인공판막 심내막염에서 판막간 섬유체 재건을 이용한 대동맥판 및 승모판 치환술 (Aortic and Mitral Valve Replacement with Reconstruction of the Intervalvular Fibrous Skeleton in Prosthetic Valve Endocarditis)

  • 대동맥판과 승모판 및 두 판막간 섬유체(fibrous skeleton)를 침범한 복잡 심내막염은 치료가 매우 어려운 질환의 하나로 여전히 술후 사망률과 이환율이 높다. 저자들은 좌측 대동맥판륜 농양 및 섬유체를 파괴한 인공 대동맥판 및 승모판막 심내막염이 동반된 55세 여자 환자에서 섬유체의 재건과 함께 대동맥판 및 승모판 치환을 하였던 예를 보고한다. 환자는 과거에 대동맥판과 승모판을 기계판막으로 두 번 치환술을 받았었다. 심장초음파 검사에서 비관상동의 판막 주위 결손 및 섬유체를 따라 비정상적인 누출 경로가 관탈되었다. 지속적으로 항생제를 투여하여도 혈액에서 staphylococcus epidermidis가 배양되고 지속되는 패혈증 증세를 보여 응급수술을 시행하였다. 대동맥 절개를 좌심방 지붕가지 확장한 다음 두 판막 및 판막간 섬유체를 완전히 절제한 다음 발견된 대동맥판륜 농양은 절제 후 소 심낭 패취를 이용하여 폐쇄하였다. 두 개의 삼각형 모양의 소 심낭 패취를 이용하여 절제된 대동맥 판과 승모판의 각각의 판륜 및 섬유체를 재건한 다음 기계판막을 대치하였다. 술후 지속적인 항생제 투여 후 심내막염 재발 소견이 없어 72일째 퇴원하였다. 술후 8개월에 시행한 심장초음파 검사에서 판막주위 누출이나 심내막염 재발 소견이 없었고 특별한 증세없이 통원치료중이다.

하드페이싱 오버레이용접 비드형상에 미치는 GMA 용접조건의 영향 (Effects of GMA Welding Conditions on the Bead Shape of Hardfacing Overlay Welding)

  • The relationship between GMA welding conditions and the bead shape of overlay weld was studied by using ${\Phi}1.6mm$ hypo-eutectic metal-cored wire designed for hardfacing against the severe metal-to-metal wear. As the welding voltage increased, the dilution also increased but the sudden drop of dilution was observed at $30{\sim}33V$. It was considered to be due to the decrease of penetration resulting from the change of transfer mode, from short circuit to spray. It was also found that the behavior of penetration with welding current was dependant on the transfer mode. The short circuit mode exerted the penetration to decrease while the spray mode did it to increase with increase of welding current. The former was considered to be responsible for the remarkable decrease in dilution at low welding voltage region. The change of transfer mode also had an effect on the behavior of bead width with welding current but it did not on the bead spreadability defined as W/H ratio. It was considered that the optimal welding conditions for multi-pass overlay welding could be obtained from the bead spreadability suitable for bead lapping and the dilution as low as possible in the spray transfer mode.

  • 전해환원공정에서 발생하는 금속전환체와 용융염을 각각 smelting하고 고화시키는 공정을 개발하였다. 진공조건에서 다단계 가열에 의하여 마그네시아 용기에 담긴 금속전환체를 잔류 용융염과 분리하고 용융시켜 금속 잉곳을 제조하는 운전방법을 제시하는 한편, 금속전환체의 분석을 수행하였다. 전해환원 공정에서 감압이송된 용융염의 경우에는 이송과 계량에 적합하게 이중 용기와 염밸브를 사용하여 일정 형상과 크기로 고화하는 신개념을 도출하였다. 본 연구의 결과는 한국원자력연구소 Advanced Spent Fuel Conditioning Process의 hot cell 실증시스템 설계에 적용되었다.

집속이온빔을 이용한 마이크로 노즐의 제작 (Machining of The Micro Nozzle Using Focused Ion Beam)

  • Micro nozzle is employed as a dynamic passive valve in micro fluidic devices. Micro nozzle array is used in micro droplet generation in bio-medical applications and propulsion device for actuating satellite and aerospace ship in vacuum environments. Aperture angle and the channel length of the micro nozzle affect its retification efficiency, and thus it is needed to produce micro nozzle precisely. MEMS process has a limit on making a micro nozzle with high-aspect ratio. Reactive ion etching process can make high-aspect ratio structure, but it is difficult to make the complex shape. Focused ion beam deposition has advantage in machining of three-dimensional complex structures of sub-micron size. Moreover, it is possible to monitor machining process and to correct defected part at simultaneously. In this study, focused ion beam deposition was applied to micro nozzle production.

모세관 인력으로 작동되는 열공압형 마이크로 펌프의 형상이 성능에 미치는 영향 (The Influence of the Geometry on the Performance of a Thermopneumatic Micropump Operated by Capillary Attraction)

  • Recently, we developed a simple thermopneumatic micropump having neither a membrane nor a valve. This micropump discharges liquid by a thermopneumatic pressure and refill by a capillary attraction. In case of the micropump driven by the capillary attraction, the flow characteristic depends mainly on the geometry of the micropump. In this paper, we investigated the influence of the geometry of the micropump on the performance of the micropump to illustrate the properness of the micropump shape. We analyzed the micropump characteristics of six types having different geometries by FVM simulation with a commercial CFD tool. Also we fabricated the micropumps with PDMS and glass by micromachining, and tested the performances. The simulation and the test results illustrate that the discharge volume and the discharge time depend on the chamber volume. The expansion angle of the inlet channel location has influence on the refill time, while the front air channel direction has influence on the backward flow loss.

