Trends in intensity-modulated radiation therapy use for rectal cancer in the neoadjuvant setting: a National Cancer Database analysis

  • Purpose: Traditionally, three-dimensional conformal radiation therapy (3D-CRT) is used for neoadjuvant chemoradiation in locally advanced rectal cancer. Intensity-modulated radiation therapy (IMRT) was later developed for more conformal dose distribution, with the potential for reduced toxicity across many disease sites. We sought to use the National Cancer Database (NCDB) to examine trends and predictors for IMRT use in rectal cancer. Materials and Methods: We queried the NCDB from 2004 to 2015 for patients with rectal adenocarcinoma treated with neoadjuvant concurrent chemoradiation to standard doses followed by surgical resection. Odds ratios were used to determine predictors of IMRT use. Univariable and multivariable Cox regressions were used to determine potential predictors of overall survival (OS). Propensity matching was used to account for any indication bias. Results: Among 21,490 eligible patients, 3,131 were treated with IMRT. IMRT use increased from 1% in 2004 to 22% in 2014. Predictors for IMRT use included increased N stage, higher comorbidity score, more recent year, treatment at an academic facility, increased income, and higher educational level. On propensity-adjusted, multivariable analysis, male gender, increased distance to facility, higher comorbidity score, IMRT technique, government insurance, African-American race, and non-metro location were predictive of worse OS. Of note, the complete response rate at time of surgery was 28% with non-IMRT and 21% with IMRT. Conclusion: IMRT use has steadily increased in the treatment of rectal cancer, but still remains only a fraction of overall treatment technique, more often reserved for higher disease burden.

공공서비스시설로서의 구민회관에 대한 사용자 이용현황과 요구도 및 만족도 분석 (Analysis on User Expectation and Satisfaction Toward Public Community Centers and the State of Use as Public Service Facilities)

  • The physical environment is one of several service characteristics that are important contributors to service quality. As far as public service buildings, public community facilities serve as arenas for education, social interaction, leisure, and health activities for community districts. The purpose of this study was to investigate the physical state of public community facilities and use of public community facilities in Seoul and Suwon and to identify expectations of users toward the physical environment and service aspects. Site visits to four public community facilities were conducted and surveys were distributed for this study, with one hundred ninety two questionnaires analyzed. Results suggest that in order to provide more equal benefits and service to the public, cities should provide several smaller instead of one large complex community facility. Additionally, facility users indicated large gaps between their expectations and the current state of facilities due to lack of storage, noise and difficulty of use. In general, physical characteristics should be improved rather than employee's attitudes and service behaviors, resulting in greater service quality.

동작 정보를 갖는 가상설비 데이터 재활용 방법론 (A Method of Reusing Kinematic Information for Virtual Facilities)

  • This paper proposes a method for reusing kinematic design data for virtual facilities, Making a virtual model of a facility involves two major activities: geometric design (virtual model visualization) and kinematic design that should be remodeled frequently whenever design changes occur, Conventionally, a virtual model of an automated facility focuses on the design level, which mainly deals with design verification, alternative comparison, and geometric model diagnosis, Although a design level model can be designed with the information of past models from PLM system, a simulation level model is not sufficient utilized to be reused for kinematic design purpose, We propose a method for reusing kinematic information of a past simulation model to cope with this problem, We use the concept or the 'center of mass', which is a point representing the mean position of the matter in a body or system. And we also use comparison method of a boundary box to identity which 3D objects have to be involved from the design model to a link structure that is contained in the simulation model. Because a proposed method only use not a historical approach but a geometrical approach, it is more effective to apply to the field.

ENHANCING UTILIZATION OF BUILDINGS THROUGH INTEGRATED ANALYSIS OF SPACE, USER, AND USER ACTIVITY

  • Enhancing utilization of buildings is gaining in importance in response to a challenging economy; thus, there is a need for a method that analyzes space, user, and user activity in an integrated way to provide project stakeholders with utilization information to support their decision-making about buildings. Conventional methods, such as architectural programming and post-occupancy evaluation, lack a formal relationship between user activity and other information, and therefore, are coarse-grained. This relationship has been formalized by two relatively new methods that provide fine-grained utilization information: workplace planning and space-use analysis. We characterize these two methods with focuses on their usage in different phases (i.e., planning, design, occupancy), required information that needs to be gathered, and the achievement and limitations in terms of three criteria, i.e., consistency, efficiency, and transparency. This characterization would not only help project stakeholders select and use a method that best meets their purposes for enhancing utilization of their buildings, but also provide researchers with promising research topics regarding enhancing utilization of buildings.

