Effect of Unstale Surface Lumbar Stabilization Exercise on Trunk Posture and Balance Ability in Patients With Scoliosis (불안정한 지지면의 척추안정화 운동이 척추측만증 환자의 체간 자세와 균형에 미치는 영향)

  • Purpose : This study was somatosensory less in patients with idiopathic scoliosis somatosensory input to the lumbar stabilization exercises carried out to determine the most effective treatment method to be stable and unstable in terms of supporting the lumbar stabilization exercises the patient's torso length and postural sway by comparing the distance from a standing position and looked for differences in effect on the balance. Methods : The subjects of the study were 18 patients who showed the symptom of scoliosis. The study classified the patients into two experimental groups, one using an unstable surface and one a fixed surface, and the patients were required to do a lumbar stabilization exercise a total of 12 times for 60 minutes per session, three times a week for four weeks. The study carried out a paired comparison t-test so as to compare differences between measurement values in each experimental group before and after the exercise. Results : Superior iliac spine on the left, there was a significant reduction in the group doing the lumbar stabilization exercise on an unstable surface (p<0.05). Regarding change in sway distance to the left and right directions in the group doing the lumbar stabilization exercise on the unstable surface, there was a significant decrease in both the condition of closed eyes or open eyes (p<0.05). As for change in sway distance in forward-and-backward direction, there was a significant reduction in the condition of either closed eyes or open eyes (p<0.05). Conclusion : The lumbar stabilization exercise on an unstable surface improved the trunk posture of patients with scoliosis symmetrically, and the static balance ability in a standing posture was discovered to be improved. In the future, the lumbar stabilization exercise on an unstable surface may be used as a posture correction and balance increase exercise for patients with scoliosis.

Choice Stepping Reaction Time under Unstable Conditions in Healthy Young and Older Adults: A Reliability and Comparison Study

  • Purpose: We aimed to analyze the reliability of the test for choice stepping reaction time (CSRT) under an unstable surface and determine whether there were differences in CSRT between support surface conditions (stable vs. unstable conditions) and between age groups (young adults vs. community-dwelling older adults). Methods: Twenty healthy community-dwelling older adults and twenty young adults performed the stepping task under an unstable condition over two visits. The mean of the two trials measured for each visit was used for the analysis. The test-retest reliability was analyzed using intra-class correlation coefficient (ICC) with a 95% confidence interval, standard error of measurement (SEM), and minimal detectable change (MDC). Differences in CSRT between support surface conditions and age groups were analyzed using the independent t-test with Bonferroni correction. Results: Excellent consistency was observed for ICC >0.90 in both groups. Moreover, the SEM and MDC values of the CSRT in older and young adults were 0.03 and 0.09 and 0.01 and 0.04, respectively. There was a significant difference in the CSRT between the age groups under stable (p<0.001) and unstable conditions (p<0.001). Conclusion: The findings demonstrated that the test for CSRT under an unstable condition had reliable results in both groups. Although older adults demonstrated longer reaction times than younger adults in all surface conditions, increasing the balance control demand by implementing a choice stepping task concomitant with a balance task had no influence on the reaction time in both age groups.

  • A fuzzy learning method to control an unstable and multivariable system is presented in this paper, Because the multivariable system has generally a coupling effect between the inputs and outputs, it is difficult to find its modeling equation or parameters. If the system is unstable, initial condition rules are needed to make it stable because learning is nearly impossible. Therefore, this learning method uses the initial rules and introduces a cost function composed of the actual error and error-rate of each output without the modeling equation. To minimize the cost function, we experimentally got the Jacobian matrix in the operating point of the system. From the Jacobian matrix, we can find the direction of the convergence in the learning, and the optimal control rules are finally acquired when the fuzzy rules are updated by changing the portion of the errors and error rates.

