Background: ATS mechanical valve is a recently introduced pyrolytic carbon bileaflet prosthesis. This report is to evaluate the results of hemodynamic and anticoagulant therapy after ATS valve replacement. Material and Method: From May 1995 to October 1998, 53 patients received 65 ATS prosthesis; 38 Mitral(27-33 mm), 27 Aortic(19-25 mm). 2 CABGs and 5 Tricuspid annuloplasty were taken concomitantly. The follow up period was 769 patient-months(mean 16.2$\pm$10.0), varied from 1 month to 39 months with 92.5% follow up rate. All patients were evaluated with Doppler echocardiography, 7-14 days after operation. Result: NYHA functional class was improved significantly, from 2.6$\pm$0.8 preoperatively to 1.3$\pm$0.4 postoperatively. The average value of peak and mean transvalvular pressure gradients were 25.7$\pm$13.5 mmHg, 12.7$\pm$8.3 mmHg in aortic position. In the mitral position, the average values of peak and mean transvalvular pressure gradient and valve area were 5.9$\pm$2.5 mmHg, 3.1$\pm$0.8 mmHg and 2.9$\pm$0.5 $\textrm{cm}^2$, respectively. In the anticoagulant therapy, mean INR was 2.5$\pm$0.6 in mitral valve replacement and 1.9$\pm$0.5 in aortic valve replacement. There was no anticoagulant related complication. During that period, there were 3 hospital death(5.9%) and 1 late death(1.9%). Conclusion: The early clinical results of the ATS heart valve replacement is quite satisfactory, and low target INR reginmen is safe. And long term follow of hemodynamic characteristics is also necessary.

Compression waves propagating in a high-speed railway tunnel develops large pressure fluctuations on the train body or tunnel structures. The pressure fluctuations would cause an ear discomfort for the passengers and increase the aerodynamic resistance of trains. As a fundamental research to resolve the pressure wave phenomenon in the tunnel, experiments were carried out by using a shock tube with an open end. A blockage to model trains inside the tunnel was installed on the lower wall of shock tube, thus forming a sudden cross-sectional area reduction. The compression waves were obtained by the fast opening gate valve instead of a conventional diaphragm of shock tube and measured by the flush mounted pressure transducers with a high sensitivity. The experimental results were compared with the previous theoretical analyses. The results show that the ratio of the reflected to the incident compression wave at the sudden cross-sectional area reduction increases but the ratio of the passing to the incident compression wave decreases, as the incident compression wave becomes stronger. This experimental results are in good agreements with the previous theoretical ones. The maximum pressure gradient of the compression wave abruptly increases but the width of the wave front does not vary, as it passes over the sudden cross-sectional area reduction.
A 6-month-old intact male Maltese dog (2.5 kg of body weight) was presented with the primary complaint of heart murmur and exercise intolerance. Diagnostic studies revealed severe pulmonic stenosis. Since the pressure gradient in stenotic pulmonic valve area was higher than 200 mmHg (maximum flow velocity = 7.1 m/s), the dog was underwent balloon valvuloplasty. With this procedure, the peak pulmonic velocity was reduced to 3.0 m/s (PG = 36 mmHg). Periodic re-evaluations performed in monthly interval revealed no further deterioration even after 4 month of procedure. This is the first case report of pulmonic stenosis fixed by non-surgical balloon valvuloplasty in Korea.

Hydraulic power steering system has been adopted in seniority passenger and commercial vehicle system for an easy maneuverability and a smoother ride. In this study, hydraulic power steering system analysis model which includes hydraulics and mechanical sub-systems was developed using commercial software, AMESim in order to predict characteristics for various steering components. Each component which constructs system was modeled and verified by experimentally obtained characteristics curves of each components. The agreement between simulation and experimental results shows the validity of the simulation model. The parameter sensitivity analysis such as valve opening area, torsional stiffness for system design are carried out by the analysis and experimental method.

