Angioleiomyoma is benign smooth muscle tumor originating from the vascular wall. While they can occur in various anatomical locations, they are rarely reported in the vallecula region of the oropharynx. We present a case of a 58-year-old female patient with a five-year history of progressive dysphagia and throat discomfort. Laryngoscopy revealed a large, soft, mobile mass located on the right side of the vallecula. Radiological imaging further characterized the lesion as a well-circumscribed, heterogeneous mass. Surgical intervention in the form of Transoral Videolaryngoscopic Surgery (TOVS) was performed, leading to the successful removal of the mass. Histopathological analysis confirmed the diagnosis of angioleiomyoma.

Angiomyolipoma is a benign tumor that is composed of adipose tissue, blood vessels and smooth muscle. The kidney is the most common location of this tumor. In the larynx, angiomyolipoma has been rarely reported in the literature. We describe the patient of 64-year-old male with an angiomyolipoma arising from the vallecula. The tumor had a smooth surface with numerous small vessels and showed heterogeneous contrast enhancement on CT. The tumor was not adhesive to surrounding tissue and was completely removed by laryngomicrosurgery with $CO_2$ laser. Microscopically, the tumor is composed of mature adipose tissue, tortuous thick walled blood vessels and thick bundles of smooth muscle.

Fish and chicken bones are the usual foreign bodies lodged in the hypopharynx, and most of them lodge In the lower pole of a tonsil, at the base of the tongue, at the lateral wall of the pharynx, or in a vallecula. Recently, the authors experienced a case of aspiration pneumonia caused by chronic local inflammatory reaction of the posterior wall of the hypopharynx originated from a special foreign body.

Background and Objectives: The purposes of this study are to find out clinical causes, clinical characteristics and treatment outcomes in relation to anatomical location of laryngeal cyst. Subjects and Method: A retrospective study of medical records was carried out for 170 patients with cysts on vocal cord, epiglottis, vallecula, arytenoid and aryepiglottic fold. Results: There were 83 cases of epiglottic cysts, 41 cases of vallecular cysts, 35 cases of intracordal cysts, 3 cases of arytenoid cysts and 2 cases of aryepiglottic cysts. Laryngeal cysts were more common in men than in women, and the ratio between men and women was 2:1. The age of patients ranged from 7 to 90 years, with their average age being 52 years. The most common symptom was voice change at intracordal cysts and globus sensation at vallecular and epiglottic cysts. The most common cause of intracordal cysts were voice abuse. But other location of laryngeal cysts doesn't have common causes. The average size of cysts was 0.3cm at vocal cord, 1.43cm at epiglottis, 1.4cm at vallecula, 0.9cm at arytenoid and 1cm at aryepiglottis. Recurrence was observed in 7 cases from 1 months to 18 months following the operation. Size of all recurred cysts was over the average. Conclusion: Physicians should be aware of changes in clinical patterns of laryngeal cysts according to location and have long follow-up period at large cysts after operation.

The supraglottic subtotal laryngectomy represents a conservation laryngeal procedure in which the upper portion of the larynx is removed without sacrificing the normal functions of the remaining larynx. The basis for this procedure rests in the embryologic derivation and consequent anatomic compartmentalization of the larynx and its lymphatics, which limit tumor spread. This procedure is performed for carcinoma involving the epiglottis and false cords, and can be extended to include carcinomas of the aryepiglottic fold and the anterior and lateral walls of the pyriform sinus and selected lesions involving the vallecula and base of the tongue. Recently the authors has experienced 4 cases of supraglottic cancer, which were performed supraglottic subtotal laryngectomy. One of which was died because of local recurrence, and the remaining cases were successful with satisfactory rehabilitation without local recurrence and impairment of voice and swallowing.
Background: The optimal sequence and extent of multimodality therapy remains to be defined for extrapulmonary small cell carcinoma because of its rarity. The purpose of our study was to assess the response to neoadjuvant chemotherapy followed by chemoradiation/radiation in patients with extrapulmonary small cell carcinoma. Materials and Methods: Four consecutively diagnosed patients were included in this study. The primary tumor site was oropharynx in three patients and esophagus in one. The patients with the limited disease were treated with chemotherapy followed by concurrent chemoradiation (n=2) or radiotherapy (n=1). The patient with the extensive disease with the primary site in vallecula was treated with chemotherapy and palliative radiotherapy to the metastatic site. Results: The median follow-up was 22.5 months (range, 8-24 months). Three patients with the limited disease (base of tongue, n=2; esophagus, n=1) were in complete remission. The patient with the extensive disease died of loco-regional tumor progression at 8 months from the time of diagnosis. Conclusions: The combination of chemotherapy and radiotherapy is the preferred therapeutic approach for patients with extrapulmonary small cell carcinoma. Induction chemotherapy followed by concurrent chemoradiation or radiation provides a good loco-regional control in patients with limited disease.

