Clinical Outcomes of Minimally Invasive Surfactant Therapy via Tracheal Catheterization in Neonates with a Gestational Age of 30 Weeks or More Diagnosed with Respiratory Distress Syndrome

  • Purpose: Minimally invasive surfactant therapy (MIST) is currently used as a method of surfactant replacement therapy (SRT) for the treatment of respiratory distress syndrome (RDS) in preterm infants with a gestational age of less than 30 weeks. However, few studies have been conducted on MIST in neonates with a gestational age of 30 weeks or more. In this study, we compared MIST with endotracheal intubation as a rescue SRT for spontaneously breathing neonates with a gestational age of 30 weeks or more who were diagnosed with RDS. Methods: We investigated the clinical characteristics of spontaneously breathing neonates admitted to the neonatal intensive care unit of the Inje University Sanggye Paik Hospital from January 1, 2014 to December 31, 2016. These neonates were born at a gestational age of 30 weeks or more and were diagnosed with RDS. The neonates who were administered surfactant by MIST were categorized into the MIST group (n=16) and those who underwent endotracheal intubation were categorized into the control group (n=45). Thereafter, the clinical characteristics between the groups were compared. Results: Compared to the control group, the MIST group was less likely to require mechanical ventilation within 72 hours (P<0.001). The frequency of bradycardia during SRT was also low in the MIST group (P=0.033). Conclusion: MIST is considered relatively feasible and safe for treating RDS for reducing the need for mechanical ventilation and decreasing the occurrence of bradycardia during surfactant administration in neonates with a gestational age of 30 weeks or more.

A Study on the development of a decision model on free flow and congested traffic conditions to determine the optimal ventilation capacity in highway tunnels (고속도로 터널의 적정 환기용량 계획을 위한 원활 및 지체조건 판별모델 개발에 대한 연구)

  • According to the local highway tunnel ventilation guideline, ventilation capacity calculation should be performed at the speed ranging from 10 km/h to 80 km/h. This is so reasonable method considering uncongested and congested traffic conditions in urban tunnels. But recently due to low traffic volume and very low congestion frequency in rural highway tunnels, it seems to be an inadequate way to apply the guideline. Therefore the calculation should be performed separately for the free flow and congested traffic cases classified by the appropriate decision model. This paper aims at determining unnecessary running speed range for reasonable tunnel ventilation design, considering free flow and congested traffic conditions. Firstly, traffic volumes in highway tunnels were collected and if any, the causes of congestion were investigated. And with concept of 'margin speed'($u-u_m$), the decision model on traffic congestion was developed. Applicability of the decision model was also analyzed with case study. According to the results, when design speed is 100 km/h, with V/C less than 0.1, then the range of unnecessary speed in tunnel ventilation design is less than 40 km/h; for $V/C{\leqq}0.35$, $V/C{\leqq}0.6$ and $V/C{\leqq}0.75$, the unnecessary speed ranges are found to be ${\leqq}30$, ${\leqq}20$ and ${\leqq}10km/h$, respectively.

Ventilation Effect of the Greenhouse with Folding Panel Type Windows (패널굴절방식 환기창 온실의 환기효과)

  • In this study, new development of natural ventilation window was accomplished to control environment of greenhouse with no use of farced ventilation during hot season. The ventilation effect of developed ventilation window was investigated in experimental greenhouse which was designed using side wall panel and folding type panel fur natural ventilation. Folding panel type ventilation window was designed to open upper part of the side wall and top of the roof using two hinges which are located bottom of the side wall and the roof panel to grab one side of each panels and guide the other side along with the guidance rail. Developed ventilation window has top ventilation part with maximum moving distance X=ι (1-cos$\theta$)=848.5 mm and side ventilation part with maximum moving distance Y=ι/2 $\times$sin$\theta$=1,184.4 mm at 45$^{\circ}$ of theoretical opening angle. It took 4.5 minutes to open roof vent fully and temperature at 1.2 and 0.8 m height decreased after 1 minute from starting opening and became equilibrium state maintaining 3-4$^{\circ}C$ difference after 2 minutes from complete opening. Air exchange rate was 15.2~39.3 h$^{-1}$ which was more than 10~15 h$^{-1}$ of continuous type and Venlo type greenhouse. The descent effect of temperature by ventilation windows was two times higher than Venlo type greenhouse.

