본 논문에서는 재생 커널 기법을 사용하여 혼합모드 균열진전 문제에 대한 연속체 기반의 형상 설계민감도 해석을 수행하였다. 재생 커널 기법은 기존의 유한요소법과 달리 요소망을 재구성할 필요가 없어, 커널 함수의 연속성을 증가시켰을 때 높은 정밀도의 형상함수를 얻을 수 있다는 장점을 가지고 있다. 균열선단 주변에서 J-적분을 수행하기 위해 선형탄성 조건이 고려되었다. 변위장과 응력 확대 계수의 설계변수에 대한 감도해석을 위하여 물질도함수를 도입하였으며 직접 미분법보다 효율적인 애조인 방법을 사용하여 설계민감도를 유도하였다. 수치 예제들을 통해서 재생 커널 기법을 이용한 균열진전 해석결과의 타당성을 확인하였으며 애조인 방법을 이용한 형상 설계민감도 해석 결과를 유한차분법과 비교하여 매우 정확하고 효율적인 결과를 얻을 수 있음을 알 수 있었다. 이를 바탕으로 간단한 모델에 대하여 형상 최적설계를 수행하여 균열이 발생될 수 있는 구조물에 대해서 균열에 의한 피해를 최소화할 수 있도록 균열을 제어할 수 있는 최적의 형상을 도출하였다.
Ebstein 기형은 복잡 심기형중 하나로 치료 또한 비정상적인 삼첨판막의 교정 또는 판막치환 등으로 방법이 다양하다. 이질환의 병리소견은 삼첨판의 중격엽과 후엽이 하방전위되어서 심방화된 심실을 형성하는 것으로 수술적 목표는 삼첨판의 기능이상을 교정하고 심방화된 우심실을 주름성형하는 것이다. 본원에서는 매우 심한 삼첨판막 폐쇄 부전증이 동반된 Carpentier type A의 Ebstein기형으로 진단된 12개월된 여아에 대해 심방화된 우심실을 수직방향으로 주름성형하고 판막을 새로이 형성된 판륜에 재부착시키는 이른바 변형적 Carpentier방법으로 성공적으로 수술하였다. 수술후 환아는 심장의 크기가 매우 줄어들었고 심초음파상 미미한 정도의 삼첨판 폐쇄부전증만 남아 술후 14일째 퇴원하였다.

Background: Orbital fractures are the most common pediatric facial fractures. Treatment is conservative due to the anatomical differences that make children more resilient to severe displacement or orbital volume change than adults. Although rarely, extensive fractures may result in enophthalmos, causing cosmetic problems. We aimed to establish criteria for extensive fractures that may result in enophthalmos. Methods: We retrospectively reviewed the charts of patients aged 0-15 years diagnosed with orbital fractures in our hospital from January 2010 to February 2019. Computed tomography images were used to classify the fractures into linear, trapdoor, and open-door types, and to estimate the defect size. Data on enophthalmos severity (Hertel exophthalmometry results) and fracture pattern and size at the time of injury were obtained from patients who did not undergo surgery during the follow-up and were used to identify the surgical indications for pediatric orbital fractures. Results: A total of 305 pediatric patients with pure orbital fractures were included-257 males (84.3%), 48 females (15.7%); mean age, 12.01±2.99 years. The defect size (p=0.002) and fracture type (p=0.017) were identified as the variables affecting the enophthalmometric difference between the eyes of non-operated patients. In the linear regression analysis, the variable affecting the fracture size was open-door type fracture (p<0.001). Pearson's correlation analysis demonstrated a positive correlation between the enophthalmometric difference and the bony defect size (p=0.003). Using receiver operating characteristic curve analysis, a cutoff value of 1.81 ㎠ was obtained (sensitivity, 0.543; specificity, 0.724; p=0.002). Conclusion: The incidence of enophthalmos in pediatric pure orbital fractures was found to increase with fracture size, with an even higher incidence when open-door type fracture was a cofactor. In clinical settings, pediatric orbital fractures larger than 1.81 ㎠ may be considered as extensive fractures that can result in enophthalmos and consequent cosmetic problems.
