The Effect of Back Pain Prevention Intervention Program on Back Pain Relief in Patients Following Percutaneous Coronary Intervention (요통예방 중재 프로그램이 경피적 관상동맥 중재술 후 환자의 요통완화에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effectiveness of back pain prevention intervention program on reducing back pain of patients undergoing percutaneous coronary intervention (PCI). Methods: The patients were divided into two groups as the experimental group and control group. In the experimental group, the participants took bedrest for 4 hours after PCI and then received back pain prevention intervention program (BPPIP). Total of 5 times BPPIP with 1 hour interval for 5-10 minutes per each intervention was administered to the patients taking bedrest for 4 hours after PCI. In the control group, total of 5 times routine nursing care with 1 hour interval was administered to the patients taking bedrest for 4 hours after PCI. The data were collected on admission in the ICU and after the 5 exercise sessions. The collected data were analyzed using SPSS/WIN program. Results: No significant differences in the occurrence of hemorrhage after the BPPIP were observed between the experimental group and the control group. After the BPPIP, back pain outcomes were significantly low in the experimental group. However, urination disorder and cortisol level did not show a statistically significant differences between two groups. Conclusion: It is clear that BPPIP is a useful nursing intervention for reducing back pain of patients undergoing PCI.

General Pharmacological Properties of Recombinant Human Parathyroid Hormone (재조합 부갑상선 흘몬의 일반약리작용)

  • General pharmacological properties of recombinant human parathyroid hormone (hPTH) were examined. The administration of hPTH (7, 35 and 175 lU/kg sc) in mice had no effects in general behavior and central nervous system, and no influences on normal body temperature, writhing syndromes induced by 0.7% acetic acid solution and chemoshock produced by strychnine and pentetrazol solution. hPTH (9 and 44 lU/kg, sc) given to anesthetized rabbits showed no effect on blood pressure of carotid artery and respiration, and it did not influence the responses produced by injection of acetylcholine or epinephrine. It showed no direct effect at 4.4$\times$10$_{-2}$IU/ml in isolated stomach fundus and uterus of rats and ileum of guinea-pig, and it also showed no inhibition of contraction produced by acetylcholine, oxytocin, serotonin and histamine in the above-mentioned preparations. It did not influence intestinal propulsion at the doses of 7,35 and 175 lU/kg sc in mice. This drug exhibited no effect on urinary excretion at the doses of 7 and 35 lU/kg sc in rats. However, at a dose of 175 lU/kg sc, it showed a decreased urination along with decreased excretion of potassium, sodium and chloride ion. These results indicate that hPTH does not exert any of serious pharmacological effects except the inhibition of urination at a highest dose used.

A Study on Syndromes of Oryeongsan(五岺散證) (오령산증(五岺散證)에 대한 연구(硏究))

  • In "Sanghanlon(傷寒論)", there are several articles to explain about Oryeongsan's syndrome. Most important articles of them are No. 72 and No. 75. Oryeongsan is explained by many doctors as a prescription to control exterior and interior, because they considered that symptoms of 72 and 75 articles were caused by exterior and interior's syndrome. Accordingly, they explained that RAMULUS CINNAMOMI(桂枝) and warm water remove exterior pathogens, and PORIA(茯笭) and others remove interior fluid retention. But considering the origin of a fluid retention, RAMULUS CINNAMOMI's effects are rather activating Yang(通陽) and warming Yang(溫陽) than removing exterior pathogens. The fluid retention is cauesd by insufficiency of Yang energy, because body fluid's spreading depends on Ynag energy. RAMULUS CINNAMOMI's effects of activating Yang and warming Yang improve the fluid retention and spread body fluid to the whole body. Some doctors use CORTEX CINNAMOMI(肉桂) instead of RAMULUS CINNAMOMI, because CORTEX CINNAMOMI's effects of activating Yang and warming Yang are better than RAMULUS CINNAMOMI's. There are many opinions about what is the main symptom of Oryeongsan's syndrome, but the difficulty in urination is understood by most doctors as the main symptom of Oryeongsan's syndrome. From the viewpoint of that, they understand that Oryeongsan's main effect is the induce diuresis. But the induce diuresis does not only mean a urination. The induce diuresis must be understood as the removing fluid retention by sweat and urine through the spreading body fluid.