Comparison of Respiratory Mechanics and Gas Exchange Between Pressure-controlled and Volume-controlled Ventilation (압력조절환기법과 용적조절환기법의 호흡역학 몇 가스교환의 비교)

  • Background : Pressure-controlled ventilation (PCV) is frequently used recently as the initial mode of mechanical ventilation in the patients with respiratory failure. Theoretically, because of its high initial inspiratory flow, pressure-controlled ventilation has lower peak inspiratory pressure and improved gas exchange than volume-controlled ventilation (VCV). But the data from previous studies showed controversial results about the gas exchange. Moreover, the comparison study between PCV and VCV with various inspiration : expiration time ratios (I : E ratios) is rare. So this study was performed to compare the respiratory mechanics and gas exchange between PCV and VCV with various I : E raitos. Methods : Nine patients receiving mechanical ventilation for respiratory failure were enrolled. They were ventilated by both PCV and VCV with various I : E ratios (1 : 2, 1 : 1.3 and 1.7 : 1). $FiO_2$, tidal volume, respiratory rate and external positive end-expiratory pressure (PEEP) were kept constant throughout the study. After 20 minutes of each ventilation mode, arterial blood gas, airway pressures, expired $CO_2$ were measured. Results : In both PCV and VCV, as the I : E ratio increased, the mean airway pressure was increased, and $PaCO_2$ and physiologic dead space fraction were decreased. But P(A-a)$O_2$ was not changed. In all three different I : E ratios, peak inspiratory pressure was lower during PCV, and mean airway pressure was higher during PCV. But $PaCO_2$ level, physiologic dead space fraction and P(A-a)$O_2$ were not different between PCV and VCV with three different I : E ratios. Conclusion : There was no difference in gas exchange between PCV and VCV under the same tidal volume, frequency and I : E ratio.

A Study on the Active Noise Control in Duct (닥트내 소음의 능동제어에 관한 연구)

  • There have been experiments dealing with the possibility of the actualization of the ANC system by means of operating the DSP adaptation filter. This filter is composed of various filters(including X-LMS algorithm, Filter-U algorithm, and Full-Feedback-Filter-U algorithm) that use ventilation fans and loudspeakers as a primary source in a circular duct as an experimental device. In this operation, the ANC system using the X - LMS algorithm was found to be more effective in reducing noise than without such system. When applying the input signal of the DSP board Full Feedback-Filtered-U algorithm system while having in mind that the additionally installed second control signal was gone through feedback and mixed into the detection microphone installed near the ventilation fan when using the first ventilation fan, the system was not emitted, but maintained stable during the operation of the control filter. At this point, noise tended to decrease at a maximum of l0dB compared to other algorithms at the frequency band of 170-250Hz.

Exposure Level of Airborne Bacteria in the University Laboratories in Seoul, Korea

  • This study evaluated the bacterial concentrations and affecting factors at the laboratories of a university in Seoul, Korea. Thirty-three samples of total airborne bacteria (TAB) and eighteen samples of gram negative bacteria (GNB) were collected from both microbiology laboratories (7) and chemistry laboratories (6). GM (GSD) of TAB and GNB concentrations were 194 (2.52) $cfu/m^3$, 24 (4.1) $cfu/m^3$, respectively. TAB concentrations in the chemical laboratories (GM (GSD): 193 (2.0) $cfu/m^3$) were not significantly different from those in microbial laboratories (GM (GSD): 202 (2.7) $cfu/m^3$, (p>0.05)). GM (GSD) of TAB concentrationsat the top of sink, the center of laboratory, and the front of ventilation ventilation device within laboratories, 182 (3.2) $cfu/m^3$, 217 (2.2) $cfu/m^3$, 176 (2.4) $cfu/m^3$, respectively, were not significantly different (p=0.48). Related factors were measured such as temperature, relative humidity, floor of laboratory, number of persons and laboratory area. TAB concentrations were significantly related to temperature (r=0.36, p<0.05), and the floor of laboratory and temperature were also significantly related (r=0.49, p<0.001). However, other factors such as relative humidity, number of persons and laboratory area did not show any significant relationship with TAB concentrations (p>0.05). TAB concentrations were affected significantly by cleaning frequency (p<0.001) and floor of laboratory (p<0.05). There was also a significant difference (p<0.01) between TAB indoor concentrations and TAB outdoor concentrations. However, other factors such as general ventilation did not affect TAB concentrations (p>0.05) in this study.

