골반저근육 운동 프로그램이 긴장성 요실금 여성의 요실금정도, 요실금량 및 최대 질수축압에 미치는 영향 (The Effect of Pelvic Muscle Exercise Program on Women with Stress Urinary Incontinence in the Degree and Amount of Urinary Incontinence and Maximum Vaginal Contraction Pressure)

  • The purpose of this study was to help the women with stress urinary incontinence lead more comfortable life, by letting them to do pelvic muscle exercise and to learn by direct experience the effect that urinary incontinence is controlled. The research design was a one-group pretest-posttest design. The subjects were 16 middle-aged women(over 38) with stress urinary incontinence. The study was conducted from August 1, to October 11, 1999. Women trained themselves for muscular strength and endurance, every the other day for each exercise for six weeks at home(that means each exercise for more than three days a week) in the pelvic muscle exercise program which was developed by Dougherty et al., and filled out exercise participation card every week. They visited laboratory once a week to get visual feedback, by means of Perineometer, of how the pelvic muscle exercise was going on. Stress Urinary Incontinence Scale that Lee, Young-Sook revised and supplemented Hendrickson's original scale was used for measuring the frequency and situation scores of urinary incontinence, "30 minute pad test" was carried out for measuring the amount of urinary incontinence, and Perineometer was used for measuring maximum vaginal contraction pressure. Percentage, mean, standard deviation and Wilcoxon signed ranks test were used for data analysis by means of SPSS/PC+ WIN 9.0 program. The results were summarized as follows : The hypothesis 1 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the weekly degree of urinary incontinence would reduce compared to before the exercise was supported. The hypothesis 1-1 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the weekly frequency of urinary incontinence would reduce compared to before the exercise was supported (p=.003). The hypothesis 1-2 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the situation scores of urinary incontinence would reduce compared to before the exercise was supported(p=.044). The hypothesis 2 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the amount of urinary incontinence would reduce compared to before the exercise was supported(p=.001). The hypothesis 3 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the maximum vaginal contraction pressure would increase compared to before the exercise was supported (p=.012). These results suggest that pelvic muscle exercise program has an effect on women with stress urinary incontinence in the degree and amount of urinary incontinence and maximum vaginal contraction pressure. So it is judged that training women with stress urinary incontinence for pelvic muscle exercise is an effective nursing intervention strategy in order to care urinary incontinence.

노인여성의 요실금 실태 (A Study on Urinary Incontinence of Elderly Women in a Community)

  • The purposes of this descriptive study were to identify the prevalence rate of urinary incontinence(UI) and the differences in frequency of incontinent and normal women by general characteristics, obstetrical history, and the conditional events for urinary incontinence of the elderly women in a community. By the results of this study, it is intended to provide nursing practice guidelines for incontinent women. The research design of this study was a preliminary descriptive study. The 173 subjects were 55 years old and over, and resided in a small city area. Data were collected from June 20 to July 20, 2001, by an interview or a self-report with questionaire. The questionaire was composed of items of general characteristics, obstetrical characteristics, and conditions of UI by the modified Henderickon's Stress Incontinence Scale(1981). The results were summariezed as follows: 1. The UI prevalence rate of the sample was 64.2%. Of the incontinent women, 31.5% had experienced UI for a period of three to five years, and 84.7% had never treated or managed their UI. Frequency of UI was once or twice times per month(46.8%). 2. The total mean of UI on the scale in the incontinent women was 25.50 of 85, ranging from 18 to 41. 3. The most frequent condition of UI was coughing, followed by laughing, sneezing, heavy exercise, and preparation of urination in descending order. 4. There were significant differences in age, education, social activity, and urinary difficulty between the incontinent women and the normal women. 5. There were significant differences in frequency of spontaneous abortion, age of menopause between the incontinent women and the normal women. 6. There were no significant differences in number of delivery, frequency of artificial abortion, age of the last delivery, and postal health management between the incontinent women and the normal women. In conclusion, the incidence of UI in this study was high, but there were no effective treatments or management. It is suggested to provide the adult women with knowledge about UI, and to educate preventive behavior and control skill of urinary incontinence. Also episodes of urinary incontinence were high in the situation of sudden increase of abdominal pressure. This data can be used for the prevention strategy of urinary incontinence, In future research it is recommended to identify comprehensive factors related to urinary incontinence including psychosocial factors, and effective strategies of urinary incontinence.

