Dentigerous cyst is a cyst arising by a seperation of follicles from around the anatomical crown of an unerupted tooth within the jaws. The dentigerous cyst is seen during routine radiographic examination. It is detected radiographically as a sharply delineated, round or oval, unilocular, homogeneous, and radiolucent area within which there is the crown of an unerupted tooth. The histological appearance of the dentigerous cyst is usually attached to the tooth at the cementoenamel junction and the sac lined by a thin, regular. stratified squamous epithelium. The dentigerous cyst can be treated by two surgical procedures: enucleation or marsupialization. The size and location of the lesion usually dictates the surgical procedures. The marsupialization consists of the removal of only a portion of the wall of the cyst to make the remaining cystic lining continous with the oral mucosa. The cyst, deprived of its supporting hydrostatic pressure, gradually shrinks until it is obliterated. The marsupialization is the best way to conserve the tooth affected by a dentigerous cyst and to permit its eruption. especially in a young person. Main advantages of the marsupialization are its relative simplicity and its conservation with respect to adjacent important structures. In marsupialization technique, a plugger can be constructed, if necessary, to maintain the opening and prevent food debris from entering the cystic cavity. However, the disadvantage is the potentiality of leaving pathologic tissues. Therefore frequent recall appointments are advisable.
The primary objective of this study is to estimate of the mesiodistal crown diameters of the unerupted permanent successors derived from the mesiodistal crown diameters of the deciduous teeth in Korean population. The subjects were 54 individuals (twenty nine boys and twenty five girls) with normal occlusion aged 6 to 13 years. The mesiodistal crown diameters of the deciduous and the successional permanent teeth were measured from the longitudinal dental cast models using the sliding calipers (Mitutoyo Co.). From the study, the results are as follows; 1. Sex differences of mesiodistal won diameters were less in the deciduous teeth, but male were more than that of female in the successional permanent teeth. 2. The mesiodistal crown diameters of the deciduous central incisors, lateral incisors, canines were smaller than that of the successional permanent teeth and the deciduous 1st molars, and 2nd molars were more larger than that of the successional permanent teeth. 3. Size differences between sum of the mesiodistal crown diameters of central incisors and lateral incisors in the decidous teeth and the successional permanent teeth were $7.20{\pm}1.79mm$ in upper, $5.38{\pm}1.64mm$ in lower and that of canine,1st molar and 2nd molar in the deciduous teeth and the successional permanent teeth were $0.56{\pm}1.19mm$ in upper, $2.22{\pm}1.19mm$ in lower. 4. In male, the correlation coefficients between the upper deciduous central incisor and the successional permanent tooth (r = 0.57) and in female, the correlation coefficients between the upper deciduous 1st molar and the successional permanent tooth (r=0.67) appeared the highest. 5. The regression constants were determined to estimate the mesiodistal crown diameters of the unerupted successional permanent teeth.
The cleft alveolus occurs about 75% of cleft lip and palate patients. The purpose of bone grafting is improve the maxillary growth, rehabilitation of continuty of maxillary arch and providing bone for periodontal support for unerupted teeth. The bone grafting for alveolar cleft defect repair are classsified; primary bone grafting, early secondary bone grafting secondary bone grafting and late secondary bone grafting. In this article, we reported the cases of PMCB grafts for repair of the alveolar clefts showed potential benifit to the patient to induce a normal maxillary growth and providing bone foor periodontal support of unerupted teeth.
Purpose: The prevalence of supernumerary teeth has been reported to be between 0.1% and 3.8%. The aim of this study was to determine the prevalence, clinical significance, and associated pathologies of fourth molars based on a retrospective study and a literature review. Materials and Methods: A 5-year retrospective prevalence study was conducted at the Department of Oral Diagnosis and Dentomaxillofacial Radiology of Okayama University Hospital, Okayama, Japan. The study involved extracting data from the digital records of patients from January 1, 2013 through December 31, 2017. The sampling frame included all patients who had panoramic radiographs, cone-beam computed tomography (CT), and multislice CT images during the period under review. Results: A total of 26,721 cases were reviewed and 87 fourth molars were identified. The prevalence of fourth molars in the 5-year study at Okayama was calculated as 0.32%. The mean age of patients with a fourth molar was 30.43 years, and the male-to-female ratio was 1:0.98. The vast majority of cases were in the maxilla (92%) and had normal shapes(89.7%); furthermore, 82.8% of cases were unerupted. Conclusion: The prevalence of fourth molars in the study population was found to be 0.32%, and fourth molars occurred with approximately equal frequency in males and females. Fourth molars were more common in the maxilla and were predominantly unerupted and small.

