We retrospectively studied 34 patients who underwent operations of ascending aortic aneurysm and aortic valve replacement from August 1979 to July 1992 at the Yonsei Cardiovascular Center. Eight patients underwent supracoroanry non-composite graft replacement and separate aortic valve replacement[group I]. Twenty six patients underwent valved composite graft replacement with reimplantation of coronary arteries[group II]. Two cases in group II died within 1 month after the operation. Among the 32 survivors 28 patients have been followed up for an average of 59 months ranging from 1 months to 159 months. During the follow up periods, a pseudoaneurysm around the ascending aorta and a newly developed dissecting aneurysm in remaining aorta were noted in group II. There were 6 late deaths: 2 cases in group I and 4 cases in group II. Three cases among the 6 late deaths have stigmata of Marfan`s syndrome[1 cases in group I and 2 case in group II]. There was no statistically significant difference in actuarial survival rates between group I and group II[p > 0.05]. This study suggests that non-composite supracoronary graft interposition with separate aortic valve replacement is a safe surgical technique in patients who have normal aortic annulus and normal position of coronary ostia. However in degenerative disease such as cystic medial necrosis, composite graft replacement is recommended because this procedure eliminates entire abnormal tissue.But it seems to be important that the suture technique and strict follow-up in patients with diseased aortic wall.
From 1958 to October 1977, 294 cases of acquired heart disease were operated. There were 68 cases of pericardium, 3 trauma, 2 foreign body, one cardiac thrombus, 3 atrial myxoma, 2 left atrial and 1 right atrial, 2 Budd-Chiari syndrome, and 214 valvular heart disease. Out of 214 cases of valve operation 73 valves were replaced in 64 patients. Male to female ratio was 1.46: 1. The youngest age was 14 years in male and 18 in female. The oldest was 54 years in male and 52 in female. Fifty-five cases of single valve were replaced, consisting of 47 mitral and 8 aortic valves. There were 9 double valve replacement cases which consist of 7 mitral and aortic and 2 mitral .and tricuspid valves. Six varieties of prosthetic valves, 3 ball and 3 disc types and 3 kinds of xenograft tissue valves were utilized. Beall, BjSrk-Shiley and Starr-Edwards prosthetic valves and Hancock valves were used mainly. For single valve 34. 5% and for double valve replacement 44% mortality were noted. There were 23 operative deaths out of 64 patients, over all mortality rate of 36.9%. Mortality for mitral valve replacement was 29.5%. But in recent 12 consecutive cases one death occurred, showing 8.3% mortality. In earlier days thrombocyte anti-adhesive drug dipyramidole-persantin-aspirin and/or SP 54 were adminstered. But in recent cases after heparinization, coumadin and Persantin were prescribed routinely.
One-hundred-and-seven patients were the consecutive cases of double replacement of the mitral and the aortic valves at the same time using the lonescu-Shiley bovine pericardial xenograft valve during the period between May, 1979 and June, 1984. They were 64 males and 43 females, and their ages ranged from 13 to 62 years [mean age, 34.011.9 years]. Eight patients died within 30 days after surgery [operative mortality rate, 7.5%], and 7 others thereafter [late mortality rate, 6.5%; or 4.21%/patient-year]. Ninety-nine early survivors were followed up for a total duration of 166.1 patient-years [mean duration, 20.116.1 months]. Two patients experienced thromboembolic complication with no death [1.20%/patient-year]; five developed prosthetic valve endocarditis [3.01%/patient-year] with one death; and three had a new development of aortic regurgitant murmur and they were, along with a mortality from endocarditis, classified into the cases of tissue valve failure [2.41%/patient-year]. The actuarial survival rate including the operative mortality was 82.24.7% at 6 years after surgery. The probabilities of freedom from thromboembolism and from valve failure were 97.61.7% and 88.67.6% at 6 years respectively. Symptomatic improvement was excellent in most of the cases at the follow-up end, showing the mean of the postoperative NYHA Classes of 1.120.33 from the preoperative one of 2.860.54. These results compares favorably with the ones reported from the major institutions. Clinical results of isolated replacement of the mitral valve and of the aortic valve were previously reported. The clinical results of a total and consecutive patients with replacement of single mitral and single aortic and double mitral and aortic valves on the mortality rate, survival rate, complication frequency, and symptomatic improvement all fully stands for the good therapeutic modalities of the valvular heart diseases with severely damaged lesions.
