Right Heart Failure during Veno-Venous Extracorporeal Membrane Oxygenation for H1N1 Induced Acute Respiratory Distress Syndrome: Case Report and Literature Review

  • A 38-year-old male was admitted with symptoms of upper respiratory infection. Despite medical treatment, his symptoms of dyspnea and anxiety became aggravated, and bilateral lung infiltration was noted on radiological imaging studies. His hypoxemia failed to improve even after the application of endotracheal intubation with mechanical ventilator care, and we therefore decided to initiate venovenous extracorporeal membrane oxygenation (VV ECMO) for additional pulmonary support. On his twentieth day of hospitalization, hypotension and desaturation (arterial saturated oxygen <85%) developed, and right ventricular failure was confirmed by two-dimensional echocardiography. Therefore, we changed from VV ECMO to venoarteriovenous (VAV) ECMO, and the patient ultimately recovered. In this case, right ventricular dysfunction and volume overloading were induced by long-term VV ECMO therapy, and we successfully treated these conditions by changing to VAV ECMO.

만성적인 에탄올 섭취 시 수종 천연 추출물 투여가 흰쥐의 혈청과 간의 지질 함량에 미치는 영향 (Effects of Herbal Extracts on Serum and Liver Lipid Levels in Chronic Ethanol Administered Rats)

  • This study had been done for the investigation of the effect of Vitis vinifera extract (VV), Schisandra chinensis extract (SC), Taraxacum officinale extract (TO), Gardeniajasminoides extract (GJ), Angelica acutiloba extract (AA) and Paeonia japonica extract (PJ) on fatty liver and hepatotoxicity which was induced by Lieber-DeCarli ethanol liquid diet. Male Sprague-Dawley rats were randomly divided into eight groups: ethanol diet (ED), normal diet (ND), ED+VV (100mg/kg), ED+SC, ED+TO, ED+GJ, ED+AA, and ED+PJ (300mg/kg/day). Rats fed liquid diets for 6 weeks showed remarkable increase in serum and hepatic lipids indicating the onset of alcoholic fatty liver. The increasing levels of GPT, ALP activities in serum were observed in the groups fed with alcohol-containing diets compared to those of the ND group. The VV, SC, TO, GJ, AA and PJ groups were decreased the levels of triglyceride, free fatty acid and total cholesterol in serum and liver and GPT, ALP activities in serum. Therefore, they can be utiliaed as a health functional food or new drug candidate for fatty liver and hepatotoxicity which was induced by chronic alcohol consumption.

New Generation of Imaging Radars for Earth and Planetary Science Applications

  • SAR (Synthetic Aperture Radar) is an imaging radar which can scan and image Earth System targets without solar illumination. Most Earth observation Shh systems operate in X-, C-, S-, L-, and P-band frequencies, where the shortest wavelength is approximately 1.5 cm. This means that most opaque objects in the SAR signal path become transparent and SAR systems can image the planetary surface targets without sunlight and through rain, snow and/or even volcanic ash clouds. Most conventional SAR systems in operation, including the Canada's RADARSAT-1, operate in one frequency and in one polarization. This has resulted in black and with images, with which we are familiar now. However, with the launching of ENVTSAT on March 1 2002, the ASAR system onboard the ENVISAT can image Earth's surface targets with selected polarimetric signals, HH+VV, HH+VH, and VV+HV. In 2004, Canadian Space Agency will launch RADARSAT-II, which is C-band, fully polarimetric HH+VV+VH+HV. Almost same time, the NASDA of Japan will launch ALOS (Advanced land Observation Satellite) which will carry L-band PALSAR system, which is again fully polarimetric. This means that we will have at least three fully polarimetric space-borne SAR system fur civilian operation in less than one year. Are we then ready for this new all weather Earth Observation technology\ulcorner Actual imaging process of a fully polarimetric SAR system is not easy to explain. But, most Earth system scientists, including geologists, are familiar with polarization microscopes and other polarization effects in nature. The spatial resolution of the new generation of SAR systems have also been steadily increased, almost to the limit of highest optical resolution. In this talk some new applications how they are used for Earth system observation purpose.