Marsupialization is widely used as a primary treatment modality for reducing size of large cysts. However, there is no recommendation for specific duration of marsupialization. In addition, Carnoy's solution usually is applied at the time of enucleation as a fixative agent. In this report, we present an appropriate marsupialization duration of ameloblastoma involving two unerupted teeth. In this present study, marsupialization using a Foley catheter was performed in two cases of ameloblastoma of the mandible involving two adjacent impacted teeth. Carnoy's solution was applied for 3-5 minutes after enucleation in both patients. Periodically during marsupialization, the size of the radiolucency was measured in panoramic view, and clinical examination was performed. No remarkable paresthesia or soft tissue injury was observed after application of Carnoy's solution or during follow-up. We recommend 12 to 16 weeks as an adequate marsupialization duration for a large ameloblastoma involving two impacted teeth based on increased radiopacity along the margins of the lesions. Poor oral hygiene was an issue after 12 weeks of marsupialization in one case. There were no remarkable complications with Carnoy's solution in either case. The Foley tube has a two-way system that is more effective for irrigating the cavity than is the conventional one-way system.
The primary objective of this study was to make the prediction percentile tabulation of the sum of mesiodistal widths of the unerupted permanent canines and premolars derived from the sum of mesiodistal widths of the erupted permanent incisors in normal and crowding groups. Crowding group consisted of 85 pairs of dental casts (42 males and 43 females) with more than 4mm of crowding in anteriors. Normal group consisted of 109 pairs of dental casts (65 males and 44 females) with less than 1mm of spacing or crowding in anteriors. The mesiodistal crown diameters of permanent teeth were measured from the dental casts using the sliding calipers (Mitutoyo Co., Japan). From the study, the obtained results were as follows: There were not significant differences between male and female in both groups on the basis of the ratios between the sum of mesiodistal widths of incisors and the sum of mesiodistal widths of canines and premolars. There was not significant difference between two groups on the basis of the ratio between the sum of mesiodistal widths of mandibular incisors and the sum of mesiodistal widths of mandibular canines and premolars and was significant differences between two groups on the basis of the ratio between the sum of mesiodistal widths of mandibular incisors and the sum of mesiodistal widths of maxillary canines and premolars. There was significant difference between two groups on the basis of the ratio between the sum of mesiodistal widths of maxillary incisors and the sum of mesiodistal widths of mandibular canines and premolars. There was significant difference between two groups on the basis of the ratio between the sum of mesiodistal widths of maxillary incisors and the sum of mesiodistal widths of maxillary canines and premolars. The percentile prediction tabulations were made in an attempt to predict the sum of mesiodistal widths of the unerupted permanent canines and premolars derived from the sum of mesiodistal widths of the erupted permanent mandibular incisors in each group. Dentigerous cysts are common odontogenic cysts that are associated with the crown of the tooth and typically develop from single lesions. Bilateral and multiple dentigerous cysts are very rare and occur in patients with syndromic conditions. This paper presents a case report of a 15-year-old male patient that experienced non-syndromic bilateral dentigerous cysts that simultaneously occurred in all four dental quadrants around the unerupted third molars. Clinicians should confirm the extent of cystic lesions using a panoramic view and computed tomography, and should keep the possibility of bilateral dentigerous cysts in mind as a potential diagnosis, even in a non-syndromic patient.
Mr. yoon, 1 20 years old man, in good health, was treated for unhealing the extraction wound, pus discharge and sensation of dull pain on maxillary 2nd molar areas. Roentgenographic examination showed unerupted 3rd molar in left maxillary sinus. The tooth was located immediatly under the zygomatic bone and directed to median line. Radical operation of its sinusitis and extraction of the impacted wisdom tooth were performed by Caldwell-Luc's operation technique. In morphological aspects, the tooth has resemblance to normal wisdom tooth.

The dentigerous cyst originates through alteration of the reduced enamel epithelium after amelogenesis is completed, with accumulation of fluid between the layers of the enamel epithelium or between this epithelium and the tooth crown. It is always associated with unerupted teeth. There are two methods of treament which are generally accepted. One is the enucleation of the cystic sac in its entirety, the other is the Partch operation, or the marsupialization, by which the cyst is uncovered or "deroofed" and the cystic lining made continuous with the oral avity or surrounding structures. The present case report showed that in instance of dentigerous cyst & odontogenic keratocyst surgical enucleation followed by careful excision of all cyst wall was successfully carried out and recovery and healing were rapid and uncomplicated.
