The purpose of this study is to evaluate the effect of additional enamel etching with phosphoric acid on the microleakage of the adhesion of self-etching primer system. Class V cavity($4mm{\times}3mm{\times}1.5mm$) preparations with all margins in enamel were prepared on buccal surface of 42 extracted human upper central incisor teeth. Prepared teeth were randomly divided into 3 groups. Group 1:no additional pretreatment with 37% phosphoric acid (NE). Group 2:additional pretreatment with 37% phosphoric acid for 10 seconds (E10s). Group 3:additional pretreatment with 37% phosphoric acid for 20 seconds (E20s). The adhesives(Clearfil SE $Bond^{\circledR}$, Kuraray, Osaka, Japan) and composite resins(Clearfil $AP-X^{\circledR}$, Osaka, Kuraray, Japan) were applied following the manufacturer's instructions. All the specimens were finished with the polishing disc(3M dental product, St Paul, MN, USA), thermocycled for 500 cycles between $5^{\circ}C$ and $55^{\circ}C$ and resected apical 3-mm root. 0.028 stainless steel wire was inserted apically into the pulp chamber of each tooth and sealed into position with sticky wax. Surrounding tooth surface was covered with a nail varnish 2 times except areas 1mm far from all the margins. After drying for one day, soaked the samples in the distilled water. Microleakage was assessed by electrochemical method(System 6514, $Electrometer^{\circledR}$), Keithley, USA) in the distilled water. At intrusion of upper anterior teeth in patient with periodontal defect, the use of three-piece base arch appliance for pure intrusion is required. To investigate the change of the center of resistance and of the distal traction force according to alveolar bone height at intrusion of upper anterior teeth using this appliance, three-dimensional finite element models of upper six anterior teeth, periodontal ligament and alveolar bone were constructed. At intrusion of upper anterior teeth by three-piece base arch appliance, the following conclusions were drawn to the locations of the center of resistance according to the number of teeth, the change of distal traction force for pure intrusion and the correlation to the change of vertical, horizontal location of the center of resistance according to alveolar bone loss. 1. When the axial inclination and alveolar bone height were normal, the anteroposterior locations of center of resistance of upper anterior teeth according to the number of teeth contained were as follows : 1) In 2 anterior teeth group, the center of located in the mesial 1/3 area of lateral incisor bracket. 2) In 4 anterior teeth group. the center of resistance was located in the distal 2/3 of the distance between the bracket of lateral incisor and canine. 3) In 6 anterior teeth group, the center of resistance was located in the central area of first premolar bracket .4) As the number of teeth contained in anterior teeth group increased, the center of resistance shifted to the distal side. 2. When the alveolar bone height was normal, the anteroposterior position of the point of application of the intrusive force was the same position or a bit forward position of the center of resistance at application of distal traction force for pure intrusion. 3. When intrusion force and the point of application of the intrusive force were fixed, the changes of distal traction force for pure intrusion according to alveolar bon loss were as follows :1) Regardless of the alveolar bone loss, the distal traction force of 2, 4 anterior teeth groups were lower than that of 6 anterior teeth group. 2) As the alveolar bone loss increased, the distal traction forces of each teeth group were increased. 4. The correlations of the vertical, horizontal locations of the center of resistance according to maxillary anterior teeth groups and the alveolar bone height were as follows : 1) In 2 anterior teeth group, the horizontal position displacement to the vortical position displacement of the center of resistance according to the alveolar bone loss was the largest. As the number of teeth increased, the horizontal position displacement to the vertical position displacement of the center of resistance according to the alveolar bone loss showed a tendency to decrease. 2) As the alveolar bone loss increased, the horizontal position displacement to the vertical position displacement of the center of resistance regardless of the number of teeth was increased.