흡기다기관 시스템의 구조진동 저감에 대한 연구 (A study on reduction of structural vibration of an intake manifold system)

  • Vibration of intake menifold is important as it could worsen the noise levels radiated from surface itself and support bracket, and it eventually leads to the failures of a Throttle Position Sensor and an Idle Air Control Valve. In this study, structural modification method is proposed to reduce structural vibration of an intake manifold system. At first, vibration problems are identified through tests on a running engine. Then modal data acquired by modal testing and finite element analysis are helpful to understand vibration mechanism of the system, and used as the design guide when structural modifications are attempted. After the system model is validated by comparison of the modal data obtained from analysis and experiment, iterative calculations are performed to find optimized structure of the system by finite element analysis. As a result, a newly designed plenum bracket is suggested in such a way that the intake manifold is stiffened, and that design of the support bracket is suggested in such a way that the intake manifold is stiffened, and that design of the support bracket is changed in terms of bolting position, thickness, shape, and minimum weight increase. Finally, it is shown that a new design achieves a significant reduction of vibration of an intake manifold system and it is confirmed by tests on a running engine.

대동맥판상부협착증: 치험 3례 (Supravalvular Aortic Stenosis - Report of 3 cases -)

  • Supravalvular aortic stenosis may be defined as an obstructive congenital deformity of the ascending aorta which originates just distal to the level of the origins of the coronary arteries It may be localized or diffuse. Enlargement of the aorta with a diamond-shaped patch of the noncoronary sinus of Valsalva was reported in 1961 by McGoon and associates But this reconstruction is asymmetric and the aortic obstruction may remain. In 1977, Dotty and associates reported the extended aortoplasty, the supravalvular ring was incised at two points in the noncoronary and in the right coronary sinuses of Valsalva closed with a tubular Dacron prosthesis of inverted Y-shape tailored to reconstruct the aorta We experienced three cases of the supravalvular aortic stenosis. The 11-year-old female and 4-year-old male with localized supravalvular aortic stenosis in William`s syndrome were operated with an inverted Y-shaped aortotomy toward the non-coronary sinus and the right coronary sinus and closed with "Hemashield`s collagen impregnated Dacron" tube graft, fashioned into "pantaloon" form patch. The 12-year-old male with localized supravalvular aortic stenosis and mitral insufficiency in William`s syndrome were operated with same procedure as two other patient above-mentioned for relief of supravalvular aortic stenosis and with mitral valve replacement. Postoperative course has been good.ourse has been good.

  • The low-emission and high-performance diesel combustion is an important issue in the combustion research community. In order to understand the detailed diesel flame field involving the complex Physical Processes, It Is quite desirable to study diesel spray dynamics, auto-ignition and spray flame propagation. Dynamics of fuel spray is a crucial element for air-fuel mixture formation flame stabilization and pollutant formation. In the present study, the diesel RCM (Rapid Compression Machine) and the Electric Control injection system have been designed and developed to investigate the effects of injection Pressure, injection timing, and intake air temperature on spray dynamics and diesel combustion processes. In terms of the macroscopic spray combustion characteristics it is observed that the fuel jet atomization and the droplet breakup processes become much faster by increasing the injection pressure and the spray angle. With increasing the cylinder pressure there is a tendency that the shape of spray pattern in the downstream region tends to be spherical due to the increase of air density and the corresponding drag force. Effects of intake temperature and injection pressure on auto-ignition is experimently analysed and discussed in detail.

주기적 연료 공급이 비애혼합 화염 특성에 미치는 영향에 관한 연구 (The Effects of Periodic Fuel Supply on the Flame Stability and Soot Formation)

  • The effects of periodic fuel supply on the nonpremixed flame stability and soot formation were experimentally studied. A solenoid valve was used to control the period of fuel supply. The laser induced incandescence technique was used to visualize cool: volume fraction profile. The flame base shape was changed significantly by the fuel supply period and partially by the fuel flowrates. The portion of bluish flame near the flame base became larger as the period increased. When the period was long, two flames coexisted within one period. It seemed that the characteristic of flame stability were repeated with 4.68m change of fuel supply line length. The soot mass measurements and soot volume fraction measurements revealed that the maximum suppression of soot by the perioic fuel supply was approximately 75% , which occurred when the occurred when the fuel supply period was relatively long.

다축제어를 이용한 모듈형 조향장치 하우징의 레이저용접에 관한 연구 (Study on Laser Welding of Automotive Modular Steering Gear Housing by using Multi-Axis Control)

  • Recently, automobile parts progress with modularization, which a great many allied products are modularized. Therefore, the purpose of this study is to develope modular housing for modularization of steering gear. Generally, steering gear housing is composed of valve housing and rack housing, it is important to combine two housings. However, housing having the pipe shape is very sensitive to welding distortion, and welding trajectory is very complicated. In order to solve this subject, cooperative control by using robots was constructed. Further, we developed the dedicated system to suit modular housing based on it, and applied laser welding to there. Moreover, welding speed was controlled in the rapid curve section so that the defect in trajectory of housing was reduced to obtain sound weldment. Accordingly, produced housing by this way is presented enough withstanding pressure to $100kg/cm^2$, and roundness and straightness are measured about 10/100 and 0.9/100 respectively.
