호로파와 야관문 복합추출물의 남성갱년기 개선에 관한 연구 (Effect of a Mixed Extract of Fenugreek Seeds and Lespedeza cuneata on Testosterone Deficiency Syndrome)

  • 본 연구에서는 야관문, 호로파 복합추출인 YHM이 남성갱년기증상의 완화에 영향을 미치는 지를 알아보기 위해 흰쥐에게 YHM을 4주간 투여한 후, testosterone, SHBG, PSA, 체지방, 혈중지방, 근육량, 운동수행능력, 정자수, 간독성 등에 미치는 영향을 조사하였다. YHM 투여 후 남성갱년기의 지표로 사용되는 남성호르몬인 testosterone이 유의적으로 증가하였고, testosterone과 결합하여 기능을 저하시키는 SHBG의 농도는 감소하였다. 또한 남성호르몬 보충요법에서 나타날 수 있는 전립선 관련 부작용을 알아보기 위해 PSA를 측정하였으나, 대조군과 별다른 차이를 나타내지는 않았다. 지방조직과 근육량을 측정한 결과 지방 조직은 줄이고 근육량은 늘리는 것으로 나타나 남성갱년기에서 흔히 있는 근육량 감소 및 체지방 증가를 완화시킬 수 있을 것으로 생각된다. 아울러 혈중지질농도에 있어서도 YHM 투여군이 대조군에 비해 총 콜레스테롤과 중성지방은 감소한 반면 고밀도 콜레스테롤은 증가하였음을 관찰하였다. 강제수영을 통한 운동수행능력 측정에서도 YHM 투여군이 대조군에 비하여 수영시간이 길었고 이는 YHM의 투여가 운동수행능력 향상에 도움을 준 것으로 볼 수 있다. 총 정자수와 활동성 정자수를 측정한 결과에서도 YHM 투여군이 유의적으로 증가한 것으로 나타났고 이는 정자형성과 활동성에 영향을 주는 testosterone 농도의 증가에 기인한 것으로 생각된다. 마지막으로 YHM의 투여가 간독성을 가지는지를 알아보기 위해 AST와 ALT를 측정한 결과 대조군과 별다른 차이를 나타내지 않음으로써 간독성을 가지지는 않는 것으로 판단하였다. 또한, 실험결과 전반적으로 저농도 투여군인 YHM-40에서 고농도 투여군인 YHM-80에서 보다 긍정적인 효과를 나타냈다. 이는 YHM의 효과가 농도 의존적이지 않다는 것을 의미하며, 최적의 생체 이용률을 나타내는 농도가 YHM 40mg/kg인 것으로 판단된다. 결론적으로 YHM의 투여는 남성갱년기에 나타나는 여러 가지 지표들을 개선시키는 것으로 나타났고, 이를 활용하여 남성갱년기 개선에 도움을 줄 수 있는 건강기능성식품 소재로 개발할 수 있을 것으로 기대된다.

필라테스 브릿지 동작 시 소도구와 발의 불안정성 차이에 따른 신체 근육의 근활성도 비교 분석 (Comparative Analysis of the Body Muscle Activity According to the Prop and Different Foot Stability during Pilates Bridge Motion)

  • 본 연구의 목적은 필라테스 브릿지 동작 시 소도구의 적용과 발의 불안정성 차이에 따른 신체근육의 근전도를 비교·분석하는데 있었다. 본 연구의 대상자는 20대 남성 18명을 대상으로 실시하였고(연령, 22.3±2.1세; 신장, 173.89±4.51cm; 체중, 72.61±4.13kg; 신체질량지수, 24.03±1.31kg/m2), 필라테스 브릿지는 소도구 적용(도구 없음, 링, 짐볼)과 발의 불안정성(기본 바닥, 폼롤러, 보수볼) 차이에 따른 9가지 동작을 수행하였으며, 표면전극 부착 부위는 신체 우측의 상복직근, 하복직근, 외복사근, 장내전근, 대퇴직근, 외측광근, 전경골근, 및 대퇴이두근으로 설정하였다. 본 연구의 결과는 다음과 같다. 필라테스 브릿지 동작 시 짐볼의 적용이 신체 근육의 근활성도에 효과적인 소도구로 검증되었고, 보수볼의 적용이 신체근육의 근활성도에 가장 높은 발의 불안정성으로 검증되었다. 이상의 결과를 종합해보면, 필라테스 브릿지 동작 시 짐볼과 보수볼의 적용이 신체 근육의 근활성도에 지대한 영향력을 미친 것으로 나타났다. 따라서 본 연구의 결과는 신체 근력 강화를 위한 효율적인 필라테스 브릿지 운동을 제시할 수 있을 것으로 기대된다.