Purpose: The aim of this study was to propose the position of maxillary anterior teeth and intercanine width measurements based on the incisive papilla in accordance with the cephalic type and gender of dentate Korean adult with normal teeth alignment. Materials and methods: 42 students with Class I normal occlusion, without crowding or spacing, were selected from the Chonnam National University School of Dentistry. The lateral skull radiographs of the subjects were taken and were classified as different cephalic types, based on their PFH / AFH ratios. 42 casts of their maxilla were prepared and both the distance between the upper central incisors and the middle of the incisor papilla was measured with a vernier caliper (A) and the distance between the maxillary canine cusp tips (B) were measured. Statistical analysis was performed using SPSS version 15 and their significance was investigated. Results: For dolichocephalic group, the mean values for A and B were 8.43 (SD: 0.61) and 36.73 (SD: 2.17), respectively. The mean value for A was 8.51 (SD: 1.27) for the mesocephalic group and 8.76 (SD 1.03) for the brachycephalic group. The mean value for B was 35.91 (SD: 1.86) for the mesocephalic group and 37.34 (SD: 2.23) for the brachycephalic group. For the male group, the mean A value was 8.86 (SD: 1.04) and the mean B value was 37.60 (SD: 0.24). For the female group, the mean A value was 8.41 (SD: 0.93) and the mean B value was 36.18 (SD: 2.01). The difference between male and female group in A values were not statistically significant (P>.05). The B values of the male subjects were greater than those of the female subjects and was statistically significant (P<.05). Conclusion: 42 students with normal dentition and occlusion in korea, the distance from the incisive papilla and the incisal edge of maxillary central incisors had no difference in cephalic type or gender. However, the distance between the cusp tip of both canines had significant difference in gender where the male showed higher values than the female, while having no difference in cephalic types.

In children, change in lifestyles such as an increase in participation of physical activities has increased the chance of receiving an injury, harming the child and their parents. This study was carried out to give basic information of traumatic injuries to the upper anterior teeth which can be used in prevention of future complications that may arise. From June, 2002 to May, 2003, of the 8,799 children in their primary and mixed dentition visiting Department of Pediatric Dentistry, College of Dentistry, Yonsei University 955 upper anterior teeth(primary 543, permanent 412) of 487 children(male 316, female 171) with traumatic injures to the upper incisors were studied. The results were as follows: 1. Trauma to the primary teeth prevailed at the age of 1-3 and to the permanent teeth at the age of 7. The male to female ratio was 1.85:1. 2. Trauma to the upper incisors for both the primary and permanent teeth occurred in the afternoon. Primary teeth were injured most frequently in the home(44.1%) and the permanent teeth outdoors, in the street(17.5%). 3. Both the primary and permanent teeth were most injured by fall-down injuries(39.8%, 12.9%), and next were collisions(22.2%, 6.0%). For the primary teeth, high fall, traffic accident, violence and sports are next in order, and for the permanent teeth, violence, sorts, traffic accidents were next. 4. Average number of 1.6 primary teeth and 2.8 permanent teeth were injured with the right central incisor most prevailing. 5. Crown fracture without pulpal exposure and subluxation injures were highest in frequency in both the primary and permanent teeth.
In orthodontic field, it is very important to understand the normal growth. Such an understanding can be derived from observation of normal growth in various samples from childhood to adulthood, and this builds a foundation from which growth abnormality or variation can be defined. Thus, a broad data collection of normal children, as well as basic study reviewing such data become necessary. The relationship between the mean values of cephalometric measurements in Growth and Development was studied among the groups(boys and girls) of Korean chidren from the ages 6-years to 16-years. 220 boys 170 girls were chosen as subjects : Cephalometric X-ray were taken for 3 years and hard tissue analysis on McNamara and Ricketts Analysis which was divided into measurements of 5 parts(Cranial base, Cranial base and Maxilla & Mandible, Maxilla and Mandible, Mandible, Dental measurements). The relationship of craniofacial growth was studied. The following conclusions were obtained: 1. There were statistically significant differences in anterior cranial base between the two sexes of 14 and 15-year grouips. 2. In comparison of growth amounts among different age groups, statistically singnificant difference in Posterior facial height exhisted among $10\~11,\;12\~13\;and\;14\~15$ year-old interval groups. This pattern increased with aging. 3. Na perpendicular to A showed earlier growth peak in females(11-12years) than males(12-13years). When horizontal measurements of point A and Pogonion are compared, mandibular growth appeared to be greater. 4. Maximum growth peak of cranial base and mandible was earlier in females (11-12years) than males(13-14 years). 5. Upper central incisor flared out with aging, and there were increases in lower incisor to A-Pg, and lower incisor inclination There was significant difference between the two sexes in 10-year-old group.

