심장과 대혈관 기원의 원발성 종괴는 매우 드물다. 그중에서도 악성 말초신경초종은 더 드물게 보고되었다. 76세 남성에서 발견된 흉수의 원인 평가를 위해 시행한 조영증강 컴퓨터단층촬영에서 우폐정맥과 좌심방을 침범하는 저음영의 종괴가 발견되었고 초음파상 우폐정맥에서 기인한 종괴로 의심되었다. 종괴는 수술적 절제를 통해 폐정맥 기원의 악성 말초신경초종으로 최종 진단되었다. 저자들은 국내에서 보고된 바가 없는 드문 증례인 폐정맥 기원의 원발성 악성 말초신경초종을 보고하고, 종괴가 폐정맥 기원임을 시사하는 영상의학적 소견에 대해 고찰하고자 한다.
일차성고알도스테론혈증은 고혈압의 원인 중 높은 비중을 차지하는 질병이다. 부신정맥채혈술은 일차성고알도스테론혈증의 원인을 감별하여 최종적인 치료방침을 결정하는 데 있어 필수적인 검사이다. 부신정맥채혈술의 성공 여부는 각 부신정맥에서 채혈한 샘플의 혈중 코티솔 농도와 말초혈관에서 채혈한 샘플의 혈중 코티솔 농도의 비를 측정하여 판단한다. 하대정맥으로 바로 연결되는 오른부신정맥에서 시술의 실패율이 더 높게 보고되며 상대적으로 왼부신정맥의 시술 실패율은 낮지만 드물게 보고된다. 본 임상화보에서는 왼부신정맥 부신정맥채혈술의 실패 사례를 소개하고 분석하여 최적의 부신정맥채혈술을 위한 고려사항에 대해 고찰하고자 한다.
Purpose: The vein graft was considered as a useful conduit for nerve defect. But the problem is that it might be collapsed in long vein graft state. A new experimental model using vein graft filled with hyaluronic acid was considered. Methods: Thirty rats were used for the experimental animal. In group I, one side of the femoral nerve was exposed and a segment was removed about 15mm. The neural gap was connected with nerve graft. In group II, the nerve gap was connected with vein graft only. In group III, the nerve gap was connected with vein graft filled with hyaluronic acid. A walking track analysis was made periodically for 2 months and NCV(nerve conduction velocity) was executed at the end of the experiment. And morphologic studies were also done for all groups Results: In a walking track analysis, the toe-spread was widen and the foot-length was lengthened. The recovery of the toe-spread and foot length was checked 2 weeks interval, periodically for two months. The SFI (sciatic function index) was $-52.5{\pm}8.2$ in group I, $-68.1{\pm}4$ in group II, $-55.3{\pm}7.9$ in group III. In electrophysiological study, NCV(nerve conduction velocity) was $26.71{\pm}3.11m/s$ in group I, $17.94{\pm}4.35m/s$ in group II, $25.69{\pm}2.81m/s$ in group III. The functional recovery in group I and III was superior to that the group II statistically(p < 0.05) Under electromicroscopic study, the number of the myelinated axons were $1419.1{\pm}240$ in group I, $921.7{\pm}176.8$ in group II, $1322.2{\pm}318$ in group III. The number of the myelinated axons were much more in group I and III than group II statistically (p<0.05). Conclusion: This study suggested that the vein graft filled with hyaluronic acid is more effective than vein graft only for the conduit of the nerve gap. It was thought that the technique could be used in clinical cases with nerve defects as an alternative method to classical nerve grafts.
The Tongyoung deposits are epithermal gold and silver bearing quartz-rhodochrosite vein type deposits of late Cretaceous. They occurs in the andesite and tuff breccia member called Gyeongsang basin. Four mineralizations can be distinguished at the mine based on macrostructures. From earlist stage to lastest stage they are: stage I, base-metal quartz vein; stage II, rhodochrosite vein (IIA) and Pb-Zn vein (IIB); stage III, barren quartz vein; stage IV, calcite-ankeritic rhodochrosite veins. Gold and silver mineralizations occur predominantly in the stage I and IIB. Electrum is closely associated with galena, sphalerite and pyrite, and has chemical compositions of 50.98-64.05 atom % Ag. Sphalerite contains 2.09-5.05 mol % FeS and 0.34-2.01 mol % MnS in the stage I, and 2.01-3.41 mol % FeS and 0.21-2.80 mol % MnS in the stage IIB. The FeS and MnS contents are in general correlated, and shows a characteristic zonal arrangement of electrum. It reveals rhat FeS contents of sphalerite which precipitated before electrum, gradually decreases in a grain during its deposition ranging from about 3.3 to 2 mol %. It may be considered from the above data that an increase of $fs_2$ caused by the oxidation of ore forming fluid is more important that the decrease of temperature.