만성질환자별 주관적 건강상태와 의료시설 이용행태 (Self-rated Health and Health Service Utilization of Chronic disease Patients)

  • 본 연구는 주관적 건강상태가 의료시설 이용에 영향을 미친다는 전제로 주관적 건강상태를 파악하고, 또한 주관적 건강상태에 따른 의료시설 이용의 차이를 만성질환자별로 분석하여 의료시설 이용 시 표준지침의 기초자료로 제공되는 것을 목적으로 하였다. 대표성 있는 고령화패널 자료를 사용하여 총 7,486명을 분석하였다. 만성질환자는 주관적 건강상태에 따라 의료시설 이용에 차이를 나타내고 있었으며, 특히 한의원 한방병원과 치과 치료에 있어서 질환별 차이가 두드러졌다. 하지만 이차자료의 활용으로 의료이용에 영향을 미치는 여러 변수들을 통제하지 못하여 후속 연구들을 제시하는데 본 연구의 의의를 두었다.

지역폐교 이용실태 및 친환경계획개념 적용사례 조사연구 (A Study on the Utilization of Regional Closed Schools applied Ecological Concept)

  • Many closed schools of about more 3,000(2006 year) have occurred all over the country according to governmental policy of 1982. These closed schools were sold or rent to institution, local government, and a private, and are used by various usage such as novitiate, institution, educational facility, art creation facilities, warehouse, and factory. But, unimproved closed school facilities are still reaching for about 450. Among practical use example of such closed school, alternative educational facility, practice facilities, art creation facilities etc. with closed schools which have general surrounding natural landscape and environment uses well preserved were applied consciousness and the practice about environmental problem to ecological concept. Also, a lot of discussions about ecological concept and study for desirable solutions about practical use of closed schools example have been achieved. However, it is hard to find various ecological practical use examples at filed. In such background, this research wishes to investigate changing space and facilities variously and its usage which is applied ecological concept with regional closed schools.

부산연안의 인공어초 시설 현황과 유효공용적 평가 (Estimation of Efficient Use of Volume and Facility Volume Distribution of Artificial Reefs deployed in the Busan Sea Region)

  • 본 연구는 부산연안에 기 시설된 인공어초에 대해서 현재 시설현황을 분석하고 조사된 결과를 바탕으로 시설된 단위어초의 유효공용적율 평가 연구를 수행하였다. 본 연구를 통해 획득되어진 연구결과는 다음과 같이 요약할 수 있다. 부산연안의 연도별 인공어초시설 추이를 살펴본 결과, 단위어초수와 시설면적은 1997년부터 2004년까지 큰 폭의 변동 경향을 나타내었다. 하지만 시설물량에서는 2003년이후 급격하게 감소된 결과를 나타내었는데 강제어초 및 세라믹어초 등 다양한 어초가 사용됨에 따라서 나타난 패턴이라 할 수 있다. 기장군 해역에 시설되어 있는 어초는 2,777개로 89%의 확인율을, 해운대 남구해역은 7,691개로 어초 확인율은 84.9%, 사하 강서해역에의 확인량은 사각형어초의 경우 905개로서 96%, 그 외 어초는 모두 100%이었다. 부산연안에 시설된 인공어초의 경우 그 보존상태가 약 85%이상으로서 매우 양호한 상태를 나타내고 있음을 알 수 있으며 해저 지반조건에 따라서 다소 침하, 이탈, 망실 등 약 15%의 손실이 발생하였다.

국립백두대간수목원의 식물자원 및 관리·활용방안 - 수목원 중점조성지역, 문수산, 옥석산을 중심으로 - (A Study on the Management and Use of Plant Resources in Baekdudaegan Arboretum - Focused on Aboretum Core area, Munsu and Okseok Mountain -)