Base isolation, one of the popular seismic protection approaches proven to be effective in practical applications, has been widely applied worldwide during the past few decades. As the techniques mature, it has been recognised that, the biggest issue faced in base isolation technique is the challenge of great base displacement demand, which leads to the potential of overturning of the structure, instability and permanent damage of the isolators. Meanwhile, drain, ventilation and regular maintenance at the base isolation level are quite difficult and rather time- and fund- consuming, especially in the highly populated areas. To address these challenges, a number of efforts have been dedicated to propose new isolation systems, including segmental building, additional storey isolation (ASI) and mid-storey isolation system, etc. However, such techniques have their own flaws, among which whipping effect is the most obvious one. Moreover, due to their inherent passive nature, all these techniques, including traditional base isolation system, show incapability to cope with the unpredictable and diverse nature of earthquakes. The solution for the aforementioned challenge is to develop an innovative vibration isolation system to realise variable structural stiffness to maximise the adaptability and controllability of the system. Recently, advances on the development of an adaptive magneto-rheological elastomer (MRE) vibration isolator has enlightened the development of adaptive base isolation systems due to its ability to alter stiffness by changing applied electrical current. In this study, an innovative semi-active storey isolation system inserting such novel MRE isolators between each floor is proposed. The stiffness of each level in the proposed isolation system can thus be changed according to characteristics of the MRE isolators. Non-dominated sorting genetic algorithm type II (NSGA-II) with dynamic crowding distance (DCD) is utilised for the optimisation of the parameters at isolation level in the system. Extensive comparative simulation studies have been conducted using 5-storey benchmark model to evaluate the performance of the proposed isolation system under different earthquake excitations. Simulation results compare the seismic responses of bare building, building with passive controlled MRE base isolation system, building with passive-controlled MRE storey isolation system and building with optimised storey isolation system.
Globally, the burden of breast cancer (BC) continues to increase. BC related lymphedema (BCRL) is currently non curable and as a life time risk it affects at least 25% of BC patients. Knowing more about BCRL and appropriate control of its modifiable risk factors can improve quality of life (QOL) of the affected patients. In this case control study to detect factors, 400 women with BCRL (as the case group) and 283 patients with BC without lymphedema (as the control group) that were referred to Shiraz University of Medical Sciences affiliated BC clinic center were assessed. The data were analyzed in SPSS. The mean age of the case group was $52.3{\pm}11.0years$ and of the control group was $50.1{\pm}10.9years$. In patients with BCRL, 203(50.7%) had left (Lt) side BC and in non- lymphedema group 151 (53.3%) had Lt side BC. Out of all BCRL patients, 204 (51%) had lymphedema in all parts of their affected upper extremities, 100 (25%) had swelling in the arm and forearm and 23 (5.7%) had edema in both the upper extremity and trunk. Edema, heaviness, concern about changing body image, pain and paresthesia were the most common signs/symptoms among patients with BCRL. In BCRL patients, the difference of circumference between the affected upper limb and non-affected limb was $4.4{\pm}2.5cm$ and the difference in volume displacement was $528.7{\pm}374.4milliliters$. Multiple variable analysis showed that moderate to severe activity (OR; odds ratio =14, 95% CI :2.6-73.3), invasiveness of BC (OR =13.7, 95% CI :7.3-25.6), modified radical mastectomy (OR=4.3, 95% CI :2.3-7.9), BMI =>25 (OR=4.2, 95% CI :2-8.7), radiotherapy (OR=3.9, 95% CI :1.8-8.2), past history of limb damage (OR=1.7, 95% CI :0.9-3.1) and the number of excised lymph nodes (OR=1.06, 95% CI :1.02-1.09) were the significant predictors of lymphedema in women with BC. Modifiable risk factors of BCRL such as non-guided moderate to severe physical activity, high BMI and trauma to the limb should be controlled as early as possible in BC patients to prevent development of BCRL and improve QOL of these patients.
