  • 도재 라미네이트 비니어(Porcelain laminate veneer, PLV)는 아주 얇은 도재를 치면에 직접 적용시키는 술식으로 전치부 심미 개선을 목적으로 전부피개관 대신에 보존적으로 사용될 수 있다. 그러나 PLV의 성공을 높이기 위해 치질 삭제량을 적절히 조절하는 것은 어려울 뿐만 아니라, 각 치아마다 법랑질 두께가 다양하고 증령에 따라 치관구조 역시 변화하기 때문에 치아 삭제시 주의가 필요하다. 즉 치아 순면을 따라 일관되게 삭제하는 기존의 방법을 이용할 경우 법랑질이 얇아진 부위에서는 상아질이 노출될 위험이 있다. 이를 방지하기 위해 다양한 치아 삭제 방법들이 소개되고 있는데 진단 납형제작을 통한 실리콘 인덱스와 아크릴릭 레진 mock-up을 이용한 방법은 최종적인 수복물의 형태에 기초하여 치아를 삭제할 수 있어 불필요한 법랑질 삭제를 줄이고 상아질 노출을 최소화할 수 있다. 또한 비정상적으로 배열된 치아 삭제시에도 적절한 삭제량 결정이 어려워 삭제량이 과도해지거나 부족해지기 쉬운데, 최종 보철물의 형태를 기준으로 실리콘 인덱스를 제작하면 이를 방지할 수 있다.

Interfacial modulus mapping of layered dental ceramics using nanoindentation

  • PURPOSE. The aim of this study was to test the modulus of elasticity (E) across the interfaces of yttria stabilized zirconia (YTZP) / veneer multilayers using nanoindentation. MATERIALS AND METHODS. YTZP core material (KaVo-Everest, Germany) specimens were either coated with a liner (IPS e.max ZirLiner, Ivoclar-Vivadent) (Type-1) or left as-sintered (Type-2) and subsequently veneered with a pressable glass-ceramic (IPS e.max ZirPress, Ivoclar-Vivadent). A $5{\mu}m$ (nominal tip diameter) spherical indenter was used with a UMIS CSIRO 2000 (ASI, Canberra, Australia) nanoindenter system to test E across the exposed and polished interfaces of both specimen types. The multiple point load - partial unload method was used for E determination. All materials used were characterized using Scanning Electron Microscopy (SEM) and X - ray powder diffraction (XRD). E mappings of the areas tested were produced from the nanoindentation data. RESULTS. A significantly (P<.05) lower E value between Type-1 and Type-2 specimens at a distance of $40{\mu}m$ in the veneer material was associated with the liner. XRD and SEM characterization of the zirconia sample showed a fine grained bulk tetragonal phase. IPS e-max ZirPress and IPS e-max ZirLiner materials were characterized as amorphous. CONCLUSION. The liner between the YTZP core and the heat pressed veneer may act as a weak link in this dental multilayer due to its significantly (P<.05) lower E. The present study has shown nanoindentation using spherical indentation and the multiple point load - partial unload method to be reliable predictors of E and useful evaluation tools for layered dental ceramic interfaces.

Effects of different surface treatments on the shear bond strength of veneering ceramic materials to zirconia