성인여성의 요실금에 관한 기초 연구 (A Study on urinary Incontinence of Adult Women : Preliminary Study)

  • The purpose of this study was to identify the incidence of urinary incontinence in adult women and to identify factors related to life style and sexual intercourse that were related to incontinence. The sample consisted of 1,065 women living in Seoul or one of five provinces. Data for this study were collected from January 16 to June 23 using structured questionnaires. The Urinary Symptom Questionnaire developed by Jackson and a demographic questionnaire were used to collect the data. The data were analyzed using frequency, percentage with an SPSS/PC$^{+}$ program. The major finding are as follows : 1. The distribution of age of subjects was as follows : 20-29(10.1%), 20-39(17.8%), 40-49(27.3%). 50-59(22.3%), 60-69(12.9%), 70-79(6.1%). 80-89(2.8%), more than 90(0.6%). The frequency of normal delivery, 0(155%), 1-2(36.0%), 3-4(29.2%), 5-6(13.0%). more than 7(6.3%). The rate of subjects with menopause was 40.8%. 2. It was reported that 50.7% of the subjects experienced urinary incontinence with stress, mixed. and urgency incontinence being 49.8%, 43.4%, 6.8% respectively. 3. The lower urinary symptoms and incidence with urinary incontinence were as follows : Daily frequency. 22.3%, nocturia. 40.8%, urgency, 71.2%, bladder pain 47.8%, unexplainned incontinence, 32.4%, noctural incontinence, 16.1%, and frequency of incontinence, 37.7%. In term of quantity of incontinence, drop/pants damp, 29.5%, dribble/pants wet, 20.5%, flood or soaking through to outer clothing, 1.7%, and flood or running down legs or onto floor, 0.2%. 4. The symptoms related to sexual intercourse and incidence of urinary incontinence were as follows : dry vagina, 39.1%, sex life trouble, 10.8%, pain during sexual intercourse, 27.4%, and urine leakage during sexual intercourse. 8.8%. 5. Life style problems related to urinary incontinence were as follows : fluid intake restriction, 20.0%, affected daily task, 24.5%. avoidance of placess and situations, 35.0%, interference in physical activities, 30.6%, interference in relationships with other people. 19.0%. interference in relationships with husband/companion, 8.1%, and time after attack of urinary symptoms, 76.9%. In term of the feeling about the rest of their lifves the women reported : perfectly happy, 11.3%. pleased, 16.9%. mostly satisfied, 20.2%, mixed feelings, 21.0%, mostly dissatisfied, 21.0%, very unhappy 8.5%, and desperate. 1.0%. In conclusion. this study was a preliminary study to provide nursing practices guidelines for incontinence in adult women. Nurse working with adult women should develop and provide adequate care for these women.n.

치과위생사들의 요중 수은함량과 관련요인 분석 (Determinants of Urinary Mercury Concentration among Dental Hygienists)

  • Objectives: The aim of this study was to evaluate the level of urinary mercury and analyze which factors would affect urinary mercury concentration among dental hygienists in dental clinics. Methods: This study conducted by questionnaire and detection of urinary mercury concentration of 268 dental hygienists working in dental clinics from July to August of 2009. Data collected from two hundred and thirty-five dental hygienists were analyzed by the geometric mean (GM). Analytical results of urine samples with less than 0.3 g creatinine/L and greater than 3g creatinine/L were excluded from statistical analysis. Results: Urinary mercury concentration of 235 dental hygienists showed the geometric distribution. The arithmetic and geometric means of urinary mercury concentration were $0.996{\mu}g/g$ creatinine and $0.755{\mu}g/g$ creatinine, respectively. From multiple regression analysis, the number of amalgam filling, the consumption frequency of raw fish and the number of amalgam handling in current workplace was revealed as increasing factors of urinary mercury concentration. Conclusions: The level of urinary mercury in dental hygienists was higher than in general Korean population. The number of amalgam filling, the consumption frequency of raw fish and the number of amalgam handling in current workplace was revealed as increasing factors of urinary mercury concentration. Therefore using resin materials instead of amalgam in dental clinics is highly desirable.