Purpose: This study was designed to help to provide the criteria of the esthetic smile in oromaxillary region on twenties, fifties and sixties. Materials and methods: The facial straight photographs of 33 adults (male 15, female 18, mean age 27.2 years old) and 20 seniors (male 7, female 13, mean age 55.6 years old) in resting and smile position were taken. The measurements and proportion of lip-teeth relation during smile were statistically analyzed, and survey of freshmen and seniors who belong to School of Dentistry, Chonnam National University were taken to select the best esthetic smile among the photographs. Results: In the relationship between the upper lip and the teeth, average smile was the most frequent, and mean of smile line ratio was 1.090 in young group and 0.90 in old group. The correlation between the buccal corridor ratio and exposed teeth count was inversed. At smile, most frequently exposed tooth was the second premolar (63.64%) in young gourp and the first premolar (35.00%) in old group. The correlation between the upper lip change ratio and exposed clinical crown length of maxillary central incisor was inverse, but there was no correlation between the lower lip change ratio and exposed clinical crown length of maxillarycentral incisor. Conclusion: In the result of survey, the students selected the case exposed to the first premolar as the most esthetic smile. The most esthetic smile between full facial photograph and the lip only photograph by the freshmen was different, but that by seniors was identical.
Craniofacial asymmetry was analyzed in 39 patients with complete unilateral cleft lip and palate(UCLP). The samples are devided into three groups nine below 9 years, twenty three from 9 years 1 month to 14 years and seven over 14 years group. Seventeen measurements were obtained from the tracing of PA X-ray cephalometric headfilms to evaluate the asymmetric characteristics and changes accdording to aging in UCLP. The obtained results were as follows. 1. Facial asymmetry in UCLP is variable(1.22-3.47 $mm/^{\circ}$) and the length from midsagitta1 reference line to maxillary 1st molar, to upper central incisor and the length of mandibular ramus showed significant asymmetry 2. Nasal septum and anterior nasal spine were deviated In the cleft side and the lower border of nasal cavity was 1ower in cleft side. 3. The deviation of nasal septum was continued significantly till after 14 years old.
This study was performed to compare the size of soft palate, tongue and airway according to the types of the malocclusion and evaluate the correlation between the size of soft palate, tongue, airway and dentofacial skeleton respectively. The sample of this study was 98 malocclusion female patients between the ages 12 and 17 years. The lateral cephalometric radiographs were taken and the distance, angle, ratio and area of the dentofacial skeleton, soft palate, tongue and airway were measured and evaluated statistically. The results obtained were as follows: 1. There was significant difference in SNB, ANB, facial angle, facial convexity, A-B plane angel, Y axis to FH, SN-MP, Wits appraisal, ODI and APDI according to the types of malocclusion. 2. The hyoid bone was more posteriorly positioned in Class II malocclusion group than other two groups and superio-inferior position of the hyoid bone was not different according to the malocclusion types. 3. The nasopharyngeal area of Class II and Class III malocclusion group was smaller than that of Class I malocclusion group, and the pharyngeal area of Class II malocclusion group was smaller than that of Class I and Class III maocclusion group. There was no difference of the area of the soft palate, tongue, oropharynx and hypopharynx according to malocclusion types. 4. The ramal height and mandibular body length(Go-Me) showed positive correlation with the area of tongue, nasopharynx, oropharynx, and pharynx. SNA did not correlated with the area of tongue and airway but SNB showed positive correlation with the area of hypopharynx and pharynx. The anterior, posterior facial height, upper and lower central incisor position to facial plane showed positive correlation with tongue area.