신경근전기자극치료가 암 환자의 근육 손실에 미치는 영향: 체계적 문헌고찰 (Effects of Neuromuscular Electrical Stimulation on Muscle Wasting with Cancer Patient: A Systematic Review)

  • Objectives The purpose of this study is to systematically review the effects of Neuromuscular Electrical Stimulation treatment on muscle wasting in cancer patients. Methods Randomized controlled trials (RCTs) were searched from seven online databases (PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure, Oriental Medicine Advanced Searching Integrated System, Korean studies Information Service System, Research Information Sharing Service. The selected RCTs were evaluated for methodological quality through the Cochrane RoB. Results A total of 126 articles were identified, and 4 randomized controlled trials were selected for systematic review. In one study, it was found that there were statistically significant improvements in the Health-Related Quality of Life (FACTH&N total score, p<0.001). Additionally, significant effects were observed in measurements that represented the size of the quadriceps muscle (cross-sectional area of Vastus lateralis and Rectus femoris, p=0.004), maximum muscle strength, the twitch response of resistance muscles, and voluntary activation (p<0.001). However, no significant differences were observed between the intervention and control groups in terms of quadriceps muscle strength in two other studies (p>0.05). Lastly, while one study showed no significant differences in muscle fiber characteristics between the two groups, it did report significant improvements in measurements related to mitochondria within muscle tissue and muscle strength in the intervention group (p<0.05). Conclusions Neuromuscular electrical stimulation can be a method used to improve muscle strength in muscle wasting of cancer patients, but it is difficult to see its effects as significant compared to other treatments.

무릎손상 엘리트 배구선수에 관절가동운동이 무릎통증, 등속성 근력, 근긴장도, 근경직 개선에 미치는 효과 (Effect of joint mobilization on improvement of knee pain, isokinetic strength, muscle tone, muscle stiffness in an elite volleyball player with knee injury)

  • 본 연구는 무릎손상 엘리트 배구선수에 관절가동운동이 무릎통증, 등속성근력, 근긴장도, 근경직에 미치는 효과를 확인한 사례연구이다. 사례자는 왼쪽 무릎관절에 연골결손과 수술경험이 있고 평소 지속적인 통증과 운동 시 통증을 호소하였다. 연구기간은 2015년 8월 5일부터 12월 24일이며, 왼쪽 무릎관절에 관절가동술을 1일 1회 15분씩 총 8회 적용하였다. 연구의 측정은 시각적사상척도를 이용하여 무릎통증, 등속성 근력측정장비를 사용하여 넙다리네갈래근과 넙다리뒤근육 동심성 최대회전력, 근긴장도측정장비를 사용하여 손상쪽에 넙다리곧은근, 안쪽넓은근, 가쪽넓은근의 근긴장도와 근경직을 중재 전과 중재 후 실시하였다. 연구 결과, 단기간 관절가동운동이 손상 쪽 무릎통증 감소, 양쪽 넙다리네갈래근과 넙다리뒤근육 동심성 최대회전력의 증가, 손상 쪽 넙다리네갈래근의 근경직 증가에 긍정적인 효과가 있었다. 특히 손상 쪽 넙다리네갈래근의 경우 관절가동운동 횟수가 증가할수록 동심성 최대회전력이 크게 증가되어 손상 쪽과 정상 쪽 다리의 동심성 최대회전력의 차이가 크게 감소되었다. 하지만 손상 쪽 넙다리뒤근육/넙다리네갈래근의 근력 비율의 개선에는 효과가 없었다. 본 연구를 통해 엘리트 배구선수에 무릎통증, 등속성 근력, 근경직 개선을 위해 단기간에 관절가동운동이 효과적인 중재방법이 될 수 있지만 적절한 근력비율을 위해 트레이닝을 함께 실시할 필요가 있겠다.

저강도 저항운동의 속도 차이가 비만중년여성의 신체조성, 근활성도 및 근력에 미치는 영향 (Effects of Low Intensity Resistance Training Speed on Body Composition, Muscle Activity and Muscle Strength in Obese Middle-aged Women)