파종시기가 Hairy Vetch(Vicia villosa Roth) 품종의 생산성 및 사료가치에 미치는 영향 (Effect of Autumn Seeding Date on the Productivity and Feed Values of Hairy Vetch(Vicia villosa Roth.) Varieties)

  • 본 실험은 중부지방에서 헤어리 벳치 품종의 봄철 건물생산성과 사료가치를 알기 위하여 실시하였다. 1. 가을철 파종시가가 빠를수록, 봄철 수확시기가 늦어질수록 건물수량이 증가되었다. 건물수량은 조기예취에서 Ostsaat > Welta > Vv4712 > Penn-02 > Common > Minnie, 만기 예취에서는 Welta > Ostsaat > Penn-02 > Common > Vv4712 > Minnie의 순으로 많았다. 2. 단백질 수량은 가을철 파종시기가 빠를수록, 봄철 수확시기가 늦어질수록 증가하였다. 조기예취에서 조단백질 수량은 Ostsaat > Welta > Vv4712 > Common > Penn-02 > Minnie 순이었고, 만기예취에서는 Welta > Ostsaat > Common > Penn-02 > Vv4712 > Minnie 순으로 많았다. 3. TDA 수량은 조기예취에서 9월 10일 파종시기에서 1,894kg/ha로 가장 많았고, 만기예취에서는 9월 20일 파종시기에서 4,119kg/ha로 많았지만 다른 파종시기 간에서는 차이가 없었다. 품종에서는 Ostsaat와 Welta의 수량이 많았다. 4. ADF 함량은 파종시기가 늦어짐에 따라 감소하였으며, 수확시기가 늦어짐에 따라 증가하였다. ADF 함량은 조기예취에서 Minnie가 32%로 가장 낮았고, Ostsaat가 34.1%로 가장 높았지만, 만기예취에서는 품종간에 유의한 차이가 없었다. 5. NDF 함량은 조기예취에서는 파종시기가 늦어짐에 따라 감소하였지만, 만기예취에서는 일정한 경향이 인정되지 않았다. 품종에서는 Vv4712와 Ostsaat가 38.9%와 39.8%로 유의하게 높았다. 만기예취에서는 Ostsaat가 43%로 가장 높은 함량을 나타냈다. 6. RFV는 조기예취와 만기예취에서 평균 157%와 132%로 모든 품종이 높은 사료가치를 나타냈다.nce, self-transcendence 점수를 보였다. 결론: 계절성과 성격 및 기질적인 부분이 서로 연관성을 보였다. 이 연구가 우리나라에서 고등학생을 대상으로 한 최초의 연구로서 여기에서 보인 결과들은 임상적 적용에 도움을 줄 수 있을 것으로 보인다.지털 이미지에 이르게 되었는지 테크놀로지 미학 안에서 자동생성주의로서 색인 이미지, 디지털 코드로서 수치 이미지의 생성, 기원, 본질 그리고 정체성을 규명하고자 한다. 특히 본 논고는 논지의 정당성을 위해 다양한 매체미학자들의 주의주장 및 이론적 쟁점을 분석하고자 한다. 또 분석틀을 통해서 테크놀로지 미학의 근간인 기계, 기술성을 바탕으로 한 사진의 생성적 측면과 문화 안에서 변형된 프로그램에 의해 창조되는 디지털 이미지의 변형적 측면의 본질을 파악하고자 한다. 이렇게 사진과 디지털 이미지의 양자의 비교를 통해서 테크놀로지 미학 안에서 올드 미디어(사진)와 뉴 미디어(디지털 이미지)의 자리바꿈은 정당한 것이라는 사실과, 이런 역설적인 구조야말로 기계, 기술을 바탕으로 삼는 테크놀로지 매체의 숙명성이라는 사실을 강조함으로써 논문의 정당성을 강화하고자 한다. 마지막으로 본 논고는 하나의 얼굴, 하나의 정체성으로 자리할 수 없다는 사실을 역사로서 확증하고, 또 사진에서 부동의 존재론과 인식론의 모습은 애초부터 불가능하다는 사실을 지적함으로써 오늘날 제기되고 있는 '사진의 죽음,' '사진의 종말'은 쟁점의 정당성에도 불구하고 매체미학의 역사를 간과하는 오도된 비평이라는 사실을 결론으로 도출하고자 한다.적인 상호교착(chiasme)의 논리를 잘 보여준다고 이해되었다. 본 연구는 먼저 현대 미술의 흐름에서 시지각과 신체의 상호작용, 혹은 신체성의 개입에 대한 변화를 확언하기 위하여 지각과 신체의 위치를 모더니즘, 미니멀리즘, 후기미니멀리즘, 그 이후의 미술이라는 사적인 흐름의 큰 틀 속에서 살펴보았으며,

