Markers of Bone Metastases in Breast and Lung Cancers

  • Aim and Background: The aim of the present study was to evaluate correlations between serum osteocalcin, osteoprotegerin and NTX (Cross-linked N-telopeptides of Type I Collagen) and urinary NTX in breast and lung cancer patients with bone metastases. These four markers are considered to have important roles in bone formation, resorption and metastases. Methods: Four markers were determined in the sera of 60 breast cancer and 21 lung cancer patients and healthy controls (n=30). Serum levels were studied using ELISA and EIA. Results: The median levels of serum osteoprotegerin (p<0.001) and osteocalcin (p=0.003) were higher in patients. Significant correlations were observed between the serum NTX-osteocalcin (r=0.431; p<0.001), serum NTX-osteoprotegerin (r=0.42; p=0.003) and serum NTX - urine NTX (r=0.255; p=0.022). Conclusion: We conclude that osteocalcin, osteoprotegerin and NTX are independent diagnostic tools. Due to the ease of urine collection, urine NTX may be applied routinely to allow early detection of bone metastases and indicate progression of the disease.

한국 여대생의 2년간의 골밀도 변화와 이에 영향을 미치는 요인 분석 (Effect of Dietary, Biochemical and Other Factors on Bone Mineral Density Change for 2 Years in Korean College Women)

  • This study was conducted to examine whether bone mineral density changes in 55 young Korean college women aged 19 to 26 years over 2 years and nutritional and biochemical factors are related. Bone mineral density (BMD) was measured in the spine (LS), femoral neck (FN), ward's triangle (WT), and femoral trochanter (FT) by dual energy X-ray absorptiometry three times at one-year intervals. Serum osteocalcin (OC), parathyroid hormone (PTH), and urinary cross-linked N-teleopeptides of type collagen (NTx) were measured. Dietary intake was assessed 8 times with 24-hour recall method. Physical activity (PA) was obtained by questionnaire and body fat content was measured by bioelectrical impedance analysis at baseline and after 2 years. Analyses were performed on 34 subjects with all three BMD measurements. The BMDs at the lumbar spine gradually increased over 2 years, while the BMDs of three sites at the femur were sustained or increased. The mean OC, PTH had a similar pattern with the change of BMD at the femur. The mean NTx decreased over 2 years but was still higher than those in other studies. BMI, body fat, vitamin A and zinc intake had a significant correlation with LS-BMD. Femur, PTH, body fat, vitamin A, vitamin B$_2$and calcium intake had a significant correlation with WT-BMD and was mostly influenced by diet. By multiple regression analysis, it was shown that the significant factors affecting the LS-BMD were BMI and vitamin A intake and those affecting FN and WT were age, BMI, PTH and calcium intake. These results indicate that some Korean women still experience increases in BMD and that this was associated with PTH and vitamin A and calcium intake. Therefore, proper diet and diet management is needed to increase changes in BMD among college women.

The effects of Cudrania tricuspidata extract on bone metabolism in ovariectomized rats

  • The effects of Cudrania tricuspidata (CT) extract on markers of osteoporosis were examined in ovariectomized rats. We classified 26 rats into five groups and provided a pellet chow diet and tap water throughout the 27-wk experimental period. During the last 15 wk, we added oral injections to each group as follows: sham-operated (SHAM, n=4) and ovariectomized-control (OVX, n=5) with distilled water, alendronate with 10 mg/kg/d of alendronate sodium (ALEN, n=5), CT (CT100, n=6) with 100 mg/kg/d of CT, and CT (CT300, n=6) with 300 mg/kg/d of CT. After the experimental period, blood, urine, and micro-CT images were assessed. The CT100 and OVX groups did not show any significant differences in urinary n-terminal telopeptide (NTx) (p<0.05 ), but with increases in CT concentration, the NTx level was slightly reduced. Serum osteocalcin was significantly higher in the CT groups than in all other groups (p<0.05 ). Notably, the serum calcium levels of all groups were within the normal range, but urinary calcium levels in the CT groups were significantly lower than the OVX group (p<0.05 ). In addition, the CT groups exhibited higher trabecular BMD than the OVX groups while showing similar BMD to the ALEN group (p<0.05 ). The Tb.Th of the ALEN group was lower than all other groups. Based on the overall analysis of results, CT prevented bone loss by inhibiting bone resorption and enhancing bone formation. Although alendronate showed a similar effect in preventing bone loss, it did so by solely inhibiting bone resorption, and its long-term use reportedly causes paradoxical effects such as hip fractures. Thus, for osteoporosis induced by ovariectomy, we conclude that CT extract is an effective natural treatment without severe side effects.