  • PURPOSE. To evaluate and compare the effect of different materials and techniques on the shear bond strength of veneering ceramic materials to zirconia. MATERIALS AND METHODS. 136 sintered zirconia cubes were prepared and randomly divided into four study groups according to corresponding methods of surface treatment and materials: GLN (grinding followed by laser scanning using Noritake Cerabien ZR), SLN (sandblasting followed by laser scanning using Noritake Cerabien ZR), GLV (grinding followed by laser scanning using VITA VM 9), and SLV (sandblasting followed by laser scanning using VITA VM 9). Spraying technique was performed to coat the core. Profilometer, SEM, XRD, EDS, universal testing machine, and stereomicroscope were used to record surface roughness Ra, surface morphology, phase transformation, elemental compositions, shear bond strength SBS values, and failure types, respectively. Specimens were investigated in unaged (not immersed in artificial saliva) and aged (stored in artificial saliva for a month) conditions to evaluate SBS values. RESULTS. Grinding and GLN as first and second surface treatments provided satisfactory Ra values in both conditions ($1.05{\pm}0.24{\mu}m$, $1.30{\pm}0.21{\mu}m$) compared to sandblasting and other groups (P<.05). The group GLN showed the highest SBS values in both conditions ($30.97{\pm}3.12MPa$, $29.09{\pm}4.17MPa$), while group SLV recorded the lowest ($23.96{\pm}3.60MPa$, $22.95{\pm}3.68Mpa$) (P<.05). Sandblasting showed phase transformation from t-m. Mixed failure type was the commonest among all groups. CONCLUSION. GLN showed to be a reliable method which provided satisfactory bond strength between the veneer ceramic and zirconia. This method might preserve the integrity of fixed dental crowns.

도재 보철물 색조의 처방에 관한 고찰 (A study on ceramic shade prescriptions for work authorizations)

  • This paper aims to examine ceramic shade prescriptions for work authorizations in all its aspectclincal information, recording information, technical considerations, extended application, practical applications. The results are as follows : 1. The basic application provides essential data for ceramic veneer fabrication. 2.Three-dimensional information divides the space for body and enamel components. 3. The second body shade selection improves shade matching in the middle one-third of the tooth and enhences the appearance of the enamel component. 4. Ceramists are encouraged to recognize the influence each component has upon the total shade composition. 5. A format is established for recording clinical information. 6. The use of available ceramic systems is seemingly unlimited. The method presented provides flexisility for recording the most detailed shade information in simple graphic and verbal terms. 7. Technical simplification is a by-product of complete, precise work information.

How will surface treatments affect the translucency of porcelain laminate veneers?

  • PURPOSE. The purpose of this study was to evaluate whether surface treatments affect the translucency of laminate veneers with different shades and thicknesses. MATERIALS AND METHODS. A total of 224 disc-shaped ceramic veneers were prepared from A1, A3, HT (High Translucent) and HO (High Opaque) shades of IPS e.max Press (Ivoclar Vivadent) with 0.5 mm and 1.0 mm thicknesses. The ceramics were divided into four groups for surface treatments. Group C: no surface treatments; Group HF: etched with hydrofluoric acid; Group SB: sandblasted with 50-${\mu}m$ $Al_2O_3$; and Group L; irradiated with an Er;YAG laser. A translucent shade of resin cement (Rely X Veneer, 3M ESPE) was chosen for cementation. The color values of the veneers were measured with a colorimeter and translucency parameter (TP) values were calculated. A three-way ANOVA with interactions for TP values was performed and Bonferroni tests were used when appropriate (${\alpha}=0.05$). RESULTS. There were significant interactions between the surface treatments, ceramic shades and thicknesses (P=.001). For the 0.5-mm-thick specimens there were significant differences after the SB and L treatments. There was no significant difference between the HF and C treatments for any shades or thicknesses (P>.05). For the 1-mm-thick ceramics, there was only a significant difference between the L and C treatments for the HT shade ceramics (P=.01). There were also significant differences between the SB and C treatments except not for the HO shades (P=.768). CONCLUSION. The SB and L treatments caused laminate veneers to become more opaque; however, HF treatment did not affect the TP values. When the laminate veneers were thinner, both the shade of the ceramic and the SB and laser treatments had a greater effect on the TP values.