  • This study was carried out to survey the vascular plants of Baekdudaegan Arboretum area. The results are as follows. The numbers of flora in this study area were summarized as 655 taxa including 95 families, 332 genera, 567 species, 5 subspecies, 72 varieties and 11 formas. The rare plants were 17 taxa including Parasenecio firmus, Rhododendron micranthum, Iris odaesanensis, Lysimachia coreana and so forth. The Korean endemic plants were 16 taxa including Weigela subsessilis, Heloniopsis koreana, Salix koriyanagi, Vicia chosenensis and so forth. Plants Adaptable to Climate Change were Northern plants 18 species, Southern plants 2 species, endemic plants 10 species. The naturalized plants were 44 taxa and ratios of naturalized index and urbanization index were estimated Core area 10.2% and 4.9%, Mt. Munsu 4.9% and 2.3% and Mt. Okseok 5.5% and 2.6%. The floristic regional indicator plants found in this area were 3taxa(2.5%) of grade V, 12taxa(11.0%) of grade IV, 22taxa(20.2%) of grade III, 35taxa(32.1%) of grade II. In life forms, hemicryptophytes were The most dominant. Next was a Therophytes. In addition, 492 taxa were categorized by usage into 8 groups, as follows: Edible 239taxa(36.5%), Fiber 10taxa(1.5%), Industrial 1taxon(0.2%), Miscellaneous 183taxa(27.9%), Ornamental 65taxa(9.9%), Pasturing 259taxa(39.5%), Siscellaneousg 14taxa(2.1%) and Timer 21taxa(3.2%).

공동주택단지 내 휘트니스 센터 시설현황 분석에 관한 연구 (The Present Status Analysis of Interior Planning in a Fitness Center Inside an Apartment Complex)

  • Lately due to the sudden growth of the construction of an apartment complex in domestic market, many construction companies are eager to provide various public facilities to increase the apartment distribution rate. An introduction of a fitness center is popularized as a part of such development. There has been a continuing development in a private fitness based on analyzing the user's data. However a public fitness is quite different from the private in terms of a user's classification, preference rate, the pattern of use, and location which requires a different facility plan. Nevertheless, all these days a public fitness in an apartment house has been developed without a specific facility plan based on relevant materials. It is worried that such development would cause the facilities not to function as appropriate resident facilities and sink to idle ones. Therefore the purpose of this paper is to make the standard considered in facility planning in future and the base to guide an applying method. In the paper the present condition of fitness centers inside apartment houses has been compared to analyze the progress of the existing facilities and the appropriateness of the organization and the structure division. The result of the research shows that public resident facilities is not the place to perform passive role for fixed activities but one to form a community through the human relationship based on the facility. Therefore a fitness center inside the apartment complex have to be designed to provide service and leisure space as one of main community facilities for residents. To obtain this purpose, when choosing facilities of an apartment complex, first of all the residents' standard of living, apartment size, residents' distinction rate of age and sex, the needs of the times should be fully considered Secondly, the size of each facility space have to be decided based on the practical data analysis in facility use such as space preference, average staying time. Also, future living culture requires the change of the function and space according to the change of social values, so continuing research and data analysis are required to related to fitness center inside an apartment complex in order to present systematic approaching method and the paper will be expected to be a little step toward it.

노인 의료복지시설의 실태분석 및 시설체계에 관한 연구 - 부산광역시 노인 의료복지시설을 대상으로 - (A Study on the Current Status and System of the Elderly Medical and Welfare Facilities with the Subjects in Busan)

  • 이 논문은 노인의료복지시설의 현황과 시설체계에 관한 논문으로 부산시를 대상으로 하였다. 현황 분석과 설문조사을 통해 그에 따른 문제점을 분석하고, 해결방안으로 새로운 유형의 시설체계를 제시하였다. 현재 노인의료복지시설들은 입소대상자들을 다양하게 수용하고 있지 못하며, 각각 독립적으로 운영되고 있어서 한 시설 안에서 전문적인 치료와 요양을 동시에 할 수 없다. 이러한 문제점들을 토대로 본 논문에서는 그 해결방안으로 좀 더 넓은 범위에 걸쳐서 한 장소에서 요양과 전문적인 치료가 가능한 'one-stop service system' 체계를 제언하였다. 설문조사 결과들을 종합하여, 구상한 5가지 유형의 시설체계는 다음과 같다. 1) 무료전문요양시설 + 노인전문병원, 2) 유료전문요양시설 + 노인전문병, 3) 노인전문병원 + 실버타운 주거공간 형식의 입원실, 4) 실비전문요양시설 + 노인전문병원 + 실버타운 주거공간 형식의 입원실, 5) 노인전문병원 + 재가복지서비스. 노인관련시설은 더 이상 특수한 시설이 아니라, 누구든 언젠가는 이용 하게 될 시설이라는 점에 착안할 때, 그에 대한 대책 마련은 시급하다고 생각된다. 앞으로 노인의 질환의 종류 및 정도가 보다 중복, 중대화 될 것으로 보인다. 따라서 보다 다양하고 전문화된 시설이 요구될 것으로 예상된다.