The author analysed tomographic and cephalometric radiographs of 82 temporomandibular joints from 41 symptomatic patients and 40 temporomandibular joints from 20 asymptomatic young adults. The results were as follows; 1. The mean condylar angulation in control group and patient group was 21.72±6.48° 20.13±9.14° respectively and there was no significant difference between two groups. 2. The mean depth of cut was 6.63±0.38㎝ in control group and 6.57±0.46㎝ in patient group. 3. Mean height and width of condylar head were 6.66±1.83㎜, 12.42±0.49㎜ in control group and 6.22±1.36㎜, 11.93±l.92㎜ in patient group. 4. The mean height of articular fossa was 10.20±2.04㎜ in control group and 9.89±1.98㎜ in patient group. The mean width of articular fossa was 21.08±2.08㎜ in control group and 21.24±3.03㎜ in patient group. 5. In centric occlusion the superior joint space was largest (4.15±0.93㎜), followed by the posterior joint space (2.99±0.97㎜) and the anterior joint space (2.70±0.73㎜) in control group. The superior joint space (3.47±1.31㎜) and posterior joint space (3.47±7.07㎜) were same in patient group. There was significant difference in left superior joint spaces between two groups (p<0.05). 6. The condylar position in articular fossa was displaced anteroinferiorly (0.99±3.65㎜ anteriorly, 1.75±1.01㎜ inferiorly) in control group and posteroinferiorly (3.20±4.69㎜ posteriorly, 1.25±1.87㎜ inferiorly) in patient group with 1 inch opening. In maximum opening, it was displaced anteroinferiorly (6.09±3.55㎜ anteriorly, 1.38±2.47㎜ inferiorly) in control group and anteroinferiorly (1.70±5.96㎜ anteriorly, 1.37±1.85㎜ inferiorly) in patient group. There was significant difference in anteroposterior position of both condyles with 1 inch opening and maximum opening between two groups (p<0.01). 7. The mean inclination of upper central incisor and the posterior inclination of articular eminence in control group was 65.60±6.04° 58.88±9.18° in control group, and 67.14±8.41°, 59.70±9.08° in patient group respectively. There was no significant correlation between two groups.

Long-term survival and prognosis of narrow-diameter implants have been reported to be adequate to consider them a safe method for treating a deficient alveolar ridge. The objective of this study was to perform case report of narrow-diameter implants with a trapezoid-shape in anterior teeth alveolar bone. A 50-year-old male patient presented with discomfort due to mobility of all of the maxillary teeth and mandibular incisors. Due to destruction of alveolar bone, four anterior mandibular teeth were extracted. Soft tissue healing was allowed for approximately 3 months after the extraction, and a new design of implant placement was planned for the mandibular incisor area, followed by clinical and radiological evaluation. Implant placement was determined using an R2GATE surgical stent. The stability of the implants was assessed by ISQ measurements at the first and second implant surgery and after prosthetic placement. At 1 and 3 months and 1 year after implantation of the prosthesis, clinical and radiological examinations were performed. Another 50-year-old male patient presented with discomfort due to mobility of the mandibular central incisors. For the same reason as in the first patient, implant placement was carried out in the same way after extraction. ISQ measurements and clinical and radiological examinations were performed as in the previous case. In these two clinical cases, 12 months of follow-up revealed that the implant remained stable without inflammation or additional bone loss, and there was no discomfort to the patient. In conclusion, computer-guided implant surgery was used to place an implant in an optimal position considering the upper prosthesis. A new design of a narrow-diameter implant with a trapezoid-shape into anterior mandibular alveolar bone is a less invasive treatment method and is based on the contour of the deficient alveolar ridge. Through all of these procedures, we were able to reduce the number of traumas during surgery, reduce the operation time and total treatment period, and provide patients with more comfortable treatment.
