Comparison of the Muscle Activities of Upper Trapezius and Middle Deltoid between Subjects with and without Elevation of Shoulder Girdle during Arm Elevation

  • Purpose: Muscle imbalance between upper trapezius (UT) and serratus anterior (SA) during arm elevation is a factor causing shoulder dysfunction. However, there is no study to compare the muscle activities of the UT and middle deltoid (MD). The purpose of this study was to compare the muscle activities of the UT and MD between with and without elevation of shoulder girdle (ESG) during shoulder abduction. Methods: The subjects without (control group=9) or with (ESG group=8) participated in this study. The muscle activities of the UT and MD were measured using a electromyography during $90^{\circ}$ shoulder abducted position in both group. The data in middle of 3-second of the 5-second periods were used. The mean value of three trials was used in the data analysis. For each muscle, independent t-tests were performed to compare for group differences. Results: The muscle activity of UT was significantly greater in ESG group, compared to that of the control group (p<0.05). The muscle activity of MD was significantly smaller in ESG group, compared to that of the control group (p<0.05). Conclusion: These findings showed that low muscle activation of MD as well as SA may contribute to hyperactivity of UT during arm elevation.

Effects of Neck Stabilizing Exercise on Muscle Characteristics, Muscle Activity and Posture in Patients with Cervicogenic Headache (목 안정화 운동이 경추성두통 환자의 근육특성과 근활성도 및 자세에 미치는 영향)

  • Purpose : The purpose of this study was to provide an effective method of exercise therapy for patients with cervicogenic headache. Methods : The subjects were divided into the following two groups according to the intervention received: cervical stretch exercise (n=15, control group) cervix-stabilizing exercise (n=15, experimental group) tone (Hz) and stiffness (N/m) of the suboccipital and upper trapezius muscles were measured. T1 slope angle and neck tilt angle were measured. After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Results : After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Conclusion : The neck-stabilizing exercise were shown to be effective in decreasing the tone of the cervical muscles by stabilizing the cervical bone and improving muscle activity, and in improving the posture by decreasing muscle tone and stiffness.

Selective Activation of Serratus Anterior Using Electromyography Biofeedback During Push-Up Plus (푸시업플러스 시 근전도 바이오피드백을 이용한 전거근의 선택적 강화)

  • Push-up plus has been advocated for increasing the activity of the serratus anterior muscle, the most critical scapular stabilizer. However, no previous study has reported the possibility of compensatory motion on the part of the pectoralis major, which could substitute for the action of the serratus anterior during push-up plus. The aim of the current study was to investigate the immediate effect of electromyography (EMG) biofeedback of the pectoralis major muscle on the pectoralis major, upper trapezius, and serratus anterior muscles during push-up plus. Fourteen healthy young subjects voluntarily participated in this study; each subject performed push-up plus from the quadruped position, in two conditions (i.e., with or without visual and auditory biofeedback). Surface EMG was used to measure pectoralis major, serratus anterior, and upper trapezius activity. A paired t-test was used to determine any statistically significant difference between the two conditions. Additionally, effect size was calculated to quantify the magnitude of EMG biofeedback in each muscle. Visual and auditory feedback reduced pectoralis major muscle activity significantly (p=.000) and increased the serratus anterior muscle activity significantly (p=.002), but did not induce a significant difference in upper trapezius muscle activity (p=.881). Thus, it is concluded that the visual and auditory feedback of pectoralis major muscle activity can be used to facilitate serratus anterior muscle activity during push-up plus.

The Analysis of EMG According to Surface Type and Elastic Band Usage During the Pilates Superman Movement (필라테스 슈퍼맨 동작 시 탄성밴드 사용유무와 지면의 종류에 따른 근활성도 분석)

  • Objective : The purpose of this study was to analyze EMG according to surface type and elastic band usage during the pilates Superman movement. Method : The subjects were 10 female university students with a mean age of 27. The independent variables were surface type (yoga mat, air filled equipment, elastic mat) and elastic band usage. In order to measure muscle activity, the Noraxon(USA) was used. Eight muscles (upper trapezius, lower trapezius, thoracic spine, lumbar spine, gluteus maximus, gluteus medius, biceps femoris, semitendinosus) activation were analyzed. For the statistical analysis, MANOVA, independent t-test and Scheffe test for the post-hoc via SPSS 20.0 was used. Results : The left and right upper trapezius muscle activities were significantly reduced when using the elastic bands. In addition gluteus medius muscle activities significantly increased with the elastic band as well. Conclusion : According to the usage of the elastic band and the different types of surfaces, different muscle groups were recruited. Elastic bands were found to have more activation on the gluteus medius muscle meanwhile there was less activation on the upper trapezius during the pilates Superman movement.