Effect of Unstable Surface Exercise on Trunk Posture and Balance Ability in Patients With Scoliosis: After six months follow-up (불안정한 표면 운동이 척추측만증 환자의 체간자세와 균형에 미치는 영향)

  • Purpose: The aim of this study was to evaluate the effect of lumbar stabilization exercise on an unstable surface on trunk posture and static standing balance ability in patients with scoliosis. Methods: Subjects included 18 patients who showed symptom of scoliosis. Patients were divided into two experimental groups, one using an unstable surface and one using a fixed surface, and the patients were required to perform a lumbar stabilization exercise a total of 12 times for 60 minutes per session, three times per week for a period of four weeks, with a six-months follow-up period. Results: A significant reduction was observed in the group that performed the lumbar stabilization exercise on an unstable surface (p<0.05). A significant decrease in both the condition of closed eyes or open eyes in the left and right directions was observed in the group that performed the lumbar stabilization exercise on an unstable surface (p<0.05). After six months, results of comparison of the length of both sides of the trunk showed a significantl decrease in the group performing lumbar stabilization exercises on an unstable surface. Conclusion: Lumbar stabilization exercise on an unstable surface improved the trunk posture of patients with scoliosis symmetrically, and static balance ability in a standing posture showed improvement. In the future, lumbar stabilization exercise on an unstable surface may be used as an exercise for posture correction and balance increase for patients with scoliosis.

Evaluation of the Balance Ability for 20 to 29 Years Old on the Unstable Platform (불안정 발판(Unstable Platform)에서 20대 연령의 균형능력 평가)

  • The purpose of this study were to evaluate and compare the balance ability at different conditions in normal 20 to 29 years old on unstable platform, KAT 2000(Breg, Inc., Vista, CA. 1994). Static and dynamic BI(balance index) were measured 3psi and 5psi surface conditions. Static tests were done on right and left leg separately, then both legs together with the feet apart 20cm with the eyes opened and closed. Dynamic tests were done on both legs together with apart 20cm with the eyes opened. A dynamic test was performed in which the subject moved platform in a circular manner to chase a moving object on a computer screen. Seventy healthy students(average 21.6 years, male, female) were tested. In this study applied the paired t-test and correlation to determine the statistical significance of result. The results were as follow: 1) The mean static balance index of the Rt leg was $119.9{\pm}75.72$ on 3psi surface condition with the eyes opened, and that of the Lt leg was $224.3{\pm}121.16$. 2) The mean static balance index of the Rt leg was $93.1{\pm}24.16$ on 5psi surface condition with the eyes opened, and that of the Lt leg was $180.5{\pm}61.76$. 3) The mean static and dynamic balance index of both legs were $76.4{\pm}31.86$, $2187.6{\pm}696.99$ on 3psi with the eyes opened, and $68.3{\pm}14.82$, $1938.7{\pm}525.41$ on 5psi respectively. 4) The mean static balance index of the Rt leg was $517.8{\pm}220.87$ on 3psi surface condition with the eyes closed, and that of the Lt leg was $588.6{\pm}204.81$. 5) The mean static balance index of the Rt leg was $271.9{\pm}192.151$ on 5psi surface condition with the eyes closed, and that of the Lt leg was $363.4{\pm}98.97$. 6) The mean static balance index of both legs was $332.6{\pm}137.31$ on 3psi surface condition with the eyes closed, and that of the 5psi was $288.5{\pm}133.07$. 7) The balance index on 3psi surface condition was significantly higher than that of 5psi (p<0.05, p<0.01). 8) The balance index with the eyes closed was significantly higher than that of the eyes opened (p<0.05). 9) The balance index on the left leg was significantly higher than that of the right leg (p<0.05, p<0.01). 10) There was no correlation between static balance index and dynamic balance index. 11) Therewas no correlation between weight or height and balance index.

  • Because sampling interval for data collection tends to be short compared with the overall processing time, in chemical process, instrumental process related tanks or furnace collected data have a significant autocorrelation. Insufficient control technique and frequent control actions cause unstable condition of the process. Traditional control charts which were developed based on iid (independently and identically distributed) among data cannot be applied on the existence of autocorrelation. Also unstable process is difficult to identity or diagnose. Because large-scale process has a lot of measurable variables and multi-step-structures among data, it is difficult to find relation between measurable variables and nonconformity. In this paper, we suggested an appicable model to diagnose the process and to find relation between measurable variables (CTQ) and nonconformity in the process having autocorrelation, unstable condition frequently, a lot of measurable variables, and multi-step-structure. And we applied this model to real process, to verify that the process engineers could easily and effectively diagnose the process and control the nonconformity.