본 연구에서는 발포 알루미늄으로 구성된 접착 구조물에서의 접합면에 대한 파괴 특성을 조사하기 위하여 각도를 변수로 정하고 TDCB 시험편을 설계하였다. 이 시험편들은 길이는 200 mm이고 시험편에 대한 접착면의 경사 각도는 $6^{\circ}$, $8^{\circ}$, $10^{\circ}$와 $12^{\circ}$인 네 가지로 모델링을 하였다. 이 시험편들의 실험 및 해석을 분석한 결과, 경사면 각도가 $6^{\circ}$, $8^{\circ}$, $10^{\circ}$와 $12^{\circ}$인 경우에 시험편들의 최대 하중은 각각 약 120 N, 137 N, 154 N과 171 N으로 발생하였다. 해석의 결과 값이 실험치에 가까워져 많은 차이를 보이지 않는 것을 확인할 수 있다. 따라서 이와 같은 연구 방법을 응용하여 실험 대신 시뮬레이션을 통하여 접착제로 접착된 알루미늄 폼으로 된 재료의 전단 거동에 관한 물성치를 효율적으로 파악할 수 있다고 사료된다.
본 최근 건축물 외벽공사에 커튼월 공법의 사용이 증가하고 있다. 외벽 커튼월 공법은 공기단축 및 경제적 효용이 크다는 장점으로 인해 건물의 외벽으로도 널리 시공되고 있다. 그러나, 커튼월 공법의 프레임과 유리를 연결해 주는 구조용 실란트에 대한 내후성 및 구조물의 거동에 대한 열화를 대체한 시험은 KS F 4910 규격에는 부재한 실정으로 안정적인 커튼월 공법 도입과 품질향상을 위해서는 필요한 요소이다. 본 연구에서는 외기환경하에서 거동에 대한 탄성복원력시험을 실시하였다. 구조용 실란트를 대상으로 열에 대한 실링재의 열화를 평가하였다. 국내에서는 구조용 실란트의 가변형 변위거동에 대한 연구는 부족한 실정으로 본 연구에서는 외기 환경에 노출된 열화조건을 대체하여 실험실 조건에서 재현한 결과를 확인하고, 설계에 반영하고자 한다. 본 연구 결과에 따르면, 기존의 구조용 실링재는 KS F 4910의 품질기준은 만족하지만, 본 연구에서 실시한 조건에서는 시험체의 파단과 표면의 균열 등이 발생하는 것을 확인하였다. 특히, 내후성 시험에서는 시험체 모두 불용의 상태를 확인하여 향후, 커튼월 공법에 사용되는 구조용 실링재의 장기적인 내구성능 평가가 필요한 것으로 판단된다. 따라서, 기존의 구조용 실링재가 현장에 적용되기 위해서는 별도의 내구성능 평가를 실시가 요구된다.
본 연구에서는 CFRP 표면부착 공법의 대안으로 최근에 관심을 끌고 있는 NSM(Near Surface Mounted)기법으로 전단 보강된 RC 부재의 전단강도를 평가하기 위한 실험과 해석을 수행하였다. 전단철근이 없는 7개의 실험체에 대해 4점 휨실험을 실시하였다. 실험변수로는 CFRP 스트립의 경사($45^{\circ}$, $90^{\circ}$)와 스트립의 간격(250mm, 200mm, 150mm, 100mm)이 고려되었다. 실험적 연구를 통해 NSM공법으로 전단 보강된 RC 부재의 전단강도와 파괴모드에 대한 각 실험변수의 영향을 평가하였다. 실험결과는 $45^{\circ}$ 경사로 스트립을 보강한 실험체들은 스트립의 파단으로 파괴된 반면, 수직으로 스트립을 보강한 실험체들은 스트립의 슬립으로 파괴됨을 보였다. 또한, $45^{\circ}$ 경사 스트립이 수직 스트립보다 전단저항력 증가시킬뿐만 아니라 파괴시의 처짐을 크게 증가시키는 것으로 나타났다. 추가적으로 RBSN 해석은 NSM기법으로 전단 보강된 RC 부재의 균열형상 및 하중-처짐관계를 적절하게 예측하였다.