A total and consecutive 163 patients underwent cardiac valve replacement using the Hancock porcine xenograft cardiac valves from 1 976 to 1984. Of 198 substitute valves, 177 were the Hancock valves. One hundred twenty-nine patients[79.1%] had single valve replacement: MVR 118, AVR 8 and TVR 3; 33[20.3%] had double valve replacement: MVR+AVR 27 and MVR+TVR 6; and a single case had triple valve replacement. Other surgical procedures were added in 34 patients. The operative mortality rate within 30 days of surgery was 6.1%, and it was, however, 4.2%, with single MVR. Late mortality rate was 6.7% or 1.95%/patient-year of a linealized mortality rate. Early survivors of 153 patients were followed up for a total of 565.1 patient-years [a mean of 44.3*27.1 months]. The linealized annual complication rates were: 1.95% emboli/patient-year, 0.89% bleeding/patient-year 1.24% endocarditis/patient-year, and 4.25% overall failure/patient-year. Primary tissue failure occurred at a rate of 1.59%/patient-year. The actuarial survival rates including operative mortality were 87.0*4.1% and 77.3*6.6% at 5 and 11 years after surgery respectively. The probability of freedom from thromboembolic complication of 89.2*3.4% at 5 years after surgery lasted unchanged upto 11 years. The probability of freedom from overall valve failure was 81.3*4.5% at postoperative 5 years, and it dropped down to 26.2*19.4% at 11 years, although the latter was statistically insignificant because of a small number of patients entering into the years approaching the follow-up end. However, the probability of freedom from the primary tissue failure was 81.3*10.6% at postoperative 9 years, which coincides closely with the speculated rate of tissue degeneration of about 20% in 10 years. These clinical results confirm the low thrombogenicity of the Hancock porcine valve and the reasonable failure rate of tissue degeneration.

Background: Mitral regurgitation is one of the leading causes of cardiovascular morbidity in pediatric patients with Marfan syndrome. The purpose of this study was to contribute to determining the appropriate surgical strategy for these patients. Methods: From January 1992 to May 2013, six patients with Marfan syndrome underwent surgery for mitral regurgitation in infancy or early childhood. Results: The median age at the time of surgery was 47 months (range, 3 to 140 months) and the median follow-up period was 3.6 years (range, 1.3 to 15.5 years). Mitral valve repair was performed in two patients and four patients underwent mitral valve replacement with a mechanical prosthesis. There was one reoperation requiring valve replacement for aggravated mitral regurgitation two months after repair. The four patients who underwent mitral valve replacement did not experience any complications related to the prosthetic valve. One late death occurred due to progressive emphysema and tricuspid regurgitation. Conclusion: Although repair can be an option for some patients, it may not be durable in infantile-onset Marfan syndrome patients who require surgical management during infancy or childhood. Mitral valve replacement is a feasible treatment option for these patients.
We have experienced 18 abdominal aortic aneurysms between March,1987 and June,1994.Unruptured patients were 11 cases and ruptured were 7.The fifth[33.4% ,sixth[27.7% and seventh[27.7% decades were most common age distributions.Sex ratio was 2.6:1[m:f .The most frequent symptoms were abdominal or back pain and palpable pulsating mass.Sixteen patients underwent resection and graft replacement.One patient refused operation.Atherosclerosis was the underlying pathology in 76%.Two ruptured patients died postoperative[28.5% .The frequency of postoperative complication was higher in the ruptured group.
Anticoagulation therapy is essential after cardiac valve surgery. However, spontaneous bleeding remains a major concern during anticoagulation therapy. Spontaneous gallbladder (GB) hemorrhage (hemobilia) is a rare occurrence during standard anticoagulation therapy. This report presents a case of GB hemorrhage that occurred shortly after initiating oral anticoagulant therapy in a patient who had undergone mitral valve replacement surgery.
A case of congenital tricuspid stenosis in 24 year old male patient is presented. The lesion was successfully corrected with prosthetic valve replacement and closure of atrial and ventricular septal defects. Especially, this is the first reported case of successful large prosthetic valve replacement to a small annulus in tricuspid valve.
We experienced a case of aneurysm of the sinus of Valsalva dissecting into the ventricular dseptum. This dissection was induced by paravalvular leakage after aortic and mitral valve replacement. This 37-year-old male was admitted via emergence room due to progressive dyspnea. He had undergone aortic valve replacement(carbomed c" 23 mm) and mitral valve replacement(carbomedic 31 mm) due to aortic regurgitation and mitral regurgitation about 6 years prior to admission and followed up regularly. The diagnosis was made by transthoracic and transesophageal echocardiography and reconfirmed by root aortography. The inlet of the ventricular septal aneurysmal sac was repaired by one layer suture with 3-0 prolene of the endocardium, epicardium and homograft muscle shoulder altogether. Postoperative course was uneventful and the patient was discharged on the 11th postoperative day. day.
Bacterial endocarditis is a well-recognized complication of prosthetic valve replacement. Postoperative fungal endocarditis of aortitis has been reported with increasing frequency. Aspergillus endocarditis or aortitis following open heart surgery has been reported in several patients. The difficulty in the diagnosis of this type of infection, the problems of therapy of prosthetic valve endocarditis0 and the relative ineffectiveness of antifungal agents account for the high mortality. Recently, we have experienced a patient with aspergillus aortitis after replacement of mitral valve. The diagnosis was finally established by histotogical examination of emboli removed from the femoral artery. The patient died after second open heart surgery for replacement of ascending aorta.