요실금 관리 자기효능증진프로그램이 요실금 정도와 자기효능감 및 삶의 질에 미치는 효과 (Effects of a Self-Efficacy Promotion Program for Management of Urinary Incontinence on the Degrees of Incontinence, Self-Efficacy and Quality of Life)

  • Purpose: This study was conducted to develop a self-efficacy promotion program and to identify the effects of the program on degree of urinary incontinence, self-efficacy and quality of life in women with urinary incontinence. Method: The study used a pre-experimental design. Twenty participants were recruited from a nursing home located in Gwangju City and included residents, employees, and volunteer workers with urinary incontinence. Participants were surveyed from September 21 to November 2, 2005. The program treatment was provided to participants once a week for 6 weeks. Results: Frequency, amount, and situation scores for urinary incontinence, maximum and mean vaginal constriction pressure, vaginal constriction time, and nocturnal frequency were significantly improved after participating in the program (p<0.05). However, there was no significant pre- and posttest difference in frequency of daily urination (p=0.097). The degree of self-efficacy and quality of life for participants in the self-efficacy promotion program at posttest were significantly higher compared to the pretest scores (p<0.05). Conclusions: The results of the study indicate that a self-efficacy promotion program can be implemented for women with urinary incontinence in community health centers or nursing homes in order to reduce their incontinence symptoms, and increase self-efficacy and quality of life.

노년기 요실금 여성의 수면의 질, 일상생활 수행능력과 우울 (The Relationship of Quality of Sleep, Depression, Late-life Function and Disability (LLFDI) in Community-Dwelling Older Women with Urinary Incontinence)

  • Purpose: The purpose of this study was to investigate the relationships among quality of sleep, depression, late-life function and disability in community-dwelling older women with urinary incontinence. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Data were collected by questionnaires, which were constructed to include lower urinary tract symptoms, quality of sleep, depression, and late-life function and disability in 128 community-dwelling older women. Results: The major findings of this study were as follow: 1) 56.3% of participants belonged to urinary incontinence group. 2) There were significant relationships between depression and sleep latency, sleep duration, daytime dysfunction, quality of sleep, function component, frequency dimension, and limitation dimension. 3) Depression was significantly associated with frequency dimension, limitation dimension in capability which explained 44% of variance in depression. Conclusion: These results may contribute to a better understanding of sleep quality, depression, late-life function and disability in the community-dwelling older women with urinary incontinence. Therefore, health programs for prompting older women's health should be planned based on results of the study.

  • Purpose: The purpose of this study was to identify the effects of pelvic floor muscle exercise on reducing the symptoms of stress urinary incontinence and improving attitude toward exercise and quality of life. Method: The research adopted was a nonequivalent control group pretest posttest design. The subjects were 55 persons who were surveyed using a structured questionnaire, and 23 persons in the experimental group among the total sample were measured for the peak pressure and the duration of PMC and trained correct pelvic floor muscle contraction using peritron in the first week. Then, pelvic floor muscle exercise was implemented for 6 weeks. The data was analyzed by $\chi^2$-test and t-test with the SPSS 10.0 program. Results: 1) The degree of stress urinary incontinence, frequency of urination, nocturia, urgency, noctural incontinence, the frequency and quantity of incontinence, outer clothing change and incomplete emptying decreased significantly more in the experimental group than in the control group. 2) Attitude toward pelvic floor muscle exercise and the qualify of life were improved significantly more in the experimental group than in the control group. 3) The peak pressure and duration of PMC increased significantly more in the posttest. Conclusion: Based on the results above, it is judged that pelvic floor muscle exercise is an effective nursing intervention in order to care for stress urinary incontinence.