교량 바닥판 설계에 대한 현 시방규정은 바닥판 슬래브가 처짐이 구속된 거더에 연속 지지되어 있다고 가정하므로, 바닥판 지간 중앙의 정모멘트와 거더 상단에 발생하는 부모멘트의 크기가 같은 것으로 간주하고 있다. 그러나 바닥판에 발생하는 휨모멘트는 거더의 처짐에 의해서 많은 영향을 받고 있으며, 거더의 처짐을 고려치 않는 현 시방규정에 의해 설계된 바닥판은 상부철근의 부식으로 인한 내구성 저하와 유지보수 비용 증가 등의 문제점을 안고 있다. 따라서 본 연구에서는 매크로 요소를 이용해 거더의 처짐 효과를 고려할 수 있는 해석법을 개발하였고, 이를 유한요소법을 통해 검증하였다. 또한, 이 해석법을 바탕으로 바닥판의 횡방향 휨모멘트에 영향을 미치는 여러 변수에 대한 분석을 수행하였다. 해석 결과, 바닥판의 지점부 모멘트는 거더의 간격뿐만 아니라 거더와 바닥판의 휨강성비 교량의 길이, 하중의 재하위치, 거더의 비틀림 강성, 가로보의 휨강성과 배치 간격 등에 많은 영향을 받고 있는 것을 알 수 있으며, 영향선을 이용해 최대하중 위치를 결정하여 몇 개의 예제교량을 대상으로 지점부의 설계모멘트를 계산해 본 결과, 현 시방규정이 다소 보수적인 값을 나타내고 있다.
The purpose of this study was to compare GRF characteristics during walking wearing jogging and roller shoes. Twelve male middle school students (age: $15.0{\pm}0.0\;yrs$, height: $173.6{\pm}5.0\;cm$, weight: $587.6{\pm}89.3\;N$) who have no known musculoskeletal disorders were recruited as the subjects. Kinematic data from six S-VHS camcorders(Panasonic AG456, 60 fields/s) and GRF data from two force platform; (AMII OR6-5) were collected while subjects walked wearing roller and jogging shoes in random order at a speed of 1.1 m/s. An event sync unit with a bright LED light was used to synchronize the video and GRF recordings. GRF data were filtered using a 20 Hz low pass Butterworth. digital filter and further normalized to the subject's body weight. For each trial being analyzed, five critical instants and four phases were identified from the recording. Temporal parameters, GRFs, displacement of center of pressure (DCP), and loading and decay rates were determined for each trial. For each dependent variable, paired t-test was performed to test if significant difference existed between shoe conditions (p <.05). Vertical GRFs at heel contact increased and braking forces at the end of initial double limb stance reduced significantly when going from jogging shoe to roller shoe condition. Robbins and Waked (1997) reported that balance and vertical GRF are closely related It seems that the ankle and knee joints are locked in an awkward fashion at the heel contact to compensate for the imbalance. The DCP in the antero-posterior direction for the roller shoe condition was significantly less than the corresponding value for the jogging shoe condition. Because the subjects tried to keep their upper body weight in front of the hip to prevent falling backward, the DCP for the roller shoe condition was restricted The results indicate that walking with roller shoes had little effect on temporal parameters, and loading and decay rates. It seems that there are differences in GRF characteristics between roller shoe and jogging shoe conditions. The differences in GRF pattern may be caused primarily by the altered position of ankle, knee, and center of mass throughout the walking cycle. Future studies should examine muscle activation patterns and joint kinematics during walking with roller shoes.
