Early diagnosis of jaw osteomyelitis by easy digitalized panoramic analysis

  • Background: Osteomyelitis is an intraosseous inflammatory disease characterized by progressive inflammatory osteoclasia and ossification. The use of quantitative analysis to assist interpretation of osteomyelitis is increasingly being considered. The objective of this study was to perform early diagnosis of osteomyelitis on digital panoramic radiographs using basic functions provided by picture archiving and communication system (PACS), a program used to show radiographic images. Methods: This study targeted a total of 95 patients whose symptoms were confirmed as osteomyelitis under clinical, radiologic, pathological diagnosis over 11 years from 2008 to 2017. Five categorized patients were osteoradionecrosis, bisphosphonate-related osteonecrosis of jaw (BRONJ, suppurative and sclerosing type), and bacterial osteomyelitis (suppurative and sclerosing type), and the control group was 117 randomly sampled. The photographic density in a certain area of the digital panoramic radiograph was determined and compared using the "measure area rectangle," one of the basic PACS functions in INFINITT PACS® (INFINITT Healthcare, Seoul, South Korea). A conditional inference tree, one type of decision making tree, was generated with the program R for statistical analysis with SPSS®. Results: In the conditional inference tree generated from the obtained data, cases where the difference in average value exceeded 54.49 and the difference in minimum value was less than 54.49 and greater than 12.81 and the difference in minimum value exceeded 39 were considered suspicious of osteomyelitis. From these results, the disease could be correctly classified with a probability of 88.1%. There was no difference in photographic density value of BRONJ and bacterial osteomyelitis; therefore, it was not possible to classify BRONJ and bacterial osteomyelitis by quantitative analysis of panoramic radiographs based on existing research. Conclusions: This study demonstrates that it is feasible to measure photographic density using a basic function in PACS and apply the data to assist in the diagnosis of osteomyelitis.

  • Health industry, a knowledge based high value-added industry, is being considered as a strategic area for the 21C and many advanced countries are making every endeavors for the promotion of health industry along with information technology, new materials, and mechatronics. Korean health industry, however, has been excluded from the governmental supports as well as bound by strict regulation so far, and there is a significant gap in technology compared with advanced countries. In 21C, technology is the main factor of national competitiveness and that is why the role of R&D institutes are so important in the high level of competition to cope with the technology protection policies of advanced countries. In this article, with Directory of Korean R&D Institutes published by Korea Industry Technology Association, I reviewed the trends of R&D institute of health industry. Main findings of the research can be summarized as follows. The portion of health industry R&D institute is 3.6% of total R&D institute but the amount of R&D investment is over than 5% This means health industry are knowledge based and R&D intensified industry, meanwhile the variations of same industry R&D institutes of health industry is huge in R&D investments and other activities. Regional distributions of health industry institutes show some kind of different patterns in each industry areas. Medical devices and Medical informatics's preference of metropolitan region are distinguished with other industry areas. Many of the institutes are located in same site of it's company rather than operating separate building for R&D specific uses. It is better for transforming ideas to products and close cooperation of research body with product lines, but it is a handicap for networking and communicating with other research institutions too. It takes 18.4yrs for bearing R&D institute on the average. For a long times 'copy products' or 'me too products' policies were easy way to maintain business entities. But recently, it is recognized that research activities are essential component of sustaining it's own business firms. This means technology itself is leading power of corporation itself in the high level of competition.

Factors Influencing Asthma in Korean Adults: A Focus on Obesity and Systemic Inflammation (한국 성인의 천식 발생 영향요인: 비만 및 전신적 염증 상태를 중심으로)

  • Purpose: This study was conducted to identify factors influencing asthma, with a focus on obesity and systemic inflammation, in Korean adults. Methods: This study was a secondary analysis of data from the sixth Korea National Health and Nutrition Examination Survey (2015). A total of 3,693 individuals aged ≥19 years were included. The prevalence of asthma was 1.6% in the normal weight group (n=23), 1.4% in the overweight group (n=13), and 2.9% in the obese group (n=39). Data included markers associated with systemic inflammation such as high sensitivity C-reactive protein level, leukocyte count, hemoglobin level, and hematocrit value based on previous studies. The results were analyzed using a complex sampling design analysis and by multiple logistic regression analysis with SPSS WIN 24.0 program. Results: In the obese group, age between 50 and 59 years (adjusted odds ratio [AOR]=14.06, 95% confidence interval [CI]=1.84-27.14); age between 60 and 69 years (AOR=3.30, 95% CI=1.34-8.14); age ≥70 years (AOR=3.22, 95% CI=1.31-7.93); female gender (AOR=2.32, 95% CI=1.12-4.78); leukocyte count (AOR=1.18, 95% CI=1.01-1.38), and hemoglobin levels (AOR=0.60, 95% CI=0.45-0.81) were identified as factors influencing asthma. Conclusion: The results can be used to develop nursing interventions to prevent asthma associated with obesity in hospitals or home-based healthcare settings.

