As the elderly population increases, they are increasingly affected by oral health problems. Therefore, efforts are being made to improve the oral health of older people, alleviate mental discomfort, and reduce unmet dental needs. This study was conducted to confirm the relationship between the National Health Insurance Elderly Denture Coverage and the unmet dental need for the edentulous elderly, as part of the protection policy. We analyzed the 2011 and 2013 Community Health Survey data of the edentulous elderly, aged 75 years or older, before 2012. In order to more precisely confirm the effects of the denture donation policy on unmet dental care, basic life recipients who were subject to the free elderly prosthetic project were excluded from the analysis. The final analysis included 20,400 subjects. According to our investigation of the factors that affect the unmet dental needs of the elderly, the National Health Insurance Elderly Denture Coverage did not affect unmet dental needs. The statistically significant variables that affected the unmet dental needs of the elderly were education and income levels, which are representative socioeconomic status variables. The lower the level of education, the unhealthier the dental care experience, and income levels showed a similar tendency. The elderly who have a low socioeconomic status are more likely to experience unmet dental needs because they lack the knowledge and socioeconomic ability to pay for dental care. Therefore, the policy for health protection of the entire elderly population should be continuously expanded. In addition, the socioeconomically vulnerable groups may have health problems due to the restriction of medical use, which may lead to quality of life deterioration.
본 연구는 손상환자의 미충족의료 관련요인을 총괄적으로 조사·파악하여 미충족의료에 관련이 있는 요인을 제시하기 위하여 시행되었으며, 연구대상은 국민건강영양조사에 응답한 손상 환자 중 미충족의료 경험이 있는 2169명을 대상으로 시행하였다. 연구방법은 빈도 백분율, Chi-square test, 다중회귀분석을 사용하였고, 이는 SPSS/WIN 23.0 Program를 이용하여 분석하였다. 연구결과, 스트레스가 있을수록, 우울감을 느낄수록, 자살생각을 할수록 미충족의료와 관련이 있는 요인으로 나타났으며, 이는 향후 미충족의료가 발생하지 않도록 의료이용 제도를 개선하고 정책 결정을 하는 데 기여할 수 있을 것으로 사료된다.
Objectives: It is crucial to detect and prevent oral problems early. Older adults, in particular, should go for regular dental checkups to maintain a healthy oral environment. This study identified the factors affecting regular dental examinations and unmet dental needs in older adults. Methods: This study analyzed 3,362 older adults aged 65 or over using the data from the 7th Korean National Health and Nutrition Examination Survey (KNHANES). Frequency analysis was performed on general characteristics. In addition, chi-square tests were done to determine differences in regular dental checkups and unmet dental needs according to general characteristics. Logistic regression analysis was performed on factors influencing regular dental checkups and unmet dental needs. Results: Factors influencing regular dental checkups were statistically significant according to residential areas, level of education, household income, and levels of physical activity (p<0.05). Factors influencing unmet dental needs were statistically significant according to residential areas, level of education, private medical insurance, and levels of dental pain (p<0.05). Conclusions: National level policy strategies should be implemented to improve the oral health of older adults. These strategies should encourage regular dental checkups to effectively lower the rate of unmet dental needs.
Unmet healthcare is an important indicator to measure accessibility of healthcare services. To examine the latest status of unmet healthcare needs in South Korea, the four different data which is composed of nationally representative sample of South Korean population were used; the Korea Health and Nutrition Examination Survey (KNAHANES, 2007-2019), the Community Health Survey (CHS, 2008-2019), the Korea Health Panel Survey (KHP, 2011-2017), and the Korean Welfare Panel Study (KOWEPS, 2006-2019). The proportion of individuals reporting unmet healthcare needs were 5.8% (KNHANES), 5.3% (CHS), and 11.6% (KHP). Annual percentage change (APC) which identifies trend for the follow-up period was -9.5%, -8.0%, and -6.5%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.1% (KNAHANES), 0.7% (CHS), 2.4% (KHP), and 0.4% (KOWEPS). The APC was -10.5%, -14.2%, -12.2%, and -19.6%, respectively. Compared to last year, the rate of unmet healthcare needs has declined in general. However, the low-income and the elderly population were reporting the highest rate of unmet health care needs, and the disparity between lowest and highest groups were remained. These results suggest that adequate benefit coverage is needed for low-income and elderly population.
Background: The purpose of this study was to identify factors affecting unmet healthcare needs due to physical accessibility by residential area by utilizing the Korea Community Health Survey (KCHS). Methods: Andersen's medical service behavioral model was applied to analyze the enabling factors, predisposing factors, and needs factors of unmet healthcare needs focusing on residential areas. This study used data from the KCHS (2017-2019, n=440,792). We used multivariate survey logistic regression analysis in order to identify affecting factors. Sub-group analysis was conducted in order to evaluate the effects of residential areas. Results: Some participants (2,621, 0.59%) had experienced unmet healthcare needs due to physical accessibility and 2,047 subjects (78.1%) of them lived in rural areas. Multivariate survey logistic regressions revealed that experience of unmet healthcare needs due to physical accessibility increased when people lived in rural areas (odds ratio [OR], 3.95; 95% confidence interval, 3.46-4.51). Conclusion: This study showed that despite the development of transportation and efforts to alleviate medical inequality, residents in rural areas may still have higher experience of unmet healthcare needs due to physical accessibility compared to the metropolitan city regardless of any other sub-group differences (OR range, 1.90-6.31). This study suggested that government and policymakers should identify the causes of the experience of unmet healthcare needs due to physical accessibility and should develop policies to alleviate those healthcare disparities.