Percutaneous screw fixation and external stabilization as definitive surgical intervention for a pelvic ring injury combined with an acetabular fracture in the acute phase of polytrauma in Korea: a case report

  • Unstable pelvic ring injuries are potentially life-threatening and associated with high mortality and complication rates in polytrauma patients. The most common cause of death in patients with pelvic ring injuries is massive bleeding. With resuscitation, external fixation can be performed as a temporary stabilization procedure for hemostasis in unstable pelvic fractures. Internal fixation following temporary external fixation of the pelvic ring yields superior and more reliable stabilization. However, a time-consuming extended approach to open reduction and internal fixation of the pelvic ring is frequently precluded by an unacceptable physiologic condition and/or concomitant injuries in patients with multiple injuries. Conservative treatment may lead to pelvic ring deformity, which is associated with various functional disabilities such as limb length discrepancy, gait disturbance, and sitting intolerance. Therefore, if the patient is not expected to be suitable for additional surgery due to a poor expected physiologic condition, definitive external fixation in combination with various percutaneous screw fixations to restore the pelvic ring should be considered in the acute phase. Herein, we report a case of unstable pelvic ring injury successfully treated with definitive external fixation and percutaneous screw fixation in the acute phase in a severely injured polytrauma patient.

Effects of the Support Surface Condition on Muscle Activity of Trunk Muscles during Balance Exercises in Patients with Stroke

  • Purpose: The purpose of this study was to examine the muscle activity of the trunk muscles during balance exercises on a stable and unstable surface in patients with stroke. Methods: Thirteen subjects (8 males, 5 females) with stroke were enrolled in the study. Muscle activity was recorded using surface EMG electrodes from the affected side of the erector spinae, external oblique, and internal oblique muscles. The exercise was performed under three conditions. For the first condition for balance exercise, subjects were instructed to sit on an exercise mat with legs extended. The second condition was to sit with legs extended, with a balance pad under their buttocks. The third condition was to sit with legs extended, have a balance pad under the buttocks and a balance cushion under the lower legs. Results: This study showed significant differences in EMG activities during both arm lifting exercise and weight shifting exercise between conditions. This study showed that the EMG activities of the erector spinae, external oblique, and internal oblique muscles were significantly higher when the trunk exercise was performed using the balance pad with balance ball than when using the stable surface. Conclusion: In conclusion, performing balance exercises using an unstable surface is a useful method for facilitating trunk-muscle strength and hence trunk stability.

On the ill - condition of reverse process from structural dynamic response data (구조계의 동적응답을 이용한 역해석에서의 악조건)

  • An approach to identifying input forces is proposed using measured structural dynamic responses and its analytical model. The identification of input forces is a reverse process and ill-conditioned problem. Its solution is unstable and generally case dependent. In this paper, the ill-condition is described considering characteristic matrix which is defined by reduced dynamic stiffness matrix. Special attention is focused on the condition number of a characteristic matrix used in the solution algorithm of this reverse process. Simple example is presented in support of the ill-condition of a reverse process.

The Life Course of Poor Female Household Heads: A Qualitative Analysis (빈곤 여성가장의 삶의 과정에 관한 질적 연구)

  • This study explored and described the life course of poor female household heads. The data was gathered by in-depth interviews with thirteen poor female heads of households. Most of poor female household heads grew up in poor families. nev could not be educated properly due to their families' poverty and boy preference, and they moved to urban areas to become informal sector laborers. After the marriages, their living conditions became worse, because many of them got married to so lazy men who had alcoholic problems that were not willing to provide their families. The poor female heads of households were under difficulties owing to low income and unstable labor Condition. The economic hardship disturbed the interaction with kins and friends. Although they got in touch with these people, they had uncomfortable feelings because they did not have enough resources which made these relationships reciprocal. Therefore, social welfare policy were essential to support the poor female heads of households.