A study was performed to investigate the size of tongue area and intermaxillary space area, and compare the sexual differences between normal Korean children and adults by introducing planimetric and linear analysis of the lateral cephalograms. The cephalograms were composed of 41 child male aged 10.8, 40 child female aged 10.5, 38 adult male aged 21.3 and 40 adult female aged 20.8 respectively. In order to study and measure the intermaxillary space area, the followings were selected, as reference items: occlusal plane, anterior intermaxillary space height, posterior intermaxillary space height, length of intermaxillary space. Among those reference items anterior intermaxillary space height and posterior intermaxillary space height were perpendicular to the maxillary plane. An. index, (equation omitted) While the tongue area was plotted by outline of tongue shadow, above a line extending from the vallecula to the most anterior point on the hyoid body, and above a line from the most anterior point of the hyoid body to the menton. The obtained results were as follows: 1. In general the measurements of male were larger than those of female in intermaxillary space area in childhood and adulthood group. but intermaxillary space area of childhood group showed no significant sexual difference, and that of adulthood group showed significant sexual difference when evaluated statistically. 2. In both groups the measurements of male were larger than those of female in tongue area and there are also statistical significance of sexual differences in both age groups. 3. Considerable growh changes between the childhood and adulthood groups were revealed in intermaxillary space area and tongue area, and the tongue had tendency to become relatively smaller when compared with the intermaxillary space in both sex.

Modern people have abundant food and various forms of life compared to the past, but they have come to form an unhealthy diet, such as skipping breakfast and not eating in time in a busy life. When these eating habits are maintained for a long time, it leads to digestive trouble. The most easily occurring symptoms are called reflux esophagitis and dysphagia. Among them, dysphagia requires quick and accurate diagnosis as they develop into various forms of complications or are also identified as presymptoms of gastric and laryngeal cancers. The result of the diagnosis is still passively judged by the doctor and each of results are different depending on the doctor. The result of the diagnosis here means the severity. When they identify treatment or complications following the results of the diagnosis, the wrong diagnosis may lead to excessive or insufficient treatment. In this paper, to figure out the severity of dysphagia in the diagnosis of dysphagia, we studied the development of a program using the K-means algorithm in the processing of X-ray images for identifying residual food in epiglottic vallecula and pyriform sinus in the section leading to esophagus.

BACKGROUND/OBJECTIVES: Understanding the mechanism of chewing and swallowing food is important when creating a proper diet for older adults. This study investigated whether texture-modified model foods can reduce the difference in chewing and swallowing parameters between healthy community-living young and older adults. SUBJECTS/METHODS: In total, 35 older and 20 young adults (mean age: 75 and 25 years, respectively), matched for sex and number of teeth, were recruited and their unstimulated salivation and tongue pressure were measured. Simultaneous assessment of chewing and swallowing characteristics was conducted using surface electromyography and a videofluoroscopic swallowing study while the participants ingested 8 g of model food with one to four levels of hardness. RESULTS: The average tongue pressure and salivation among older adults were 61% and 49.7%, respectively, of the corresponding values observed in young adults. The older adult group used significantly (P < 0.05) increased muscle force with more chewing cycles (P < 0.05) than the young adult group, which was maintained even when consuming foods with the lowest hardness, although without chewing. However, the age effect on oral processing time existed only for the hardest foods. Swallowing difficulties among older adults were demonstrated by the significant increase in vallecula aggregation time. The total food intake duration was significantly (P < 0.05) longer in older adults than in young adults, regardless of food hardness. CONCLUSIONS: There were measurable differences in the process of chewing and swallowing food between young and older adults, which can be improved with food hardness control. Adjusting food hardness may help food intake in healthy older adults.