Mass-production of Eleutherococcus seoulensis Seedlings Through Somatic Embryogenesis (체세포배 형성을 통한 서울오갈피(Eleutherococcus seoulensis) 묘목의 대량생산)

  • This study was conducted to establish the optimal condition for acclimatization from somatic embryos of Eleutherococcus seoulensis. Torpedo-shaped embryos of Eleutherococcus seoulensis were cultured on 1/3 MS and WPM media supplemented with $GA_3$ (3.0, 5.0 mg/L) for 4 weeks. Plentlets were transferred to 1/2 SH solid medium with 1.0 mg/L $GA_3$ and 0.2% activated charcoal for shoot and root elongation and them elongated plantlets further developed on 1/2 SH medium for 4 weeks. Developed plantlets further elongated into well-shaped leaf and root system on 1/3 SH medium under ventilation condition for 4 weeks. Plantlets grew normally on 1/3 SH basal medium, were acclimated on various soil. Survival frequency of plantlets was influenced by soil type(peatmoss+perlite, perlite, soil on Nam mountain). The highest survival rate to soil was more than 70% when plantlets were 1/3 SH medium under ventilation condition in Nam mountain soil. These results indicate that the systematic procedure of plant production in Eleutherococcus seoulensis could be practically applicable for mass propagation.

Factor Associated with Injury Related to Home Mechanical Ventilation in General Ward Patients: A Retrospective Study (가정용 인공호흡기 관련 안전사고 특성 및 손상 영향 요인 분석: 상급종합병원 일반병동 환자 중심으로)

  • Purpose: This study aims to describe the characteristics of safety incidents and factors associated with injury for patients with Home Mechanical Ventilation (HMV) at the hospital. Methods: This is a retrospective study. Data were collected from the work log of respiratory home care nurses and the patients' electronic medical records were investigated. In order to compare group differences, independent t-test and χ2 test were used. Associated factors with injury development were identified by generalized mixed modeling analysis controlling for age and gender. Results: A total of 304 patients on HMV were included in this study, among which 129 (42.4%) experienced 352 HMV-related incidents. Mean frequency of incidents for each patient was 5.11±3.98, ranged from 1 to 15 times. In 19.0% of the incidents, injury was developed. Types of incident and persons involved in the incidents were significantly associated with the patient's injury. In the case of the safety incidents, patient's injury was significantly higher in accidents caused by respiratory circuit problems compared to those caused by problems with the ventilator operation by the medical staff (coefficient=1.25, p=.020). In addition, in the case of those involved in the safety incidents, patient's injury was significantly higher in the accident caused by the patient family members or caregivers than that caused by the medical personnel (coefficient=1.25, p=.019). Conclusion: In order to minimize injury caused by incidents in patients with HMV, hospitals need to provide systemic education to their medical staff and caregivers to enhance awareness of the importance of reporting and safety management.

In vitro propagation of endangered species, Hylotelephium ussuriense (Kom.) H. Ohba (멸종위기종 둥근잎꿩의비름 (Hylotelephium ussuriense (Kom.) H. Ohba)의 기 내 증식)

  • To establish the system of in vitro plant regeneration, the different explants (stem with axillary bud and stem without axillary bud) of Hylotelephium ussuriense were cultured on the Murashige and Skoog's medium containing 6-benzylaminopurine (BA) and indolebutyric acid (IBA). The adventitious shoot induction was more effective in the stem with axillary bud explants than the stem without axillary bud explants, and was the best on MS medium containing 3.0 mg/L BA and 0.01 mg/L IBA. Frequency of plantlet growth was not significantly treated on MS and sucrose. Total chlorophyll contents under ventilation treatment were higher than those in control (non-ventilation). This in vitro propagation protocol will be useful for conservation and mass propagation of this endangered plant.

The Administration Patterns of Sedatives for Sedation of Mechanically Ventilated Patients and Patient Assessment by Nurses (기계환기기 치료 환자들의 진정상태유지를 위한 투약양상 및 사정내용에 대한 일 조사연구)

  • Purpose: The purpose of this descriptive study was to investigate the administration patterns of sedatives and paralytic agents for mechanically ventilated patients. Method: The participants were 18 adult patients who received mechanical ventilation therapy for more than 1 day in the MICU. Collected data were type of injected medications, administration interval, injected amount, administration purpose and patient assessment by nurses. Result: Mean age of the patients was 61 years old. Seventy-two percent were male. Only 2 drugs (midazolam and norcuron) were administered to most of subjects. The type of medication administered was less heterogeneous than in previous studies. Most of the patients received sedatives and paralytics intermittently. Six different methods were used by nurses to administer sedatives and paralytics. The mean injection frequency was 9.3 times/day for sedatives and 10.8 times/day for paralytic agents. The most common purpose for administration of the drugs was full sedation of the patient (57%). Conclusion: Since a few patients received sedatives and paralytic agents higher than usual dose and a few Patients received those drugs lower than usual dose, nurses need to use proper protocols and guidelines for sedation to avoid oversedation and undersedation.