A Septic embolism is a type of embolism infected with bacteria containing pus. These may become dangerous if dislodged from their original location. Embolisms of this type in the azygos vein are potentially fatal. The diagnosis of septic azygos vein embolism is difficult, so rapid diagnosis and treatment is important to avoid complications. Generally, treatment is enough for appropriate antibiotic therapy without anticoagulant therapy. We report a case of staphylococcal septic embolism in the azygos vein, which was discovered in a 51-year-old man exhibiting chest pain, dyspnea and fever. The patient was treated with antibiotic therapy alone without the use of anticoagulants.
Minor elements such as Ag, As, Au, Bi, Cd, Cu, Co, Ni, Pb, Rb, Sb, Sr and Te were analyzed by atomic absorption spectrophotometry and induced coupled plasma spectrophotometry in order to investigate pathfinders for gold in quartz porphyry, granite porphyry and vein materials in Jeonjuil gold - silver mine, and in altered biotite granites and vein materials in Dukum gold - silver mine. In Dukum gold - silver mine, it is observed that Au contents have positive relation with As, Co, and Rb contents, but negative relation with Bi contents in altered biotite granites. Au contents have positive relation with Ag, As, Co and Te contents in vein materials. In Jeonjuil gold - silver mine, it is observed that Cd, Rb, Sr and Te are enriched near ore vein in quartz porphyry and granite porphyry. Au contents have positive relation with As, Cd, Cu, $Fe_2O_3$ and $K_2O$ in vein materials.
This represents a case report of the retained polyethylene catheter fragment in superior vena cava. A 39 year old male was admitted to this Korea University Hospital a short time after compression wound on abdomen with heavy cement material in emergency room, a polyethylene catheter was introduced into the right subclavian vein through a needle. But when the polyethylene catheter was attempted to withdraw the catheter was severed by the beveled tip of the needle. Later that day, chest X-ray disclosed the presence of the fragment extending from right subclavian vein to the superior vena cava. {Fig. 1 and Fig. 2]. Local exploration by way of an infraclavicular incision was unsuccessful in locating the catheter fragment. Another attempt was then made remove the catheter by means a biotome, which is originally a device for the biopsy of the myocardium, introduced through the right great saphenous vein. This procedure, though well tolerated by the patient, was in vain. After 11 days later, during that time he was taken a laparotomy with drain, another operation for removal of retained catheter fragment was performed through median sternotomy. After exposure of the right subclavian vein, innominate vein, and superior vena cava, an incision 1 cm in |length was made directly over the palpated catheter. The catheter immediately was picked upward and removed. The length of the catheter was approximately 8 cm. [Fig 3 ] There was no evidence of thromboembolism from the catheter or other complications. The patient made an uneventful recovery, and was discharged asymptomatic on the 9th postoperative day.
Background: The relationship between the degree of expression of matrix metalloproteinases or tissue inhibitor of metalloproteinases and venous reflux remains to be investigated. Materials and Methods: Primary varicose vein tissues were obtained from 23 patients, 18 females and 5 males, aged from 19 to 73. Cephalic or basilic veins were obtained for the control group from 10 patients who underwent vascular access for maintenance hemodialysis. Two operative techniques (high ligation with stripping or endovenous laser coagulation) were used. The expression of matrix metalloproteinase-2 and 13 and tissue inhibitor of metalloproteinase-4 in the varicose vein group and control group was assessed semi-quantitatively by immunohistochemical slides stained with primary antibodies. Results: Twenty (87%) of the varicose vein group patients had greater or lesser saphenous vein diseases with reflux. The focal weak (+) stain for matrix metalloproteinases-2, and 13, and tissue inhibitor of matrix metalloproteinase-4 was dominant in the varicose vein group; the focal or diffuse strong stain (++ or +++) was prevalent in the control group. The differences were statistically significant (p<0.01). The degree of reflux and the duration of symptoms were not significantly related to the expression of MMP-13 (p=0.317 and p=0.654, respectively). Conclusion: Further study should be performed to investigate the relationship between the clinical characteristics related to venous hypertension or reflux and expression of MMPs and TIMP in varicose veins.
Knowledge of variations of veins of head and neck in relation to internal jugular, external jugular, anterior jugular and facial veins is important to surgeons doing head and neck surgery as well as to radiologists doing catheterization and to clinicians in general. In the current case, congenital atresia of the left internal jugular vein is reported. During neck dissection, superficial veins of neck and left external jugular vein was engorged and the left internal jugular vein was look like a fibrotic band. The left common carotid artery and vagus nerve was normal.