Four Cases Report of the Female Lower Urinary Tract Infection with Oryeong-san (오령산(五苓散)으로 호전된 여성 하부 요로감염증 환자 치험 4례)

  • Purpose: This paper aims to report the effects of Oryeong-san on lower urinary tract infection. Methods: Four women were admitted to Dunsan Oriental Hospital with chief complaint that was not supposed to be a symptom of urinary tract infection. During the admission, urine test showed pyuria with white blood cell and their chief complain also accompanied. We prescribed Oryeong-san three times a day, and then took follow-up 4 days later. Results: After treatment, symptoms of lower urinary tract infection were decreased and the results of urine test improved also. Conclusions: These cases indicate that Oryeong-san is effective in treatment of the lower urinary tract infection.

A case of disseminated strongyloidiasis diagnosed by worms in the urinary sediment

  • Strongyloidiasis is a chronic infection caused by the intestinal nematode parasite Strongyloides stercoralis and is characterized by a diverse spectrum of nonspecific clinical manifestations. This report describe a case of disseminated strongyloidiasis with urination difficulty, generalized weakness, and chronic alcoholism diagnosed through the presence of worms in the urinary sediment. A 53-year-old man was hospitalized for severe abdominal distension and urinary difficulties that started 7-10 days prior. The patient also presented with generalized weakness that had persisted for 3 years, passed loose stools without diarrhea, and complained of dyspnea. In the emergency room, approximately 7 L of urine was collected, in which several free-living female adult and rhabditiform larvae of S. stercoralis, identified through their morphological characteristics and size measurements, were detected via microscopic examination. Rhabditiform larvae of S. stercoralis were also found in the patient's stool. During hospitalization, the patient received treatment for strongyloidiasis, chronic alcoholism, peripheral neurosis, neurogenic bladder, and megaloblastic anemia, and was subsequently discharged with improved generalized conditions. Overall, this report presents a rare case of disseminated strongyloidiasis in which worms were detected in the urinary sediment of a patient with urination difficulties and generalized weakness combined with chronic alcoholism, neurogenic bladder, and megaloblastic anemia.

The Effect of Korean Medical Treatment on the Overactive Bladder Syndrome with Coughing: Case Report (기침을 동반한 과민성 방광 증후군에 대한 한방 치료 1례)

  • Objective: Overactive bladder syndrome refers to the symptom of frequent urination, as the function of the bladder is so sensitive that the bladder muscles contract regardless of the patient's intent and the patient feels the need to urinate quickly. Overactive bladder syndrome is common among the elderly, especially in women, and the prevalence increases with increasing age. Overactive bladder can decrease social activity, increase isolation, and decrease quality of life due to sleep disorders caused by night urination; therefore, active treatment is required. In this study, we report the effects of Korean medical treatment on a Korean hospital patient with overactive bladder symptoms, including urine frequency, urine incontinence, and night urination that began at a similar time to an onset of coughing. Methods: We treated the patient with herbal medicines and acupuncture therapy. The International Prostate Symptom Score (IPSS), Overactive Bladder Symptom Score (OABSS), and a numeric rating scale (NRS) were used to assess symptom changes. Results: The patient with urine frequency, urine incontinence, and nocturia was hospitalized for 8 days, and the overall symptoms of overactive bladder syndrome were improved. Conclusion: The results indicate that Korean medical treatment is effective in patients with overactive bladder syndrome.

Cystopexy to Treat Urinary Incontinence Due to Urethral Sphincter Mechanism Incompetence in a Male Dog (수컷 개에서 발생한 요도 조임근 기능 부전에 의한 배뇨실금의 방광 고정술을 이용한 외과적 치료 증례)