Computational study of the Nitrogen-16 source term in the ITER vacuum vessel cooling circuit through the coupling of system-level analysis code and CFD

  • In ITER, the evaluation of the activated water radiation source and its impact on the radiological levels is necessary to demonstrate compliance with the safety requirements. The use of simplified or conservative approaches often results in the application of expensive constraints on the installation that impact its economics, operations, and construction schedule. In this work, we propose a novel methodology to calculate the activated water source term with a higher degree of realism. The methodology is based on the coupling of a system-level code with a Computational Fluid Dynamics (CFD) code in an explicit, one-way approach. We apply this methodology to the evaluation of the16N radioisotope within the ITER Vacuum Vessel Primary Heat Transfer System (VV-PHTS) cooling circuit in a steady-state and transient scenarios. We chose this system since previous analyses of the VV-PHTS were done with simple, ad-hoc calculations that yielded results that differed by up to a factor of five, underscoring a higher level of uncertainty. As a result, we generate a computational model of the source term that can be used to evaluate the radiological condition surrounding the cooling systems during the operations.

Estimation of Rice Growth Using RADARSTA-2 SAR Images at Seosan Region

  • Radar remote sensing is appropriate for monitoring rice because the areas where this crop is cultivated are often cloudy and rainy. Especially, Synthetic Aperture Radar (SAR) can acquire remote sensing information with a high temporal resolution in tropical and subtropical regions due to its all-weather capability. This paper analyzes the relationships between backscattering coefficients of rice measured by RADARSAT-2 SAR and growth parameters during a rice growth period. We examined the temporal variations of backscattering coefficients with full polarization. Backscattering coefficients for all polarizations increased until Day Of Year (DOY 222) and then decreased along with Leaf Area Index (LAI), fresh weight, and Vegetation Water Content (VWC). Vertical transmit and Vertical receive polarization (VV)-polarization backscattering coefficients were higher than Horizontal transmit and Horizontal receive polarization (HH)-polarization backscattering coefficients in early rice growth stage and HH-polarization backscattering coefficients were higher than VV-polarization backscattering coefficients after effective tillering stage (DOY 186). Correlation analysis between backscattering coefficients and rice growth parameters revealed that HH-polarization was highly correlated with LAI, fresh weight, and VWC. Based on the observed relationships between backscattering coefficients and variables of cultivation, prediction equations were developed using the HH-polarization backscattering coefficients.

L, C, X-밴드 레이더 산란계 자동측정시스템을 이용한 콩 생육 모니터링 (Monitoring soybean growth using L, C, and X-bands automatic radar scatterometer measurement system)

  • 본 연구에서는 다편파 레이더 산란계 자동 측정시스템 이용하여 콩 생육변화를 관측하고 레이더 시스템에서 얻어진 후방산란계수과 콩 생육인자들과의 관계분석을 통하여 콩 생육추정 가능성을 모색하고자 하였다. 2010년도 농촌진흥청 국립식량과학원 연구지역에 다편파 레이더 산란계 관측시스템 (L, C, X-밴드 안테나, 네트워크분석기, RF switch, 입사각 $40^{\circ}$)을 구축하고 콩 파종시기에서 수확기까지 10분단위로 콩 생육변화를 자동 측정하였다. 모든 안테나 밴드, 편파에서 콩 생육초기 (6월초~7월 중순)에는 VV-편파가 HH-, HV-편파보다 후방산란계수가 높게 나타났고, 그 이후 HH-편파와 다른 편파들 간의 cross-over 현상이 일어났는데 그 시기는 L-밴드가 7월 20일 (DOY 200), C-, X-밴드의 경우에는 7월 30일 (DOY 210)로 밴드에 따라 차이를 보였다. 모든 밴드 및 편파에서 9월 29일 (DOY 271)까지 후방산란계수가 증가하다가 그 이후 감소하였고 특히 종실비대기 (DOY 277, R6) 이후 감소폭이 크게 나타났는데 이 현상은 콩 생육인자 (초장, 엽면적지수, 건물중 등)변화와 일치하였다. 밴드에 따른 후방산란계수와 콩 생육인자들과의 관계를 분석한 결과 L-밴드 HH-편파에서 LAI ($r=0.93^{***}$), 초장 ($r=0.95^{***}$), 건물중 ($r=0.94^{***}$), 꼬투리중 ($r=0.92^{***}$)등 콩 생육인자들과의 상관계수가 가장 높게 나타났고 이에 비해 X-밴드 편파에서는 콩 생육인자들과의 상관계수가 상대적으로 낮게 나타났다. 후방산란계수 (L-밴드 HH-편파)를 이용하여 콩 생육인자 추정을 위한 회귀식을 작성하였다.

