right ventricle. Under such circumstances, the electrode catheter tip was left in tricuspid annulus after being sutured at its entrance and burying the loop of lead in generator pocket. New other one electrode was then reimplantation through left external jugular vein.

Fermented ginseng, GBCK25, ameliorates hemodynamic function on experimentally induced myocardial injury

  • In the present study, we investigated whether treatment with GBCK25 facilitated the recovery of hemodynamic parameters, left ventricle systolic pressure, left ventricular developed pressure, and electrocardiographic changes. GBCK25 significantly prevented the decrease in hemodynamic parameters and ameliorated the electrocardiographic abnormality. These results indicate that GBCK25 has distinct cardioprotective effects in rat heart.

  • We have studied the effect and application of CST which is the function and disease of the autonomic nerve system based on recovery of flexibleness of the autonomic nerve. CST purpose to keep sustenance of human body by recovering joints have limitation of mobility and controlling autonomic nerve system by diminution tense of the sympathetic nerve. CST makes well flowing of CSF by pressure the fourth ventricle of the brain or relaxation of soft tissues of the base of a cranial bone. There is no result of studies about CST in Korea yet. It will bring a benefit if it prove the effect of CST in countries depend on drugs way high such as Korea. It needs positive approach and definite research for the improvement.

Myogenic Autoregulation of Coronary Vessels and Heterometric Autoregulation of the Myocardium

  • 1) At the isolated perfused guinea-pig and rat heart heterometric autoregulation of the myocardium and myogenic autoregulation of the coronary vessels were induced by means of stepwise increases of perfusion pressure. 2) According to this loading test Frank-Starling function curves of the left ventricle and pressure-flow curves of the coronary vessels can be drawn. This graphic evaluation gives more information about the condition of the heart and the coronary vessels than simple evaluation under hemodynamic equilibrium. 3) There are significant differences in both curves between animal species and between different perfusate Mg concentration. 4) Myogenic autoregulation is not affected by the cyclooxygenase inhibitors indometacin and me- clofenamate. Thus it appears unlikely that prostanoides are involved in myogenic autoregulation. 5) Ca antagonists (Gallopamil, prenylamine) depress myogenic autoregulation dose-dependently. Enhanced myogenic autoregulation, induced by low extracellular magnesium, can be reduced effectively by Gallopamil. 6) Ginsenosides from Panax ginseng as well as the ginsenoside 'Rg' are effective inhibitors of myogenic autoregulation without major negative inotropic effects.

  • Most of clinical morbidity in cardiology are associated with abnormalities of the left ventricle. Several methods have been developed to measure the left ventricular function, including cardiac catheterization with cineangiography, echocardiography, and systolic time interval. But these methods have many limitations. ECG gated cardiac blood pool scan provides a safe, noninvasive, repeatable method for determining the left ventricular function. Utilizing the cardiac blood pool scan, we measured the left ventricular function in 165 cardiac patients, and in 26 normal subject. 1. Left ventricular ejection fractions were measured by cardiac blood pool scan, and compared in 20 patients with that measured by x-ray cineangiography. Correlation coefficient was 0.885. 2. Ejection fractions were classified by funtional class made in New York Heart Association. Ejection fractions well represented the functional status. 3. Ejection fractions decreased in cardiomyopathy ($20.1{\pm}4.8%$) and ischemic heart disease ($34.4{\pm}16.7%$). Impaired ejection fractions in myocardial infarction were associated with the extent of infarction. 4. Regional left ventricular wall motion was evaluated from the end-diastolic and end-systolic images. In cardiomyopathy diffuse hypokinesia was noted and in myocardial infarction akinesia was noted on the infarcted areas.