Background: Attaining an adequate effective orifice area (EOA) is definitive goal in aortic valve replacement (AVR). The simple interrupted suture (SIS) technique could be a solution to achieve this goal, but limited data are available in the literature. This study aimed to compare hemodynamic differences between the SIS and non-everting mattress suture (NMS) techniques. Methods: From our database, 215 patients who underwent AVR for severe aortic stenosis were extracted to form the overall cohort. From March 2015 to November 2016, the SIS technique was used in 79 patients, while the NMS technique was used in 136 patients. Hemodynamic outcomes were evaluated, as detected by transthoracic echocardiography and computed tomography. Results: There were no significant differences in baseline characteristics between the 2 groups. On immediate postoperative echocardiography, the SIS group showed a significantly wider EOA (1.6±0.4 vs. 1.4±0.5 ㎠, p=0.007) and a lower mean pressure gradient (PG) (13.3±5.4 vs. 17.0±6.0 mm Hg, p<0.001) than the NMS group. On follow-up echocardiography, the SIS group continued to have a wider EOA (1.6±0.4 vs. 1.4±0.3 ㎠, p<0.001) and a lower mean PG (11.0±5.1 vs. 14.1±5.5 mm Hg, p<0.001). There was no significant difference in paravalvular leakage. Conclusion: The SIS technique for AVR was associated with a wider EOA and a lower mean PG. The SIS technique could be a reasonable option for AVR.
Background: Numerous surgical devices for mitral repair have been used in the past with good results. In this study we describe a simple annuloplasty technique with using a new device ($Mitracon^{(R)}$). The aim of this study was to assess its efficacy and surgical results with using $Mitracon^{(R)}$. Material and Method: From May 2003 to October 2005, 46 patients (21 women and 25 men (mean age of $51.4{\pm}17.8$ years) with mitral regurgitation from various causes were treated with either the $Mitracon^{(R)}$ (the $Mitracon^{(R)}$ group) or the Capentier Edward rigid ring (the CE group). The median follow-up duration was 18.9 months. Result: The mean grade of mitral regurgitation before and immediately after surgery in the $Mitracon^{(R)}$ group and the CE group decreased from $3.2{\pm}0.8$ to $0.6{\pm}0.7$ and $3.4{\pm}0.7$ to $0.3{\pm}0.5$, respectively. There were no significant changes in the ejection fraction either between the two groups or before and immediately after surgery. No deaths were seen in either group. Early postoperative echocardiography of all 46 patients showed only trivial mitral regurgitation or none at all. Echocardiography at a median of 18.9 months also showed no progression in mitral regurgitation. The mean grade of mitral regurgitation in the $Mitracon^{(R)}$ group at this time point decreased from $3.2{\pm}0.8$ to $0.8{\pm}0.7$ (p<0.05). The CE group also showed a similar degree of decrease from $3.4{\pm}0.7$ to $0.3{\pm}0.6$ (p<0.05). The mitral valve area in the $Mitracon^{(R)}$ group at 1 year follow-up was $3.3{\pm}0.9cm^2$. The mitral valve area in the CE group was $2.7{\pm}0.6cm^2$. The mean mitral pressure gradient in the $Mitracon^{(R)}$ group at 1 year follow-up was $3.1{\pm}1.3$ mmHg. The mean pressure gradient in the CE group was $4.5{\pm}2.1$ mmHg, although any statistical significant difference for this between the groups was not reached. Conclusion: The present study showed the described technique to be safe and effective in the intermediate term. Because long term results are unavailable, a more extensive prospective randomized multicenter trial may be warranted to determine whether this procedure should be generally applied for repair of mitral valve disease.
From June 1995 to May 1997, we have implanted 52 Sorin Bicarbon mechanical valves in 41 patients. They were 16 men and 25 women, and their mean age was 47.4${\pm}$14.8(range; 18∼74 y.o.). 35(27 mm∼31 mm) were in mitral position, 15(19 mm∼25 mm) in aortic position, and 2(31 mm) in tricuspid position. 3 CABGs and a tumor excision were taken concomittantly. 35 patients were primary operation, and 6 were re-do operations. By intraoperative transesophageal doppler echocardiography, transvalvular peak/mean pressure gradient was 6.1${\pm}$2.7/2.4${\pm}$1.4 mmHg in mitral position and 27.6/10.7 mmHg in aortic position. The effective valve opening area in mitral position was 3.2${\pm}$0.6 cm2. Follow-up was total 508.6 patient-months, and mean follw-up was 12.7${\pm}$9.2 months. NYHA class was improved from 2.6${\pm}$0.6 to 1.2${\pm}$0.3 in average postoperatively. During that period, there was no operative death. 2 late non-valve related deaths were occurred. One was died of COPD, and the other was possible acute myocardial infarction. Among 7 postoperative complications, one valve related complication(minimal paravalvular leakage) was noticed. In conclusion, Sorin Bicarbon mechanical valve is believed one of the safe choice in clinical settings. It showed excellent hemodynamic and mechanical functions, and very low postoperative valve related complications in short term clinical experience.
A 4-month-old intact male Boxer dog(9 kg of body weight) was presented with the primary complaints of heart murmur and exercise intolerance. Diagnostic studies revealed severe subaortic stenosis. Since the pressure gradient(PG) in stenotic aortic valve area was higher than ${\sim}120mmHg$(5.4 m/s of peak velocity), the dog was underwent balloon valvuloplasty. With this procedure, the peak aortic velocity was reduced to 3.9 m/s(${\sim}60mmHg$ of PG) and the clinical condition of dog was markedly improved. Periodic re-evaluations performed at monthly interval revealed no further deterioration. This is the first case report of subaortic stenosis treated by non-surgical palliative balloon valvuloplasty in Korea.
Purpose: Although several reports have described the relationship between periodontal disease and cardiovascular disease, information about the association between periodontal disease and the progression of degenerative aortic stenosis (AS) is lacking. Therefore, we performed a retrospective, single-center, pilot study to provide insight into this potential association. Methods: Data from 45 consecutive patients (19 men; median age, 83 years) with mild or moderate degenerative aortic stenosis were analyzed for a mean observation period of 3.3±1.9 years. The total amount of Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis and titers of serum immunoglobulin G (IgG) against periodontal bacteria and high-sensitivity C-reactive protein (hs-CRP) were evaluated. Aortic valve area (AVA), maximal velocity (Vmax), mean pressure gradient (mean PG), and the Doppler velocity index (DVI) were evaluated. The change in each parameter per year ([ParameterLATEST-ParameterBASELINE]/Follow-up Years) was calculated from the retrospective follow-up echocardiographic data (baseline vs. the most recently collected data [latest]). Results: No correlation was found between the concentration of periodontopathic bacteria in the saliva and AS status/progression. The anti-P. gingivalis antibody titer in the serum showed a significant positive correlation with AVA and DVI. Additionally, there was a negative correlation between the anti-P. gingivalis IgG antibody titer and mean PG. The hs-CRP concentration showed positive correlations with Vmax and mean PG. Meanwhile, a negative correlation was observed between the anti-P. gingivalis IgG antibody titer and ΔAVA/year and Δmean PG/year. The hs-CRP concentration showed positive correlations with Vmax and mean PG, and it was significantly higher in patients with rapid aortic stenosis progression (ΔAVA/year <-0.1) than in their counterparts. Conclusions: Our results suggest that periodontopathic bacteria such as A. actinomycetemcomitans and P. gingivalis are not directly related to the status/progression of degenerative AS. However, inflammation and a lower immune response may be associated with disease progression.