지르코니아 코어 두께에 따른 전부도재관의 상부도재 축성방법이 보철물 색조에 미치는 영향 (The Effect of Veneering Techniques on the Color Parameters of Y-TZP Based All Ceramic Restoration of Varying Ceramic Core Thickness)

  • 이 연구에서는 지르코니아 코어의 두께를 달리 제작하여 동일한 색조의 상부도재를 축성함에 있어 제작방법을 다르게 하여 두 상반된 시편을 각각 분광측색장치를 이용하여 CIE $L^*,a^*,b^*$ 색체계로 산출하여 객관적으로 색차를 비교, 분석하여 다음과 같은 결론을 얻었다. 1. 지르코니아 코어의 두께가 두꺼워 지고 상부도재의 두께가 얇아지면서 $L^*$ 값의 수치는 높아지며 $a^*,b^*$ 값은 낮아지는 경향을 띠었다. 상부도재 축성방법을 다르게 제작한 시편에서도 같은 결과를 보였다. 동일한 색조 시편에서 HP군은 PST군에 비해 $L^*$ 값의 수치는 낮게 측색되어 어두운 경향을 보였으며 $a^*,b^*$ 값도 낮은 수치를 보여 황색의 경향을 띠었다. 2. HP군, PST군 모두 동일한 색조에서 지르코니아 코어 두께와 상부도재 제작방법 간에 교호작용은 없었다(P=.083). 지르코니아 코어의 두께에 따라서는 통계적으로 유의한 차이는 존재하였으며(P=.001), 두께가 0.3 mm의 시편 A1, A2 색조에서 색차 값($E^*$)은 3.7 이상으로 나타났고, 그 이외 시편에서는 3.7 이하로 분석되어 임상가능 허용치로 나타났다. 이상의 결과로 볼 때 지르코니아를 이용한 전부 도재관 제작 시 같은 색조의 재료 일지라도 제작방식과 상부도재의 재료에 따라 색차 값이 존재하므로 이를 감안하여 보철물 제작에 사용해야 할 것으로 사료되며, 특히 코어의 두께가 얇은 보철물에서는 임상 허용치 이상의 색차 값을 보이므로 코어의 두께를 좀 더 추가하여 설계한 보철물이 바람직하다고 생각된다.

Comparison of the translucency of shaded zirconia all-ceramic systems

  • PURPOSE. The purpose of this study was to evaluate and compare the translucency of shaded zirconia allceramic systems. MATERIALS AND METHODS. Translucency of 3 different zirconia all-ceramic systems colored by different techniques was compared with a lithium disilicate glass-ceramic (IPS e.max Press). Square-shaped specimens with 0.5 mm thickness were fabricated from In-Ceram YZ, ICE Zirkon and Katana systems in A1, A2 and A3.5 shades according to Vitapan Classical shade tab (n=11). Specimens were then veneered and glazed with corresponding veneer ceramic recommended by each zirconia system manufacturer and the total thickness was set to 1.5 mm. Translucency measurements were performed with VITA Easyshade Compact spectrophotometer after each stage and translucency parameter was calculated. Data were statistically analyzed with repeated measures ANOVA and Tukey multiple comparison test. RESULTS. The control group was significantly more translucent than the zirconia systems (P<.05). ICE Zirkon cores showed the least translucency; neither In-Ceram YZ nor Katana systems were superior to each other in terms of translucency. Translucency of all specimens was decreased after veneering, and the translucency rankings were changed. CONCLUSION. Coloring technique did not have a significant effect on translucency of zirconia cores. Although zirconia systems were less translucent than lithium disilicate glass ceramic, they had partial translucency and there were translucency differences among the zirconia systems. Chroma affected the translucency of precolored zirconia cores.

샌드블라스팅 처리와 라이너가 지르코니아와 전장도재의 전단결합 강도에 미치는 영향 (Effect of sandblasting and liner on shear bond strength of veneering ceramic to zirconia)

  • Purpose: This study aimed to compare the shear bond strength between zirconia cores and veneer ceramics as per the sand blasting and liner treatments. Methods: The following 4 groups of zirconia-veneering ceramic specimens were prepared: (1) Group I, untreated; (2) Group II, with 110 ㎛ aluminium oxide (Al2O3) sandblasting; (3) Group III, with liner (IPS e.max ZirLiner; Ivoclar Vivadent); and (4) Group IV, with 110 ㎛ Al2O3 sand blasting and liner. Surface roughness was measured for all the prepared specimens, and the surface morphology was observed using a scanning electron microscope. All the samples (n=40) were fixed with measuring jigs, and shear bond strengths were obtained using a universal testing machine with a crosshead speed of 0.5 mm/min. The shear bond strength data were analyzed using one-way analysis of variance and t-test. The post hoc comparison was performed using the Tukey's test (α=0.05). Results: A significant difference in the surface roughness was observed between the specimens of groups I and II (p<0.05). Surface treatment with liner and sandblasting showed higher shear bond strength between zirconia core and veneering ceramic (p<0.05). Conclusion: The sand blasting and liner treatment increased the shear bond strength between zirconia core and veneering ceramic.