중추성 요붕증 1례 (Central Diabetes Insipidus. A Case Report)

  • Central diabetes insipidus(CDI) results from deficient vasopressin(antidiuretic hormone) secretion and causes polydipsia and polyuria. Its etiologic diagnosis is confirmed with an increase of urine osmolality by administering desmopressin(DDAVP) after water restriction. Because cm is caused by deficiency of vasopressin, up to now, desmopressin, a synthetic analog of vasopressin, has been the drug of choice in the treatment of CDI. However, under such treatment, CDI patients suffer from the continual administration of DDAVP throughout one's life and high cost of the treatment We administrated oriental herb medicine on a cm patient in a state of discontinuance of DDAVP. Prior to the study, brain sella MRI was scanned to exclude germinoma. In addition, urine analysis, serum and urinary osmolality, daily urinary volume, serum electrolyte levels were measured. Chungsimyunjatang was administered for 15 days, and urine analysis, urine osmolality, daily urinary volume, serum Na were measured several times again during the therapy, As a result, urinary frequency increased, serum Na slightly elevated, but specific gravity of urine, urinary osmolality severely decreased and daily urinary volume substantially increased. However, the frequency of DDAVP treatment was reduced from four times per day to once or twice a day with the continual administration of the Chungsimyunja-tang for two months after the discharge.

여성 요실금 환자의 요실금 중증도 및 삶의 질에 대한 영향요인 분석 (Factors Affecting the Severity of Urinary Incontinence and the Quality of Life of Women with Urinary Incontinence)

  • Purpose: This study was conducted to investigate the factors that affect the severity of urinary incontinence and the quality of life of women with urinary incontinence. Methods: An exploratory correlation research design was applied and the sixty women who responded that they currently experience involuntary urine leakage at least once per week were invited to participate in the study. Results: According to the study results, the factors that significantly contribute to the severity of urinary incontinence were age, number of parity, and number of normal deliveries. Other influencing factors, such as obesity, menopausal state, hysterectomy, and number of episiotomies, were not significant to predict the severity of urinary incontinence. Urinary incontinence frequency per week and duration of experiencing urinary incontinence were shown to be factors that significantly affect quality of life. Conclusion: In women above 60 years old, multiparity, and multiple experiences of vaginal delivery tended to present a higher level of severity of urinary incontinence. In addition, women with higher frequencies of urinary incontinence per week and a longer duration of urinary incontinence showed a lower level of quality of life.

복합성 요실금과 복압성 요실금의 특성: 하부요로증상과 요역동학 검사결과의 관계 (Characteristics of Mixed Urinary Incontinence and Stress Urinary Incontinence: Relationship between Lower Urinary Tract Symptoms and Urodynamic Parameters)

  • Purpose: The purpose of this study was to analyze the relationship between lower urinary tract symptoms and urodynamic parameters to investigate the characteristics of mixed urinary incontinence (MUI) and stress urinary incontinence (SUI). Methods: The subjects were 318 women with MUI and 128 women with SUI. Data were collected retrospectively from electronic medical records including Bristol Female Lower Urinary Tract Symptoms-Scored Form (BFLUTS-SF), Incontinence Quality of Life Instrument (I-QOL), voiding diaries, and urodynamic parameters. Results: Compared with the SUI group, the MUI group was older and showed lower I-QOL and more severe urinary tract symptoms. The MUI group had more urinary frequency, more nocturia, and a higher urgency score than the SUI group. In the correlation analysis, the greatest difference between the two groups was that urgency was associated with Qmax, maximal cystometric capacity, and detrusor overactivity only in the MUI group (r = -.175, p= .004; r = -.281, p< .001; r= .232, p< .001, respectively). Conclusion: As a result of this study, we propose that a customized management program that emphasizes the control of urgency for the MUI group, and one that effectively strengthens the weak pelvic floor for the SUI group.