SRD5A2 Gene Polymorphisms and the Risk of Benign Prostatic Hyperplasia but not Prostate Cancer

  • Background: Testosterone, a primary androgen in males, is converted into its most active form, dihydrotestosterone (DHT), by $5{\alpha}$-reductase type 2 (encoded by the SRD5A2 gene) in the prostate. DHT is necessary for prostatic growth and has five times higher binding affinity than testosterone for androgen receptors. We hypothesized that polymorphic variations in the SRD5A2 gene may affect the risk of benign prostatic hyperplasia and prostate cancer. Materials and Methods: We analyzed SRD5A2 gene polymorphisms in 217 BPH patients, 192 PCa cases, and 171 controls. Genotyping was undertaken using direct DNA sequencing. Genotype data were compared between cases and controls using a Chi square statistical tool. Results: We found that the A49T locus was monomorphic with 'AA' genotype in all subjects. At V89L locus, the presence of 'VV' showed a marginally significant correlation with increased BPH risk (p=0.047). At the $(TA)_n$ locus, longer TA repeats were found to be protective against BPH (p=0.003). However, neither of these polymoprhisms correlated with the risk of PCa. Conclusions: We conclude that A49T is monomorphic in the study population, VV marginally correlates with BPH risk, and longer $(TA)_n$ repeats are protective against BPH. None of these polymorphisms affect the risk of PCa.

A Study on Prevalence and Risk Factors for Varicose Veins in Nurses at a University Hospital

  • Background: Lower-limb varicose veins (VVs) are common and known to have a higher prevalence among people who work in occupations requiring prolonged standing. In the Republic of Korea, however, VV-related occupational factors have seldom been examined. This study was conducted to assess the prevalence of VVs among nurses, an occupational group considered to be at high risk of VVs, and determine the occupational risk factors of prolonged standing. Methods: Between March and August 2014, a questionnaire survey coupled with Doppler ultrasonography was conducted on the nurses working at a university hospital. Results: A total of 414 nurses participated in the survey and diagnostic testing. From the survey analysis and test results, the prevalence of VVs in nurses was estimated to be 16.18%. Significant factors for venous reflux were age [odds ratio (OR) = 1.06, 95% confidence interval (CI) = 1.03-1.10], pregnancy (OR = 2.15, 95% CI = 1.17-3.94), and delivery (OR = 2.02, 95% CI = 1.08-3.78). The statistical significance of these factors was verified after risk adjustment for sociodemographic factors (OR = 3.40, 95% CI = 1.27-9.09). Conclusion: Factors significantly associated with venous reflux were increasing age and prolonged working hours (${\geq}4hours$) in a standing position (OR = 2.80, 95% CI = 1.08-7.25), even after risk adjustment for sociodemographic factors. This study is significant in that an objective diagnosis of VVs preceded the analysis of the risk factors for VV incidence, thus verifying objectively that VVs are associated with occupations requiring prolonged hours of working in a standing position.

IVET-based Identification of Virulence Factors in Vibrio vulnificus MO6-24/O

  • Vibrio vulnificus is an opportunistic pathogen that causes septicemia in humans. To identify the genes associated with its pathogenicity, in vivo expression technology (IVET) was used to select genes specifically expressed in a host, yet not significantly in vitro. Random lacZ-fusions in the genome of V vulnificus strain MO6-24/O were constructed using an IVET vector, pSG3, which is a suicide vector containing promoterless-aph and -lacZ as reporter genes. A total of ${\sim}18,000$ resulting library clones were then intraperitoneally injected into BALB/c mice using a colony forming unit (CFU) of $1.6{\times}10^6$. Two hours after infection, kanamycin was administered at $200{mu}g$ per gram of mouse weight. After two selection cycles, 11 genes were eventually isolated, which were expressed only in the host. Among these genes, VV20781 and VV21007 exhibiting a homology to a hemagglutinin gene and tolC, respectively, were selected based on having the highest frequency. When compared to wild-type cells, mutants with lesions in these genes showed no difference in the rate of growth rate, yet a significant decrease in cytotoxicity and the capability to form a biofilm.