Standard based Deposit Guideline for Distribution of Human Biological Materials in Cancer Patients

  • Background: Human biological materials from cancer patients are linked directly with public health issues in medical science research as foundational resources so securing "human biological material" is truly important in bio-industry. However, because South Korea's national R and D project lacks a proper managing system for establishing a national standard for the outputs of certain processes, high-value added human biological material produced by the national R and D project could be lost or neglected. As a result, it is necessary to develop a managing process, which can be started by establishing operating guidelines to handle the output of human biological materials. Materials and Methods: The current law and regulations related to submitting research outcome resources was reviewed, and the process of data 'acquisition' and data 'distribution' from the point of view of big data and health 2.0 was examined in order to arrive at a method for switching paradigms to better utilize human biological materials. Results: For the deposit of biological research resources, the original process was modified and a standard process with relative forms was developed. With deposit forms, research information, researchers, and deposit type are submitted. The checklist's 26 items are provided for publishing. This is a checklist of items that should be addressed in deposit reports. Lastly, XML-based deposit procedure forms were designed and developed to collect data in a structured form, to help researchers distribute their data in an electronic way. Conclusions: Through guidelines included with the plan for profit sharing between depositor and user it is possible to manage the material effectively and safely, so high-quality human biological material can be supplied and utilized by researchers from universities, industry and institutes. Furthermore, this will improve national competitiveness by leading to development in the national bio-science industry.

Knowledge Reasoning Model using Association Rules and Clustering Analysis of Multi-Context (다중상황의 군집분석과 연관규칙을 이용한 지식추론 모델)

  • People are subject to time sanctions in a busy modern society. Therefore, people find it difficult to eat simple junk food and even exercise, which is bad for their health. As a result, the incidence of chronic diseases is increasing. Also, the importance of making accurate and appropriate inferences to individual characteristics is growing due to unnecessary information overload phenomenon. In this paper, we propose a knowledge reasoning model using association rules and cluster analysis of multi-contexts. The proposed method provides a personalized healthcare to users by generating association rules based on the clusters based on multi-context information. This can reduce the incidence of each disease by inferring the risk for each disease. In addition, the model proposed by the performance assessment shows that the F-measure value is 0.027 higher than the comparison model, and is highly regarded than the comparison model.

Practical Understanding of Gross Examination Techniques (육안검사기술의 실무적 이해)

  • Gross examination techniques (GETs) of specimens collected from cancer surgery or endoscopy comprise the act of recording visual information about cancer for accurate histopathological diagnosis and collecting sections of the lesion to create microscopic specimens. GETs must include concise and accurate expressions, appropriate structuring, sufficient resections, error-free standardization of important information, and photo-diagramming of complex specimens. To increase the satisfaction of pathological interpretation, it is a task that must be performed accurately and carefully to gain confidence on a theoretical and practical basis with a sufficient understanding of gross examination. Based on the experience of clinical pathologists in the field of GETs, additional specimen types should be identified as viable candidates. Also, their needs and concerns regarding treatment should be carefully considered. In addition, departments at each institution should review the national focus on clinical partnerships, continuous professional training, diagnostic errors, and value-based healthcare provision.

Standardization of Time of Smartphone Use in Adolescents : Using Data from Korea Youth Risk Behavior Web-based Survey of 2017 (청소년의 스마트폰 사용시간 표준화 연구 : 2017년 청소년건강행태온라인조사 원시자료를 이용하여)

  • The purpose of this study was to investigate the standardization of daily average smartphone use time among adolescents. The data were analyzed using the data from Korea youth risk behavior web-based survey of 2017, and the total number of students analyzed was 54,603. Frequency analysis, independent t test, Pearson correlation analysis, and one-way ANOVA were used for statistical analysis. As a result, the correlation coefficient between smartphone use time, age, and grade was very low (r =.025~.039). There was no significant difference in the usage of smartphone between middle and high school students(p>.05). Time of smartphone use was significantly higher in middle school students as the grade increased(p<.05). In high school students, second-year students had significantly more smartphone usage than first and third grade students(p<.05). The researchers described 54603 adolescents' average(minute) daily smartphone usage about sex(male, female), period of use(weekday, weekend), school(middle school, high school), and grade(first, second, third) as a 7th quartile. We suggest that the standard value of smartphone use time can be used as a basis for the physical and mental health management in adolescents. It is necessary to present a standard value that can prove the future overuse of smartphone.