The Effectiveness of Upper Limb Offload Dynamic Taping Technique on Scapular Muscles Activation During Elevation in Healthy Subjects (상지 부하감소 다이나믹 테이핑 기법이 정상인의 어깨 올림 시 어깨뼈 주위근의 근활성도에 미치는 영향)

  • Background: For performing various movements well, cooperation between the muscles around the scapula and shoulder has been emphasized. Taping has been widely used clinically as a helpful adjunct to other physiotherapy methods for shoulder pathology and dysfunction treatment. Previous studies have evaluated the effect of taping techniques using dynamic tapes on shoulder function and pain. However, no study investigated the electromyographic (EMG) changes in the shoulder muscles. Objects: This study aimed to investigate the effect of the upper limb offload taping technique using a dynamic tape on EMG activities of the upper trapezius (UT), lower trapezius, serratus anterior (SA), and middle deltoid (MD) muscles during scaption plane elevation. Methods: A total of 26 healthy subjects (19.85 ± 6.40 years, male = 20) volunteered to participate in this study. The subjects were instructed to perform scaption elevation with and without dynamic taping on the shoulder. Shoulder elevation strength tests were performed at 100%, 75%, 50%, and 25%, for the maximal isometric contraction force. Results: There were statistically significant interaction effects between the taping application and shoulder scaption elevation force in EMG activities in the UT (p < 0.05) and MD (p < 0.05). EMG activities in the UT showed significant increases in 50%RVC (reference voluntary contraction, p < 0.05) and 25%RVC (p < 0.01). Furthermore, the EMG activity of the SA significantly increased in 50%RVC (p < 0.01) and 25%RVC (p < 0.01) after dynamic taping. For the MD, the EMG activity level significantly decreased in 100%RVC (p < 0.05). Conclusion: These results indicated that upper limb offload dynamic taping application affects the muscle activities of some shoulder muscles depending on different scaption elevation strength levels. Therefore, we suggest that the upper limb offload dynamic taping can be applied to the shoulders when patients need middle deltoid inhibition or upper trapezius facilitation, such as patients with shoulder impingement syndrome.

Effects of the Additional Scapular Posterior Tilt Movement on Selective Muscle Activation of the Lower Trapezius during Prone Shoulder Extension

  • Background: Although the scapular posterior tilt movement could facilitate the lower trapezius (LT) muscle activity, no study identified the effects of the scapular posterior tilt movement on the selective activation of the LT muscle during prone shoulder extension. Objectives: To examine the influences of additional scapular posterior tilt on electromyography (EMG) of the upper trapezius (UT) and the LT muscles during prone shoulder extension. Design: Cross-sectional study. Methods: There were 15 asymptomatic male participants in this study who performed prone shoulder extension with and without scapular posterior tilt movements. For the scapular posterior tilt movements, participants performed visual biofeedback training for scapular movement using motion sensor. During the exercises, the EMG activity of the UT and LT was recorded using surface EMG system. Results: The EMG activity of the LT significantly increased during prone shoulder extension with scapular posterior tilt compared to that of general prone shoulder extension, whereas that of the UT was not significantly different between the two exercises. Moreover, scapular posterior tilt application significantly decreased UT/LT muscle activity ratio. Conclusion: Scapular posterior tilt movement may be emphasized during exercise when facilitating LT muscle activation.

  • The purpose of this study was to investigate the effect of wearable sensor wear on the muscle contraction of cervical erector spinae and upper trapezius causing the forward head posture induction in order to reduce the stress induced by the use of smartphone. This study was to investigate the muscle activity of healthy adults in the 20th to 30th generations by dividing them into the control group using the smartphone, the non-wearing group conscious the posture of the head posture, and the wearing group wearing the wearable sensor. There were no differences in muscle activity between cervical erector spinae and upper trapezius compared to the control, non - wearing, and wearing groups. In addition, the changes in muscle activity of cervical erector spinae muscles were increased in all groups, but the muscle activity of upper trapezius muscles were in the wear group compared to the non-wear group and the control group, but there was no statistical significance. That is, wear of the wearable sensor may be effective in controlling the conscious posture, but it may cause the compensation of another part.