  • 이 연구에서는 비만중년여성을 대상으로 저강도 저항운동의 속도 차이가 신체조성, 근활성도 및 근력에 미치는 영향을 검토하는데 그 목적이 있다. 연구대상은 비만중년여성 24명을 일반속도의 저항운동집단(normal resistance training speed group; NSG, n=12) 및 느린속도의 저항운동집단(slow speed resistance training group; SSG, n=12)으로 무작위 분류하였다. NSG는 신장성 수축 1초, 등척성 수축 1초 및 단축성 수축 1초의 총 3초간으로 구성하여 35회 3세트를 실시하였으며, SSG는 신장성 수축 3초, 등척성 수축 1초 및 단축성 수축 3초의 총 7초간으로 구성하여 15회 3세트를 실시하였다. 1RM 30%의 저강도 저항운동은 두 집단 모두 동일하게 세트 당 105초, 세트 간 1분 및 운동 간 3분의 휴식시간, 그리고 주 2~3회의 4주간에 걸쳐서 실시하였다. 신체조성, 근활성도 및 근력은 운동전과 운동 4주후에 각각 동일한 방법으로 측정하였다. 신체조성에서 NSG는 WHR에서, SSG는 체지방률에서 운동 전과 비교하여 운동 후에 각각 유의하게 감소하였다(p<.05). 근활성도에서 NSG 및 SSG는 위팔두갈래근, 안쪽넓은근 및 가쪽넓은근에서 운동 전과 비교하여 운동 후에 각각 유의하게 감소하였다(p<.01, p<.05). 근력에서 NSG 및 SSG는 biceps curl 및 leg extension에서 운동 전과 비교하여 운동 후에 각각 유의하게 증가하였다(p<.01). 하지만 신체조성, 근활성도 및 근력은 집단 간의 비교에서 통계학적으로 유의한 차이가 나타나지 않았다. 따라서 이 연구에서 신체조성, 근활성도 및 근력은 저항운동의 속도차이(반복횟수)보다는 저항운동의 수행시간(근수축 발생시간)과 밀접한 관련성이 있을 가능성이 시사되었다.

흰민들레 발효추출물의 남성 갱년기 개선에 대한 효과 (Effects of Fermented Dandelion (Taraxacum coreanum) Extract on Male Climacteric Syndrome)

  • 본 연구에서는 민들레 추출물(DE) 및 민들레 발효추출물(FDE)이 남성갱년기 증상의 개선에 영향을 미치는 지를 알아보기 위해 22주령된 고령 흰쥐에 추출물을 4주간 투여한 후, testosterone, SHBG, PSA, 체지방, 혈중지방, 근육량, 운동수행능력, 정자 수 및 정자 활동성 등에 미치는 영향을 조사하였고 세포 실험을 통해 그 기작을 알아보았다. DE 및 FDE 투여 후 남성갱년기의 지표로 사용되는 남성호르몬인 testosterone의 혈중 농도가 증가하였고, testosterone 과 결합하여 생체 내 활성을 저하시키는 SHBG의 농도는 감소하였다. 또한 남성호르몬 보충요법에서 문제가 될 수 있는 전립선에 대한 부작용을 알아보기 위해 전립선특이항원(PSA)을 측정하였으나, 대조군과 별다른 차이를 나타내지는 않았다. 지방조직과 근육에 대한 영향을 측정한 결과, 지방조직량은 줄이고 근육량은 늘리는 것으로 나타나 남성갱년기에서 흔히 나타나는 근육량 감소 및 체지방 증가에 긍정적인 효과를 갖는 것으로 생각된다. 또한 testosterone의 감소에 흔히 동반되는 혈중지질대사 이상에 있어서도 민들레 추출물 및 민들레 발효추출물 투여군이 총 콜레스테롤과 중성지방은 감소시킨 것에 반해 고밀도 콜레스테롤은 증가시켰음을 관찰하였다. 강제수영을 통한 운동수행능력 측정 결과에서는 민들레 추출물 및 민들레 발효추출물 투여군이 대조군에 비하여 수영시간이 증가하였고, 이는 민들레 추출물 및 민들레 발효추출물의 투여가 운동수행능력 향상에 기여한 것으로 사료된다. 총 정자 수와 활동성 정자 수를 측정한 결과에서도 민들레 추출물 및 민들레 발효추출물 투여군에서 증가한 것으로 나타났고, 이는 정자 형성과 활동성에 영향을 주는 testosterone 농도의 증가와 관련이 있는 것으로 여겨진다. 마지막으로 이러한 동물실험 결과의 기작을 알아보기 위해 세포실험을 통해 testosterone 생합성 과정에 관여하는 유전자의 변화를 측정한 결과 투여군에서 모두 증가함을 나타냄으로써, 추출물이 testosterone 합성을 직접적으로 촉진 시킬 수 있음을 알 수 있었다. 전반적인 결과로 민들레 추출물 보다는 발효추출물이 더 좋은 효과를 나타내었고, 이는 발효 과정을 통해 민들레 유효 성분의 생체 이용률이 증가한 것으로 사료된다. 따라서, 추후 연구에서 발효에 의해서 증가하는 활성 성분에 대한 분석이 필요할 것으로 생각된다. 결론적으로 본 연구에서는 민들레 추출물 및 발효추출물이 남성갱년기에 긍정적인 개선 효과를 나타냄을 보였고, 이를 활용하여 남성갱년기 개선에 도움 줄 수 있는 소재로 개발 할 수 있는 가능성을 제시하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.