Changes in upper limb muscle activity during smartphone usage while in stable and unstable positions and during gait

  • Objective: The objective of this study was to describe the influence of smartphone usage on the upper limb muscles while in stable and unstable positions and during gait. Design: Cross-sectional study. Methods: The study was conducted with 20 right-handed university students between 20 and 27 years of age. Experiments were carried out on students who have used cell phones for more than a year. In this study, experiments were performed with one-handed and two-handed smartphone operations while on stable ground, and the same parameters were measured during smartphone use on unstable ground and during gait. Subjects were instructed to write a text message in Korean on the smartphone for 3 minutes. This was repeated 3 times, with a rest period of 10 seconds given between each 3-minute period. Electromyography was used to record the muscle activity of the upper trapezius, extensor carpi radialis, extensor pollicis longus, and abductor pollicis (AP) during phone operation. Results: The muscle activity value for the right AP in one-handed and the left-AP in two-handed operations was statistically higher than the other muscles in the stable position and during gait (p<0.0001). Also, the right AP in the one- handed operation condition was statistically higher than the two-handed condition (p<0.0001). Conclusions: The use of smartphones while in a stable position resulted in high muscle activity of the right upper limb AP. However, in an unstable position, there were no significant differences from the other muscle activities. Using smartphone while assuming various positions may lead to musculoskeletal disorders in different places.

A Study on the Efficient Flow of Health Examinees (건강검진 수검자의 동선 효율화에 관한 연구)

  • The purpose of this study is to optimize the patient(examinee) flow in a health examination center via a simulation model and to improve operational efficiency. Two experimentation scenarios were implemented into the simulation model to determine which proposed scenario provides better improvement in terms of the following performance measures: LOS(Length of Stay), staff utilization, and occupancy level. The simulation results demonstrated that there was no significant difference in response results of two scenarios. Although the original motivation of this study was suggest optimal policy for a patient(examinee) flow, the insight into applying simulation in efficiently managing hospital operations is of more value. Simulation approach is a powerful technique that supports efficient decision-making compared to traditional healthcare management approach based on past experience, feelings, and intuition. Therefore, the proposed experimentation model has wide applicability in healthcare systems.

A Study on the Spatial Structure Change of Elderly Care Facility according to Introduction of Long-Term Care Insurance (장기요양보험제도의 도입에 따른 노인요양시설의 공간구조 변화에 관한 연구)

  • Purpose : Long-term Care Insurance sets up facility standard and installation standard of aged care facilities and decides the presence of minimum number of rooms and its size in care facilities by using systematic instruments. Therefore, most aged care facilities had renovation in expansion and reconstruction following the revised regulations and even facility space structure and space composition are continuously improving. The study is to determine the purpose and trend by comparing before and after space composition of facilities which followed the implementation of Long-term Care Insurance and also to suggest hierarchical space composition suitable for aged care facilities through derived problems and to provide basic materials to plan the most appropriate facility for the aged. Methods : J-graph based on Space Syntax Theory will be schematized through in-site facility survey and before and after facility floor plan. Space composition trend will be analyzed by comparing indexes through S3 program. Results : As a result of 5 cases analysis, the following results were found; the average of whole space depth is increasing due to the Extension, the number of volunteers is decreasing and rooms for geriatric care helpers are being created due to the geriatric care helper introduction duty. Also, there are solariums being created to improve the health of the aged and dispensaries are being placed on every floor with the increase of documentary work for geriatric care helpers. With the policy implementation, care facility space composition and structure are changing with facility standard and it was analyzed that facilities were mostly put under the control of limited number of people in care room and total ground area per person. It was also found that there is increase in care space integration through before and after comparison of whole integration value. This is considered as the important result not only in facility standard satisfaction, but also in care support of geriatric care helpers and the aged, its main users. Implication : In order for elderly care facilities to have quality improvement and to develop as suitable facilities for characteristics of the aged, independent environmental facility standard preparation of elderly care facilities is needed through mutual cooperation of construction fields with regulation and policy related researches.