Comparison of Muscle Activities Serratus Anterior and Upper Trapezius Muscle During Scapular Protraction in Quadruped Position at Legs Difference (네발기기 자세에서 어깨뼈 내밈 운동시 다리들기에 따른 앞톱니근과 위등세모근의 근활성도 비교)

  • Background: This study was conducted to investigate the effect of leg lift difference on the serratus anterior muscle and the upper trapezius muscle when a subject with winged scapula performs a scapula protraction exercise in a four-leg crawling posture. Method: Twenty normal adults and 20 subjects with winged scapula participated in the experiment. Surface EMG recordings were collected from serratus anterior muscle and back trapezius muscle during scapula protraction exercises. Scapular winging is measured with the lifting distance of scapula retraction to the back using an electronic digital caliper. In two groups of four-leg crawling posture, the two legs support, the dominant leg lifting, and the non-dominant leg lifting, including the scapula protraction, were performed. To examine the difference between groups in the variance analysis, the Bonferroni correction was used (significance level ${\alpha}=.017$). Statistical significance level ${\alpha}$ was .05. Results: There was a significant difference in serratus anterior muscle and upper trapezius muscle during push-up plus exercise in leg lifting in four-leg crawling posture, but there were no significant differences in muscle activity between serratus anterior muscle and upper trapezius muscle, and there was no significant difference according to the presence or absence of scapular winging. Conclusion: For the shoulder stability of the ipsilateral side with the serratus anterior muscle, the leg-lifting posture is effective in the four-leg crawling, and also when a subject with winged scapula chooses an exercise, lifting the ipsilateral side of leg with scapula protraction exercises at the same time may have a positive effect on scapula dysfunction.

Analysis Characteristic the Using Surface EMG of Scaling Working of the Dental Hygienist with Upper Body Musculoskeletal Pain (상반신의 근육뼈대계 통증이 치과위생사의 스케일링 작업에 미치는 특성에 대한 표면 근전도 분석)

  • Purpose : The current research examines the muscle activity that happens during scaling practice subject to 20 dental hygienic students with musculoskeletal pain and then propose a basic data according to the working attitude of the Dental Hygienist. Method : The Nordic-style questionnaire is used to define experimental group with musculoskeletal pain and control group. During the scaling the surface EMG device is used to measure the muscle activity of experimental and control group. Study design : The surface EMG is measure RMS(root mean square) of suboccipital muscle, biceps brachii, upper trapezius, and brachioradialis muscle activity. Results : In the experimental group, the RMS of upper trapezius and brachioradialis is increased during scaling practice(p<0.05), but the control group's RMS is not changed(p>0.05). Conclusion : Musculoskeletal pain may contribute to increase muscle activity of neck & arm during scaling practice. In the future we think there is a need to raise the office efficiency by subjecting to dental hyginiest that are in the clinics and performing experiments.

The Effect of Postural Correction and Visual Feedback on Muscle Activity and Head Position Change During Overhead Arm Lift Test in Subjects with Forward Head Posture

  • Purpose: This study aimed to investigate the immediate effects of posture correction and real-time visual feedback using a video display on muscle activity and change of head position during overhead arm lift test in individuals with forward head posture. Methods: Fifteen subjects with forward head posture and fifteen normal subjects who volunteered were included in this study. During both groups performed the overhead arm lift test, the muscle activity of the upper trapezius, serratus anterior, sternocleidomastoid, and lower trapezius muscle were measured using electromyography, and head position change was measured using photographs. Then, forward head posture group was asked to perform overhead arm lift test again after posture correction and real-time visual feedback using a video display respectively. One-way analysis of variance (ANOVA) was used to analyze four conditions: pre-test, posture correction, real-time visual feedback, and the control group. Results: The upper trapezius and lower trapezius muscle activity significantly decreased posture correction, real-time visual feedback, and control group than pre-test of forward head posture group (p<0.05). The sternocleidomastoid muscle significantly decreased real-time visual feedback and control group than pre-test of forward head posture group. Head position change significantly decreased three conditions than pre-test of forward head posture group and real-time visual feedback and control group significantly decreased than posture correction. Conclusion: This study recommend for maintaining cervical stability during the overhead arm lift test, postural control using real-time visual feedback is more effective in subjects with forward head posture.