DICOR와 G-CERA PORCELAIN LAMINATE VENEER의 전단결합강도에 관한 비교연구 (A COMPARATIVE STUDY ON THE SHEAR BOND STRENGTH OF DICOR AND G-CERA PORCELAIN LAMINATE VENEER)

  • Cermic has been widely used because of its excellent esthetics and strength. The recently introduced castable ceramic system is regarded as the more esthetic and biocompatible restorative material. The purpose of this study was to compare the shear bond strength of Dicer & G-Cera porcelain laminate veneer according to the type of cement and surface treatment and to observe the surface of bonding failure with SEM. Total forty disks(3.5mm $diam.\times2.0mm$ thickness) were prepared. Forty extracted human maxillary central incisor teeth were stored in saline solution. Ten teeth were bonded to Dicer specimen with Dicer ZPC cement and ten teeth were bonded with Dicer resin cement. Ten silicoated G-Cera specimen and ten non-silicoated G-Cera specimen were bonded to teeth with G-Cera resin cement. Bonded units were mounted in a plastic tube with hard stone and stored in a humidor at $37^{\circ}C$ for 24 hours. Shear bond strength was measured by Instron Universal Testing Machine (Model 1125) and all the specimen were observed with SEM(JEOL, JSM-T2000)and modes of failure were recorded. The obtained results were as follows: 1. The mean shear bond strength of Dicer bonded with Dicer resin cement was 11.62 MPa and that bonded with Dicor ZPC cement was 0.88 MPa : Shear bond strength of Dicer bonded with Dicer resin cement was significantly increased(P<0.05). 2. The mean shear bond strength of silicoated G-Cera was 13.10 MPa and that of non silicoated G-Cera was 10.93 MPa : Shear bond strength of silicoated G-Cera was not significantly increased (P>0.05). 3. Shear bond strength of Dicer and G-Cera porcelain laminate veneer was not significantly different (P>0.05). 4. In observation of bond failure with SEM, Dicer bonded with Dicer ZPC cement exhibited adhesive failure. Dicer bonded with Dicer resin cement and silicoated and non silicoated G-Cera exhibited cohesive failure.

The effect of zirconia surface architecturing technique on the zirconia/veneer interfacial bond strength

  • PURPOSE. The purpose of this study was to evaluate the effect of the zirconia surface architecturing technique (ZSAT) on the bond strength between veneering porcelain and zirconia ceramic. MATERIALS AND METHODS. 20 sintered zirconia ceramic specimens were used to determine the optimal surface treatment time, and were randomly divided into 4 groups based on treatment times of 0, 1, 2, and 3 hours. After etching with a special solution, the surface was observed under scanning electron microscope, and then the porcelain was veneered for scratch testing. Sixty 3 mol% yttria-stabilized tetragonal zirconia polycrystal ceramic blocks were used for tensile strength testing; 30 of these blocks were surface treated and the rest were not. Statistical analysis was performed using ANOVA, the Tukey post-hoc test, and independent t-test, and the level of significance was set at ${\alpha}=.05$. RESULTS. The surface treatment of the zirconia using ZSAT increased the surface roughness, and tensile strength test results showed that the ZSAT group significantly increased the bond strength between zirconia and veneering porcelain compared to the untreated group (36 MPa vs. 30 MPa). Optimal etching time was determined to be 2 hours based on the scratch test results. CONCLUSION. ZSAT increases the surface roughness of zirconia, and this might contribute to the increased interfacial bond strength between zirconia and veneering porcelain.
