  • As intervention techniques have been developed recently, minimal invasive treatment for uterine myoma using uterine artery embolization is receiving attention increasingly. Uterine rupture occurs rarely, but once it occurs, it may cause fatal results in both mother and fetus. Although the relationship between uterine artery embolization for treatment of uterine myoma and uterine rupture has not been clearly revealed yet, a case implying that the embolization for treatment of uterine myoma can be a risk factor of uterine rupture like previous caesarian delivery or myomectomy.

  • Uterine rupture is uncommon in small animals and can be caused by trauma, infection, dead fetus, or inappropriate obstetric techniques. A 4-year-old, intact female mixed-breed dog weighing 3 kg presented for elective ovariohysterectomy with a history of a motor vehicle accident two months previously. The patient showed no clinical signs of uterine abnormalities or evidence of pregnancy during physical examination. Clinical examinations, radiography, and ultrasonography revealed no significant findings. During ovariohysterectomy, a completely transected left uterine horn and dome-shaped ends were identified. The uterine layers were everted such that the endometrium could be seen outside the uterine horn. The dog recovered without post-operative complications and was discharged the following day. This report describes a rare case of an asymptomatic uterine rupture caused by trauma in a non-gravid dog.

Spontaneous Uterine Rupture in a Greyhound Bitch during Parturition

  • A 6-year old, Greyhound bitch was presented with vaginal hemorrhage and dystocia. Physical examination revealed severe vaginal hemorrhage, abdominal pain, pale mucous membranes and the presence of solid structures to abdominal palpation. A hematological test revealed a marked hemorrhagic anemia, and abdominal radiography and ultrasonographic examination showed two dead fetuses in the uterus. Median laparotomy revealed a rupture of the left uterine horn adjacent to the bifurcation, region of weakened uterine wall in the right uterine horn, blood clots and uterine fluids in abdominal cavity without septic peritonitis. The bitch underwent ovariohysterectomy and recovered without complication.

  • Spontaneous uterine rupture of the unscarred uterus during the second trimester of pregnancy is rare, but it is a surgical emergency. Because it results in rapid deterioration of patient and high mortality despite of prompt operation and massive transfusion, early diagnosis and proper management are critical for optimizing patient care. We present a case of spontaneous uterine rupture with fetal death in 14 weeks gestation with a brief review of literatures.

Uterine Rupture with Retained Placenta in a Primiparous Bichon Frise Bitch

  • This report describes a case of postpartum uterine rupture associated with retained placenta and sequential acute metritis, not dystocia. A 14-month-old Bichon Frise bitch presented with continuous vaginal bleeding. She delivered five puppies 3 days ago and expelled four placentas on the next day. After then, anorexia, acute depression with continuous vaginal haemorrhage developed. Plain radiography showed decreased serosal detail, and ultrasonography showed irregularly thickened uterine wall and ascites. Blood-tinged peritoneal fluid was an exudate with bacteria. Ovariohysterectomy was curative for the patient, and four full-thickness perforated holes were identified on both sides of the uterine horns.

  • In this report, we described a case of acute colic with uterine rupture in a Thoroughbred broodmare during parturition. A 14-year-old broodmare showed acute colic characterized by rolling just after delivering of a healthy foal. Large intestine was prolapsed through vulva, contaminated with soil and its serosa was heavily damaged. We performed a euthanasia according to the owner's request. Post mortem examination revealed about 15 cm rupture on ventral side of the uterus. In addition, the orphan foal follow his owner after necropsy on his dam.

  • Traumatic uterine rupture is uncommon but can be fatal and life-threatening for both the mother and infant. In addition to complications caused by trauma itself, such as pelvic fracture, gestational complications such as placental abruption, abortion, premature labor, rupture of membranes, maternal death, and stillbirth can occur. In particular, fetuses have been reported to have a high mortality rate in cases of traumatic uterine rupture. A 35-year-old pregnant female patient fell from the fourth floor and was admitted to our trauma center. We observed large hemoperitoneum, pelvic fractures, and spleen laceration, and the fetus was presumed to be located outside the uterus. The pregnant woman was hemodynamically unstable. Although the fetus was stillborn, angioembolization and surgical treatment were properly performed through collaboration with an interventional radiologist, obstetrician, and trauma surgeons. After two orthopedic operations, the patient was discharged after 34 days. This case report suggests the importance of a multidisciplinary approach in the treatment of pregnant trauma patients.

Canine Uterine Rupture with Septic Peritonitis Results from Adenocarcinoma in a 2-year-old Maltese Dog

  • A 2-year-old female Maltese dog was presented with a history of anemia and vaginal hemorrhagic discharge. Physical examination revealed severe vaginal hemorrhagic discharge, abdominal pain, pale mucous membranes, low blood pressure and dehydration. Results of serum biochemistry, hematology, venous blood gas, and electrolyte canine C-reactive protein (CRP) test revealed severe normocytic normochromic anemia, severe neutropenia, a high level of CRP, hypoglycemia, and imbalanced electrolytes. Abdominal ultrasound examination showed focal hypoechoic defect with loss of layering in uterine horn wall. A laparotomy revealed a clear reddish fluid in the abdomen, the fistula of left and right uterine horn, the purulent discharge from fistula, and symptoms of septic peritonitis near by the fistula site. The bitch underwent ovariohysterectomy and recovered without complication. Histopathological diagnosis of the uterine fistula site was adenocarcinoma.

  • Intramural pregnancy is an unusual ectopic gestation located within the uterine wall, completely surrounded by myometrium and separate from the uterine cavity, fallopian tube, or round ligament. It is known to be difficult to diagnose, and associated with a high rate of uterine rupture. We report a case of intramural pregnancy in which early diagnosis was made and successful treatment was done by dilatation and curettage. Diagnostic laparoscopy confirmed the absence of uterine rupture during the procedure. Therefore, conservation of fertility can be possible with early diagnosis of intramural pregnancy. To our knowledge, this is the first case report of intramural pregnancy following IVF-ET in Korea.