Chronic Alcohol Consumption Induced Tibial Bone Loss and Resulted in Osteopenia in Growing Young Male Rats

  • To determine the deleterious effects of chronic alcohol consumption on bone especially in adolescents or young adults, 8 week-old Sprague Dawley male rats were fed with Lieber-Decarli ethanol liquid diet, containing $36\%$ of energy as ethanol, ad libitum (ethanol group) or isocaloric normal liquid diet (control group) for 7 weeks. Body weight was significantly lower in ethanol group than that in control group after 1 week of feeding to the end. liver weight and the ratio of liver or kidney weight to body weight in ethanol group were significantly increased when compared to those in control group. Ethanol group showed significantly lower serum protein and albumin levels (p<0.05), higher total cholesterol and HDL-cholesterol levels (p<0.05), and AST, ALT and BUN activities than control group, but serum triglyceride, Ca and phosphate levels were not different. Ethanol group had significantly lower tibial trabecular bone area and serum osteocalcin level than control group (p<0.05), but urinary Ca and NTx (cross-linked N-telopeptide of type I collagen) concentrations and serum testosterone and parathyroid hormone levels were not different. In conclusion, chronic alcohol consumption in growing young male rats may result in osteopenia through the reduction of bone formation as well as liver malfunction.

한국인에 있어서 알렌드로네이트 제제의 제형간 유효성 및 순응도 비교 (A Comparison of Effectiveness and Compliance among Alendronate Pharmaceutical Products in Koreans)

  • Alendronate is a bisphosphonate that selectively inhibits osteoclast-mediated bone resorption. Dosing convenience is an important element for the enhancement of patient compliance and the effective management of osteoporosis. The purpose of this study was to compare the effectiveness and compliance among alendronate pharmaceutical products (oral once-weekly alendronate 70 mg, daily alendronate 10 mg, and once-weekly alendronate 70 mg with Vitamin $D_3$ 2800 IU) in terms of the change in bone mineral density (BMD), biochemical markers, and compliance estimates. A retrospective chart review was conducted in patients with osteoporosis who received alendronate 70 mg (Group 1), alendronate 10 mg (Group 2), or alendronate 70 mg with Vitamin D3 2800 IU (Group 3) at the endocrinology department of a hospital in Korea from Jan. 1, 1998 to Mar. 31, 2008. The primary endpoints were the increases in spine antero-posterior BMD T-score and femur trochanter BMD T-score, and the compliance of alendronate products. Secondary endpoints included changes in bone turnover-related biochemical markers including bone-specific alkaline phosphatase, urinary N-terminal telopeptides (NTX) and osteocalcin, and in serum vitamin $D_3$ concentration. There was no statistical difference in the BMD increase among the three alendronate products; spine BMD T-score increased by $0.49{\pm}0.52$, $0.39{\pm}0.48$ and $0.50{\pm}0.41$, and femur trochanter BMD T-score by $0.29{\pm}0.42$, $0.21{\pm}0.53$ and $0.24{\pm}0.22$ in Group 1, 2 and 3, respectively. With respect to the increases in femur trochanter BMD T-score and the decreases in NTX and osteocalcin, 70 mg once-weekly group was remarkably superior to 10 mg daily group (p < 0.05) The compliance of 70 mg once-weekly group was significantly higher than that of 10 mg daily treatment group (p < 0.001). In conclusion, all three alendronate treatment groups were equivalent in effectiveness, and the compliance of 70 mg once-weekly group was better than that of 10 mg daily treatment group.