  • A castrated male Yorkshire Terrier dog was presented for urinary incontinence and constipation. On physical examination, the dog showed difficult urination. There were no neurological abnormalities and no bacterial detection on urinalysis. Rectal examination revealed a regular, normal-sized prostate. Urethral catheterization was performed easily. Excretory urography and retrograde positive contrast urethrocystography showed displacement of the urinary bladder to the intrapelvic region. There was no evidence of an ectopic ureter. A tentative diagnosis of urethral sphincter mechanism incompetence accompanied with a pelvic bladder was made. Cystopexy was decided to place the urinary bladder to its normal position. The neck of the urinary bladder was anchored to the body wall and prepubic tendon using mattress sutures. Additional sutures were placed to appose the lateral part of the urinary bladder and abdominal wall. A simple interrupted suture was placed to tack the apex of the urinary bladder on the abdominal incision line. A urinary catheter was placed in the urinary bladder to provide post-operative evacuation. The catheter was removed when the dog was able to urinate with minimal straining at 3 days post-operatively. The owner reported that the dogs showed normal urination without straining at 3 days after the catheter was removed. Excretory urography revealed that the urinary bladder was located on its normal position at 2 months post-operatively. Subsequent communication with the owner by telephone revealed that the dog was in good urination at 3 years 11 months post-operatively.

Study on Therapeutic Effect of KMST (Korean Medicine Steam Therapy) -Included Korean Medicine Combination Therapy about Leukorrhea Patients (熱氣熏法 및 침, 뜸, 한약 치료가 帶下를 주소로 내원한 환자의 증상변화에 미치는 영향에 대한 연구)

  • Objectives This study aimed to investigate therapeutic effect of Korean Medicine Steam Therapy (KMST) for leukorrhea patients. Methods From December 1st 2013 to Nov 31st 2014, 16 leukorrhea outpatients treated with KMST filled out questionnaire before and after treatments. The questionnaire was composed of 8 question items such as sleep, appetite & digestion, urination, defecation, emotion, cold hypersensitivity, menstrual pain, leukorrhea and patients were told to evaluate their symptoms as NRS score. We analyzed mean NRS score of each question item before and after KMST by using Wilcoxon's signed rank test of SPSS ver. 19. Results Mean age of participants was 40.8±13.0 years and mean value of treatment numbers per person was 3.8±1.8 times. 43.75% (n=7) of the patients had history of vaginitis caused by gardnella vaginalis, ureaplasma urealyticum or candida albicans. All patients were treated with acupuncture, moxibustion and Korean medicine. In addition, there was no side effect reported by participants. NRS score of quantity, odor of leukorrhea, perineal unpleasant sensation such as itching, dryness, burning sensation remarkably decreased (p<0.01, p<0.05, p<0.01 each). NRS score of symptoms including sleep, urination, cold hypersensitivity, menstrual pain also remarkably decreased (p<0.05). Conclusions Symptoms of leukorrhea patients including sleep, urination, cold hypersensitivity, menstrual pain, leukorrhea showed significant decrease after treated with concurrent KMST.

Development of a novel combination tablet containing silodosin and solifenacin succinate for the treatment of urination disorder (배뇨 장애 치료를 위한 실로도신과 솔리페나신 숙신산염 함유한 새로운 복합 정제 개발)

  • This study was undertaken to develop a new combination tablet containing silodosin and solifenacin succinate for treating urination disorders, for which a simultaneous analytical method of silodosin and solifenacin succinate was established. The aqueous solubility of silodosin and solifenacin succinate was determined to be higher than 1 mg/ml in various buffers, and dissolution of the silodosin and solifenacin succinate commercial products was accomplished within 30 minutes. The drug-excipients compatibility test was subsequently evaluated using differential scanning calorimetry. Excipients without compatibility were selected, and various combination formulations were prepared applying the wet granulation method. Of these, the formulation comprising silodosin, solifenacin succinate, lactose hydrate, MCC PH101, sodium lauryl sulfate (SLS), Povidone K30, crospovidone and magnesium stearate, having a weight ratio of 8/10/56/112/2/6/6/2, respectively, showed equivalence comparative to the dissolution achieved with the commercial products of silodosin (Thrupas tab) and solifenacin succinate (Vesicare tab). Thus, we propose that compared to the currently available commercial products, this novel combination tablet containing silodosin and solifenacin succinate is an effective alternative for the treatment of urination disorders.

  • A seven year old dog with a history of vomiting for one month, icterus for one week yellowish urination for a few weeks, weight loss (1.5 kg loss/wk), and swelling of submandibular, prescapular, axillary, inguinal and popliteal lymph nodes and then a 3 year old dog with mild vomiting history and multifocal mass(neck, shoulder, mammary glands) were diagnosed as lymphoma by radiography, ultrasonography and ultrasound-guided fine needle aspiration and died all thereafter without any chemotherapeutical treatment.