이 연구는 지역사회에 거주하고 있는 중고령자의 미충족 돌봄요구 현황을 파악하고 관련요인을 밝히고자 수행한 이차자료 분석연구로, 전국의 45세 이상 일반가구 거주자를 대표하는 표본(n=10,254)을 표집하여 조사한 2006년 제 1차 고령화연구패널을 이용하였다. 미충족 돌봄요구는 기본적 일상생활수행과 도구적 일상생활수행에 있어 다른 사람의 도움을 필요로 하는 상태임에도 돌봐줄 수 있는 사람이 없는 경우로 정의하였다. 모든 분석은 대상 인구집단을 대표하는 추정 통계치를 구하고자 가중치를 적용하여 수행하였다. 연구 결과, 중장년의 7.3%, 젊은 노인의 14.5%, 그리고 고령 노인의 41.8%가 돌봄을 필요로 하는 상태였으나, 이 중 각각 34%, 33%, 24%가 돌봄요구를 충족받고 있지 못한 것으로 나타났다. 미충족 돌봄요구 관련요인은 연령 그룹에 따라 달랐는데, 중장년에서는 교육과 소득수준이 미충족 돌봄요구와 음의 상관관계를 갖고 있었던 반면, 젊은 노인과 고령 노인층에서는 독거만이 음의 상관관계를 가진 요인이었다. 결론적으로, 우리나라 지역사회 거주 중고령자 중 상당수가 미충족 돌봄요구가 있으며, 그 관련요인은 생애주기에 따라 차이가 있었다. 연구결과는 지역 사회 거주 중고령자의 미충족 돌봄요구를 최소화할 수 있도록 지역사회기반 재가서비스의 확충이 시급하며, 특히 교육수준이 낮은 중장년과 독거노인 대상의 서비스 개발 및 보급이 중요함을 시사한다.
Objectives : This study investigated the prevalence of unmet health care needs among Korean adults and related factors. Methods : The study participants were adults over the age of 20 mental health experience from the Korea Health Panel in 2012(n=4,730). Statistical analysis methods used in this study were the ${\chi}^2$-test, Logistic Regression Analysis and other basic statistics such frequency-and percentage using SPSS version 22.0. Results : (1)Significant variables of stress: Factors were age, economic activity, subjective health status, and activity limitation. (2)Significant variables of depression: Factors were age, income class(low) and activity limitation. (3)Significant variables of suicidal impulse: Factors were age, chronic diseases, income class, and activity limitation. Conclusions : Stress, depression, and suicidal impulse can be unmet medical factors; therefore improvement measures and mental health counseling programs in response to suicide impulses, should be developed. Thus there is a need for a health sciences approach.
이 연구는 빈곤층 노인의 미충족 의료에 영향을 미치는 융합 요인들을 규명하고자 하였다. 분석자료는 2011년 한국의료패널이며 Anderson모형에 기초하여 각 요인에 속하는 변수들을 선정하였고 빈도분석을 통해 일반적 특성을, 교차분석을 통해 변수간의 연관성을 검정하였다. 마지막으로 로지스틱 회귀분석으로 미충족 의료에 영향을 미치는 요인들을 분석하였다. 분석 결과, 빈곤층 노인이 미충족 의료를 경험할 가능성이 소인성 요인에서는 무학 및 초등학교 졸업 노인이 고졸이상 학력의 노인보다 약 1.5배 이상 높았다. 가능성 요인에서는 수입목적으로 일하지 않은 노인과 고용주 자영업자 노인이 무급가족종사자보다 1.5배 이상 높으며, 필요요인에서는 일상 활동에 제한을 받는 노인이 그렇지 않은 노인보다 2.9배 높았다. 이 연구의 결과를 통해 빈곤층 노인의 의료 이용시 경제적 부담 증가가 빈곤층 노인의 미충족 의료를 발생시킬 수 있다는 것을 확인하였다.

The proportion of individuals reporting unmet healthcare needs were 6.4% (KNHANES), 5.4% (CHS), and 12.2% (KHP). Annual percentage change (APC) which identifies trend for the follow-up period was -9.9%, -9.1%, and -5.5%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.0% (KNHANES), 0.4% (CHS), 2.2% (KHP), and 0.4% (KOWEPS). The APC was -11.3%, -17.0%, -12.2%, and -21.2%, respectively. Overall, the low-income and the elderly population reported a higher rate of unmet health care needs. Although the overall experience rate of unmet medical care due to cost decreased over the past decade, the disparity between the lowest and highest income groups still remained in 2020. Disparity between income levels and age groups is a challenge to address in healthcare system, and these results suggest the need for adequate health coverage for the low-income and the elderly populations.
Objectives: This study was designed to evaluate whether employment status is associated with the experience of unmet care needs. Methods: This study utilized the Korea Health Panel data from 2012 to 2016. A total of 4,083 workers were selected as baseline subjects in 2012 and were followed for four years. This study used the GLIMMIX procedure under the marginal model while adjusting for covariates. Results: A total of 12.4% of 4,083 people said they had failed at least once to have a treatment or checkup despite the needs. Those more likely to experience unmet care needs were women and people of lower income level, with worse health conditions and chronic disease. Precarious workers, the self-employed, and the unemployed were more likely to experience unmet care needs caused by economic burden than permanent workers (Odds Ratio: 2.14, 2.07, 2.74, respectively). Conclusions: This disparity means that precarious workers and the unemployed are more likely to face barriers in obtaining needed health services. Given their insecure employment status, meeting their needs for health care is an important consideration.
